2 MODEL : NAVAHO BOD CROLL FAST HARNESS The results of this PPE inspection are provided to you subject to the condition that the components to be inspected do not come into any of the categories listed below, any of which would require concerning the checking of the components history, which must be done by the client. Condition of webbing (cuts, wear, burns, marks, chemical contamination, other damage) Condition of load-bearing stitching (cut, worn or pulled threads) Condition of the attachment points (deformation, marks, wear, corrosion) Condition of the adjustment buckles (deformation, marks, wear, corrosion) Condition and compatibility of the connector (see connector form) Condition of protective components (tubular protective cover of tie-in point, thigh protection) CHECK OF COMFORT COMPONENTS Condition of padding of the waistbelt and leg-loops, jacket, spreader, gear loops etc. Condition of non load-bearing stitching Check that the webbing straps are properly threaded through the buckles Verify the function of the spring in the FAST quick-connect buckle Operation of the adjustment buckles VERDICT: The product is fit to remain in service PASS (tick) VERDICT: The product is unfit to remain in service FAIL (tick) Put status of component in the column above

5 MODEL : ASAP MOBILE FALL ARRESTER These PPE inspection results are valid only if neither of the following conditions are true of the equipment to be inspected: - Product has received a fall. - Component has exceeded it s lifetime The inspector declines all responsibility for inaccuracy in the information concerning the historic verification that must be done by the user. VISUAL VERIFICATION OF SAFETY ELEMENTS Condition of the body, attachment holes (cracks, marks, deformation, wear, corrosion) Condition of the articulated arm (cracks, marks, deformation, corrosion) Condition of the anti-return stop (marks, deformation) Condition of the locking wheel (worn, fouled, or missing teeth) Condition of the spring (breaks, cracks, deformation, corrosion, contamination by foreign objects) Condition of the protective cover (cracks, contamination by foreign objects) Compatibility and condition of the connector (see connector form) Check keeper cord is under 11cm (if present) FUNCTIONAL TEST Effectiveness of the articulated arm return spring (return strength and mobility) Turn the locking wheel through a complete 360 rotation in both directions The wheel should turn smoothly, without catching The ASAP should slide easily up and down the rope Fall arrest function: the fall distance of a 5 kg mass falling from the height of the carabiner must be less than 0.5 m. (Unlock the locking wheel)

Condition of the rope (cuts, wear, burns, marks, chemical contamination, soft spots) Condition of webbing (cuts, wear, burns, marks, chemical contamination) Condition of the load-bearing stitching (cut, worn, torn or pulled threads) Condition of the metal pieces (deformation, marks, cracks, wear, corrosion) CHECK OF THE COMFORT COMPONENTS Condition of the protection components (strings, stitching protectors, protective cover) COMPATIBILITY CHECK Condition and compatibility with the connector (see connector form)
