United Dance Center Competition Team Audition Packet 2018-2019 Requirements, Expectations, Terms and Policies, Registration Form and Agreement

Requirements and Expectations: Placement Audition: All dancers (including current students) who wish to participate on a competition team are required to attend the Placement Auditions. The purpose for holding auditions is to insure that dancers are placed with the correct team for the upcoming year based off of their current level of dance technique. Students must be in Kindergarten+ as of August 2018 to participate. Placement auditions will be held May 22nd, 23rd, and 24th during the following times (audition times are based off of your dancer s CURRENT GRADE as of May 22nd): Current 8th - 11th Graders: Audition on 5/22 and 5/23 from 3:30-5:00 p.m. (students must attend both days) Current 6th - 7th Graders: Audition on 5/22 and 5/23 from 5:15 p.m. - 6:45 p.m. (students must attend both days) Current 4th - 5th Graders: Audition on 5/22 and 5/23 from 7:00-8:00 p.m. (students must attend both days) Current 2nd - 3rd Graders: Audition on 5/24 from 5:45-6:45 p.m. Current Pre K, Kindergarten & 1st Graders: Audition on 5/24 from 4:30-5:30 p.m. Dancers need to wear a fitted tank top and fitted dance shorts or fitted dance pants / leggings to auditions. Bare feet are acceptable. If your student already owns ballet shoes or foot undeez please have them bring them to the auditions, as well as any type of sneakers. Auditions will run like a regular dance class. Dancers will warm up and show flexibility and do jazz, ballet and hip hop technique. Dancers do not need to prepare any type of routine prior to the audition. Dancers must bring the last 3 pages of this packet to auditions with $25.00 cash or check written to United Dance Center attached to the packet. Students must have these items to audition. The $25.00 cash or check covers: $5.00 Audition Fee + $20.00 Registration Fee for the 2018-2019 dance year. The Registration Fee for families is $35.00. Families with multiple dancers auditioning should pay $35.00 for the Registration Fee + $5.00 per student, for the Audition Fee. Dancers will be placed on teams based off of technique level. Students are not placed on teams based off of parent opinion or wish. The placement auditions are closed to parents. As our studio has quickly grown, we may not be able to place every dancer that auditions onto a competition team. For those dancers who do not make a competition team, they will be required to take one full year of both ballet and jazz classes before auditioning again in May 2019. We will try our best to put all students on a competition team, however please know that this may not be possible. Dancers of all technique levels are welcome to audition. We will announce the members of each competition team on our website on Friday, June 1st by 10:00 a.m. Other important information will be available such as class schedules for the required 4 week summer course (July 9th - August 3rd) and class schedules for the 2018-2019 dance school year. (We are not able to create the class schedules until auditions are over and teams are created. Attendance: It is expected that dancers who are placed on a Competition Team commit to both the 4 week summer course, and the entire 2018-2019 school year. When a student does not fulfill their obligation to complete the entire school year, it effects the entire team. Dancers who quit will be charged full tuition for the 2 months following the month after class attendance ends. This policy begins when the student attends the Placement Auditions in May. There are no exceptions. Parents and dancers should take this into consideration prior to auditioning. If a student quits in the summer at any time, including prior to the summer course starting, the same rule applies and 2 full months of tuition will be required for the parent to pay. Consistent class attendance is extremely important for a dancer to progress, grow and improve. One student s absence effects the entire class of students, therefore all dancers are expected to come to class each week, and on time. Absences will be monitored. If a student is absent 2 times in any required class, parents will be contacted. If a student is absent 3 times in any required class, parents will be contacted and students will be considered on probation. If a student is absent a 4th time, the dancer may end up being removed from the team at the Directors discretion. We understand that emergencies and illness happen. If your daughter must miss class we ask that you let us know ahead of time via email so that the absence will be marked as excused prior to missing class. We do not have make up classes for students and tuition will not be discounted for missed classes. Students should not miss class for reasons such as birthday parties, going shopping, extended family in town, or orthodontic appointments (please schedule these appointments at other times).

Priority: Dancers must make competition dance practice their priority. All other extra curricular activities will need to be secondary to our required team class times. Summer Course Requirement: We will hold a 4 week Summer Course from July 9th - August 3rd. Competition Dancers are required to attend the summer course to keep up and improve technique throughout the summer. If a dancer has a family vacation or girls camp already planned during that time, please contact Megan (UDC Director) to discuss the conflict. Dancers who are placed on a competition team will automatically be registered for the summer course after they are placed on a team. The pricing for the 4 Week Summer Course will be similar to the monthly tuition that you are willing to commit to on the last page of this packet. Competition Routines: Each Competition Team will have 1-3 competition routines for the upcoming school year. Each team will also have one ballet routine for our Studio Spring Recital. Please view the following pages to mark the number of hours, days, and competition numbers you would like to commit to. Competitions & Recital: Competition Dancers are required to attend two local Competitions in the Spring of 2019. We attend one competition in March and one Competition in April. These competitions are no further than 60 minutes from our Studio. All dancers who are on a team with 2 or more competition routines will also attend one competition in St. George. All competitions charge a fee of approximately $30.00 per dancer, per routine. The competition fee will be due on August 7th, 2018 for the upcoming season. All United Dance Center students will participate in our Annual Spring Recital in 2019. We do not charge a Recital Fee. Tickets will be approximately $5.00 for all spectators to help cover building rental costs. Costumes, Warm Ups, and Class Attire: Each routine dancers participate in will require a costume. We try very hard to find high quality, appropriate costumes at a good price. Competition costumes are approximately $55.00 per dancer, per routine she is in. For teams who compete a hip hop routine, costumes are $75.00 because tennis shoes will be included with the costume (unlike other costumes). The costume fee will be due October 7th, 2018 for competition dancers. Dancers will be required to purchase the correct dance shoes/foot undeez and tights for Competitions. More information will come. Warm up jackets cost approximately $60.00 and pants cost approximately $30.00. Purchasing warm ups is optional. You may purchase the jacket only, or the set. Purchasing a costume bag is also an option for about $30.00. We will begin sizing for warm ups in September. Wearing the correct class attire is important! Dancers are expected to wear the correct attire to every class. Ballet Attire - Pink or Black Leotard of any color. Leotards may have a short cap sleeve, or tank top type straps. Leotards may be purchased anywhere. Light Pink Tights and Light Pink Canvas shoes must be purchased at The Glass Slipper in American Fork or online. Short (mid thigh), thin ballet skirts are optional. Hair must be pulled back into a bun or a pony tail. Jazz/Contemporary/Lyrical Attire - Dancers must wear fitted tank tops, or a fitted cap sleeved shirt. No baggy t-shirts are allowed. Dance shorts or dance pants may be purchased at The Glass Slipper or online. Bare Feet, Foot Undeez, or Jazz Shoes are acceptable for class. As noted above, dancers will be required to purchase the correct shoes and tights for competition/ performances. More information will come in October for performance shoes/tights. Hair must be pulled back into a bun or a pony tail. Hip Hop Attire - Dancers may wear the attire listed above for Jazz, with the option of adding on baggy, stretchy pants, and a baggy t-shirt. Sneakers are required for this class (any sneakers that the dancers wear to school, work great).

Terms and Policies / Contract 2018-2019: Please initial below next to each policy stating that you are aware of and agree to the following terms: I hereby release United Dance Center and all staff members and officials from any claims of damages or injury suffered by the registered student(s) in connection with or by their association with United Dance Center. This includes my heirs who may act in our behalf. I give my permission to United Dance Center to use photos of my dancer online (for the website, Facebook, Instagram, etc) or in an advertisement. I understand that tuition is determined on an annual basis and is not pro-rated monthly. Some months have more classes than others due to holidays, extra practices, longer/shorter months, etc. There are no classes held on days that there is no school. Tuition is due on or before the seventh day of each month. A $10 late fee will be assessed for all payments made after the seventh of the month. No refunds are given for any reason. Tuition is not lowered if classes are unattended. If tuition is more than 45 days late the student will not be able to participate in class. All balances on your account must be paid in full before receiving any costumes or performing. If an account goes 90 days past due, it will be sent to collections. I understand that if for any reason my dancer does not complete the dance year (through May 2019), I will be required to pay 2 full months of tuition after my student stops taking class. This policy applies during the summer of 2018 as well as throughout the 2018-2019 school year. This policy begins May 2018 following team auditions, and ends at the end of May 2019. I understand that a $20.00 Registration Fee is due with this form + $5.00 for the Audition Fee. I understand that consistent class attendance is extremely important and that all dancers are expected to come to class each week on time. I understand that my signing this form as a parent/guardian, I am committing myself and my dancer to attend classes and all competitions for the entire 2018-2019 school year. I understand that after my dancer is placed on a team following auditions, an online account will be created by the studio director and I will be registered in the software used by United Dance Center for the 2018 Summer Course and the 2018-2019 dance year. (Parents are welcome to use this software to make payments online, but are not required to. Details to come.) Company Agreement As a parent/guardian, and as a dancer in membership with the 2018-2019 United Dance Center Competition Teams, we have read through the Competition Requirements, Terms and Policies, and understand the above Rules and Requirements to participate with United Dance Center. Failure to follow the Requirements and Terms will result in dismissal from the Company. I understand the time commitment and financial commitment associated with registering to participate on a Company Team. I understand that if I do not fulfill this contract through May 2019, I will be responsible for 2 months of tuition, after I stop coming to class. As the dancer, I understand that having a positive attitude is of the highest priority in class and crucial for a successful team. As the dancer I understand that gossip, excluding other dancers, and any type of bullying will not be tolerated and will result in immediate dismissal from the Company. Competition Teams - Time and Financial Commitment At UDC, some of our teams have weekly practice on Saturdays (between the hours of 8:00 a.m. and 1:30 p.m). Please mark if you are ok with weekly Saturday practices. Yes No As previously mentioned, our teams who compete 2 or more routines will be required to attend a competition in St. George. This does not ensure that your dancer(s) will take exactly this amount of hours. They may be placed in a group taking less hours. We will try our best to keep close to what is marked below. Ballet is the foundation of technique. All competition dancers are required to take ballet. For those on pointe or soon to be on pointe, they are required to take ballet 2-3 times throughout the week (or 2-3.5 hours of ballet per week). Kindergarten, 1st, & 2nd graders are required to take 2 hours per week. Most K-2nd graders do great with 2 hours per week. 3rd - 4th graders are required to take 3 hours per week, with the option of taking more hours. 5th+ graders are required to take 4 hours per week, with the option of taking more hours. Dancers who are currently on pointe are required to take 7+ hours of class. Please check the option below that you are willing to commit to.
