3 1. Pedestrian Safety Road accidents may not only involve drivers & passengers but also pedestrians us. Although we may have own vehicles but at some point of time we will be pedestrians. Many accidents could be because of bad driving habits but pedestrian s responsibilities cannot be ignored. As a pedestrian we have certain responsibilities to avoid putting ourselves in road dangers. Always look for cars or other vehicle for yourself although you may be crossing in groups. Choose the safest routes to walk with the fewest and safest streets to cross. Avoid crossing busy or high-speed roads. Use sidewalks or footpaths while walking along the streets. Always use footbridge or subway if available for crossing the road. After stopping, look, listen, think. Take enough time to look and listen. When there is no footpath, then walk facing the traffic as far from the traffic as is practicable. Cross the streets at marked crosswalk or intersections; do not cross mid block. If walking along with children, keep them away from traffic as adults are more visible.
4 When crossing the road: Stop at the curb and look right, left and right again for traffic. Wait until no traffic is coming and begin crossing. Keep looking for traffic until you have finished crossing. Walk, don t run across. Avoid distractions such as cell phones. Avoid music which may disable you of someone alerting about dangers. Cross in a well-lit area at night. Be Bright at Night. Wear light coloured clothing. Never play near the traffic Stand clear of buses, hedges, parked cars or other obstacles before crossing. Obey all traffic rules / signals Don t solely rely on traffic signals; look for vehicles before crossing

Lessons Learned 2.1 Electrical Shock Division Date of Incident Type of incident Incident Time Location Injury Nature : BORL Bina :21 st Nov 15 : Lost Work Day Case :10:07 Hours : Sub Station 10, PMCC 301 (415 V) Board, Motor Feeder of Seal Pot Blower#1 (Boiler # 1). : Burn injury on Face, both hands up to elbow (2 nd degree 12% burn) Description of Incident: Incident revealed that on 21 st Nov 2015 prior to occurrence of the incident, electrical isolation permit (EIP No ) was received by shift electrical team (to permit Seal pot Blower-1 discharge valve gland replacement by mechanical maintenance). This Seal pot blower-1 having feeder no. - 4-LM , in PMCC -301 switch board is a SFU (Switched Fuse Unit) type module & located inside switchgear room of Sub-Station -10, at CPP. During isolation process, he first turned the SFU isolator handle from ON to OFF position. Then he opened the door of the panel & switched OFF the control MCB (both AC & DC) & removed 2nos of power fuses successfully. While removing the 3rd fuse using plier, it touched the indicating Lamps mounting plate & thus resulted in short circuiting between R-Phase to earth; causing flash over.
6 KEY OBSERVATIONS / FINDINGS 1. The person turned the SFU switch handle to OFF position but the internal switch mechanism did not turn to OFF position. During the switching process of SFU isolator handle, it slipped to OFF position without doing the SFU electrical isolation, which was not realized by the injured person. 2. One of the door lock of SFU panel was in damaged condition resulting in slip between operating handle and operating rod.

During removal of the 3rd fuse using plier, as the indication lamp mounting plate (metal) was not moved away from its position (necessary before removal of fuse), plier touched the metal mounting plate & resulted in short circuiting between Phase to earth; causing flash over. 4. He was not wearing Arc flash suit, safety goggles & insulated rubber hand gloves while doing the isolation of the feeder. He has only worn Safety helmet, Safety Shoes & Boiler Suit. He was accompanied by engineering supervisor. 5. SOP was not followed, as the feeder OFF condition should be cross checked through Multi-meter prior to removal of control & power fuses. 6. In this case over confidence of the injured person is a matter of concern. Root Cause Analysis Human Factors: 1.1 Violation by Individual SOP was not followed: The feeder OFF condition was not cross checked through multi-meter prior to removal of control & power fuses. Arc flash suit was not used. 2.2 Improper Use of Tools: Pliers was being used to remove the power fuses. Physical Factors 5.7 Inadequate isolation of equipment SFU Isolator Handle moved to OFF position without changing the switch position: One of the door lock was in damaged condition. 7.3 Work exposure to Energized Electrical System- There was no indicator available in the SFU feeder module to indicate electrical isolation. System Factors: 8.2 Inadequate Preventive Maintenance One of the door lock remained in damaged condition. Fuses were having broken notches. Recommendations 1. Present SOP should be reviewed and modified in consultation with M/s BORL considering the findings of this incident. Further necessary training should be imparted to all electrical Engineers. 2. Ensure strict adherence to Standard Operating Procedure (SOP). 3. Enhance the competency level by training 4. All fuses used in substation need to be checked for availability of both notches. 5. PM checklist of switchgear should be revised to ensure healthiness of door locks, Isolator handle and condition of fuse notches. 6. Modification should be done in consultation with BORL on Contactor Type module in order to get feedback of isolation.

12 5. Corporate & Divisional Safety Highlights Corporate Safety Conducted weekly, monthly & quarterly field safety audit at following sites: 1. Transmission (Borivali, Salsette & Kolshet) 2. Trombay (Trombay Unit#7 air intake system & scaffolding job, Conveyor 15A/B dismantling) "Stree Shakti" program organized as part of woman safety for Trombay Woman employees 6 th Incident Investigation Knowledge Sharing Workshop Tata Power, India s largest integrated power company has always been committed to safety for all its stakeholders and has a vision of achieving ZERO injuries. Safety has been its core value and always the top most priority for us in Tata Power. Many initiatives have been taken within our organization to improve the safe working environment for employees and partners in the last several years. We believe that life of fellow colleagues & associates is dear and more important than production. In connection with this Tata Power Incident Investigation Corporate Sub Committee organized 6th Incident Investigation Knowledge Sharing Workshop on 18th December at Haldia. The objective behind this workshop was to:- 1. Brainstorm with the site teams of Incident Investigation committee for various initiatives to be taken to prevent incidents 2. Improve quality of incident investigation to get effective action points 3. Conclude unified solutions to avoid recurrence of incidents 4. Bench marking the process with other divisions and companies. Mr. Sethi, COO&ED in his message to the team emphasized the need to conduct such sessions to involve the employees and build commitment towards safety.
13 Mr. Sharad Baijal Chief Corporate Operations (Thermal), inaugurated the session and released the booklet on Lessons learned from FY-15 incidents which will be used as a ready reference to identify and implement the recommended recommendations from incident investigations. All Tata Power divisions participated in the workshop and shared their valuable learning s & thoughts. The team finally came out with a 10 point action plan to be implemented across Tata Power based on the common themes emerging from analysis of the incidents. Photographs:- 1. Garlanding to J. N. 14 Action Points from 6 th Incident Investigation knowledge sharing meet. 1. Explore use of low voltage/battery operated hand tools. 2. Reflective tapes should be used on electrical incomer panel s for identification during black out. 3. Portable tools wire should not be more than 3 meter in length. Electrical board should be provided with ELCB to connect portable tools. This electrical board should be connected by wires to the main power supply board. 4. Use LED hand lamps/helmet with torch/lighting at remote sites. 5. Revise divisional Safety Induction module incorporating changes based on lessons learned. A uniform training module should be used across Tata Power. 6. Arrange training sessions on Incident Investigation to improve quality of investigation. Maintain & circulate the list of trained investigators. 7. Duties of Electrical Safety officers to be circulated in line with CEA guidelines. 8. Color coding to be done to identify new employee based on experience. 9. Explore the use of Harsco Scaffolds at all company locations for erection and certification beyond three meters. 10. Exploring the use of Load cell during lifting of equipment s (Ex: EOT crane). Mobile Application Tata Power SURAKSHA for reporting of Unsafe Observations / Near miss is rolled out
15 6. Security Highlights I. Trombay Training activities Police officials in civil with arms hidden in their body from various Police Stations approached Main Gate to seek an entry as a decoy on 03 occasions and tried to penetrate in our security system but failed to succeed due to proper checks at entrance. Failing in their attempt to enter they showed their identity cards and disclosed their police batch numbers and said that they tried to penetrate the security of the plant with arms but failed.
16 BCDMP Awareness session for HODs Trombay conducted on 11th Dec 2015 Mock Drill Conducted on scenario heavy leakage of water is developed in the interconnection line between u#7 and u#8 Pipeline Area on 15 th Dec 2015 from 1100 hrs to 1131 hrs. Necessary participation was made by the Security, QRT, Fire and Medical Teams. New Safety Induction Training Venue for Contractor staff at FOT A Area duly inaugurated by Chief Trombay in the presence of Union Representatives & Security/CMD/EMD staff. The training venue is having the seating capacity of 50 persons with LAN Connection/PC/Projector and other amenities.
