Birch House Upton Hospital: Albert Street, Slough SL1 2BJ With the church and graveyard on the left the hospital is located just past this. If reaching roundabout, the patient has gone too far. Brookside Group Practice: Brookside Surgery, Brookside Close, Earley,, RG6 7HG Surgery is next to the Brookside church. Road entrance to surgery is on the bend of Gipsy lane. The Downlands Practice: East Ln, Chieveley, Newbury, RG20 8UY Large Parking at the rear of the practice for both staff and patients. Free to use. Surgery closes between 1-2 The Boathouse Surgery: Whitchurch Road, Reading, RG8 7DP Plenty of parking outside surgery or there is a car park opposite over the road. Tilehurst Surgery: Tylers Place, Pottery Rd, Tilehurst, RG30 6BW Behind the Tyler s rest pub. Parking is very limited at surgery. Pay and display parking available at pub. Twyford Surgery: 6 Loddon Hall Road, Twyford, Reading, RG10 9JA Just off London Road, ample free parking. West Community Hospital: London Road, Benham Hill, Thatcham, RG18 3AS Travelling along London Road from the Newbury direction, the hospital is on the left hand side after the traffic lights. Parking is available and costs 1 for one hour. Go to outpatients reception

5 Wokingham Medical Centre: 23 Rose Street, Wokingham, RG40 1XS No parking at surgery. Use local short stay car parks nearby. Woosehill Medical Centre: Fernlea Drive, Woosehill, Wokingham RG41 3DR On the left hand side, behind the Morrisons, plenty of parking at the supermarket.

There is car parking available a short walk away behind Waitrose Chipping Norton War Memorial Hospital: Russell Way, London Road, Chipping Norton, OX7 5FA Driving out of Chipping Norton towards Southcombe on the A44, the hospital is on the left hand side of the road, on the same site as new Chipping Norton Health Park Horsefair Surgery: South Bar House, South Bar, Banbury, OX16 9AD Front of practice visible from main road through Banbury, car parking behind Surgery. On the junction with Bloxham road, please go to reception on the first floor. Didcot Health Centre: Britwell Road, Didcot, OX11 7JH Opposite large bingo hall. From Shell garage direction Centre is on the left hand side. From the Didcot shops it is past the fire station on the right. Wellington House Surgery: 5 Station Rd, Chinnor, OX39 4PX Free parking available at the surgery Townlands Community Hospital: York Road Henley-on-Thames Oxfordshire RG9 2EB Travelling along Kings Road from the roundabout/town centre, the left/right turn up York Road leads to the hospital car park. Witney Community Hospital: Welch Way Witney Oxfordshire OX28 6JJ On the corner of the cross roads opposite the fire station on the main road. A free multi-storey car park is on the opposite side of the road next to the cinema. Wantage Health Centre: Church Street, Mably Way, Grove, Wantage, OX12 9BN Turn right at the mini roundabout and the centre is on the right hand side, on entrance parking is on the left hand side of the building. On arrival, please go upstairs to the Outpatients reception area.
