2 TRAINING OBJECTIVE To ensure that all VIH Helicopters Ltd. Maintenance personnel and Pilots are aware of the Canada Labor Code requirement for Fall protection when working above 2.4 meters. Also, to ensure that t all personnel who may be working on the KA-32A11BC above 2.4 meters are properly trained in the use of fall restraint equipment in accordance with the VIH policy.

3 WORKING AT HEIGHT Policy To ensure that t workers are in compliance with the Canada Labour Code while working above 2.4 meters in height on the Kamov KA-32A11BC helicopter. Purpose - To improve on the Three Points of Contact t procedure presently in effect while working at height on the Kamov helicopter, and to ensure that workers are properly protected against falls when working above 2.4 meters. Personal Protective Equipment Consists of the Following American National Standards Institute (ANSI) approved equipment. 1. ANSI Compliant Harness 2. ANSI Compliant D ring extension strap and Carabiner. 3. ANSI Compliant Personal fall limiter 4. ANSI Compliant Nylon web anchor straps The following Procedure is in accordance with the VIH policy

4 Personal Protective Equipment - Safety Harness Safety Harness, ANSI, Z359.1 compliant Shoulder and leg straps are adjustable. The main D ring for attachment is on the back. There are 2 D rings on each side of the harness in the area of your hips. There is also a ring on the chest. Each Aircraft kit will have two harness Note: The chest and hip rings are intended for use only during repositioning while working at height.

5 Personal Protective Equipment Extension Strap D ring Extension Strap, ANSI, Z359.1 compliant A twist lock aluminum Carabiner is used in conjunction with the strap to connect the D ring end of the strap to one of the D rings of the harness. Each Aircraft kit will have two extension straps.

6 Personal Protective Equipment Fall Limiter Upper Snap hook Personal Fall Limiter, ANSI, A compliant with self retracting strap and locking snap hooks at both ends. Each Aircraft kit will have two Personal Fall Limiters.

7 Personal Protective Equipment Anchor Strap Nylon Web Anchor Straps, ANSI, Z359.1 compliant. The Anchor Strap shown in the adjacent photo has the protective Hi-Vis cover. The identification tag of each strap is covered with flexible clear vinyl to verify manufacture date etc. A quantity of two each, 2, 3, 4 and 5 foot Straps will be in each kit.

8 Personal Protective Equipment Protective Storage Case The adjacent photo shows the open storage case indicating all of the equipment in it s proper place. NEXT, WE ARE ON TO THE PROCEDURE

9 PROCEDURE Caution: Prior to starting work at height where Nylon Web Anchor Straps will be attached to the Rotor Mast of the helicopter, install the Red Maintenance sock on the pilots cyclic.

10 PROCEDURE, HARNESS WITH FALL LIMITER Chest Ring Hip Ring Prior to putting the harness on, attach the upper swivel hook of the Fall Limiter to the D ring on the back of the harness. Put the harness on, and adjust the straps to fit. Note the location of the Chest ring, which is height adjustable for personal preference. Note the location of side or hip D rings. The Extension Strap may be attached to either chest or hip ring for repositioning.

11 PROCEDURE, CONTINUED Fall Limiter upper hook Note the Fall Limiter attached to the D ring on the back. The Carabiner of the extension strap is attached to the back as well. You may need the assistance of a co-worker to help remove or install the Carabiner of the Extension strap to your back. The extension strap may be attached to the chest, or either hip D ring while repositioning i while working at height. Place two Anchor Straps in your pocket, or tuck them in to the straps of the harness. This will allow your hands to be free while climbing on to the helicopter.

12 PROCEDURE, CONTINUED If you are going to be working in an area and not be required to move to much, such as installing blades. You may use just the extension strap attached to the D ring on your back.

13 PROCEDURE, CONTINUED An appropriately Rated ladder with enough height must be used to safely climb up, and step on to the top of either the right or left hand # 5 tank platform area. Alternatively, the fuselage steps and landing gear struts t may used to gain access to the top of either the right or left hand # 5 tank. The Three Points of Contact procedure must be used at all times when using either the ladder or fuselage steps.

14 PROCEDURE, CONTINUED Once on top of the # 5 tank, and while maintaining the Three Points of Contact open the required Nacelle cowling.

15 PROCEDURE, CONTINUED In preparation to step up on to the top of the Nacelle, and as shown below, loop and choke one of the Lower Slider arms with a Nylon Web Anchor Strap.

16 PROCEDURE, CONTINUED Prior to climbing on to the upper Nacelle. Using gyour personal preference of either of the hip D rings, or Chest ring of the harness, attach the Carabiner of the Extension strap. Attach the Extension Strap locking snap hook to the Nylon Anchor strap that is choked to the Arm of the Lower Slider.

17 PROCEDURE, CONTINUED Once you are secured to the arm of the lower slider, use the Airframe step attached to the Gearbox aft firewall, and step up on top of the Nacelle. Once you are on top of the Nacelle, and as shown in the adjacent photo, loop and choke another Nylon Web Anchor Strap around one of the outboard feathering hinge sleeves of the Upper hub, and connect with the Fall Limiter locking snap hook.

18 PROCEDURE, CONTINUED You may now remove the D - ring extension strap from the Lower slider arm to allow your free movement to carry out the required Rotor Mast maintenance.

19 PROCEDURE, CONTINUED There are numerous options for connecting while repositioning Here are a couple of more photos showing a worker at height. NOW IT S TIME TO GET DOWN

20 PROCEDURE REMOVING THE EQUIPMENT AND DESCENDING The procedure for getting back down from the Helicopter is simply the reverse of climbing up. First you re-attach the Extension strap to the Anchor Strap on the Arm of the Lower Slider. Next, you remove the locking snap ring of the Fall Limiter that was attached to the Sleeve of the Upper Hub, and connect it to one of the harness hip D rings. CAUTION: Next, remove the Nylon Anchor strap from the Sleeve of the Upper Hub. You may now step down to the Nacelle cowling, and on to # 5 tank platform and disconnect Extension Strap from the Anchor Strap. CAUTION: Next, remove the Nylon Anchor strap from the Arm of the Lower Slider.

21 PROCEDURE FINAL CHECK Caution: Prior to climbing down from the g aircraft, ensure that the Nylon Web Anchor Straps are removed from the Rotor Mast.

22 PROCEDURE - FINAL CHECK Caution: Double check that all of the Fall Restraint Equipment is in it s proper place within the Protective Case.
