1 MAHOGANY S 3rd ANNUAL 3 ON 3 HOCKEY TOURNAMENT 2019 REGISTRATION PACKAGE REGISTRATION DEADLINE: **Early Bird: January 9, 2019 Regular: January 23, 2019
2 The tournament will be made up of two divisions. Competitive and Social. With a maximum of 20 teams. (10 each division) Entry Fee: $ per team (consisting of at least 1 member of the community of Mahogany). $ per team with no Mahogany residents. Early Bird Registration receives $25.00 off team fee!! Fees are made out to Mahogany Homeowners Association in the form of a cheque or CC. Registration forms will be accepted as of Friday December 20, Completed Registration forms can be dropped off at the Mahogany Beach Club to a staff member, ed to shaun@shaunfrench.ca or completed online at All registration forms must be complete prior to being submitted. If a registration form is submitted and is incomplete the team will not be registered until all forms are complete. Mahogany Beach Club will not hold or reserve team slots for anyone for any reason. A complete registration form consists of the following information: Names, addresses, phone # s, ages and emergency contact information for all members on the team. The maximum number of players per team is 9. The minimum is 4 (3 players, 1 goalie). An address of the team captain. A signed waiver with signature of all players on the waiver form. Entry fee CHEQUE made payable to Mahogany Homeowners Association. CREDIT in person at Mahogany Beach Club. Once a team is registered there will be no changes made to your roster under any normal circumstances. Please have all team members read the rules and regulations for the tournament before submitting your registration package. All team captains will be ed 1 week prior to the tournament with full tournament details consisting of scheduling, event location, map, nearby amenities and prize info. This registration policy will be strictly adhered to and there will be no exceptions. *Registration Deadline will be January 23, 2019 Any questions or concerns please contact Shaun at or shaun@shaunfrench.ca
3 Equipment: Goalies: Full equipment is mandatory. Players: Helmets, shin pads, elbow pads, gloves, skates and sticks are all mandatory. *Full cage helmets are recommended. Rules: All team members must be a minimum of 16 years of age. All team members must be registered on the team registration form. No new players under normal circumstances are permitted to join a team once registration package is complete. Teams are to consist of a minimum of 1 goalie and 3 skaters. All players are to be present at their designated rink (there will be 2 surfaces) at least 10 minutes before the start of their game. Due to tight scheduling the game start times will be strictly adhered to. If a team is not present at game start time a penalty shot will be awarded to the opposing team. After 5 minutes the late team will forfeit that game. All round robin and crossover games will consist of a 5 minute warm up before each start and 20 minutes of run time for each game. The semi-finals and final game will be a 5 minute warm up with 25 minutes of run time for each game. There are no timeouts and the clock will not stop for any reason. Players referee their own games and call their own penalties. Penalties will result in a penalty shot. Following the penalty shot the defending team is awarded possession with a 5 sec rule in effect. There will be no body checking or fighting. If this occurs the Mahogany Residents Association reserves the right to eject the team from the tournament. Slap shots are not permitted and will result in a penalty shot. Any player consistently being called for penalties will be removed from the game at the discretion of both teams. Coincidental penalties will result in both players subbing off immediately and a face off at center ice. The only face off will be at the start of the game. Therefore, after a goal or freezing of the puck by the goalie the defending team gets possession with a 5 second rule in place.
4 Rules Continued: There is no icing or off sides. There will be no stoppages in play so all substitutions will be made on the fly. In the case of a tied game a shootout will determine the winner. A 3 shooter format will be followed and must consist of 3 different shooters. After the 3 shooters if it is still tied shooters can repeat as many times as they like until the game is decided by sudden death format. There will be 2 points awarded for a win and 0 for a loss. If there is a tie in the point standings it will be decided by a goals for and goals against format. If there is still a tie it will be decided by head to head result from those teams in the round robin. ABSOLUTELY NO ALCOHOL PERMITTED! If a team or member of a team is found with alcohol on Mahogany Residents Association Property they will be immediately disqualified from the tournament. Given that this is a community tournament we expect that you maintain a level of absolute sportsmanship at all times. Significant unsportsmanlike conduct could result in the removal of your team from the tournament. Notables: There will be a team prize for first place, second place and an individual prize for the top player of the tournament as well as top goalie. All entries will receive a welcome package There will be a silent auction table set up at Canadian Brewhouse with various items. All proceeds go back to the Mahogany Residents Association to help fund this event and remaining funds will help facility expansion needs. Winners will have their names engraved in the Annual Mahogany 3 on 3 Tournament Trophy and will receive recognition with team photo on the HOA Website as well as social media sites. The tournament is weather permitting. If it is cancelled due to poor weather all registration fees will be returned.

I, including my next of kin, heirs, and assigns, agree to release and hold harmless the MHOA, its insurers, successors, assigns, employees and volunteers (including without limitation, coaches, instructors, managers or any other responsible persons), from all liability, causes of action, claims, and demands for damages whatsoever, arising out of or in any way connected with this program, including any claims for negligence, but not including action, claims and claims of intentional wrongdoing or gross negligence. 7.
