3 WARNINGS This instruction manual is intended to meet the Manufacturer s Instructions as required byosha and ANSI. It must be used as part of an employee training program: provide to all users prior to use, part of user safety training and kept on file for reference and inspection. Employers have a responsibility to ensure all users of this equipment must be trained, read, understand and obey all manufacturer instructions, company safety regulations and state and federal regulations, instructions, markings, warnings and product limitations. Federal and State laws require that the user of this equipment receive Safety Training prior to its use. ALL COMPONENTS MUST BE INSPECTED: PRIOR INSTATION AND PRIOR TO EACH USE. A COMPETENT PERSON OTHER THAN THE USER SHALL INSPECT AND RECORD THE DETAILS AT LEAST ANNU- ALLY. Use of compatible components is mandatory. Failure to properly use, inspect, maintain and remove from service damaged or defective equipment, as well as, PLEASE READ THE FOLLOWING WARNINGS: The first rule of safety is: If it is unsafe, don t do it. If you have a use, care, inspection, compatibility or any other questions, contact Riggers Safety. These instructions must be provided to all users to read, understand and follow accordingly. This manual should be available at all times for reference with extra copies on file. A competent person, fully aware of applicable safety regulations for use, inspection and maintenance should ensure all fall protection Anchor systems are installed correctly and connected to a compatible Personal Fall Arrest System (PFAS), to prevent injury to the user or damage to the equipment. All PFAS are required to comply with ANSI Z and prevent free fall or limit it to 6 feet or less. Plan and confirm that there is adequate fall clearance and that there are no obstructions below the work area to prevent the user from striking lower levels. (Note: The amount of appropriate fall clearance is dependent on the PFAS used.) This equipment when used properly can reduce, but cannot eliminate the 3

4 possibility of roll-out. PFAS components are for user fall protection only and NEVER to load, hang, support materials or tools. Consult a physician if there is concern or doubt about employee fitness level, health or ability to absorb shock from fall arrest prior to using this product. Before operating, a worker should be: mentally and physically fit for the purpose, especially at heights or in confined spaces. Free from the influence of alcohol or drugs. Trained under safe conditions for working under the above requirements Safe Conditions: Training must include: equipment use, limitations, andmaintenance, inspection and rescue protocols. Rescue protocol must be determined in advance and put in writing by the authority in charge of safety. Training must be carried out under conditions that exclude the risk of injury or fall. Do not allow PFAS to be used near any physical hazards like electrical, welding, heat, severe cold, corrosive, chemical, moving machinery, sharp edges or any other hazard that can injure the employee, cause a fall or damage the equipment. Never disassemble PFAS components or use an altered or misused anchor, lanyard, harness or other PFAS components. All PFAS components associated with the use of an anchor system must comply with ANSI Z Riggers Safety assumes no liability for product failure due to adequacy of installations, alterations of the product or use of non-compliant components. These are non-repairable anchors must be inspected prior to use and annually. Any failure of inspection requires the anchor be removed from service then disposed of to prevent reuse. Definitions and Functions: Personal Fall Arrest: Anchorage Connectors are used as components of a personal fall arrest system (PFAS) to prevent a fall or protect the user should a fall occur. PFAS typically include an anchor, full body harness, connecting subsystem (shock-absorbing lanyard) and rescue procedure. Impact forces and free fall distance is limited. WARNING: USE OF BODY BELTS FOR FALL ARREST IS PROHIBITED BY LAW. 4

5 PFAS Applications Anchorage Connecting Subsystem (self-retracting lifeline) Anchorage Connector Restraint Lanyard Full Body Harness Back D-Ring Fall Protection Fall Restraint Anchorage Cable Back-up Fall Arrest System Anchorage Connector Full Body Harness Ladder Cable Sleeve Full Body Harness Work Positioning Ladder Climbing Roof Anchor/D-Ring Anchorage Connector: Designed to function as an attachment between a personal fall arrest, restraint, work positioning, personnel riding or rescue system and an anchorage to provide a single user with a single anchorage point. Harness: Designed to be incorporated in a PFAS where worker fall protection is a required. The function of a full body harness is to prevent a fall or support the user during and after a free fall and to distribute impact forces. Shock-absorbing lanyard: Designed for use as a connecting device in a PFAS. The lanyard connects the user s harness to an anchorage point, anchorage connector or rope grab with self-locking hooks. The shock-absorbing component extends the deceleration distance minimizing the momentum energy. Carabiner: Designed to be incorporated in a personal fall arrest system where worker fall protection is required. The primary function of a carabiner is to connect compatible components of a PFAS. Fall arrest: The action or event of stopping a free fall or the instant the down- 5

6 ward free fall has been stopped. Fall Restraint: Anchorage Connectors are used as components of a restraint system to prevent the user from reaching a fall hazard. Restraint systems typically include a full body harness and a lanyard or restraint line. User is not exposed to a fall hazard and vertical free fall is prevented. Work Positioning: Anchorage connectors are used typically attached to the front of the body as components of a work positioning system to support the user at a work position. Work positioning systems typically include a full body harness, positioning lanyard and a back-up PFAS for the purpose of working hands free. Free fall distance must be limited to 2 ft. Personnel Riding: Anchorage Connectors are used as components of a personnel riding system to suspend or transport the user vertically. Personnel riding systems typically include a full body harness, boatswain s chair or seat board and a back-up PFAS. Vertical free fall is not permitted. Rescue: The process of removing a person from danger, harm or confinement to a safe location. Rescue components vary depending on the type or rescue. Vertical free fall in not permitted. System Requirements: The Riggers Safety Anchorage Connectors are intended for use with approved subsystems components (i.e. harnesses, shock absorbing lanyards, self-retracting lifelines...etc) that must meet ANSI Z , ANSI A Substitutions made with non-compliant components may jeopardize equipment compatibility and the safety of the user. All PFAS components must meet ANSI Z , ANSI A PFAS and Anchorage Connectors are designed for use by persons with a combined weight (incl. clothing, tools etc.) of no more than 310 lbs. Only one employee PFAS system may be connected an anchor at a time and must be rigged to limit the fall arrest forces and free fall distance to six feet or less. Connectors (hooks, carabiners, D-Rings...etc.) must be compatible in size, shape and strength; capable of supporting at least 5,000 lbs. Care must be taken to insure the compatibility of connection hooks and the connection point. Roll-out can occur when non-compatible connectors are used. Only ANSI Self-locking snaphooks or self-locking carabiners must be used. For fall arrest applications please insure that the Anchoring Point of attachment where the anchor will be attached must be capable of supporting static loads in the directions permitted by the PFAS when in use of at least: 3,600 lbs. where certification exists or 5,000 lbs. in the absence of certification. 6

When attaching more than one roof anchor to a structure the strengths given above must be met at each point independently. Fall Clearance Calculation: Before installation or use of this anchorage a Hazard assessment should always be conducted by a Qualified Person to determine installation location. Swing fall, unprotected edges and Fall Clearance hazards are some of the hazards that must be considered. Standard Fall Clearance Calculation Anchorage Back of Worker Before Fall 6 ft. Length of Lanyard Front of Worker After Fall Energy Absorber Elongation (3.5 ft.) 3.5 ft. Deceleration Distance 6 ft. Height of Worker 3 ft. Safety Factor 7

8 Specifications: A111 Riggers Safety, ROOF ANCHOR, NAIL-IN WOOD ANCHOR, YELLOW ZINC: Forged D-ring which is attached to two hinged steel plates (Legs), 20 each Vinyl Sinker 16D nails, 3 ¼. A212 Riggers Safety, D-RING 2-HOLE ANCHOR PLATE KIT. Bolt on Anchorage Connector Components: Forged D-Ring, Two Hole Mounting bracket, User must provide a Minimum of Two (2) SAE Grade 5 3/8 Bolts, Four (4) Flat 5/8 SAE Grade 5 Washers and Two (2) SAE Grade 5 Locking 5/8 Nuts are required. Higher grade hardware can be used. Additional Tapered Washers or hardware may be required to complete the installation. Bolts must be long enough to be able to pass through all hardware and anchor plate with a mimium of ¼ extending beyond the connecting nut. Alloy Steel rated to 5,000 lbs. Meets OSHA 1926 and ANSI Z Limitations: Anchors are designed to function as an attachment for personal fall arrest, restraint, work positioning, personnel riding or rescue system for a single user. 1. Wood Roof Frame Structure: The Riggers Safety A111 nail in roof anchor is a temporary anchor should only be installed on wood frame roof structures that meet the anchorage strength requirements set by ANSI Z Never leave installed for future use. 2. Steel Structure: The Riggers Safety A212 Two Hole steel anchor should only be installed into Steel structures that meet the anchorage strength requirements set by ANSI Z Capacity: The Riggers Safety anchors are intended for use by one person with a combined weight (including body weight, clothing, tools etc.) of less than 310 lbs combined weight 4. Only one personal fall arrest system may be connected to the roof anchor at one time. 5. PFAS: Personal fall arrest system s selected for use with this anchorage device must meet compliance criteria and include components deemed compatible with ANSI and this anchorage device. 6. Free Fall: PFAS used with the Riggers Safety roof anchor must limit the free fall to a maximum distance of six feet. 7. Fall Clearance: Enough clearance must exist in the fall path to prevent user from striking an object. The amount of clearance necessary is dependent on the subsystem in use and the anchorage location. Please refer to the fall clear- 8

9 ance diagram above. 8. Corrosion: Use near sea water or in corrosive environments may require more frequent inspections and servicing (including replacement) to insure that they roof anchor device continues to perform optimally. 9. Chemical Hazards: Solutions containing acid, alkali or other caustic chemicals, especially at elevated temperatures can damage the equipment. Contact Riggers Safety if concerns exist regarding the proximity of chemical hazards to equipment. 10. Electrical Hazards: Do not install Riggers Safety roof anchor where the worker or equipment may come into contact with electrical power lines. 11. Training: The Riggers Safety roof anchor is intended to be installed and used by workers who have received proper training on appropriate application use, limitation, and care and maintenance equipment. Installation: Installation: A111 Riggers Safety, NAIL-IN WOOD ROOF ANCHOR 56 (2.20) 77 (3.03) 76 (3.01) mm. (in.) 362 (14.28) WARNINGS Never use power actuated tools or nail-gun to install anchors. Select only wood structure capable of withstanding a 5,000 lb. static load in the intended use direction. This is not a Permanent Roof Anchor system. This nail-in roof anchor may only be used as a temporary anchorage connector between wood frame structures and compatible PFAS/restraint system that limits the fall forces to 900 lbs or less. Take caution not to use excessive force and cause damage the roof or anchorage structure when installing anchors. 9

10 Inspect anchor for cracks, deformation or damage to insure that it is undamaged prior to each use. Never attach to unsupported sheathing, gables, overhangs or facia boards. Anchor Location:Before installation of this or any anchor, an Anchorage location and Hazard Assessment should always be conducted by a Qualified Person. Avoid side loading anchor or mounting too close to un-protected edges that would create equipment failure or swing fall hazard. Anchor must be located a minimum of 5 from any exposed edge. Installation RIDGE BOARD ROOF EDGE MOUNT ANCHOR DIRECTLY INTO SUPPORTING TRUSS ROOF EDGE WORK ZONE SWING FALL HAZARD For each anchor installation, always use 20 each new 16D Vinyl Sinker Nails 3 1/4 To Install: Open Hinged roof anchor and place each leg directly over a supporting roof truss on each side of roof peak. Ensure the structure meets the minimum load and that the anchor location is a minimum of 5 from the roof edge. Nail six (6) new 16D, 3.25 Vinyl Sinkers nails into the center row of each leg of the anchor. The nails should pass through the anchor legs, wood sheathing into the supporting roof truss. There should be no space under the nail heads or the roof anchor legs. Add four (4) new additional 16D, 3.25 Vinyl Sinkers nails to four outside holes for a total of ten (10) nails in each leg. 10

11 The Anchor should be complete secured to the supporting structure with no movement to the Anchor plate. To Remove: Use claw or hammer to remove nail head only. Do not pry under the anchor legs to remove the nails because this will cause damage to the anchor. Inspect the anchor for cracks, deformation or damage and destroy if the product fails inspection. Dispose of and never reuse 16D vinyl sinker nails to connect anchor. Installation ROOF PEAK ROOF ANCHOR CARABINER ROPE GRAB ROOF EDGE ROOF EDGE REDIRECT LIFELINE TO REDUCE SWING FALL HAZARD WARNINGS If inspection reveals a defect, remove unit from service immediately and destroy it or contact Riggers Safety for information. If this equipment has been subjected to forces resulting from the arrest of a fall it must be immediately removed from service and destroyed. 11

12 Although the A111 Nail in Roof Anchor is not designed for side loading or suspension of horizontal lifelines, you can use multiple nail in roof anchors to redirect the Lifeline to an anchorage closer to the work zone and fall zone. This can help minimize the Swing Fall Hazard. These redirect nail in roof anchors must be nailed in just like the anchor at the ridge. Installation: Installation: A212 Riggers Safety, D-RING 2-HOLE ANCHOR PLATE KIT: Riggers Safety D-ring 2-Hole Anchor Plate Kit can be used as a temporary or permanent anchor to connect compatible PFAS/restraint system that limits the fall forces to 900 lbs or less. Anchor Location: Before installation of this or any anchor, an Anchorage mm. 13 location and hazard assessment should always be conducted by a Qualified Person. Anchor must be located far enough away from the exposed edge as to avoid blowing out anchorage point from the side. Before installation or use, carefully inspect anchor equipment for damage. Select only steel structure capable of withstanding a 5,000 lb. static load in the intended use direction. Avoid side loading anchor or mounting too close to un-protected edges that would create equipment failure or swing fall hazard. USER MUST PROVIDE FASTENERS TO THE SPECIFIC INSTALLATION NEED. A Minimum of Two (2) SAE Grade 5 Bolts, Four (4) Flat SAE Grade 5 Washers and Two (2) SAE GRADE 5 Locking Nuts are required. Higher grade hardware can be used. Additional Tapered Washers or hardware may be required to complete the installation. Bolts must be able to pass through all hardware and anchor plate with a minimum of ¼ extending beyond the connecting nut. Refer to Hardware suppliers Torque and Tension settings to achieve a properly fastened anchor. 14 Training: Users of this equipment must be properly trained, read, understand and obey all manufacturer instructions, company protocols and safety regulations as well as state and federal regulations, instructions, markings, warnings and product limitations prior to use. Training shall include instruction on use, rescue, limitations, care and inspection of all fall protection components used. Use this product manual to record application and Fall Protection Safety Training. Use of compatible PFAS components is mandatory. Users must be trained under safe conditions and not exposed to a hazard during training. Follow OSHA requirements for ongoing and refresher training. Before operating, a worker should be: mentally and physically fit for the purpose, especially at heights or in confined spaces and be free from the influence of alcohol or drugs. ANSI and OSHA specify PFAS requirements comply with specifications regarding Anchorage Strength, Maximum Fall Arrest Forces, Swing-Fall, Free Fall Distance and Total Fall Distance, Body Connection, Body Support, Rescue and more. Consult applicable standards and a safety professional to develop your PFAS and write a Standard Operating Protocol. Provide comprehensive training to all users prior to using this product. Training: If the anchor or any Fall Protection equipment fails inspection or is exposed to dynamic fall forces, it must be removed from service and destroyed. The user must visually inspection anchor prior to each use. A formal inspection of the Roof Anchor and D-ring Anchorage Connector and its connection structure must be performed at least annually by a competent person other than the user. This formal inspection should be carried out more often if components are exposed to conditions that could cause damage. Record inspection date, location and results in this manual. Before use and at least annually inspect Anchors using the following steps: Closely inspect and remove from service anchors Hardware, Rivets and D-Rings that fail inspection for: rust, damage, distortion, cracks, corrosion, painted or pitted surfaces. Modification by the User or missing parts. Anchor plates should lie flat without distortion. Inspect and confirm anchors are securely connected; hardware is in place, properly torque and not deformed or fatigued. Inspect manufacturers tags are in place and legible. 14

15 Confirm connection structure will support the intended loads of 5,000 lbs. Replace Wood, Steel or Concrete structure that does not meet the load requirement or relocate anchor. Remove from service failed anchors or any PFAS components. Keep all PFAS clean with light detergent and water then dry completely using cool air or towel. Metal components can receive a light coat of oil to prevent corrosion. Record All Inspections: Date of Manufacture: Date Put In Service: Serial # Date Inspector Pass / Fail Maintenance Performed 15
