11 2007 Assessment May 2007, ASEPA requested ERS assistance for a hazardous materials assessment at the site. Field screening for heavy metals with the XRF and PCBs with Chlor-noil test kits. Measureable amounts of Pb, As, Cr, Fe & Sb found that exceeded Industrial PRGs, but not enough to warrant a Superfund removal action. 15 th Annual OSC Readiness Training Program 10
12 In October 2007 the American Samoa Power Authority (ASPA) awarded a contract to have the seven acre site cleared. During the initial period of scrap metal removal, a cylinder (most likely anhydrous ammonia) was crushed by a piece of heavy equipment. Local Gov t Contract Award Over one dozen site workers were exposed to the ammonia gas and had to be sent to the hospital for observation. No one was seriously harmed Following the incident the Contractor made a sincere effort to segregate all hazardous materials they encountered. 15 th Annual OSC Readiness Training Program 11

17 Contract Close-out Problems Early 2010 March 2010, approx. 800 gas cylinders had been accumulated. Contractor began indiscriminately removing cylinder valves and venting them to the air. ASEPA with FBI immediately shut the site down ASEPA advised ASPA to require a written plan from their contractor before any work removing the cylinders could resume. The first attempt by ASPA s contractor was horribly inadequate. 15 th Annual OSC Readiness Training Program 16
