1 HOW TO SIGN UP TO BE A RIDER ON THE U of A PARENTS & FAMILY ASSOCIATION EL TOUR DE TUCSON TEAM 1. Complete the Special Application for Team U of A PFA. This form will be turned in by Captain Earl to the El Tour organizers, so answer all the questions fully and completely. Incomplete forms will have to be returned for your completion. Step by step instructions are on the back of this page. 2. Complete the Assumption of Risk and Release Form. Get a copy of this form at: 3. Enclose a check for $150 made out to El Tour de Tucson/ PFA. This includes your El Tour registration and a non-deductible contribution to the Parents & Family Association of $30. If you are ordering an El Tour jersey your check could be more. See the reverse of this form for a detailed explanation, under # Return the check, the Assumption of Risk form and the Special Application for Team U of A PFA form to: Earl Mendenhall P. O. Box Tucson, AZ All paperwork must be returned to Captain Earl by OCTOBER 7 th. We must have 20 applications for our team by October 7 th or PFA will lose the $30 per person donation mentioned above. We can take late registrations after October 7 th, but there is a late registration shall we say donation of $10 just to keep you motivated. So and get your paperwork turned in on time. Absolutely no registrations can be taken after October 30 th. 6. To RSVP for the no host legendary after ride party, 7pm at Wild Garlic Grill (2530 N 1 st Ave) please Lee O Rourke at lorourke@ .arizona.edu. 7. Go out and get sponsors/donors in support of your ride. After all, you deserve some support!! 2 HOW TO COMPLETE YOUR EL TOUR DE TUCSON REGISTRATION FORM (the Special Application for U OF A PARENTS & FAMILY ASSOCIATION ) 1. Review the front page 2. On the back page complete the basic registration information at the top. 3. Check off your preferred distance in #1 4. Check of the type of bicycle in #2 5. Check off your T-shirt size in #3. All participants will get an El Tour T-shirt as part of your basic registration package. See below if you wish to order a PFA Team T-shirt. 6. Check Level 1 entry in #4. We are riding as a team, so everyone will be in Level 1 7. Read and check all boxes in #5 8. Check where you are staying in #6 9. If ordering an El Tour jersey in #7, check your type and size. 10. Please ignore the box on the right labeled How Much Are You Enclosing Now. Captain Earl will complete that section for you. 11. Write a check to El Tour de Tucson/PFA as follows: a. $150 for on time registrations before October 7 th b. $229 for on time registrations WITH an El Tour Jersey c. $160 if late but still before October 30 th, no jersey d. $239 if late but still before October 30 th WITH an El Tour Jersey

In signing this waiver as a participant in El Tour de Tucson, I understand and accept that I must obey all laws of the State of Arizona that may apply to my activities during this event, especially traffic laws. Unless instructed to the contrary by a law enforcement official, I will comply with all traffic regulations including traffic signals, devices, signs and other traffic rules. El Tour closes at 6:00 p.m. on Ride Day and all course support will end at that time. I understand that if I continue on the course after 6:00 p.m. I am solely and completely responsible for my own safety and support and must obey all laws, including proper lighting on my bicycle after dusk. In consideration of my signing this agreement, I, the undersigned, intending to be legally bound, hereby, for myself, my heirs, executors and administrators, waive and release any and all rights and claims for damages, including, but not limited to, the loss of my bicycle, helmet or any other personal items, I may have against the State of Arizona, Arizona Department of Transportation, Arizona Department of Public Safety, City of Tucson, City of South Tucson, Pima County, Tu Nidito Children and Family Services, The Leukemia & Lymphoma Society of America, Perimeter Bicycling Association of America, Inc., Town of Oro Valley, Town of Marana, Sol Casinos, Catalina Community Services, any other associated agency or beneficiary, any and all governmental and tribal agencies, and any and all underwriters and their representatives, successors, and assigns for any and all injuries suffered by me as a result of taking part in this bicycling event and any related activities. I attest that I will participate in this event as a bicycling entrant; I will wear an CPSC-approved bicycle helmet; that I am physically fit and have sufficiently trained for the completion of this event and that my physical condition has been verified by a licensed medical doctor. Further, I hereby grant full permission to any and all the foregoing to use any photographs, videotapes, motion pictures, recordings, or any other record of this event for any legitimate purpose. I understand that I will receive periodic communication from Perimeter Bicycling in the form of USPS or electronic mail and that I may unsubscribe from such mailings at any time by directly contracting Perimeter Bicycling. The event utilizes open public roads, which may or may not be maintained by local municipalities, cities, counties, and/or the State. The event is not responsible for the condition and maintenance of the roads. Road hazards are always possible and in entering this event, and signing this waiver, I agree and understand that I must be alert to all road hazards, including but not limited to: pot holes, uneven pavement, road cracks, road debris, unfinished construction, and motor vehicle traffic. I understand that aero-type and other similar handlebars are prohibited in this event and that utilizing such bars may result in my disqualification. Furthermore, I am responsible for all my personal items including, but not limited to cameras, cell phones, clothing, bicycles, etc. I understand that there are no refunds or transfers of registration to another person or event. I understand that I will receive a chip (transponder) so I can be timed in this event. I understand that I am responsible for properly mounting the device to my bicycle and that improper mounting may result in NO TIME being recorded.

I fully recognize that there are dangers and risks to which I may be exposed by participating in the Program. I understand that the University of Arizona and its governing board, officers, employees, and agents (collectively the University ) does not require me to participate in the Program, but I want to do so, despite the possible dangers and risks and despite this Release. I therefore agree to assume and take on myself all of the risks and responsibilities in any way associated with the Program. 2. Health & Safety. I understand and agree that the University does not have medical personnel available at the Facility, which is the site location for my participation in the Program. I understand and agree that the University is granted permission to authorize emergency medical treatment, if necessary, and that such action by the University shall be subject to the terms of this Agreement. I understand and agree that the University assumes no responsibility for any injury, damage or cost which might arise out of or in connection with such authorized emergency medical treatment. I have consulted with a medical doctor with regard to my personal medical needs. There are no healthrelated reasons or problems that preclude or restrict my participation in this Program. I have arranged, through medical insurance or otherwise, to meet any and all needs for payment of medical costs while I participate in the Program. I understand that neither the University nor the Facility is obligated to provide transportation in connection with the Program. I understand that I am expected to carry my own automobile liability insurance coverage. 3. Standards of Conduct. I will comply with the University's rules, standards and instructions for student behavior, including the Student Code of Conduct, as well as the Facility s standards of conduct. I understand that I am not permitted to consume alcohol, possess/use illegal substances or weapons, or engage in sexual/illicit behavior while participating in the Program. I waive and release all claims against the University that arise at a time when I am not under the direct supervision of the University or that are caused by my failure to remain under such supervision or to comply with such rules, standards, and instructions. I agree that the University has the right to enforce the standards of conduct described above, in its sole judgment, and that it will impose sanctions, up to and including expulsion from the Program or from the University, for violating these standards or for any behavior detrimental to or incompatible with the interests, harmony, and welfare of the University, the Program, the Facility or other student participants. The University has the right to make changes in the format and administration of the Program. I understand that the University has no control over the operations or premises of the Facility. 4. Assumption of Risk, Covenant Not To Sue, and Release of Claims. Knowing the risks described above, and in consideration of being permitted to participate in the Program, I agree, on behalf of my family, heirs, and

6 personal representative(s), to assume all the risks and responsibilities surrounding my participation in the Program. To the maximum extend permitted by law, I release, indemnify, and covenant not to sue the University from and against any present or future claim, loss or liability for injury to person or property which I may suffer, or for which I may be liable to any other person, during my participation in the Program (including periods in transit). I have carefully read this Release Form before signing it. No representations, statements, or inducements, oral or written, apart from the foregoing written statement, have been made.
