5 Health & Safety Policy Statement of Intent It is the firm policy of APW Glazing (herein after referred to as the Company) to take all reasonable steps to ensure the safety, health and welfare at work of all its employees and any others who may be affected by its undertaking, in accordance with current legislation. The Company will provide such information, instruction and training as is necessary to ensure that all its employees are aware of their own duties and responsibilities under relevant legislation and to ensure implementation of this policy. Risks assessments will be undertaken / reviewed on a regular basis to ensure the identification of all significant hazards and corrective action required to reduce the risk to an acceptable level. Sufficient resources will be provided to meet the requirements of Health and Safety legislation. Our general intentions are: To provide adequate control of the health and safety risks to employees and others arising from our work activities; To consult with our employees on matters affecting their health & safety; To provide and maintain safe premises, plant and work equipment; To ensure safe handling and use of substances; To provide adequate information, instruction, training and / or supervision for employees; To maintain safe and healthy working conditions; and To review and revise this policy at regular intervals Page 5 of 41

6 Construction Information Scope of Work Covered by this Risk Assessment The installation of Rationel composite windows and doors including any ancillary items within our order. Work Activity Information General Site Safety All operatives, including subcontractors, working on site will be made aware of the Health and Safety risk assessments of APW Glazing. Instructions will be given regarding specific site conditions and procedures which must be adhered to. Hard hats, high visibility jackets, gloves and safety footwear must be worn at all times where instructed or signed on site. All powered hand tools will be 110v or battery operated. The relevant test certificates and identification numbers of any hired in plant will be submitted to SITE SUPERVISOR prior to use on site. No smoking on site. All operatives and visitors to sign in and out of the site. Site Welfare Facilities Site welfare will be provided by Higgins Construction Unloading & Storage Arrangements Materials must be arranged to arrive at site only when requested by the Site supervisor. Program of Works Ground floor windows to 1-6 and come in first over a 19 week program. Environmental Issues All waste is to be removed by the main contractor using the skip provided and collected by a licensed waste carrier. Guidelines for the correct procedures for off loading, lifting and distribution are laid out within the Method Statement. The use of mechanical lifting equipment such as dolly trucks, glass suckers and pallet trucks, safety equipment and correct lifting methods should significantly reduce the risk of injury to the installation team. Windows will be vertically lifted by means of hoist and the horizontal distribution by means of dolly trucks around the scaffold. Page 11 of 41

Reduce carrying distances to the minimum practicable and wear the PPE provided (gloves and safety shoes). Check walkways are clear of obstructions, trip and slip hazards prior to lift. Be aware of weather conditions (wet or slippery conditions). Velux windows will generally be in cardboard boxes and therefore even in rain the grip should be good. Always lift within your capabilities pregnant workers or those with a history of back problems should not lift these items. Always lift in accordance with training received. Avoid poor posture. Whilst the sequence of erection may vary between the differing elements, the construction is similar to each. General Installation Method Statement 1. All windows will be delivered to site by others, on pallets, protected on all corners and shrink wrapped in polythene. Pallets are to be unloaded from lorry trailer by forklift or crane by the Main Contractor with a certified driver. 2. Pallets should be moved to a safe, secure and dry storage area designated by the Main Contractor where they will not impede any construction works or be hazardous to any other operatives. 3. All pallets/windows are to be hoisted by either crane or hoist provided by main contractor to their relevant floors. Windows should be palleted to suit the requirements and set up of the building and should not require re-palleting to enable hoisting. Re-palleting does not form part of our scope of works. 4. Window units will be distributed to window openings either by pallet trucks, trolleys or manhandling when required to be fitted into respective openings. If windows exceed the weight guidelines recommended by the Health and Safety Executive, then the window units can be de-sashed to allow safe lifting and installing. This will be at the discretion of the installers. 5. Protective packaging will be removed immediately prior to installation procedures.the area in and around the installation will be clear of any hazards and extra task lighting brought in if required. 6. Windows are placed on a protected bench where all corner protectors are removed and the window is prepared by screwing all necessary straps/brackets to the timber mainframe or drilling counter bore holes in the timber mainframe, whichever method is required or both methods if necessary. 7. If a pre-compressed expanding neoprene tape (Compriband 600) is to be used as the primary external seal, then this can be applied to the front edge of the timber mainframe while on the bench or when the window has been permanently fixed. The Compriband 600 tape must be of a suitable size to suit the gap between the timber mainframe and the structure and be applied in accordance with the manufacturer s recommendations which are clearly indicated on its packaging. We have only allowed for a Compriband 600 tape sufficient in size to fill a gap size of 15mm maximum. Other sizes are available but at an additional cost. Page 34 of 41

35 8. The window is offered into the opening by 2 operatives. one operative will hold the window in place while the other operative sets the window into its correct position. Apw will use Ideal Combi 300 series drawings Datum's and grid lines provided by the Sisk contractors to achieve the correct position. The installation operative will then wedge Permanent packers placed under each corner and mullion and the window is positioned ensuring that the perimeter gap between the window and the structure is kept as uniform as possible subject to the tolerances of the structural opening. 9. The window is then permanently fixed using 45mm 5.5 gauge fixings into the SFS through the fixing straps/brackets. all D.P.C s are in the correct position, using suitable fixings. Packing s are to be placed at every fixing point, ensuring that the window is plumb, level, square and true and that all sashes line up and are working correctly. 10. The secondary internal seal of expanding PU foam or suitable insulation can now be applied, as recommended by the manufacturer, to the perimeter and intermediate joints. Where joints are to be left visible then a suitable backer rod and silicone is to be applied as well as the expanding PU foam or insulation. All silicone must be applied in accordance with the manufacturer s recommendations and to be of sufficient depth to suit the joint width. Excess expanding foam may need to be trimmed off after it has set. 11. Where silicone is to be used as the primary external seal then this is to be completed after the permanent fixing of the window. Ensure that the structure that is to be sealed against is complete, clean and dry. Apply a suitable backer rod and the specified silicone ensuring that the correct depth of silicone is applied to suit the joint width. Silicone must be tooled to give a neat appearance and any excess cleaned off the window and the structure. 12. Ensure working area is free from debris before moving to the next window opening. Report any damage immediately to the Main Contractor whether caused prior, during or after window installation. 13. After the installation of the windows, aluminum cills, corner pressings and pressings in general are then fitted. This is done by screw fixing the cills or pressings with roundhead screws. The ends of cill pressings are finished with suitable silicone to ensure the integrity of the seal around the windows. It is often the case that these items are fitted at a much later date to avoid the risk of damage. 14. All hand tools are to have a daily inspection by the site operative before first use and after every break. All defects should be reported to the site supervisor, all defected tools should be removed from site immediately. Drills Page 35 of 41

37 Method Statement - Working At Height Method of Work Avoid working at height if possible. When using the scaffold on site that has been provided by the Principle Contractor make sure all scaffold has an up to date scaffold tag, which says it has been tested by a competent person and is safe to use. Never make any adjustments and report anything that may not be safe. When using scaffold towers make sure it has erected and checked by a competent person, make sure all toe boards are in place and the tower is on a solid base. If the tower is moved or has been erected for more than 7 days this must be checked again by a competent person. If a MEWP is to be used, only an IPAF trained person can drive the machine, If the MEWP is being moved around site a banksman must be used, the area around the machine must be barriered off when in use. 40 Site Induction Rules 1. All site staff and visitors must sign in and out of the site using the site diary. 2. All deliveries are to be recorded in the site diary 3. Hard hats and protective footwear are to be worn on site at all times unless relaxed by the site supervisor. 4. All materials not to be used immediately must be stored as per the direction of the site supervisor. As a rule all materials must be fixed within 48 hours of delivery. 5. Sub contractors remain responsible for the safe keeping and insurance of equipment and materials left on site in connection with their trade. 6. Emergency procedures are contained earlier in this document and must be read and understood. 7. You shall not intentionally or recklessly interfere or misuse anything provided to safeguard your safety or that of others. 8. Do not enter unauthorized areas as signed. 9. Do not start work unless safe entry and exit routes are available 10. No alcohol, drugs or other illegal substances are allowed on site. Anyone suspected to be under the influence of any of the above substances will be removed from the site. 11. Never climb a defective ladder or an unsecured leaning ladder 12. Keep clear of all removals / demolitions works unless you are involved in that work. 13. Keep clear of working plant and never stand under a suspended load. 14. Do not use any tools unless trained and authorized to do so. This includes cartridge tools and abrasive wheels. 15. Ensure all manufacturers instructions are followed. 16. Never use portable appliances other than 110V or battery powered. All such equipment should be visual inspected prior to use and regularly PAT tested. All trailing cables should be adequately routed to reduce exposure to tripping hazards. 17. Hot work is subject to a permit to work and should be discussed with the site supervisor. No hot work is permitted in the last hour of work in any day. 18. Do not allow rubbish to accumulate, ensure it is disposed of appropriately. 19. Report all accidents to the site suppervisor 20. All roof work must be authorized by the site supervisor. 21. The site is a no smoking area. 22. Ensure you are aware of the hazards associated with the work and the risk assessments which are available from the site supervisor at all times. Page 40 of 41
