2 Information Afternoon! Thank you for your interest in Gower Hockey Fest, Gowerton s first ever 7-a-side tournament. About the Day As stated above, the tournament is a 7-a-side format with mixed gender teams, however, each team must have a minimum of two females in their squad. Unfortunately, this year we are going to have to restrict players entry to 14+ but we look to open up to juniors in future events. Your team can have a maximum of 13 players in total. If you are not a team of hockey players and cannot provide two females, we may be able to provide you with two from the Gowerton Ladies team on a first come, first served basis. The game is to played half-pitch with no need to provide a goal keeper. The day will be organised as a pool-system with teams progressing to the finals at the end of the day. The winning team will receive a small token prize. In addition to the tournament, there will be a BBQ with burgers, hot dogs and veggie options available for purchase as well as soft drinks, teas and coffees. Please bring cash and as much change as possible for the day to run as smoothly as possible. Due to the event being held in July, it could be a hot day (but being in Swansea this is unlikely) so please bring plenty of water with you to stay hydrated. There will also be water available for purchase on the day. There will also be a raffle with tickets available for 2 each. The raffle prizes include Crossfit memberships, a signed Scarlets rugby shirt as well as two tickets for a Scarlets match in the 2017/18 season. For the day, it would be ideal if you have one or more players that would be willing to umpire during the day. You do not need to be a qualified umpire.

3 Equipment All players will need to provide their own shin pads and we recommend players wear a gum shield however this is optional for the tough nuts among us and should be taken at one s own risk. If you are not a hockey player and do not have your own stick, we can provide one for you. Please state whether you need a stick to be provided for on the registration form below. Registration and Payment Information When registering you will need to state a team name as well as the names of the members of your squad. You may wear a team colour although this is optional since there will be bibs available on the day. Fancy dress is also encouraged, but once again, not compulsory. The deadline for registration is Friday 16th June. The event is 5 per person and will need to be paid by Friday 16th June. When paying for the event please pay as a team and have your team name as the payment reference. Further, I am aware that hockey is a vigorous sport involving severe cardiovascular stress. I understand that hockey involves certain risks, including but not limited to, death, serious head injuries and serious injury to all bones, joints and muscles; and that equipment provided for my/my child's protection may be inadequate to prevent serious injury. I further understand that hockey involves a particularly high risk of knee, head, and neck injury. I am voluntarily allowing me/my child to participate in this activity with knowledge of the danger involved and hereby agree to accept any and all inherent risks of property damage, my child's personal injury, or my child's death. I further agree to indemnify and hold harmless Gower Hockey Fest, Gowerton Ladies, Elba Sports Complex, or its participants and others listed for any and all claims arising as a result of my child engaging in or receiving instruction in the Gower Hockey fest activities or any activities incidental thereto, wherever, whenever, or however the same may occur. In the event of a zombie apocalypse I will not hold Gower Hockey Fest, Gowerton Ladies, Elba Sports Complex, or its participants responsible for any

6 injuries caused and will proceed to use the youngest member of my team as bait in order for all participants to escape. I understand that this waiver is intended to be as broad and inclusive as permitted by the laws of the United Kingdom and agree that if any portion is held invalid, the remainder of the waiver will continue in full legal force and effect. I further agree that the venue for any legal proceedings shall be in the United Kingdom. I affirm that I am of legal age and am freely signing this agreement for me/ my child to participate in the Gower Hockey Fest. I have read this form and fully understand that by signing this for I am giving up legal rights and/or remedies which may be available to me for the ordinary negligence of Gower Hockey Fest, Gowerton Ladies, Elba Sports Complex, or its participants, or any parties listed above.
