Hazard Assessment and Control A hazard assessment is a foundation of an effective safety plan. The hazard assessment identifies hazards and the associated controls. A hazard assessment is required before any field activity and should be reviewed whenever there is a change or new process. The hazard assessment must be reviewed periodically. The hazard assessment is a requirement of the Alberta Occupational Health and Safety Act and of University policies (reference reference). A hazard assessment is often developed as a table/matrix and includes a risk evaluation process. (see examples here and here). All other aspects of your field activity plan will naturally flow from this hazard assessment (emergency response plan, comms plan, admin and insurance plans, etc.) See the following pages for the hazard assessment for this project.

2 Name: King Lab - Remote Field Work Hazard Assessment Date Created: Jun 26, 2018 Description: Hazard assessment for a summer research project in the NWT involving canoe-based fieldwork where we are dropped off by floatplane in a location, and then transit between field sites using canoes. Fieldwork will take place over 5 weeks. Last Published: Jun 26, 2018 Task Air Travel Both commercial flights between YEG and YZF as well as small floatplane travel between YZF and field sites. Remote Location Field work will be conducted in a remote location away from major centers (and inaccessible other than flying). This means that any major incidents will require several hours if not a full day for evacuation. Canoe Travel Movement between sampling locations will be via canoe watercraft. Both water travel and portaging is included under this task. Emergency Escalation Protocol if no Check-in: Grace period before starting emergency protocol: No message received from both SPOT and/or InReach for two hours after check-in (with no additional contact via satellite phone). Steps When to call Who to call 1. If after grace period, still no check-in, send message to: Name of Supervisor: Greg King 24/7 Contact Address (InReach device): Call Satellite Phone: XXX-XXXXX 2. 7 Potential Emergencies/hazards: Procedures for dealing with Potential Emergencies: Identification of, location of and operational procedures for emergency equipment: Emergency response training requirements: Location and use of emergency facilities: Fire protection requirements: Alarm and emergency communication requirements: First aid services required: Procedures for rescue and evacuation: -watercraft activities (canoe movement between field sites) -wildlife encounters (bears, caribou) -injury while accessing field sites -illness while in field / away from home Our field team is a group of 6 individuals working in remote locations. At most times we will be in one group or perhaps 2 groups when accessing field sites (always camping together). Our procedures would depend on the severity of the injury and may involve arranging for a helicopter/floatplane pick-up. All members of our expedition have wilderness first-aid training to ensure proper care in case of delay prior to evacuation. In case of severe injury requiring evacuation we would contact local RCMP/hospital/Air Tindi to arrange this. When calling an evacuation emergency, procedure is to give location co-ordinates first, in case sat. phone contact is lost. Comprehensive first aid kits will be carried by researchers. A smaller kit for field work and a larger kit is kept in camp at all times. Bear spray will be carried on each person at all times in the field. Each group will also have access to wildlife deterrents including bear bangers and air horns. Each group of researchers will also always be carrying a communication device to reach other groups (i.e. radios if in different locations) and to communicate with contacts not in the field (i.e. SPOT, InReach, or SatPhone). All team members have wilderness first-aid training. Both UofA members have Bear-Aware training and canoe paddling training/experience. Yellowknife Hospital: (emergencies) Wekweètì Nursing Station: NWT wildfire map will be monitored for fire activity and will be communicated to us via InReach device or Sat Phone. Field crew will carry a satellite phone, InReach and SPOT device. In event of emergency, Yellowknife hospital. If required, contact will be made with the local RCMP detachment in Yellowknife (activating SPOT rescue will also lead to RCMP contact). If not possible to reach the RCMP we will make efforts to directly contact several potential flight/helicopter companies for emergency evacuation. The following contacts have been programmed into our satellite phone: Air Tindi Yellowknife - 24-hr dispatch (867) Yellowknife Float Base - (867) Canadian Helicopters Yellowknife Designated rescue and evacuation workers: All staff have adequate first aid training and have been informed of evacuation procedures Page 4, Field Activities Plan Template, Version date: February, 2018
