Completion of this form is not mandatory, but is strongly encouraged. This form is part of our duty of care to our officials and their welfare and will not be used for any purpose other than obtaining medical and health information, to be readily available on site if an incident or accident was to occur. Official Requirements As the events will be conducted over a number of areas, Surf Life Saving NSW is looking for capable Officials who meet the following requirements: Current financial member of a club for 2018/2019 season Hold a minimum of a current Core/Junior Official accreditation When appointments have been finalised, all Officials who nominated will be notified by an Official Appointment Memo. Officials Quota Please be aware that there is an Officials Quota System applied to Clubs for the Age, Open & Masters Championships on the number of Officials supplied. To fulfil your club s quota, you are required to officiate for two or more days to help run a successful event. 3 Additional Information All Officials should ensure that they are physically capable to carry out their important functions over the days of the Championships, and are encouraged to undertake a medical check prior to the Championships. All appointed Officials are encouraged to attend the Official s Briefings. Times and locations will be outlined in the Officials Appointment Memo. Appointed Officials are responsible for all costs associated with travel and accommodation for the Championships. Appointed Officials will receive an Official shirt however, requested sizes are not guaranteed. Any late nominations will be placed into the Official s Pool and positions may be allocated if they become available. Officials clothing may be unavailable to Officials appointed through the Official s Pool. While every effort is made to appoint you to your preferred position, it is not always possible to do so.
