3 1. Why use Helmet? Facts about 2-wheelers in India- 69 % of total motorized vehicle are 2 wheelers Two wheeler rider is 30 times more likely to be killed than a car driver Death most common in young people. 72% in age group of years 2-wheeler riders are more vulnerable to accidents because- Lack of physical protection Less visible Lack of proper education about road rules & safe driving Share same road with fast moving cars, buses and trucks Most common cause of death for 2-wheelers in accident is Head-Injury 75% to 88% of the deaths are due to Head Injury. Head injuries are serious because- Treatment is very costly Requires long hospitalization & follow up High chances of disability even after treatment / recovery What can be done? Prevention is better than cure. By practicing defensive driving, accidents can be avoided We can prevent Head-Injury to great extent by using HELMET. They are effective in reducing the likely hood of Head-Injury as well as their severity (by reducing impact force) Advantages of helmets- Reduces the likelihood of death by 39% Reduces risk and severity by 72% Reduces cost of health care Prevents collision with flying insects Riding in rain possible Helps in controlling sun burn Protects your eyes from the blast of moving air & dust

4 Outer hard coat Mechanical barrier- no direct contact Impact absorbing layer (1.5cm to 3cm) Absorb some of the impact Head comes to a halt more slowly so reduced deceleration less injury Comfort Padding Keeps Head comfortably -- Helmet fits snugly Spread the impact over a greater surface area so they are not concentrated on a particular area of skull Chin Strap should not be loose, otherwise Helmet will come out during accident

Lessons learned 3.1 Electrical shock Date : 31 th October 2012 Time : 17:10 Hrs Location : Malad Sub Station Injury Details : Burn injury on left hand & shoulder Description : A problem was encountered while taking back 22kV / 11kV 10MVA DT # 2 into service. A and C phase GOD arms were closing properly whereas B phase GOD arm was not. The victim climbed on the H Frame of 11 kv BMC lagoon # 2 / BMC pumping # 2 GOD in order to trouble shoot which was half LIVE as the cable was back charged. He came in the arcing zone and sustained burn injuries on his left shoulder (left dorsal) and left hand. Root Cause Analysis: Human Factors 1.2 Violation by group - The Victim did not inform the NMMC engineer that he is attempting for trouble shooting and same time the Engineer failed to stop the victim from climbing on H Frame 4.1 Improper decision making or lack of judgment The victim undermined the risk of arcing Physical Factors 5.1 Inadequate guards or protective devices - No barricades to prevent inadvertent entry of personnel on wrong equipment 7.3 Energized electrical systems - the 11 kv BMC PPG # 2 / BMC # 2 GOD was half live System Factors 10.1 Inadequate technical design - design input obsolete 11 kv BMC PPG # 2 / BMC Lagoon # 2 GOD cables live jumpers are in close proximity with H-Frame Marshalling box of 11 kv BMC PPG # 2 / BMC Lagoon # 2 GOD provided easy access 8.2 Inadequate leadership / Supervision - The NMMC engineer was not vigilant enough to ensure that the victim does not climb on H frame. Proper job planning was not done Lessons learned: 1. Live Line Indicators to be provided on all back to back cables for visual indication of cable charged condition. 2. Physical Barricades of permanent nature to be provided near 22kV & 11kV cable terminations to prevent inadvertent access to employees to live portion.

8 3. Strengthen the process of Tool Box Talk (TBT) / Job Safety Analysis (JSA) communication to emphasize Safety expectations. 4. Permanently demarcate Distribution and Transmission assets by means of permanent barricades & handing over the Distribution Assets to Distribution Customer Services (DCS) for Carrying out Operation & Maintenance activities 5. As lack of Operational Discipline is evident from the investigations & interviews of concerned employees, appropriate action as per Consequent Management policy is recommended. 6. AIS with indoor switch gears & 22kv & 11kv RMUs are to be replaced as they are outdated. 3.2 Fall of RCC beam- Near-miss Date : 7 th November 2012 Time : 13:30 Hrs Location : NGT area, Saki GIS While lowering the dismantled RCC beam using 20mm PP rope, the rope slipped from the workers hand as they could not controlled the momentum. As a result, the beam landed on the ground by force. Root Cause Analysis: Human Factors: 1-2 Violation by Group Safety control measures not deployed as per job safety analysis 1-10 Shortcuts Process followed for breaking earlier beams was by passed 4-1 Improper decision making or lack of judgment Misjudgment of beam weight & beam lowering method Physical Factors 7-9 Clutter or debris Fall of RCC beam System Factors: 5-7 Supervisor implied haste - Intense work pressure from supervisor to finish the demolition work 11-1 Inadequate work planning Safety control measures not deployed as per job safety analysis Lessons learned 1. Review manual lowering and use of PP rope procedure in the light of object weight and shape 2. Ensure effective supervision and follow of work procedure / job safety analysis 3. Lesson learned shall be communicated to all contractors personnel

Security Highlights Women Safety Government initiates: The Government has initiated some measures like improvement in public transport system at night, GPS on buses, verification of staff and dress and id badges for them, increase in the number of PCR vans and equipping these with GPS, deployment of specialized police staff at night, crack down on all vehicles with tainted glasses and curtains, 24 hour police cover for areas around malls, discos, eateries, cinemas, and urging women to call 100 to seek assistance to go home at night. India Inc beefs up security for women: A list of measures adopted by India Inc to strengthen protection of women includes (but not limited): Travel security: Vehicle tracking system in cars; single-button press facility in phones to send distress call to a pre-arranged list; armed escort till home; confirm safe arrival at home post late night shifts; enforcement of stringent processes related to first female board and last drop process whereby no woman ever travels unescorted in a cab and encouraging employees to travel in groups. Training and awareness: workshops in awareness and self-defence; cobra pepper spray (an instant spray that injures the attacker). Sensitising male employees to enable them to provide help when required. First preference to women in parking lots, irrespective of the designation, so that they do not park in unsafe areas. While travelling on work, allowing women to upgrade to better rooms and accommodation, if they are not satisfied. Updating and sharing database of blacklisted vendors and drivers so that they are not hired. What should women do? Do not rely solely on the state or your company to protect you. Round the clock 24 hrs protection may not be feasible. Trust your instincts about people and situations; be alert and aware about your surroundings. If you sense danger or feel someone is following you, step into a crowded place (but not too crowded). Call a friend or relative to escort you back. Tinted glasses and curtains, and suspicious vans are a strict no-no. As soon as you enter a bus or a van, or enter an area which appears risky or unknown, note the number of the vehicle or place and quietly sms to a friend. When getting into or out of your car, watch out for anyone hanging around or a single man in any car parked near yours. Run back to safety if you are suspicious. Walk confidently, do not appear nervous or afraid. Criminals tend to target scared-looking, nervous women.

12 If assailant has a gun, run away as fast as possible; there is a fair chance that he will not shoot; and even if he does there is an even greater chance that the bullet would miss you. Keep Police Control room number or another emergency number of a friend or relative on your speed dial. You could keep a pepper spray handy; if not, at least have a perfume or hair spray. Learn self-defence techniques; the law permits self-defence, including use of force. Be polite, but careful. Never open the door to a stranger when alone nor stop to help a man in deserted area; you can call the cops or inform office 24x7 control room if you want. When a mechanic or plumber or electrician is due for work at home, please ask your husband, neighbour or friend to be present Do not leave door keys in obvious places (like near the main door). Dress appropriately; do not flash wealth. Avoid eye contact with suspicious strangers. Keep away from mobs. in a mob, every individual has the potential to turn into a violent criminal. 16 II. T & D Security & Vigilance week was observed from 26/11/12 to 2/12/12 at Dharavi, Carnac, Parel, Mahalaxmi, Borivali, Kalyan & CENTEC-Technopolis. III. CGPL To regularize the traffic movement, parking and to curb the over speeding in the plant premises traffic team conducted various check Nakas and drills. Mock drill was conducted depicting a scenario of fire and worker sustaining minor burn injuries at JNT#30 conveyer belt. On 8th Nov 12, Labour Head Count Exercise (LHC) was conducted wherein all access locations were covered for the accurate manpower strength. A total of 1847 labours (199 at field hostel gate, 1050 at main gate & 598 at Tunda Wicket gate) were physically checked by Security Team. On 1st Nov 12 security meeting held in between CGPL security team members and official from SISA security agency.

17 IV. JOJOBERA Security awareness classes were given to 221 contract workmen to bring awareness of the Do's and Don ts in the plant with respect to security. Fire fighting training was given to 34 security personnel of GURU Security Force. New parking for employee vehicles started with the help of civil department. Visitor Management System started.
