1 Competitive Program Information Packet Thank you for your interest in Alpha s All-Star Cheer and J2 Productions All- Star Dance program! As we continue to strive to be the TOP in our industry, we want children to walk away with the experience of a lifetime! Learning new skills, progressing individually as an athlete, and working as a team is always encouraged and taught here at Alpha Cheer & J2 Productions! As we begin our 21st year in the business of Cheerleading and Dance, we look forward to having you as a part of our All-Star Cheer & Dance Family! CHEER Our Cheer program has sent athletes to Cheer at Tech, Baylor, SMU, Angelo State, UNT, OSU, UT Austin and more. * 2 NCA Titles (Level 2 & 3) * Over 100 National Titles DANCE Last year our Hip Hop program was the MOST decorated program in the All-Star WORLD! * 5 USASF World Championships * 3 USASF Silver Medalist * 5 USASF Bronze Medalist * 12 NDA Titles MISSION STATEMENT I can do ALL things through CHRIST who gives me STRENGTH. 2 CONTACT INFO 1600 Idlewilde Dr Midland TX Office Hours: 4:00-8:30pm BILLING INFO Statements are generated on the 25th of each month. You will be sent a statement for the balances due for the upcoming month via for your convenience. We will need all current and accurate addresses on file. It is your responsibility to check your statement for any mistakes that could have occurred during the billing process. Your addresses are private and are only used for billing and communication processes. LATE FEES We will be running credit cards (VISA, MC & DISCOVER ONLY) for all All-Star Athletes. All balances will be drafted on the 1 st of every month. It is the parent s responsibility to make sure that we have the correct and most accurate information on file. If your balance for the month is not paid by the 10 th, your child will have to sit out of practice until the balance is paid. Unfortunately there will also be a $35 late fee for all insufficient card numbers, insufficient funds, card cancellations & declines. All financial obligations must be paid by April 2018; failure to do so will result in being turned over to collections. Parents please understand that it is very uncomfortable for the Alpha & J2 Productions staff to contact you regarding your unpaid balances. We understand that unfortunate financial situations happen, however, we have deadlines to collect balances for a reason and that is why we have to enforce rules to be fair to everyone involved. Please understand that if your financial obligation gets out of hand we will have to collect your balances before your child may return to practice.

3 PARENT MEETING The parent meeting (all returning parents and NEW parents) will be held Wednesday, April 19 at 6:00 pm to go over this year s packet of information. All parents are encouraged to attend this meeting in order to get the most accurate information and to clear up any questions you may have about this year! SIGN-UPS Sign-ups for the season will start on Monday, April 3rd and run through April 30. Please be prepared to pay your $40 membership fee and your athlete(s) USASF membership fee of $35 when you sign up. All tuition financial obligations will start May 1, In order for your child to be evaluated you must sign up first! You will need to print off this packet from our website, or pick up one from the office and fill out all required information & turn it in to the front office.

4 EVALUATIONS Evaluations will begin Tuesday, May 2 and run through Thursday, May 11, After evaluations, our staff will work together to come up with teams based on age, skill ability, and potential for a successful competition season. It is mandatory for each child to attend evaluation dates (listed below) to be placed on a team. Once teams are determined, we will send out an and post them to our website Sunday, May 14 by 6:00 pm. Practice times will also be posted at this time. Team practices will begin Monday, May 15, DANCE Tuesday, May 2 & 9 The schedule of evaluations are listed below: Ages 4 to 9: 5:30-6:30pm Ages 10 to12: 6:30-7:30pm Ages 13-19: 7:30-8:30pm CHEER Thursday, May 4 & 11 The schedule of evaluations are listed below: Ages 4 to 8: 5:30-6:30pm Ages 9 to11: 6:30-7:30pm Ages 12 to18: 7:30-8:30pm FLYER Wednesday, May 3 & 10 The schedule of evaluations are listed below: Ages 4 to 8: 5:30-6:30pm Ages 9 to11: 6:30-7:30pm Ages 12-18: 7:30-8:30pm *Age determined by the child s age as of August 31, 2017

5 PROGRAM COSTS We have an inclusive monthly price that covers most of the expenses that an athlete would normally incur throughout the year. This system makes financial planning much easier for our customers. You will know exactly how much will be charged, and when it will happen. Please carefully consider the financial commitment involved and read this form in its entirety. MEMBERSHIP FEES All students must be registered through the office with the appropriate fees paid to participate in All-Stars, developmental classes, as well as private lessons. Your annual membership fee to Alpha Cheer & Dance Co./J2 Productions is nonrefundable and is due annually. ALL-STAR TUMBLING All-Star Cheer (prep & level teams) A 1-hour/1 time per week tumbling class will be included in the monthly tuition. Days & times are TBA. Classes to be held September through April. TRICKING CLASSES All-Star Hip-Hop (prep & level teams) A 1-hour/1 time per week trick class will be included in the monthly tuition. Days & times are TBA. Classes to be held September through April.

6 TUITION RATES May May 2018 $40 Membership fee & $35 USASF Membership fee May April 2018 ALL-Star CHEER Levels athletes will pay an all-inclusive price of $250 tuition ALL-Star CHEER Prep athletes will pay an all-inclusive price of $200 tuition (Includes team tuition, competition fees, choreography/music fees, practice clothing, coach s fees) ALL-Star HIP HOP athletes will pay an all-inclusive price of $300 tuition ALL-Star HIP HOP Prep athletes will pay an all-inclusive price of $200 tuition (Including team tuition, competition fees, choreography/music fees, practice clothing, team uniform, coach s fees) Tuition does not fluctuate based on the number of duration of practices in any month. Monthly tuition is due beginning May 2017 and will run through April If any athlete joins after the initial start of the season, additional fees will be collected to cover competition fees, practices, choreography/music fees, and uniform costs. Cheer - This rate includes your monthly team tuition, team tumbling class, competition fees, choreography/music fees, coach s fees & practice clothing. Tuition does not include Summer Skills Camp, your annual membership, your USASF fee, uniform, team warm-ups, competition hair bow, shoes, competition make-up or travel costs. Hip-Hop - This rate includes your monthly team tuition, team tricking class, competition fees, choreography/music fees, practice clothing, coach s fees & hip-hop uniform. This does not include Summer Skills Camp, your annual membership your USASF fee, team warm-ups, shoes, competition make-up, or travel costs. Discounts: All siblings will receive a $20 discount on tuition. Any additional siblings will receive a $40 discount on tuition! All athletes who choose to do both competitive cheer and hip-hop will receive a $50 discount off of their second tuition fee.

8 USASF ANNUAL MEMBERSHIP FEE TENTATIVE COMPETITION SCHEDULE Benefit of Athlete Membership: Alpha Cheer & Dance Co/J2Productions requires all All-Star cheer and dance athletes to have a current USASF athlete membership in order to compete for the season. Below is more information on what the USASF membership does for your athlete! Athlete Identification (All athletes identified with a membership number and verification of date of birth) Additional Excess Accident Medical Insurance at all USASF Sanctioned Events Additional Catastrophic Accident Insurance at all USASF Sanctioned Events Membership Fees & Terms: The athlete membership fee is $35 per membership term The membership term begins August 1 and ends on July 31 Membership fees are paid on an annual basis, beginning August 1 Proof of Age and Identity: An image of the original birth certificate is required to be uploaded into the athlete identification system by the gym owner or individual parent. A current photo will need to be uploaded by parent or gym owner. Any athlete found to have provided an incorrect birth date and/or birth certificate that has been falsified or altered in any manner may subject to disciplinary action including but not limited to restriction of membership and eligibility. Athlete Membership: Belongs to the athlete (regardless of who registers the athlete and pays for membership) Allows participation in USASF Member Event Producer Sanctioned Events Is a way to identify the athletes in all star cheer and dance Is a way to verify age of participating athletes Is a way to legitimize all star cheer and dance in the sports world We will collect each athlete s USASF membership fee of $35 at the time of All-Star signups, as well as a copy of each athlete s birth certificate.

10 TRAVEL EXPENSES Hotel and travel accommodations/expenses will be the sole responsibility of the parent/guardian. All competition vendors REQUIRE you to stay at hotels that they have selected for their competition under the Varsity Connections Housing Requirement. Our Staff helps with these accommodations by providing you with a list of room blocks for each specific competition. CHOREOGRAPHY CAMPS Choreography camps are MANDATORY! We will know the exact dates and times shortly after the teams are selected. All Cheer & Dance teams will learn their choreography in June/August TBA. 11 STAFF Our staff offers quality instruction, ensuring proper coaching, as well as safety in technique and skills as your athlete progresses competitively while performing their All-Star Cheer and/or Dance routine. The quality of our product is important to us; the staff here at Alpha Cheer & Dance Co and Jordan Johnson Productions are all USASF Certified in cheer, dance or both, making your All-Star experience enjoyable, worthwhile and successful! COMMUNICATION Our staff is always willing to talk to the parents about any information regarding ALPHA & J2 PRODUCTIONS. If you have any questions or would like to speak with a coach, please do so at the appropriate time (before or after practice). If you are not able to speak with them at that time, please do not contact them directly. You may call or the office and let us know with whom you need to speak with and we will have them get back with you as soon as possible!

In addition to giving my full consent for my child s participation, I do hereby waive, release and hold harmless Alpha Cheer & Dance Co./Jordan Johnson Productions, it s coaches, supervisors, and representatives for any injury that may be suffered by my child in the normal course of participation in cheerleading/dance and the activities incidental thereto, whether the result of negligence or any other cause. In case of an accident, I agree to allow Alpha Cheer & Dance, Co./Jordan Johnson Productions to call 911-EMS for my child, at the expense of parent of guardian. 15 ALPHA CHEER & DANCE CO./JORDAN JOHNSON PRODUCTIONS CHEER/DANCE COMPANY MEMBER CODE OF CONDUCT All Company members of the Alpha Cheer & Dance Co/Jordan Johnson Productions Dance Company is expected to adhere to the Alpha/JJP Code of Conduct as outlined below. This list is not meant to be all-inclusive. A member who does not adhere to the Code of Conduct will be subject to suspension or dismissal from the company at the discretion of Alpha/JJP. Each Alpha Cheer & Dane Co./Jordan Johnson Productions Dance Company member, Parent, and Guardian: 1. Must take pride in being a member of the Company, always display team spirit, and be a positive representative of the Alpha Cheer & Dance Co./Jordan Johnson Productions Dance Company. 2. Must display good sportsmanship in class, rehearsals, practices, performances, and competitions. 3. Will follow all rules and regulations of the Alpha Cheer & Dance Co/Jordan Johnson Productions Dance Company Contract including but not limited to Class, Practice, Performance & Competition, Attendance, and Uniform procedures. 4. Will always give 110% during all classes, practices, and performances. 5. Will seek additional cheer/dance instruction on their own time to master the required Company routines. 6. Athletes and Parents/Guardians must be respectful of Cheer/Dance Company Director; Cheer/Dance Company Assistants; Staff; Company mates; chaperones; and all other faculty/administrators/personnel associated with Alpha Cheer & Dance Co./Jordan Johnson Productions Dance Company. 7. Must display a good general positive attitude regarding the Alpha Cheer & Dance Co./Dance Company, Jordan Johnson Productions Dance Company, and others. Adversarial, argumentative behavior and comments will not be tolerated. 8. When wearing any Alpha Cheer & Dance Co/Jordan Johnson Productions Dance Company apparel cheer/dancer will act with class and dignity so as not to discredit himself/herself, the Cheer/Dance Company, or Alpha Cheer & Dance Co./Jordan Johnson Productions Dance Company. 9. Will not use cell phones during any class, practice, or performance without the direct permission from the Cheer/Dance Director. 10. Will treat all Alpha Cheer & Dance Co./Jordan Johnson Productions Dance Company staff, and chaperones with respect and follow any given instructions without comment. 11. Parents/Guardians and Cheer/Dance Company Members will be responsible for checking our website and , and staying informed of all Cheer/Dance Company activities and deadlines. 12. Will respect to all Cheer/Dance Company, school, and other Company member property. 13. Must not engage in any unfavorable, questionable, or illegal manner through electronic media (i.e. websites, personal home pages such as My Space or Facebook, blogs, text messages, chat rooms, or similar websites) or use electronic communication devices (i.e. cell phones, camera phones, digital photos) in such a way as to bring discredit, dishonor, or disgrace on the Alpha Cheer & Dance Co./Jordan Johnson Productions Dance Company or Company members - including yourself - or any other Alpha Cheer & Dance Co./Jordan Johnson Productions Dance Company members. 14. Will not engage in any vulgar or inappropriate conduct, or use vulgar language, while representing Alpha Cheer & Dance Co./Jordan Johnson Productions Dance Company while in uniform or Company jacket. This includes class, practices, performances, events, and competitions - on or off Alpha/JJP property with or without the Company. 15. Will never speak disrespectfully to or about any Company members. 16. Will not engage in any fraternization with any other members of the Company. Fraternization is defined as pursuing to date, dating, or having romantic relationships with other Company members.
