3 INTRODUCTION Tri-City Storm Soccer Club (TC Storm) is dedicated to providing the most comprehensive soccer experience in our region. TC Storm has enjoyed success with our involvement in youth soccer and learned that one key ingredient of success is clear, consistent communication between the club, its teams, coaches, players and parents. This document is one way of explaining, in detail, our expectations of you as a member of TC Storm. It will help you answer questions about the club s decision-making process in the Select Program. As a member of TC Storm, please take the time to read the following pages carefully. The Code of Conduct Agreement must be signed by player and parent to be accepted in the club. This agreement covers the seasonal year of August 1 st through July 31st. TC Storm is committed to providing the proper environment to help our athletes achieve their goals. We believe that there are different stages of development in each player s soccer career. By joining the Select Program, you have joined a group of players who are looking for a training program that is designed for the youth athlete who is serious about soccer and interested in playing at the highest competitive level available. The training program is geared to prepare the players to compete both technically and tactically at the highest level. This handbook will outline some of our policies and expectations in order to achieve this common goal. Members of TC Storm agree to put the club ahead of the team. When decisions are made regarding points of contention between the club and a particular team, the club will come first. Decisions regarding soccer issues will be made by the Select Program Director. Issues beyond soccer or if you feel any issue is not being addressed properly, may be directed towards our office at rd St South Suite A, Fargo ND or ed to execdirector@tricitystorm.org. Members of TC Storm agree to put the team ahead of the individual player. If conflicts arise between the team and a player or parent, the team coach under the direction of the director of coaching will be responsible to make decisions for the good of the team. These decisions will be made after gathering all relevant information and consulting with those involved. The decision of the team coach will be objective and will be based on his/her best evaluation of the facts. Once a decision has been made, it is final and without appeal. Players and parents must agree to abide with the final decision. PLAYER INFORMATION Commitment Joining TC Storm is voluntary, but also brings with it a commitment. We hope that you and your parent(s) will consider all of your options and choose what will be best for you, taking into consideration your goals and aspirations. Once you commit to join TC Storm you agree to abide by the policies of the club and your team. This commitment is for one soccer season/year only (ending July 31 st, 2017). Once your commitment is made, the club expects your full participation and support for the entire soccer year, even in the event team results, personal playing time, personal relationships, etc. do not meet your expectations or are not to your liking. Take into account that when you make your commitment, our coaches may need to inform other potential players they did not make the team. Please be sensitive to others and consider your decision carefully. If you have committed to the club, the club will also be honoring its commitment to you. No player will be released from a team during the soccer year unless it is for disciplinary reason or failure to fulfill your financial and commitment obligations. The exceptions would be when that players family has moved or severe injury. The Select Director and Team Coach will make decisions regarding temporary suspension from the club after carefully consulting with the Board of Directors. Decisions by the Select Director and/or Board of Directors are final. Season training sessions are normally conducted three times per week (number of training sessions will vary pending the time of year) and will be determined by the coach, director or team manager. Every effort is made to establish regularly scheduled sessions. However, this is not always possible due to training availability, weather and coach availability. Players should 3 P a g e

4 always be dressed and ready to start practice on time. Players are expected to be consistent in their practice attendance. Participating in Multiple Sports TC Storm is flexible with players who play multiple sports. We have many players currently in the club who play football, basketball, volleyball, baseball, hockey, softball, and track. TC Storm tries to work with the other sports schedules as much as possible, but there must be good communication between the parents and the coaching staff on which practices and games that player will miss as a result of participating in multiple activities. Player Responsibilities The players in all of the TC Storm programs have the following responsibilities: Act in a respectable manner. Without an opposing team or referees there wouldn t be any games to play, so in the spirit of competition be sure to treat teammates, opposing players, coaches, referees, and spectators with the utmost respect. Play within the rules and spirit of the game of soccer. Strive to always win, but don t mistake winning with achieving success. Successful players display humility in victory and gracefulness in defeat. Focus only on that which you have control over. Accept the decisions of the referee and avoid making excuses about things you have no control over such as the other team, referees, spectators, etc. Be a positive role model for all around you by practicing good sportsmanship at all times. Remember the phrase, We are what we repeatedly do. Therefore, excellence is not an act, but a habit. Remain dedicated to the team and your teammates. Once you accept a spot on a team you are committing to attend all training sessions and events. It is the responsibility of the player/parent to notify the coach at the earliest possible moment if you are unable to attend a practice or event. Report all injuries to the team coach and the Select Director as soon as possible. Do not under any circumstances hide an injury from your coach. Keep your uniform in a respectable fashion. The responsibility of uniform condition lies with the player. Replacement of a damaged or lost uniform will be the responsibility of the individual at full cost. Academic Standing Although TC Storm is not affiliated with any school district or school, TC Storm recognizes the importance of academic achievement. TC Storm does not require a specific level of academic achievement for participation in any of the TC Storm programs. Instead, TC Storm supports parents setting academic standards for their children. TC Storm coaches are expected to honor temporary limits to a player s participation in soccer to support academic studies, if necessary. Substance Abuse TC Storm prohibits all forms of substance use. Any cases of substance use can effect a player s participation or result in termination of membership with TC Storm. Injury Policy Please seek medical attention if an injury to a player renders them unable to participate. TC Storm recommends the players follow the advice of their physician and only return to play when it is deemed appropriate or safe. Injured players are encouraged to continue to attend team events during their recovery. If a player visits a medical professional for any injury they have sustained, they will need to submit a doctor s note stating the extent of the injury and a timeframe for when the athlete can return to full participation to the Select Director. 4 P a g e

5 UNIFORM AND APPAREL Uniform Policy The cost of uniforms is separate from the club fee because TC Storm works with specific vendors to provide player uniforms. In addition, by keeping the costs separated, uniforms will not have to be purchased every year and may be used as long as they are in acceptable condition. Uniforms will need to be ordered through a designated club official, will be paid for by each player. Players will be able to order specific sizes to meet their individual needs. The basic required uniform kit includes: two jerseys (home and away), one training jersey, one pair shorts, and two pairs of socks. Players may purchase additional items at their own discretion. Players numbers will be assigned but the club and be determined by age group. Only TC Storm approved logo(s) and jersey number can be on TC Storm uniforms (jerseys, shorts, etc.) All players should arrive to a game wearing their uniform or other warm-up gear as specified by their coach. All players should have their complete game uniform sets with them, along with their soccer bags, soccer balls, and water or sports drinks. Soccer bags should be lined up neatly on the sidelines. Club Logo and Spirit Wear Policy The logo of TC Storm is considered intellectual property and is proprietary to TC Storm. The logo may only appear per Board Guidelines: Only approved vendors are allowed to produce spirit wear items. Spirit wear is defined as all items, including apparel and merchandise that carry the TC Storm logo and/or name. All spirit wear merchandise for sale to the membership falls under the direction and control of TC Storm. TC Storm members cannot use the TC Storm logo, name, list, and tax ID without the prior approval of the Board of Directors. Teams/members must have prior approval of the Board of Directors to sell spirit related items such as stickers, banners, pins or flags. GAMES AND EVENTS During the course of the soccer season players participate in practices, scrimmages, and games, during which the coach will make decisions about player selection, lineup, player s position, amount of playing time, and tournament participation. Players are responsible for maintaining a positive presence on their teams in fulfilling the role they are asked to perform, even if that role has them coming off the bench, playing a different position or not playing the desired minutes. Playing Time TC Storm recognizes the importance of player development as one of the pillars of our club s philosophy. This is especially important in the younger age groups. While player development occurs during team practices and individual work, certain skills are best evaluated/tested in game settings. For this reason, TC Storm has placed the following guidelines in place regarding playing time during games: U12 and below: TC Storm will practice equal playing time per game to the best of the coach s ability. U13: and above: 1 st teams field players will play at least 33% of every game to the best of the coach s ability. 2 nd teams or lower each field player will play at least 50% of every game to the best of the coach s ability. 5 P a g e

6 All playing time requirements are subject to modification by the coach for disciplinary reasons such as failure to attend practices, work ethic, attitude, arriving late to games or injury. In the event that a player would like to discuss their role on the team, the player or family should contact the coach to setup a time to discuss the situation. GUEST PLAYERS The use of guest players is permitted when a team is short of available players. Guest players are not to be used to replace players currently on the roster. Guest players should be the exception, not the rule. A player who regularly guest plays for a team should reasonably be considered part of the team for fee purposes. Guest players for tournaments will be responsible for their share of tournament expenses. If a player would like to guest play for another club, they will need to approval and proper paperwork from the Select Director. Please contact the Select Director as early in the process as possible so they can provide assistance. PARENTS, FAMILY, & GUARDIANS Expectations and Responsibilities Parents and Guardians of players should conduct themselves in a mature and responsible manner whenever representing TC Storm at practices, games, or outings. Negative remarks about teammates, opponents, referees, or coaches reflect poorly on our club and may lead to an unhealthy environment within the team. Always be aware of the example you are setting as an adult and role model for our players. The parents of players in all of the TC Storm programs have the following responsibilities: Be respectful to everyone on the field, regardless of which club they support. Without an opposing team or referees there wouldn t be any games to play, so in the spirit of competition be sure to treat teammates, opposing players, coaches, referees, and spectators with the utmost respect. Be supportive of not only your own child, but also all other players on both teams. Cheer on all players in reaction to play, not anticipation of what will happen. For example, great pass! is an acceptable comment. Pass it! is not an acceptable comment. Do not coach or give directions from the sidelines. Shouting directions from the sideline will only distract the player from focusing on the task at hand. Additionally, comments from the sideline may contradict the message from the coach, which creates more confusion for young players. Provide transportation to and from all practices and games and ensure the player is prompt in arriving and departing. Ensure that the player has the proper equipment, including shin guards, appropriate ball size, uniform, shoes and weather appropriate clothing. Promote an active and healthy lifestyle for your child. Children pick up on a lot that their parents do so if you as parents lead a healthy lifestyle the kids are likely to follow the same path. Encourage your child to play soccer on their own as well as other sports/activities and prepare healthy meals to support their growing bodies. Volunteer your time at club functions. Volunteers are a crucial part of the club and as such we rely heavily on the parents in the Select Program to help provide exceptional service to our members. Pay program fees in a timely manner. Report all injuries to the team coach and the Select Director as soon as possible. Do not under any circumstances hide 6 P a g e

7 an injury from your coach. Keep your child s uniform in an acceptable condition. The responsibility of uniform condition lies with the player/family. Replacement of a damaged or lost uniform will be the responsibility of the individual at full cost. Communication Parents should feel free to communicate and ask questions to their coach about their child, but please be mindful of when it is appropriate to do so. Parents, players and coaches need time to reflect and process performances following a game. Therefore, parents are asked to refrain from approaching coaches to discuss issues/concerns for a period of 24 hours after the event. Also, please take the initiative to encourage and teach your player to speak up and communicate for themselves whenever possible. Players U14 and above should start to have these conversations with and the coach as opposed to their parents. Parents should also be aware that coaches use the time before and after training sessions in different ways. Understand that individual coaches will feel more comfortable communicating at different times and in different ways (phone call, personal visit, and/or text). Consult with your player s coach before the season to see what methods work best for them. Issues and Conflicts If there is any issue or conflict that arises during the season (whether between players, parents, or other), please follow these steps to properly handle the problem: Discuss the problem with the coach/manager. If not resolved then Discuss the problem with the Select Director. If not resolved then Discuss the problem with the Executive Director. Scheduling Parental support and involvement in the club are essential for our continual growth. Parents of players must make sure their child attend all possible club and team functions (practices, meeting, games, etc). We understand there will be times when a conflict cannot be avoided and the player must attend another event. The club requires your communication, planning and understanding so we can minimize these potential conflicts. Game Expectations TC Storm encourages all parents to attend as many games as they can. Please be aware that you are representing your club, city, state, or country depending on the event/opponent, and to act accordingly. There should be no coaching by parents from the sidelines, as this may conflict with instructions given by the coach and could confuse the players. It is important that players are given one set of instructions by one voice. Your vocal support, cheering and positive encouragement are strongly encouraged. There should be no yelling or complaining to referees or officials during or after games. The display of good sportsmanship extends to interactions with opponents as well. Act appropriately; do not taunt or disturb other fans or players. Profanity or objectionable cheers and gestures are offensive. Complaints in regards to sideline conduct will be taken seriously and investigated by the Directors and BOD. TC Storm has the right to remove a parent from the sidelines, if necessary for behavior not acceptable to TC Storm s Parent/Player Agreement, Financial Agreement and any other NDSA/MYSA or USYSA/US Club rules that apply to sideline behavior. This removal can be for one game, one month and/or the season/year. The duration of this removal will be at the discretion of the Board of Directors for TC Storm. No one other than those listed on the official game roster may sit on or near the team bench before or during games. Please respect the space and privacy necessary for the coach and team to carry out their game responsibilities. 7 P a g e

8 SOCIAL MEDIA Social Media Use TC Storm parents, players, family members, guardians, coaches, referees, and staff are all representatives of the club and community. The club requires all members to exercise good judgment in their use of social media websites and to conduct these activities in a responsible and respectful manner. Best Practices and Reminders for use of social media: Think twice before posting. If you wouldn t want your parent, player, coach, or others involved in TC Storm to see your post, don t post it. Be respectful and positive. Remember, many different audiences with different viewpoints or senses of humor can view your post. The Internet is permanent. Even if you delete something, it s still out there somewhere. Be in the right state of mind when you make a post. Do not post when your judgment is impaired. TC Storm Staff and BOD members may at any time monitor social media activity. Posts containing derogatory, inappropriate or offensive language towards another member, opponent, the community, or club will be placed on file. Penalties ranging from temporary suspension to club expulsion will be at the discretion of the Program Directors and BOD. ANTI-BULLYING Definition of Bullying Bullying is a pattern of aggressive, intentional or deliberately hostile behavior that occurs repeatedly and over time. Bullying behaviors normally fall into three categories: physical, emotional, and verbal. Bullying may include, but are not limited to, intimidation; assault; extortion; oral or written threats; teasing; putdowns; name-calling; threatening looks, gestures, or actions; rumors; false accusations; hazing; social isolation; and cyber-bullying. All reports of actions falling into the definition above will be investigated and dealt with seriously. Consequences for those involved shall be determined base of the severity and nature of the bullying behavior, the developmental age of the individuals involved, and any history of problematic behaviors or performance. Every effort will be made to correct the problem behavior, prevent another occurrence of the behavior, and protect the victim. VOLUNTEER PROGRAM POLICY Volunteers are a crucial part of the TC Storm club, without them, it would not be possible to host club events. Volunteer opportunities allow us to keep our registration fees at low rates as well as build a positive presence in the community. As our club has grown it has become more important to be able to count on the volunteers that are critical to our continued success. Volunteer hours go way beyond just helping out in the concession area. There are opportunities to volunteer as a recreational coach, help out as a Select team manager, helping out with administrative work, setting up fields for games/events, etc. If you are looking for volunteer hours be sure to check the DIBS section on the website or contact the office for more opportunities. Parents and guardians of all TC Storm Select Program players are required to fulfill annual volunteer hours and submit a volunteer deposit check in the amount of $100 per player. In the event that you do not complete your volunteer hours by the end of the programming year the check will be cashed. If you complete a portion of your volunteer hours, the check will be cashed and the remaining amount based on the number of unfulfilled hours will be credited to your account. Those families with multiple children will be billed for any unfulfilled volunteer hours at the end of the programming year. The 8 P a g e

9 requirements are as follows: Number of Players in Select Program One Two Three Plus Total Hours Required 8 hours 14 hours 20 hours Although families can volunteer for more hours, these commitment levels must be met each year. The volunteer hours can be performed by any member of the family as long as the activity selected is appropriate for the age and maturity level of the person performing the activity. The TC Storm BOD reserves the right to make the final determination as to what is age appropriate and safe. Children under the age of 14 must be supervised by an adult parent or guardian. Volunteer assumes responsibility for his/her physical fitness and capability to carry out the agreed upon volunteer activities, and assumes all risk of injury or damage that may result from such involvement. Volunteer hours are not transferable between families, and may not be rolled over from year to year. Members registering late have the same volunteer goals as those families who registered on time. Once you have signed up for a volunteer time slot, you are responsible for fulfilling that obligation. If you cannot do so, you must notify the TC Storm Office at least one week in advance. It is the responsibility of each family to verify that its volunteer time has been recorded properly. At most volunteer events and activities there will be a sign in sheet and a person designated to manage volunteer activities. For smaller events or activities where there is no sign in sheet, be sure to coordinate with the event or activity manager to be sure that your service is recorded properly. COACHING STAFF TC Storm coaches are instructed to conduct themselves first as teachers and second as soccer coaches. Our coaches cannot overlook the impact they have on players lives. Coaches are responsible for the conduct of the team on and off the field whenever the team is together. We insist our players are polite, well behaved and respectful. Players and parents can expect honesty, communication, consistency and reliability from the coach. The club and coaches are committed to giving players the tools and skills to enable them to become as good as their ability, desire, commitment and effort allow. Coaches are committed to creating an environment that is conducive to the development of quality, competitive soccer. We recognize there are things more important in life than soccer. In terms of priority, family and school work come first. Because TC Storm is dedicated to player development, it follows guidelines for the hiring/placement of coaches including qualifications and playing experience requirement. The Club has created clear duties and responsibilities for its paid staff. This will help in ensuring that each team is assigned the appropriate level coach. It is the philosophy of the TC Storm that coaches will lose their effectiveness over a long period of time with the same group of players. As such, the TC Storm reviews its coaching assignments each year to ensure that the players under the guidance of their coach continue to develop and are challenged. Coaches will be evaluated regularly by the Select Program Director who will use information obtained through direct observation, self-evaluation, solicited feedback from all players, and informal/unsolicited feedback from members as part of the evaluation process. 9 P a g e

10 FINANCIAL POLICIES Financial Assistance for Club Fees A limited number of scholarships are available to help assist with the fees for players who demonstrate financial need. These scholarships can be awarded on a full or partial basis. Applications must be completed and submitted to the TC Storm office. Contact the Office or Executive Director of Club for further information. Scholarships are only available for the TC Storm program fees, not uniforms, tournaments or team fees. For more information and the application form please visit the website. There may be situations where TC Storm would approve services in exchange for credit for the payment toward club fees. Criteria for approving the situations should include a demonstrated financial need of the club member and a recognized financial benefit to the club of the service rendered. Interested members will be required to complete the TC Storm Scholarship Application Form and contract for in-kind services rendered. All scholarship program policies apply. Note: TC Storm reserves the right to evaluate services and amend contract as needed at any point in time. Refund Policy Evaluation Fee Non refundable in any circumstance. $250 Commitment Fee Non refundable in any circumstance. Select/Competitive Fees - Tri-City Storm Soccer Club will not refund evaluation, commitment or registration fees. As a Non-Profit organization that must plan the use of resources and staff in advance, we do not refund Select/Competitive fees.* * In the case of a season ending injury, fees for tournaments that are paid in advance may be considered for refund. The injured player must be able to present a physician's note state the date they are eligible to compete and conduct physical activity. All requests must be made in writing and documentation of injury are required. Proration of Club Fees A player who joins the club midway through a soccer year shall be responsible for paying the full program fee. An exception to this policy will pertain to players who are joining TC Storm for the first time; these players shall pay a portion of the annual fee, prorated to the fraction of the soccer years remaining at the time they join the club. Delinquent Fees Unless other arrangements have been made with the club or Select Director, any player who is delinquent in any payment, whether club fees or team fees, will not be permitted to participate in any practices/games/tournaments until the delinquent fees have been paid. The club will review fees on a periodic basis each year and any player whose club or team account is delinquent on the review date will not be eligible to participate in any practices/games/tournaments until the accounts are current. Note: Penalty fees may apply to past due accounts and/or returned checks. Additional Tournament Fees Outside of Program Inclusions Teams electing to add tournaments, not included in the price of the Select program, will be responsible for all fees associated with the tournament, including but not limited to registration fees, coaching fees, coaching per diem, and coach s travel fees. At the time of team registration, all players indicating availability/commitment will be responsible for payment of associated fees. 10 P a g e

11 FUNDRAISING Club Level TC Storm is looking towards the future. Two of our long-term goals are to keep program costs down for our members and to secure an indoor facility to improve developmental opportunities throughout the entire year. Fundraising is essential in keeping costs reasonable and ensuring opportunities for the youth we currently serve and for those in the future. To effectively pursue a facility, our club must be in a good financial position and therefore the members of TCSSC will participate in a fundraiser to help fund these goals. There will be a mandatory fundraiser in the amount of $100 of raffle tickets sold per player. There is a Multiple Player Family Cap of $250. The 1st and 2nd player have $100 minimums. Additional players are only responsible for total of $50 more. Further details regarding the fundraiser will be presented in the upcoming season. Team Level Teams are allowed to conduct/host fundraising events to benefit the members of their team, subject to approval by the Board of Directors. Teams will not be allowed to use the TC Storm logo in conjunction with their individual fundraising events. Teams will not be allowed to solicit sponsorship(s) to benefit their individual team members. This includes both cash and/or in-kind donations. Individual teams will be responsible for determining how funds raised from team level fundraising events are distributed. Once approved by the sponsorship committee, the fundraising proposal will be forwarded to the TC Storm Board of Directors for approval. Details, including but not limited to, team requesting the event, complete event details, and reason for request, must be submitted to the committee at least 1 month prior to the fundraising event. You will be contacted within 7 10 business days from the time they received the proposal. The event cannot be advertised prior to approval by TC Storm. FACILITY AND EQUIPMENT POLICIES Use of Soccer Complexes Pepsi Soccer Complex, Moorhead Soccer Complex, Soccer Central and any other site designated by the club contain space scheduled by the club for specific usage. Scheduling/rescheduling of any games, scrimmages or practices will be done through the office or in accordance with the league or facilities policy. Any scrimmage or game between TC Storm and another club will require a referee. The TC Storm field coordinator schedules the use of the TC Storm training complex in Fargo or Moorhead. Individual players and teams ordinarily may not request use of the TC Storm training complex outside of the training sessions, games, clinics, etc., already scheduled for that facility. Substance Free Environment TC Storm facilities and TC Storm training, games, tournaments, special events and other TC Storm-sponsored events are pet, tobacco, alcohol, and substance free environments. Coaches, players, parents, spectators and staff are expected to refrain from use of tobacco. MISCELLANEOUS 11 P a g e

12 Website Policy In order for a photo of any kind to be posted on the TC Storm Website, the photo must first follow the following eligibility criteria: TC Storm will not allow posting of players without parent permission. Permission is included in the Player Contract. The caption for each photo may contain the names of players. We will make every effort to protect a player s privacy. Groups affiliated with TC Storm may request permission to post their announcements on the website. Please contact the TC Storm Soccer Office for additional information. Content posted on any teams/player/member's website must be consistent with the Mission, Values, Policies and Procedures of TC Storm. Risk Management All adults in positions of responsibility or direct contact with members of the TC Storm, including coaches, assistant coaches, team managers, board members, TC Storm staff, etc. will complete a Risk Management form as provided by the affiliated soccer groups under US Soccer. These forms must be filed as directed by the timetable released by the Board of Directors or Staff. TC Storm works to ensure that all individuals required to complete the form do so in a timely manner. Insurance Claims In the event of injury that may be subject to benefits through the NDSA or US Club or another player insurance policy, contact the office within a week after the incident. 13 TRI-CITY STORM SOCCER CLUB POLICIES AND GUIDELINES Player & Family Handbook 2016/17 Please read the agreements listed below. You will need to accept these terms in the online registration in order to become a member of the Tri-City Storm Select Program. I have read the TC Storm Player Handbook and agree to abide by the provisions of the handbook as long as I am a member of a TC Storm team. I agree to put the club ahead of team, the team ahead of player. I agree to accept the decision of the Board of Directors regarding the code of conduct. I understand that if I violate the Code of Conduct, it may result in my dismissal from the club. The club also requires all members to exercise good judgment in their use of social media websites, and to conduct these activities in a responsible and respectful manner. I understand that there is no discussion, confrontation or acting on concerns with coaches, staff, or team managers for 24 hours after an event or incident. If I violate this policy I will not be allowed on the sidelines for a period of time to be determined by the TCSSC staff and Board of Directors and MAY affect my player s future participation. I understand that TC Storm has the right to remove a parent from the sidelines, if necessary, for behavior not acceptable to TC Storm's Parent/Player Agreement, Financial Agreement and any other USYSA/US Club/NDSA/MYSA or US Soccer Affiliates rules that apply to sideline behavior. This removal can be for one game, one month and/or the season. The duration of this removal will be at the discretion of the TC Storm's Board of Directors. Each family has volunteer hours to complete before the end of the season, I understand that this is mandatory by each family and the hours are determined by the amount of children in the program as outlined in the handbook. All families are required to submit a $100 deposit check. This check will only be cashed at the end of the programming year if any volunteer hours are left unfulfilled. I agree and understand that I am aware of and required to participate in this year s raffle fundraiser. It will be implemented in the spring of the programming year soccer season for the amount of $ Every Select player is required to participate in the fundraiser, but if families choose not participate they will have the option to buy out their raffle obligation for a fee of $100 per Select player up to a maximum of $250 per family. I hereby grant permission to TCSSC to use my child s photograph without payment or other consideration in print or online materials designed for news, informational, or educational purposes related to TCSSC this does include website and social media. I hereby authorize TCSSC to publish such photos with or without associated names for any lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness or my child s likeness appears. I waive the right to royalties or other compensation arising from the use of the photograph for TCSSC purposes. 13 P a g e
