1 Fall Schedule 2018 and Registration Form For the CBT Racquet Club of Memphis Sanderlin Ave Home for Children's Ballet Theater and Memphis Youth Musical Theater All classes are open to public for boys, girls and adults. 1 of 9 General Information Select your classes below. Register & pay on our registration page to ensure quick spot placement. Current students: Due date is August 6 to avoid $50 late fee. As an option, you can print the forms and mail with the check to: Children s Ballet Theater 5345 Panda Lane Memphis TN Or you may bring to Racquet Club to the Pro Shop desk at 5111 Sanderlin Ave. Discounts and Information Discounts: Students with siblings in the program receive $10 off your total price. Racquet Club member discount for Fall 2018 will be $10 off your total price. (You do not have to be a member to be in the program). Withdrawals: 75% refund is given if a student withdraws within 24 hours prior to a second class. No refunds will be given after this time except in the case of a physician-verified illness. There is no pro-rating and no makeup classes. Tuition is not transferable to another session. Students cannot attend class until full payment is made. There is a $30 fee for cancelled checks. CBT reserves the right to cancel classes due to inclement weather conditions or teacher illness. In the event of a weather condition, call the CBT office one hour prior to your class time at for a recorded message. There are no makeup classes for weather conditions. Payment Plan: If you are taking 3 or more classes a week, you have option to pay ½ tuition now and ½ by October 15. For the half payment option, info@balletchildren.com for a form. It is recommended to stay in the same level for fall/spring to get the most out of the program year olds are the exception if the child has aged up. All new students going into Levels 2 and up will need to contact the School to see if a placement evaluation is required. Class demonstrations for all levels, a special PKJK show, and a recital on stage, will be held in the spring. Children s Ballet Theater Nutcracker at GPAC, directed by Mary Van Dyke Students who are in SK and older, up to 18 years, are invited to perform in the Children's Ballet Theater Nutcracker on the gorgeous GPAC stage for 2018! CBT s highly acclaimed professional performance features children and teens from Children s Ballet and the CBT Junior Company in many of the major roles. Furthermore PKJK can be flower presenters! 8 Children s Ballet Theater Nutcracker at GPAC, directed by Mary Van Dyke Students who are in SK and older, up to 18 years, are invited to perform in the Children's Ballet Theater Nutcracker on the gorgeous GPAC stage for 2018! CBT s highly acclaimed professional performance features children and teens from Children s Ballet and the CBT Junior Company in most of the major roles. Auditions are held in September for roles including Clara, Party Girls, Dolls, Russian, and more. Exciting news: two Clara s will be selected for Mary Van Dyke s new choreography. The young Clara will be from level 3 or 4 for the Party Scene and older Clara from Level 5 or 6 for the Mice Scene and Snow Scene pas with the Prince. The general public is also welcome to audition for all roles. Auditions open to the public and Costume fittings: SK-1st, Level 1 and Level 2: Saturday September 8th Mandatory Costume Fitting and Meeting with your parent. No Audition is required and students will be assigned parts. SK-1 students will be assigned one night to perform. Levels 3-6: Saturday September 22nd Auditions and Sunday September 23rd Mandatory Costume Fitting and Meeting with your parent. PKJK Students are invited! We invite any PK & JK students who are attending the performance to present flowers to the ballerinas and Clara at the end of the show. Students can wear their prettiest dress and a CBT headpiece. More information will be ed. There will be a $75 Nutcracker fee due at the time of costume fitting and meeting, by check or cash. Ticket sales cover only a small portion of expenses for this professional production. Your fee helps support the presentation of the Nutcracker each year at a quality venue such as the GPAC, and helps cover costs of costumes, student rehearsals at the theater, and more. Rehearsals: Rehearsals are held during class times along with MANDATORY FULL CAST PRACTICES. Save these dates: the first three in November and first weekend in December at the RCLUB. (SK-1st and Level 1 will only attend one of these). MANDATORY stage rehearsals will be held December 13th and 14th. Full schedule handed out after the auditions. Immediately after audition date, anyone in Level 3-6 should save time for possible rehearsal between 11:00 am - 1 pm on, based on casting. Exact schedule TBA. The older Clara should be prepared to commit to any extra days/rehearsals necessary, and optional class fees will apply. Performances: December 15th at 6pm and December 16th at 2:30pm on the gorgeous GPAC stage. Tickets go on sale in September. 10 a CBT student, please include their name and put FRIEND WAIVER OF LIABILITY, INDEMNITY AGREEMENT, AND ASSUMPTION OF RISK In consideration of permission to use, today and on all future dates the property, facilities, and services of Children s Ballet Theater, Memphis Youth Musical Theater, Racquet Club, Christ Methodist School or Church, Roulhacs, St Agnes, or Second Baptist (CBT programs) I, on behalf of myself, my heirs, personal representatives, or assigns, do hereby release, waive, discharge, and covenant not to sue, CBT programs, its directors, officers, contract workers, volunteers, independent contractors, and agents from liability from any and all claims arising from the ordinary negligence of any agent of Children s Ballet Theater or any of the aforementioned parties. This agreement applies to 1) personal injury (including death) from accidents or illnesses arising from the participation in our Ballet School activities including, but not limited to, organized activities; classes; observation; performances at GPAC theater, Buckman Theater or any other theater; and individual use of facilities, premises, or equipment; and to 2) any and all claims resulting from the damage to, loss of, or theft of property. Indemnification and Hold Harmless: I agree to HOLD HARMLESS AND INDEMNIFY Children s Ballet Theater from all claims resulting from negligence and to reimburse them for any expenses in investigating and defending a claim or suit if my claim is withdrawn, or to the extent a court or arbitration determines that Children s Ballet Theater is not responsible for the injury or loss. Severability and Venue: The undersigned further expressly agrees that the foregoing waiver and assumption of risks agreement is intended to be as broad and inclusive as is permitted by the law of Tennessee and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. Likewise, I agree that if legal action is brought, it must be brought in Memphis, Shelby County, Tennessee. Acknowledgement of Understanding: I have read this waiver of liability and indemnification agreement and fully understand its terms. I understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily, and intend my signature to be a complete and unconditional release of all liability to the greatest extent allowed by the State of Tennessee. The undersigned parent and natural guardian does hereby represent that he/she is, in fact, acting in such capacity, has consented to his/her child or ward s participation in the activity or event, and has agreed individually and on behalf of the child or ward, to the terms of the accident waiver and release of liability set forth in Section 1. The undersigned parent or guardian further agrees to save and hold harmless and indemnify each and all of the parties referred to in Section 1 from all liability, loss, cost, claim, or damage whatsoever which may be imposed upon said parties because of any defect in or lack of such capacity to so act and release said parties on behalf of the minor and the parents or legal guardian. I hereby waive any claim of liability on behalf of Children s Ballet Theater programs, for any efforts to obtain treatment from a doctor or hospital for my child in event that my representative or I cannot be reached.
