5 THE RESULTS SPEAK FOR THEMSELVES I recently met Kyle during the Memorial Tournament and found him to be a solid young man, with a great desire to work hard and excel at whatever he does a real overachiever I know when someone has a desire to be good and work hard, they usually accomplish what they are set out to do. -Jack Nicklaus was the inaugural Elite Junior Champions Academy, and the play and growth of our juniors were terrific. OUR TECHNOLOGY Trackman Launch Monitor used for every shot Used by more than 100 players on the PGA TOUR, Trackman is the pioneer in launch monitor technology. It is used by the PGA Tour, R&A, USGA, and all the major networks for all ball and swing data when watching golf. Once used as a missile defense system, Trackman is now capable of tracking ones golf shots. 7 Program Details Perfect Proper Practice for Faster Results Good feelings do not create proper swings; proper swings create proper feelings. The Elite Junior Champions Academy emphasizes high-quality practice rather than high-quantity practice. Rather than mindless hours of raking and hitting on the range, juniors will practice like the pros, with each shot having a purpose and a consequence. Juniors will learn the difference between block and random practice and when to employ each concept to maximize success. In addition, instead of solely relying on constant coaching feedback during supervised practice sessions and hoping it translates to correct independent practice, drills and equipment are used so the player immediately knows if they made a proper swing based on the success of the drill. For example, if trying to correct an over-the-top swing, players may be instructed to avoid hitting a noodle or cone in their swing. If they miss the noodle the swing was not over-the-top; if they hit the noodle it was. The immediate feedback of the drills allow the player to confidently practice independently at a high level. Course Management Seminars to Help Think like a Pro Did you know that 80% of double bogeys come from errors on tee shots? The worst player on tour from inside 30 yards from any lie is better than the average player from 60 yards in the fairway? Our coaches teach your junior golfer how to manage the golf course with help from 5 time Tour winner Kyle Morris, and psychologist to the pros Dr. Todd Kays. We use Shotbyshot.com to help quantify and verify how each player is tackling the course and how to go about playing an event for their best on course performance. It takes more than the correct technique to become a champion golfer. It includes mastering the art of managing oneself and the golf course. Our coaches have all played or taught the game at the professional level and understand what is necessary to compete with the world s elite. To this end, a significant amount of each student's time is spent training and competing on the golf course under various conditions, all under the watchful eyes of our elite coaching staff. 8 Fitness Training Sports-specific training is essential to the success of any athlete. Our Junior Champions train like the pros, learning how to properly move, the correct mechanics for lifting, as well as what exercises are best and which can be hindering your golf performance. In addition to speed and power exercises to help your junior bomb it off the tee and gain college coaches attention, our twice weekly workouts will include exercises specifically designed to train the muscles and movements needed for golf. Many swing faults- lateral sway, loss of posture, over-the-top, to name a few- are due to physical limitations. The Tour pros all do exercises in the gym to help minimize these faults and maximize their swing efficiency. In collaboration with the rest of the team, your junior will perform exercises that allow the body the potential to do what is being asked of them in their swing instruction lessons. Comprehensive golf health, including sleep, nutrition, and hydration will also be emphasized to ensure optimal recovery for athletic and academic performance. Lindsay Becker knows first-hand from her time spent working with players at the highest levels of the game how the pros train during tournament weeks and in the off-season. She will ensure your player has the strength, power, and hardiness needed to endure the rigors of competitive golf. Tournament Preparation Since mental preparation is a major part of golf, one of the instructor's roles is to assist students in developing formal routines on and off the course. Formalized routines make for more consistent play and allow students to focus more clearly while competing. We will teach them how to properly warm up, correct nutrition before, during and after rounds, post round reaction, and proper dress. Students practice in all types of weather and are trained to handle all types of inclement conditions during tournament play. Because all of our staff knows what it takes to win at a professional level, we are able to fully prepare you junior golfer for the rigors of competing against the game's elite players in high school, college, and beyond. Tournament Observation The academy recognizes the immense value in of being able to watch juniors play under tournament conditions. Much can be learned about their game that cannot be seen on the lesson tee. Our team will attend select events to watch and study your junior champion to help better understand their competitive strengths and weaknesses. 9 Titleist Performance Institute Physical Assessment and Screening Lindsay Becker, a certified TPI trainer, will conduct a full physical screening on each player to determine their strengths and weaknesses. Combined with K-Vest 3D technology and finding the players efficiency patterns in their swing, Lindsay will create a customized workout plan for your junior golfer to succeed as they work out together twice a week. If the body does not want to get to the proper position due to lack of strength or flexibility, the swing will fight it like a virus, making it sometimes nearly impossible to improve ones swing, For this very reason, we make the physical aspect a mandatory requirement for all of our Elite Junior Champions and why our players increase an average of 9 MPH of club head speed each winter. Adding the desire to play college golf, at the right academic institution, for the right coach with a good program, while taking into account so many other factors can make the task very uncertain and risky. With the help of Second Nine Consulting, and the advice from Coach B, the process becomes much more clear and concise. The Second Nine Consulting program is designed to mentor talented junior golfers and their families on the recruiting process and navigating junior golf. We will aim to create a lasting impression via our mentorship on each junior that will benefit them during their junior golf career, college career and help with long term success in life "after golf." Coach B's 4 seasons at West Point have given him an incredible vantage point on how to develop character, leadership and discipline. During the program, Coach B will aim to impart what he has learned on each client that will hopefully lead to long term growth and development that will help in both junior and college golf. We will take a positive, measured approach to finding the right college fit. We will work with each family to create customized priorities during their search and use industry best practices to reach our desired goals.

10 (Second Team All-American Package) Mental Training Program for Golf Room Elite Junior Champions Academy Mental training in golf is many times one of the key missing pieces to success. Golfers are more diligent about working on their swing and physical fitness when it comes to tournament play, but their mind unfortunately fails them because of a lack of preparation of it. All of that hard work on swing and fitness, but to no or limited avail. They have the skill, but are not performing to their skill level because of fear, pressure, lack of confidence, overthinking, worrying, not able to let go of mistakes and self-doubt to name a few all mental lapses and hindrances. As part of the Elite Junior Academy at the Golf Room, your player will develop mental abilities to better perform to their skill level on a consistent basis. As a parent, coach or supporter of your junior, I am quiet certain if you are like most, you are tired of seeing your junior not perform to his or her capability. You know they have the swing, but are not executing when it counts. Do not let this be the case any longer. Here is the training your junior will obtain: Six individual meetings with Dr. Todd Kays: In the first meeting, Dr. Kays will help the player assess his or her mental strengths and weaker areas and begin to establish an individual program based on this assessment. Golf Personality Profile: Each player will be administered a Golf Personality Profile so that Dr. Kays and the rest of the coaches at the Golf Room can better understand and teach the player. This profile provides critical information for players to understand themselves better, but also helps us as coaches deliver individually-tailored instruction in the best possible manner. Six group meetings with Dr. Todd Kays: Players will meet with all of the other members of the Elite Junior Academy along with Dr. Kays. These sessions are designed to address what the juniors are challenged by most from a mental perspective. Specific topics are determined by all of the coaches at the Golf Room. This is one of the most powerful pieces of this program because the juniors get to bond with other golfers and also receive great support and challenge from each other. One 9-hole Focus Band session: Dr. Kays will walk nine holes at the golf Club of Dublin or your junior s home course and obtain direct feedback regarding their brain and neurological performance. Dr. Kays uses the Focus Band to do so and is continually give feedback to the player. Again, one of the most powerful pieces of the mental training program. 11 A Typical Day as a Junior Champion The Elite Junior Champions Academy is just that, elite. Players are surrounded by a team of renowned coaches 4-5 days per week working to develop not only their games but their minds and characters. Twelve- fifteen select juniors will participate in the Academy and work together in small groups. Each junior will spend at least 2 hours per week with Master Instructor Kyle Morris, 2 hours under the watchful eye of Physical Performance Coach Lindsay Becker, as well as (if added) bi-monthly sessions with Mental Performance Coach Dr. Todd Kays. All students will also attend mandatory monthly seminars with Dr. Kays learning key assets to playing at peak performance. Players will work on their golf game either Monday/Wednesday and fitness Tuesday /Thursday or vice versa inside our state of the art 4,000 sq. ft facility or at The Golf Club of Dublin, weather permitting. On days of dedicated golf instruction, Kyle Morris creates perfect practice for all players to ensure the player has to make proper swings. Through perfect practice stations using specific drills and feedback tools, the player can learn what proper swings truly feel like and therefore train the body/mind the proper feelings to recreate on the golf course. Kyle covers everything from the essential fundamentals such as tilts, turn, and alignment to the lesser-known subtleties such as how to play golf with your eyes learned over a 20-year career of competitive golf. Kyle draws on his own experiences of wins as a collegiate and professional golfer to help players shoot low scores competitively. In addition to invaluable personal experience, players will be surrounded with industry-leading technologies to help objectively assess and monitor the key metrics needed for optimal performance. Players will have access to the same tools and statistics used by Tour players to maximize their efficiency, track each shot, discover areas of weakness, and monitor consistent improvement over the course of the Academy. Physical performance training will be coached by physical therapist and strength & conditioning specialist, Lindsay Becker. Golfers will first undergo a golf physical assessment to determine strengths and limitations in range of motion, strength, power, and coordination that may be affecting their golf performance. Sixty minute, twice weekly workouts will focus on correcting these limitations as well as improving endurance, speed, power, and flexibility for athletic and competitive golf. Having worked extensively with collegiate and Tour players at the highest level, Lindsay knows exactly the training necessary to make your junior an athletic, powerful, and consistent golfer while minimizing the risk for injury and breakdown. Dr. Todd Kays provides mental performance training both on-site as well as on the course. Many junior players lack the mental know-how of thinking properly on the golf course and have no formal routine for dealing with pressure. With over 20 years of experience, combined with cutting-edge FocusBand technology, players will learn how to trust their training on the course and the art of staying out of their own way. In addition to one-on-one sessions, Dr. Kays will also hold monthly seminars for all players in the academy about key intangibles needed for peak performance on and off the course.

14 MEET THE TEAM Kyle Morris: The Golf Room s Director of Instruction and Junior Champions Director is Kyle Morris, a 7 year touring professional and former member of the PGA TOUR. For 7 years, Kyle traveled around the world playing in events on the PGA Tour Latin America, PGA Tour Canada, and Web.com tour. While touring, Kyle accumulated 5 professional wins, over 25 top 5 s, and over 50 top 15 s, with a career low round of 60. Kyle brings to life the art and science of the swing through mentorship by Mike Bender, Stan Utley, Pia Nilsson, Lynn Marriott, and James Siekmann (All top 25 of Golf Digest Instructors). As a player, Kyle understands that there is more to golf than just the swing and takes a full coach philosophy. He will teach you about the art of playing the game at your peak level and what that looks like for each player. Kyle is also a master of showing you how to practice perfectly when not with Kyle. Lindsay provides world-class rehabilitation and performance training to golfers of all skill levels, from juniors to players on the PGA Tour. Lindsay provides medical coverage during the Memorial Tournament and was a Team Trainer for the 2013 President's Cup and worked with multiple members of the PGA Tour. Kays, Ph.D. and president of the Athletic Mind Institute, a sport and performance consulting firm in Dublin, Ohio. Dr. Kays is a licensed psychologist and is devoted to helping athletes and people of all walks of life achieve peak performance on a consistent and frequent basis. He attended the University of Notre Dame for his undergraduate degree and obtained his doctorate at the State University of New York at Buffalo. Dr. Kays is a leader in the field of Sport Psychology. The co-author of Sport Psychology for Dummies (2010), Dr. Kays' training and guidance have helped thousands of athletes eliminate the most common mental errors and breakdowns in sport. For five years, he was the sport psychologist for the Columbus Crew, the major league soccer team in Columbus, Ohio and has worked with numerous soccer players and coaches throughout the country. Dr. Kays has consulted with athletes and coaches from all different levels of sport, including Major League Baseball, Major League Soccer, United States Tennis Association, Professional Golf Association, and Ladies Professional Golf Association. He consults regularly with the athletic departments at The Ohio State University and Ohio University, both of which he also serves as an adjunct faculty member. Dr. Kays was the first sport psychology Doctor at The Ohio State University, training several athletes, coaches, and teams, including National Champions, All-Americans, and Olympians. Dr. Kays has appeared on radio, television, and newspaper, including ESPN, Golf Channel, Fox Sports, and New York Post. He is and has been a consultant to top businesses, including NAPA Auto Parts, Huntington Bank, Mount Carmel Health, and Ohio Health. Dr. Kays brings an elite set of skills of transferring golf psychology into practice in the heat of competition. Through quantitative analysis, goal setting, and teaching the art of recognizing the moment we are in during a tournament, one can correctly assess and correct their current mind set to play at peak performance. Richard Brazeau The Golf s Room is proud to announce its partnership with Richard Brazeau and Second Nine Consulting, one of four AJGA College advisors. Rich is a former college golf coach, player, AJGA employee and is an overall fanatic for college golf. Richard has 10+ years experience in the college golf industry that has helped him frame a vast knowledge, philosophy and network that allows him to help junior golfers and their families find their best fit for college golf. His time with the AJGA has allowed him to gain an even deeper understanding of junior golf, tournament scheduling and how this can directly affect college recruitment. This can be a very stressful process for families and his job is to MAKE IT SIMPLE! His philosophy on college placement revolves around finding the "best fit" for each player and family based on their priorities in finding a school. Each player and family will have different wants and needs from an academic, athletic, social aspect. After spending over 6 years as a Division 1 coach, Rich has seen hundreds of recruits go through this process and knows what it takes to play at every level (Division 1, Division 2+3 and NAIA). He will establish a great list of "target schools" based on the players needs, an incredibly important part of the process and often overlooked.

21 Method of payment Invoice through squareup.com will be sent and expected to be paid by due date. UNCONDITIONAL OBLIGATIONS If the Student is asked to withdraw, voluntarily withdraws, or is dismissed for any reason after enrollment, the undersigned agrees to pay, and will be held financially responsible for the unpaid balance of the applicable charges for the remaining balance of the academy. PROLONGED SICKNESS OR INJURY In the case of prolonged sickness or injury with supporting documents from a licensed doctor, funds can be transferred to future training. CANCELLATION POLICY In the case where the undersigned provides written notice of cancellation to THE GOLF ROOM JUNIOR CHAMPIONS prior to first day of practice, the undersigned is responsible for payment of one month fee amount as liquidated damages due to the actual damages in securing the resources necessary to secure the training program offered by THE GOLF ROOM JUNIOR CHAMPIONS on behalf of the Student are uncertain and cannot be determined in advance. Any amounts already paid on Student s account over one month of the full tuition will be refunded. PHOTO RELEASE AND WAIVER: I/We hereby understand and agree to allow the TEAM to use my child s image and/or likeness, including without limitation, his/her photograph in promotional material(s) produced by the TEAM including, but not limited to, the TEAM's Internet site, brochures, advertisements, YouTube and video(s). GOVERNING LAW / JURISDICTION: The validity, effect and operation of this Agreement shall be determined according to the laws of the State of Tennessee. ATTORNEYS FEES Any expenses incurred on behalf of the TEAM for the enforcement of this Agreement, and/or any of its provision will be borne by the undersigned, including all costs and reasonable attorney fees. NOTICES All notices required herein must be in writing and shall be deemed to have been given on: a. the date of delivery, with receipt of overnight service; b. the date of personal delivery to the TEAM with receipt; c. three (3) days after mailing by prepaid certified mail with receipt; d. on the date of ing only with receipt of acknowledgement from the recipient. Mailing shall be to the respective Party s address as first stated above. Any changes of address shall be provided to the other Party pursuant to this subsection. SEVERABILTIY In the event that any provision(s) of this Agreement is/are held by a court of competent jurisdiction, or by an administrative tribunal, to be invalid or unenforceable, the reminder of the provisions will continue in full force and effect, provided the purposes for which the parties entered into this Agreement are not substantially frustrated thereby. ENTIRE AGREEMENT / AMENDMENT This Agreement constitutes the entire understanding between the Parties and supersedes and prior agreements, proposals, commitments, negotiations and understandings, whether written or oral, and all other communications between the Parties, relating to the subject matter hereof. Any addendum which may be subsequently entered into by the Parties will be incorporated by reference herein. No amendment to this Agreement, or any addendum, shall be valid or enforceable unless in writing and signed by all Parties. SUBJECT HEADINGS The subject headings of this Agreement are included solely for purpose of convenience and reference, and they shall not be deemed to explain, modify, limit, amplify or aid in the meaning, construction or interpretation of any provisions of this Agreement. COUNTERPARTS / COPY This Agreement may be executed in any number of counterparts, each of which will be deemed to be an original and all of which together will constitute one and the same instrument. A photo copy or other electronic copy of this Agreement and the signatures thereon shall for all purposes shall be deemed original and have same force and effect as the original. RULES OF INTERPRETATION It is agreed no rules of interpretation shall be applied against any Party as the drafter of its Agreement, the Parties acknowledge that this Agreement is the result of arms-length negotiations between the Parties being of equal bargaining power, and that the academy drafted this Agreement for the convenience of the Parties. If Parents, Legal Guardians, Legal Custodians, or Sureties of the Student are legally divorced or separated and have joint custody or share financial responsibility, then each financially responsible party must sign the Agreement before the Student will be considered eligible for enrollment.
