What to stock? Formthotics require minimal storage space. By holding a few models in a range of sizes, clinicians have found they are able to fit most of their patients. During a patient visit they can be provided immediately to give improvement in comfort and relief for pain. Formthotics Original models fit the majority of patients for a standard volume shoe (eg. a casual or running shoe). When deciding which models to stock, consider the majority of patients who are seen in the practice. The top selling Formthotics Medical Original models are listed below: The top Formthotics Medical models to stock Original Dual Medium Alternative: Original Dual Medium (Perforated) The number one, most popular model worldwide because of its versatility and excellent balance between comfort and support The dual layer of foam has a softer, cushioning top layer and a medium hardness base for better compliance and comfort Suitable for conditions where additional cushioning is required without compromising stability or functionality Same foam density layers, support and functionality as the Original Dual Medium but a different color Forefoot perforations for increased airflow to help keep feet cool and dry Provides optimum patient comfort Original Single Medium Alternative: Original Single Firm Does not have the additional cushioning layer of Original Dual Medium forming a closer contact with the sole of the foot for proprioception For shoes that are a closer fit. There is less bulk than the Original Dual Medium Suitable for effective use in a broad range of clinical applications where management of patient biomechanics and function will improve outcomes A supportive intermediate density foam but a slightly harder density than the medium (blue) density foam Color will blend seamlessly into most business, hospitality, leisure and darker sports footwear Lightweight and durable enough to wear everyday

Original ShockStop What the Formthotics Medical colors mean Generally, the different base colors relate to different Formax foam densities. Foams are listed from softest to hardest. For dual density models, the top color foams vary in color but are all the same density. Beige X Soft density Burgundy Soft density Revolutionary ShockStop foam protects and reduces impact, providing superior shock absorption Highly beneficial for at risk patients and those who require extra care; history of arch blistering, elderly, diabetes, arthritis, atrophy or boney prominences Original Dual Hard Blue Medium density Black Firm density Red Hard density Green Medium density, for models with different shapes than the Original High density semi-rigid base with a 60% softer top layer Excellent biomechanical support and durability with enhanced comfort and cushioning from the soft top layer Size charts Adult range size chart Sizes XXS XS S M L XL XXL XXXL UK Euro US (Women) US (Men) Mondo Pt Junior size chart Sizes J1 J2 J3 J4 UK Euro US Foot length (cm) Approx age (girls) Approx age (boys) Youth size chart Sizes Y1 Y2 Y3 UK Euro US Mondo Pt

Formthotics versatility A wide variety of shapes and sizes If you have a one off need for a patient and don t want to stock the model, check with your distributor, you might be able to order in time for the next appointment with your patient. Examples of Formthotics applications: different models in 23 the medical range At risk feet (diabetes, arthritis, neuropathy or fat pad atrophy) High risk feet need accommodative, full contact and cushioning qualities for orthoses. Models: Original Single Soft, Original Single X-Soft, Original ShockStop Excessive pronation in adults For those who have pain related to over pronation, requiring support and comfort. The shape choice of Formthotics also depends on the shape of the footwear (eg. a narrow shape for soccer boots). Models: Consider a medium to hard density eg. Original Single Medium or Original Dual Medium in either medium, firm or hard densities. Note: flat feet with excessive pronation may require a low profile model. Excessive supination/high arches (more rigid foot type) Excessively supinated, rigid foot types generally need cushioning and full contact support to increase the weight bearing surface area and reduce joint load. Models: Original Single Soft, Original Single X-Soft Tight toe boxes Some shoes are too tight at the forefoot or toe area and simply cannot fit full length orthoses. ¾ models are available. Models: Comfort Single Medium, ¾ Dual Medium Wide feet If the patient s feet are wider than the Original shape Formthotics, the Widefit shape is the alternative. It is also great for wide fitting shoes such as waterproof boots or work boots. Model: Widefit Dual Medium Low volume shoes For close fitting shoes including business and casual shoes. A more pointed toe area is able to fit similarly shaped shoes. Models: Low Volume Single Medium, Low Volume Single Firm Soccer boots A narrow shape to fit into narrow, close fitting soccer boots. Models: Low Profile Dual Medium, Low Profile Dual Hard Cyclists An exclusive performance model available for medical professionals only for the competitive cyclist. Model: Cycle Single Firm Shock reduction The patient is often in an environment with hard concrete floors, or often pounding the pavements. Shock reduction is a common feature sought. Models: Original ShockStop is the most shock absorbing, or dual density models Children Two Formthotics Medical ranges are available for children: Junior (approx. 3 9yrs) and Youth (approx. 4 to 14yrs). These offer support, full contact with feet, and cushioning for Sever s disease type heel pain. Junior: Soft, flexible ¾ length foot orthoses, specifically designed for younger children. Available in 4 different sizes. Youth: Available in 3 different sizes and foam densities. Models: Youth Original Single X Soft, Youth Original Dual Medium, Youth Original Single Medium Formthotics Additions A comprehensive range of forefoot and rearfoot modifications designed to fit to Formthotics. These include rearfoot wedges, heel raises, arch pads and metatarsal domes. Available in 5 pair packs in S, M and L sizes. Please note: These model options are a guide only. A more detailed recommendation would require information on a case by case basis, including patient demographics, pathology, and differential diagnosis. More information: formthotics.com/medical/products Formthotics Heating Machine available Push one button to automatically and safely heat Formthotics to the optimal thermoformable temperature inside your patient s shoes in just 3 minutes.

The Formthotics difference Formthotics are made by Foot Science International in New Zealand, using a unique design and manufacturing process that ensures the ultimate customizable orthotic. Support Heel cup The deep heel cup provides increased support around the heel, assisting with shock absorption and minimizing ground reaction force at heel strike. Functional support The graded medial support ensures the foot is stabilized by providing longitudinal, lateral and transverse arch support. Foam features Antimicrobial Formthotics contain Ultra-Fresh, which inhibits the growth of bacteria and fungi that create foot odor and staining, keeping your feet healthy. Foam types Formax Foot Science International s own foam is made from a range of high grade closed cell foams, designed to provide the most flexible, lightweight and supportive orthotic possible. Thermoformable Formthotics are thermoformed (heat molded) at relatively low temperatures and retain the molded shape. Molding an orthotic to the shape of your foot provides a truly customized fit. ShockStop Some special Formthotics models are made from this unique foam produced by Foot Science International. Made from a hybrid of EVA and polymer foams to provide maximum shock absorption and cushioning, this minimizes the impact on muscles, joints and tendons and eases recovery time after performance. Construction 3D milled Formthotics are 3D milled from a block of foam, ensuring they provide a consistent spread of support across the skeletal system of your foot and any pressure points.

Formthotics Custom Medical Orthotics are prefabricated customizable orthotics for medical professionals. Patients love wearing Formthotics They are lightweight and fully flexible, providing comfortable dynamic support they heat mold to the foot, supporting every moment during gait. They re insulating, waterproof, easily cleaned and hypo-allergenic. Around the world the pain and discomfort in the lower extremity in millions of patients has been eased with Formthotics, aided by good patient compliance. Don t wait until the next appointment seek a patient s immediate relief A patient does not need to wait until their next appointment to have orthotics fitted or spend time considering an expensive fully custom lab produced orthotic. Formthotics can be quickly fitted within 15 minutes and a clinician can receive immediate patient feedback. Why are Formthotics Custom Medical Orthotics? Formthotics heat mold to the patient s individual shoes and feet Formthotics are thermoformable. For the fastest adoption and optimal fit to the feet and shoes, we recommend Formthotics are heated with the Formthotics Heating Machine prior to wearing for the first time or after modification. For small volumes of Formthotics, a hairdryer may be used instead (instructions ions available). However, Formthotics still mold to the feet for a custom fit just by using the natural body heat of the feet and many clinicians find this acceptable. Formthotics are very easily customized Quickly and easily modify the underside of Formthotics with rearfoot and/or forefoot modifications to change support and therefore, function as the patient requires. There is a range of Formthotics Additions available. Formthotics can be used in a variety of ways Formthotics can be used in many types of shoes, enabling the clinician to customize the fit to the activity for that shoe. Saving the patient changing orthoses between shoes ensures the patient is wearing supportive devices for the greatest amount of time possible, not just when, for example, running or playing football. Formthotics They just work Clinicians have confidence they are using a product proven in research and worn by millions of people worldwide over the past 35 years. With the Formthotics Medical System s 6 Tests, 6 Steps to assist the assessment, selection and fitting of Formthotics, medical professionals from entry to advanced levels trust Formthotics in their practice. Formthotics can be used for short term relief from an injury in many cases without the patient needing to purchase laboratory-made custom orthoses. Formthotics can be used for pathologies that require longer term support and comfort. Formthotics can be used to test whether a change in support shows improvement before possibly providing a laboratory-made custom device later. These make me a hero in my practice. Formthotics are the clinicians tool They have told us why: Effective Reliable Fast-to-fit Proven Formthotics are versatile, assisting treatment of a variety of lower extremity problems. Used by millions across the world over the past 35 years, Formthotics are a trusted, consistent high quality device. Formthotics are quick and easy to fit. In one visit, the patient can be fitted and is able to assess their effectiveness, providing immediate feedback. Independent, peer reviewed research has proven Formthotics reduce pain, treat and prevent injuries and improve patient comfort. 8 Formthotics Medical System Get the best results for your patient. A patient centric process of assessment, utilizing your expertise to quickly and effectively select, fit and adjust Formthotics. The system is a comprehensive clinical method to improve lower limb function with custom-fitted Formthotics and includes: 6 Tests Objective clinical tests 6 Steps Select, fit and adjust Formthotics Custom Medical Orthotics Test 1: Foot morphology and motion test Relates to the general principles of open kinetic chain foot biomechanics; joint position and passive range of motion. Test 2: Alignment test The foot and lower limb skeletal alignment is examined in stance to determine if and/or how it compensates when weight bearing. Test 3: Windlass (Jack's) test Assessment of the windlass mechanism using the Jack s test. Looks at the functional dorsiflexion range of motion availability of the hallux associated with heel lift and effective propulsion. Test 4: Supination resistance test Relates to the position of weight bearing axis of the subtalar joint. To assess, the clinician manually supinates the foot and grades the force required. Test 5: Proximal control knee bend test Assesses lumbopelvic postural stability and proximal control systems. The clinician observes the stability of balance, duration of balance, and the loss of balance (medial or lateral). Test 6: Neuromotor balance test Relates to propulsion. The clinician observes the relative ease of rising onto the toes, stability and the tendency of inversion instability. There are 3 options depending on the patient s strength and balance ability and what is being assessed. Option 1: Open/closed eye, single limb balance test Option 2: Star excursion balance test Option 3: Forefoot stability test Step 1. Formthotics selection and fitting Select the Formthotics model and size - consider foam density and shoe shape and then fit Formthotics into the patient s shoes. Step 2. Patient adjustment period Allow a 3 to 7 day break-in period for the patient to adapt to Formthotics. Step 3. Rear foot modification The rearfoot is adjusted by functional modification by way of applying a wedge to the underside of Formthotics. Step 4. Forefoot modification The forefoot is adjusted by functional posting. The improvement of function should be verified by the patient. Step 5. In-shoe testing Check the patient s stance and gait to assess the functional effects of Formthotics while the patient is wearing them in their shoes. Step 6. Follow up and ongoing adjustment Formthotics should be checked and modified at 3 to 6 month intervals especially in the initial period.
