Hazards must be eliminated or the uncontrolled risk level reduced through substitution, isolation, engineering or a design change. Ensure that the highest possible controls have been applied (e.g., elimination, substitution, isolation, engineering, several administrative controls and PPE). TASK MUST BE ED The Construction Manager, Site Manager, Superintendent or equivalent MUST review controls and ensure they are appropriate and effective before the task can start. Each step should accomplish some major task and be in a logical sequence There is a risk that.. because.. STOP - No. of people to do the task - Licenses, Qual s, Permits - Training requirements - Approvals, Plans & Permits - Environmental controls - Lift Studies / Calculations - Safety Equipment - PPE - MSDS/Hazsub/DG s STOP Wherever possible list who specifically (by name) and when this needs to be done. 1 Prior to unloading equipment on site. Site visit to inspect the safe unloading of the directional drill from the float and the general housekeeping of the equipment over the worksite during the project Confliction with traffic when parking vehicles, chance of being hit by through traffic. Park vehicles well off the road, use flashing lights/ hazard lights and be aware of through traffic on the road. If we have to get out on the road, avoid having to cross the road to inspect the worksite. 3 WORK SEQUENCE BASIC JOB STEPS RISK & HAZARD / ISSUE DESCRIPTION UNCONTROLLED CONTROLS CONTROLLED WHO WILL DO THIS? 3 Drill set up and drill the pilot bore. Install pilot head to drill, peg out drill line, enter inlet pit excavation to adjust sonde housing height, connect hoses, start mixers. Check all underground services have been exposed and identified, begin to drill the pilot bore. Heavy pilot housing, over 25kg, manual handling injury. When pegging out string line for pilot bore, risk of being struck by traffic if marking out crossing in a road. Entering an open excavation over 1.5m deep may result in being buried alive, fall from height. Connecting of the hoses may inflict injury to hands as camlocks may have burrs on them. The Hatz diesel engines are noisy over 95 decibels and could cause loss of hearing. Unidentified services especially underground power can cause death. When drilling is underway the drill has moving rods which can entangle and crush causing death. We need 2 people to lift any tooling over 15kg. Our gloves should be on and use standard manual handling techniques. When marking out bore line make sure either traffic management or another team member is spotting the safety from through traffic. If the entry hole is over 1.5m. Do Not enter if the excavation is not shielded or benched well. A ladder should be installed if over 1.5 m. PPE. gloves should be worn when connecting hoses. Our headsets should be worn and in ON position when starting the Hatz engines. Only trained personal should be in the facinity of the working drill. PPE should be worn while operating. 4 WORK SEQUENCE BASIC JOB STEPS RISK & HAZARD / ISSUE DESCRIPTION UNCONTROLLED CONTROLS CONTROLLED WHO WILL DO THIS? 4 Upsize the hole Install the reamer to open up the hole and start reaming. Evacuate drill slurry from the exit hole Excavate solid clay from the exit hole Reamers are heavy and can crush you and cause a major manual handling injury and death when spinning in the exit hole Slurry is slippery when spilt. If not properly cleaned up we may slip and worse fall from height if the exit hole is greater than 1.5m. The suction hose could cause injury if allowed to get in contact with skin or clothing Using an excavator to dig clay from the exit hole may cause serious bodily injury or death as the result of hitting personal with a moving bucket or boom Reamers are to be lifted by 2 people, where heavier than 30kg use lifting slings with a mechanical aid. Maintain good communications with internal radios between driller installing reamer and drill operator. The remote lockout should be in possession of the driller installing the reamer. Install tooling by hand, a spotter to be outside the hole. Drillers should exit the hole as soon as the reamer is installed. Take time to clean up all slurry even if it seems minimal. Wear PPE and never put hand over the end of the suction pipe. Start All Stand well clear of the swing area of the excavator, maintain communications and eye contact with the operator PPE # Example Transport 50kg reo bar to work area. 5 WORK SEQUENCE BASIC JOB STEPS RISK & HAZARD / ISSUE DESCRIPTION UNCONTROLLED CONTROLS CONTROLLED WHO WILL DO THIS? 5 Pull the pipe in the bore hole Bring the pipe into place and connect Vaccum drill slurry Disconnect the pipe When bringing the pipe into place the pipe may whip with the aid of a mechanical aid and cause injury to personal, public and property A moving pipe in the exit hole prior to connection could crush hands and limbs. A slippery, wet pipe in the entry hole could cause slips and other minor lost time injury Have a discussion with all about the most safest way to bring the pipe to the exit hole. Have as many personal as eyes as possible Where possible, push reamer on top of ground and make the connection of the pipe to the reamer on top of the ground After the bore is through, where possible disconnect the installed pipeline on top of the ground.
