3 UC Santa Cruz Automated External Defibrillator Program The UCSC Automated External Defibrillator (AED) program is managed by the Office of Emergency Services (OES). For ques ons about this program, please contact OES at AUTOMATED EXTERNAL DEFIBRILLATOR (AED) PROGRAM The purpose of the Automated External Defibrillator (AED) program is to ensure proper oversight, compliance, installa on, maintenance and response for AEDs. AEDs treat vic ms who are experiencing sudden cardiac arrest. An AED shall only be applied to vic ms who are unconscious, not breathing normally, and showing no signs of circula on such as normal breathing, coughing or movement. An AED will analyze the heart rhythm and advise the operator if a shockable rhythm is detected. If a shockable rhythm is detected, the AED will charge to the appropriate energy level and deliver a shock. The UCSC AED Program and Manual sa sfies the State of California and Santa Cruz County requirements for public AEDs. For further explana on of these requirements, visit the Office of Emergency Services web site: htps://oes.ucsc.edu/public-educa on/cpr-training.html Aside from AED registration and AED record keeping, this manual does not apply to the UCSC Police Department, due to varying compliance standards in training and victim records. OVERSIGHT, ROLES AND RESPONSIBILITIES This sec on describes the roles and responsibili es for the UCSC AED Program. CAMPUS AED PROGRAM MANAGER RESPONSIBILITIES The Campus AED Program Manager is the Director of Emergency Services. The Program Manager is responsible for: Compliance, oversight and management of the UCSC AED program Annual no fica on to the campus of AED loca ons and training Management of the AED roster Promulga on of procedural changes No fica ons to Santa Cruz County Emergency Medical Services (no ce of AED program, loca ons, and use of AEDs) Ensure records of maintenance and tests are appropriately maintained. Providing emergency response system protocols for AED Emergency Response Teams. AED DEPARTMENT COORDINATOR RESPONSIBILITIES Departments intending to place an AED in their building assume the responsibility to maintain compliance with the UCSC AED program, including assigning a Department AED Coordinator. 4 The AED Department Coordinator shall: 1. Read and implement the requirements and guidelines of this UCSC AED Program manual, including the AED Compliance Checklist (which is included below). 2. Atend AED Coordinator training and review updated procedures when distributed. 3. Complete an AED Emergency Response Plan for your building (see template in appendices). 4. Every 30 days, conduct a maintenance check of your AED units and report inspec ons on the campus AED tracking tool. See the Maintenance Requirements sec on of this manual. 5. A er an AED ac va on, remove the unit from service and no fy the Office of Emergency Services (831) Do not download the AED ac va on record. Instruct the AED responder to complete a pa ent record form (included in appendices and on OES web page). A erwards, secure the pa ent record form and the AED, then call the Office of Emergency Services for these items to be picked up. Do not or fax the pa ent record form. AED USER RESPONSIBILITIES 1. A er an AED ac va on, remove the AED from public service and no fy the Office of Emergency Services (831) , or OES@ucsc.edu. 2. Complete a pa ent record form and submit it to the AED Department Coordinator. If you are unable to contact the AED Department Coordinator, secure the pa ent record form and call the Office of Emergency Services at (831) to arrange for the record to be picked up. AED User Manual/Guide (see individual manufacturer s instruc ons). PROCURING AN AED Specific AED models have been pre-selected for FDA approval, quality, service and cost. Visit the Offices of Emergency Services web page to access the preferred vendor and AED model(s). Departments should not purchase AED models outside of this recommenda on. 5 PROCUREMENT COSTS AND MAINTENANCE FEES (ESTIMATES) Central funding may not exist for elec ve AED purchases. Before purchasing an AED, departments should determine if they could support the procurement and maintenance fees associated with owning an AED. Responsibility $1,200 x $35 x Per person $55-90 x $200 x $219 x $1,744 x PLACEMENT RECOMMENDATIONS The majority of UCSC buildings are not required to provide a public AED. However, the Office of Emergency Services encourages the provision of AEDs for the enhancement of public safety. A department that opts in to providing an AED, must then follow all requirements of this manual, including the AED Checklist in order to ensure compliance with applicable laws and regula ons. Visit the OES web site for a complete explana on of public AED requirements as relates to: Health Studio Facili es (Gyms) Health Facili es New Construc on Though most UCSC buildings are exempt from AED requirements, the Office of Emergency Services encourages the addi on of AED units if your building meets the following descriptors: 1. Group A assembly buildings with an occupancy of greater than Group B business buildings with an occupancy of 200 or more. 3. Group E educa onal buildings with an occupancy of 200 or more. 4. Group F factory buildings with an occupancy of 200 or more. 5. Group I ins tu onal buildings with an occupancy of 200 or more. 6. Group M mercan le buildings with an occupancy of 200 or more. 7. Group R residen al buildings with an occupancy of 200 or more, excluding singlefamily and mul family dwelling units. Page 3 of 13
6 An AED may already be in close proximity to your building. See the OES web page for AED loca ons. Please contact OES with specific ques ons regarding the building descriptors or the placement loca ons of a public AED. INSTALLATION, LOCATION AND MAINTENANCE LOCATION AND INSTALLATION The following guidelines provide summary informa on regarding loca on and installa on of AEDs. OES will review and approve actual installa on loca ons on a case by case basis, in consulta on with UCSC Physical Planning, Development and Opera ons. This review is conducted in order to ensure compliance with applicable laws and regula ons, including the Americans with Disabili es Act (ADA). The AED is located in an area accessible to all employees in an unlocked cabinet. An alarm will sound when cabinet door is opened and silences when cabinet door is closed. Install the AED at a point that is central or common to the building s popula on (i.e. near main entrance or near other emergency equipment). Op mally, this loca on is within a ninety-second brisk walk from the furthest populated area of your building. High traffic areas are ideal. The AED will have one set of defibrilla on electrodes connected to the device and one spare set of electrodes with the AED; these are located in the AED case, in lid under flap. One resuscita on kit will be connected to the handle of the AED. This kit will include two pairs of latex-free gloves, one razor, one set of trauma shears, and one facemask barrier device. Adhere to ADA Guidelines: ADA Accessibility Guidelines (ADAAG) specify that objects such as automated external defibrillator wall cabinets shall not protrude more than 4 inches from the wall into walks, corridors, passageways, or aisles. Ensure that the AED is installed in an area with consistent phone service Ensure that the AED is clearly visible and unobstructed with appropriate signage and labelling. Affix use and repor ng instruc ons with each AED (see AED Instruc ons and Repor ng annexes within this manual) Automated External Defibrillator Moun ng Heights The height to reach the handle of an automated external defibrillator (AED) in a public gathering place should be no more than 48 inches above the floor. ADA guidelines specify maximum reach ranges for health equipment such as automated external defibrillators and other life safety devices. For safety equipment with an unobstructed approach, the maximum forward reach to the equipment is 48 inches above the floor. The maximum side reach for an unobstructed approach to an AED is 54 inches. Page 4 of 13
7 MAINTENANCE COMPLIANCE A department owning an AED assumes the responsibility to regularly maintain and test the AED according to the guidelines set forth by the manufacturer and according to any applicable State Laws or Civil Codes. In addi on to the manufacturer s instruc ons, ensure the following are completed. Every 30 days, verify that the AED is operable. The first Monday of every month, record this verifica on via the tracking mechanism provided by the Office of Emergency Services. For access to the AED Team Google Drive, contact the Office of Emergency Services at OES@ucsc.edu. Post AED instruc ons next to any installed AED. (Use the manufacturer s user instruc ons for this handout and pos ng). The wall pos ng must be in no less than 14- point type font. OPTIONAL CPR/AED TRAINING While CPR and AED training is not required in order to have an AED unit placed, for every AED unit acquired, up to five units, the Department Coordinator may wish to have one employee trained in cardiopulmonary resuscita on and AED use. All trainings must comply with the regula ons adopted by the Emergency Medical Service Authority and the standards of the American Heart Associa on or the American Red Cross. The AED Department Coordinator shall record trainings on the AED Team Drive and forward no fica on to the Campus AED Program Coordinator. The Campus AED Program Coordinator will maintain training records. a copy of training cer ficates to: oes@ucsc.edu. ACTIVATION, REPORTING AND RESTORATION AED deployments require no fica on, repor ng and restora on. Complete the following steps a er each par al or full AED use. 1. The AED responder shall no fy the Department Coordinator and the Office of Emergency Services (831) of the AED use. Prepare a No fica on of AED Use form (see annex) and note any procedural complica ons. Documenta on shall be ini ated whether or not defibrillator shocks were delivered. Contact the Office of Emergency Services for pickup of the Notification of AED Use Form at oes@ucsc.edu. 2. The Office of Emergency Services will pick up the AED to process the data report. A er examining and refurbishing the unit, OES will return the unit to the department. 3. Do not restore the unit to public access un l OES has picked up and serviced the unit. 4. Rendering aid can be a difficult experience. For grief counseling or mental health services, UCSC s Human Resources Employee Assistance Programs. Page 5 of 13
8 AED COMPLIANCE CHECKLIST The AED coordinator is responsible for reading and implemen ng all requirements and guidelines of the following AED Compliance Checklist. Item Task 1 Read the full content of the AED Program manual. 2 Comply with all regula ons governing the placement of an AED (see manual). 3 Determine if your department has funding to purchase, install and maintain an AED unit. 4 Purchase the AED model recommended and supported by UCSC s Office of Emergency Services. 5 Register your unit with the Office of Emergency Services oes@ucsc.edu and on the AED Team Drive. (OES will register your unit with Santa Cruz County Emergency Medical Services). 6 A manufacturer or retailer shall provide to the acquirer of the AED all informa on governing the use, installa on, opera ons, training, and maintenance of an AED. 7 Access the AED Team drive to maintain your AED maintenance records (request access by ing oes@ucsc.edu). 8 OPTIONAL: Have 1 trained AED user per 5 units. 9 Prepare a department AED Emergency Response Plan. 10 Install your AED (or coordinate installa on) in accordance with the manufacturer s and this manual s installa on guidelines. 11 Ensure that the AED is maintained and tested in accordance with the opera on and maintenance guidelines set forth by the manufacturer. 12 Ensure that all records of service, maintenance and tes ng are recorded on the AED Team Drive roster. 13 Ensure that an inspec on is made of all AEDs on the premises at least every 30 days for poten al issues related to operability of the device, including a blinking light or other obvious defect that may suggest tampering or that another problem has arisen with the func onality of the AED. Document on the UCSC AED Team Drive. 14 OES will provide an annual no fica on to the campus, detailing the loca ons of campus AEDs, instruc ons for AEDs use, and where voluntarily AED and CPR training is offered. 15 Annually, have at least one member of your AED response team receive an AED demonstra on. In this training, they will walk through how to use an AED properly during an emergency. Register for training at the UC Learning Center; see Office of Emergency Services training courses. 16 Next to the unit, post AED Instruc ons and Repor ng that came with the unit. Must be in no less than 14-point type. 17 A er the AED is used, remove the unit from public access, contact the Office of Emergency Services (831) and complete a No fica on of AED Use Report. 18 If the AED is used, contact OES to complete the Restore of your AED. 10 AED EMERGENCY RESPONSE PLAN (POST AT AED AND SHARE WITH BUILDING RESPONSE TEAM) Building Occupants: Voluntary CPR or AED training is available through the Office of Physical Educa on, Recrea on and Sports (831) , htps://opers.ucsc.edu/index.html. Annex A: Breathing Victim Emergency Response Procedures Step Assess and Respond 1 First person to respond shall ini ate the Chain of Survival by calling out for help, state that there is a medical emergency! Call 911 and delegate someone to go outside to escort the paramedics to the scene. The AED and any other medical supplies are to be brought to the pa ent. 2 No fy trained personnel. If trained, the responder will ini ate CPR un l the AED arrives. Ini al protocol for the unconscious vic m is as follows: 3 Upon arrival, assess the scene safety; use universal precau ons (no electrical, water or criminal hazards). 4 Assess pa ent for unresponsiveness a) Call 911(you will call if you are alone) b)send someone to get the AED (you will get it if you are alone) 5 Assess breathing a) If not breathing give 2 breaths. 6 If warranted, perform CPR un l the AED arrives, then begin AED treatment. 11 Annex B: AED PROCEDURES Step Procedure 1 Turn on AED and follow the visual and/or audio prompts. 2 Open the person's shirt and wipe his or her bare chest dry. If the person is wearing any medica on patches, you should use a gloved (if possible) hand to remove the patches before wiping the person's chest. 3 Apply defibrilla on pads. Make sure the AED pads are placed in the proper loca on and that they make good skin contact with the chest. Do not place AED pads over the nipple, medica on patches or implanted devices. 4 Make sure no one is, including you, is touching the person. Tell everyone to "stand clear." 5 Push the "analyze" buton (if necessary) and allow the AED to analyze the person's heart rhythm. 6 If the AED recommends that you deliver a shock to the person, make sure that no one, including you, is touching the person and tell everyone to "stand clear." Once clear, press the "shock" button. 7 Begin CPR after delivering the shock. Or, if no shock is advised, begin CPR. Perform 2 minutes (about 5 cycles) of CPR and continue to follow the AED's prompts. If you notice obvious signs of life, discontinue CPR and monitor breathing for any changes in condition. Ini al and current condi on of the vic m. 2 Complete a No fica on of AED Use (find form on the Office of Emergency Services web page or in the AED case) 3 Remove the deployed AED from public access. 4 Call the Office of Emergency Services for AED pickup (831) The Office of Emergency Services will complete an incident debriefing, file reports, refurbish and return the AED when opera onally ready. 7 If you have provided aid to a vic m and feel overwhelmed from the experience, visit the UCSC Human Resources web site for counseling services.
