Guide to using and filling out Club or Team or Player Registration Forms

1 This guide will hopefully help you, when you wish to know how to fill in the registrations forms that can be found on the Garforth League website. There are currently 3 forms available to clubs for registration purposes these are:- 1. FORM C10 GJFL CLUB APPLICATION FORM (Click Here To Download) 2. FORM T10 GJFL TEAM APPLICATION FORM (Click Here To Download) 3. FORM P10 GJFL PLAYER REGISTRATION FORM (Click Here To Download) This guide will take each form in order, however each form is easier to fill in if you first of all download it from the website (links above) and then open it in the latest version of the free adobe acrobat reader (Download Latest Adobe Acrobat Reader). Once you have followed the instructions to install the software you can begin. All the forms when filled in using acrobat reader will guide you from field to field and drop down fields will show the required input were necessary. Mandatory field are also shown with a red outline border. I would advise you now to download each form and save it to your computer, maybe naming the forms as C10, T10 and P10. The downloaded forms can then be opened and filled in on your computer. When you have completed all the required sections in the form you opened, you can then save the form again as a different name Maybe C10yourclubname as an example, for club forms, T10yourteamName for example for the team form and P10playername for player form. 2 C10 GJFL CLUB APPLICATION FORM One of these forms has to be completed for each club entering team or teams into the league. The information supplied will be checked with the relevant authorities. The league has been awarded Charter Standard League status. And because of this, ALL member clubs must have already attained, or be in the process of attaining, at least the minimum level of FA Charter Standard Status before the commencement of the coming season. Clubs that are new to the GJFL for the coming season will be granted 12 months to attain Charter Standard accreditation and should be accredited by the start of the following season. Clubs are warned that the GJFL will verify all Charter Standard status with the WRCFA. Any Club or Official that submits misleading or false details will be reported to the WRCFA and could be penalised. 1. Football Club Details b. FA Charter Standard Status:- Select from dropdown list c. FA Charter Standard Level:- Select from dropdown list d. Date Charter Standard Expired:- Enter date of expiry of Charter Standard 2. County FA Affiliation Details a. Name Of County Association Club Affiliated to:- Select from dropdown list b. Name Of District Association Club Affiliated to:- Select from dropdown list c. County Affiliation Number: - Enter your CAN or UNKNOWN if awaiting it. 3. Club Secretary Details a. Enter contact details of the club secretary, the Name, Address Line 1, Town, 4. Club Treasurer Details a. Enter contact details of the club treasurer, the Name, Address Line 1, Town, 5. Club Welfare Officer Details a. Enter contact details of the club welfare officer, the Name, Address Line 1, Town, b. The club welfare officer must be Criminal Record Bureau (CRB) checked therefore the CRB Number, CRB Expiry Date, WRCFA CRB ID Number, WRCFA CRB ID Expiry Date and Date of attendance of the Welfare Officer Workshop must be entered as they are Mandatory. 6. Agreement b. READ THE AGREMENT. c. Save and Name the form EG save as C10YourClubName. d. Print the form. e. Chairman and Secretary of the club should sign the agreement on behalf of the club entering the league. 7. Finish a. The now signed document can now either be scanned and ed to Club Registration (Preferred Method) b. Or posted to GJFL Registrations Season , 52 Bishop Way Tingley Wakefield WF3 1UA. No monies need to be sent as the Garforth League Treasurer will invoice clubs. The registration Secretary will acknowledge receipt of the forms. Please to Club Registration if you have not received your confirmation more than a week after you ed / posted your application.
3 T10 GJFL TEAM APPLICATION FORM One of these forms has to be completed for each team wishing to play in the league. The information supplied will be checked with the relevant authorities. Any Club or Official that submits misleading or false details will be reported to the WRCFA and could be penalised. 1. Team Details a. Name of Team: - Select from dropdown list or type in if you re new to league. b. Age Group & League Format: - Select from dropdown list. c. Name and Address of Ground: - Enter the name and address of the ground you play your home match on excluding postcode. d. Postcode of ground: - Enter the postcode of the ground you play your home matches on. e. Home Kit Details: - Enter the colours of your home kit, Shirt, Shorts and Socks. 2. Team Manager Details a. Enter contact details of the Team Manager, the Name, Address Line 1, Town, b. If extra coaches are required please entered same details on separate sheet. 3. Coaching Qualification Details a. Highest Coaching Qualification: - Select from dropdown list. b. Highest Coaching Qualification Date: - Enter date of qualification c. Have you completed a FA Safeguarding Children Workshop: - Select YES or No d. Have you completed a FA Emergency Aid Course: - Select YES or No e. The team manager must be Criminal Record Bureau (CRB) checked therefore the WRCFA CRB ID Number and WRCFA CRB ID Expiry Date must be entered as they are Mandatory 4. Agreement b. READ THE AGREMENT. c. Save and Name the form EG save as T10YourTeamName. d. Print the form. e. Chairman and Secretary of the club should sign the agreement on behalf of the club entering the league. 5. Finish a. 4 P10 GJFL PLAYER REGISTRATION FORM One of these forms has to be completed for each player wishing to play for a team already registered to play in the league. The information supplied will be checked with the relevant authorities. Any Club, Official or Player that submits misleading or false details will be reported to the WRCFA and could be penalised. This form is used to registered players for your team. HOWEVER AT THE END OF THE SEASON AT THE END OF APRIL THE REGISTRATION SECRETARY WILL ISSUE REGISTRATION FORMS FOR CURRENT PLAYERS REGISTERED FOR YOUR TEAM THESE SHOULD BE USED IF THE PLAYER NAMED IS TO CONTINUE PLAYING FOR YOUR TEAM IN THE NEXT SEASON AGE GROUP (I.E. MOVING FROM UNDER 14 TO UNDER 15) THE FORM JUST NEEDS PHOTO AND SIGNATURE. IF A PLAYER IS NOT CONTINUING WITH YOUR TEAM PLEASE CROSS DESTROY THE FORM. 1. Type of Registration a. The top of the form contains the selection of the type of player registration that you will be using the form for: - Select EXISTING, NEW or TRANSFER b. Selecting EXISTING means you have the players Last Season ID Card with his current GJFL ID Number from the previous season. You have to fill in the relevant box this number else the player is a NEW player (see c below). c. Selecting NEW means the player being registered did not play for a team, which played in the GJFL during from the previous season. You will have to supply a COPY of proof of age document i.e. Passport or birth certificate etc. d. Selecting TRANSFER means the player being registered currently plays for another team currently playing in the in the GJFL. Please complete the form and return it with the registration card of the player for his current team. If this is not possible then the transfer process will take longer as communications will need to be taken between the two clubs involved As well as completing a Player Transfer Form available to be requested from the Registration Secretary. 2. Player Details a. Enter ID Card Number which can be on the bottom right hand of the Players ID Card from the previous season. b. Players Sex:- Select from dropdown list c. Players Forename: - Enter ALL the forename s of the player as show on proof of age document d. Players Surname:- Enter surname of the player as shown on proof of age document e. Players Address:- Enter the current address of the player including town and postcode f. Date of birth: - Enter the player s date of birth as shown on the proof of age document. g. Players Photo: - If you have already got a photo of the player on your pc you can click in the box and select the photo to import. If not the form can be printed later and the photo attached. 3. Team Details a. Team Number: - Each team has its Own Number which can be found on any of last year s player registration ID cards in brackets at the end of the team name. b. Age Group & Format: - Select from dropdown list. c. Team Name: - Select from dropdown list or if new type in the name. d. Club Last Played for: - If the player is being transferred from another GJFL team then the name of the club the player is being transferred from should be entered. e. Other clubs played for this season: - Enter any other clubs the player has played for during the current season.
5 4. Declaration (Only to be completed if the player is playing in the under 12 s and above) a. Have the player registered for another club outside England: - Choose YES or No b. If you answered YES to last question then answer the question has the player obtained international transfer certificate from the FA : - Choose YES or No c. If you answered YES to last question then please give the approximate date. 5. Signatories Details 6. Finish a. Enter the person about to sign the forms position within the club. b. Enter the date. c. The form can now be electronically signed and saved (and printed if required). a. The now signed document and proof of age document (if required) can now either be scanned and ed to Player Registration (Preferred Method) b. Or posted to GJFL Registrations Season , 52 Bishop Way Tingley Wakefield WF3 1UA The ID Cards are handed out for your team at the start of the season when you attend the manager s pre-season meeting. During the season as you register players an will be send to the address supplied confirming that the player(s) you have sent forms in for have been registered and that the ID card is ready to collect. (Unless you sent stamped address envelope or request posting). Cards not collected will be periodically posted to the club secretary.
