4 July 23, 2018; First Day of official practice IMPORTANT DATES TO REMEMBER August 06, 2018; Rostering Begins! First day to certify rosters August 24, 2018; FINAL day to add a player (no Roster movement after this date) September 01, 2018; FINAL day for any corrections to Rosters August 25, 2018; Week 1; 1 st game of the season Note: Roster Certification will begin as of Monday, August. 6th. exact dates, times and location for each association will be scheduled and communicated.

5 FOOTBALL TERMINOLOGY Player Certification: There is no player certification in any division. All roster-certified players will continue to be verified at check-in prior to each game. Property Player (Returning player): A player will become property of an association after 1 year Roster Certification: Conference organized; the process where documentation is verified i.e. birth certificate, report cards, physicals, CPR & First Aid etc. Roster certification of players and coaches explains in detail.

6 ROSTER CERTIFICATION OF TEAM, PLAYERS AND COACHES What information is inputted into Blue Sombrero? What to bring? How many copies? What does conference keep? How should I have my paperwork organized? How to complete the certification process? Roster certification consists of verifying player eligibility. Age based means the player must fall within the age parameters of the division certifying for and cannot exceed the max age specified for that division. Players can play up. NO player will be allowed to play down if age requirement is not met. Coaches must meet their requirements and certain aspects will be verified by SDYFC Eligibility prior to rostering any coach, all other coaching requirements is subject to verification by the SDYFC Coaches Director. Any redlines to your rosters must be corrected by August 31 st.

7 BLUE SOMBRERO ROSTER MANAGEMENT What information do I need to have in Blue Sombrero for Players?: Parent Information i.e. name, , phone number(s) PLAYER Legal First and Last Name Home Address (NO P.O. boxes) PLAYER Date of Birth Program/Division/Team Jersey Number School Name Current Grade G.P.A. ** Important Notice: Any player(s) entered into the rostering database after the deadline dates set forth by SDYFC will not be roster certified. 8 CERTIFICATION OF TEAM What I need to bring for each Team: Four (4) SDYFC Blue Sombrero Certified Rosters Extra jersey number must be listed (otherwise known as blood jersey ) *Two (2) will be returned to association 1 stamped original 1 stamped copy *Two (2) will be retained by SDYFC; Records & Little Scholars Note: Be sure you have already made copies of any/all documents that are being retained by SDYFC

9 What I need to bring to Roster Certify each Player: CERTIFICATION OF PLAYERS Two (2) Completed Player Contracts with photo attached (no stapling/taping/ or white out) Photo must be a headshot and must be unobstructed i.e. no hats, sunglasses, helmets, etc. *One (1) original will be returned to association (for binder) *One (1) copy will be retained by SDYFC Note: Original signed contract must have COLOR photo (features unobstructed) Copy can be black and white but must have photo be clear & visible *would help to note if player is property or new at the bottom of contract One (1) Copy of Birth Certificate (NEW participants only) *will be retained by SDYFC Two (2) Physician Completed Sports Physical ORIGINAL IS REQUIRED** Use SDYFC 2018 Physical form: ok to staple MD electronic form to SDYFC form but top portion must be completed by parent Chiropractor or Holistic/Homeopathic/Naturopathic, International conducted sports physicals are NOT acceptable Nurse Practitioner (NP) and Physician Assistant (PA) signed sports physicals OK. No WELL CHILD visits Physicals must cover entire season July 2018-December 31, *Physicals are good for 1 year *One (1) original will be returned to association (for binder) *One (1) copy will be retained by SDYFC One (1) Copy of End of school-year ( ) report card Progress reports not accepted *retained by SDYFC for Little Scholars One (1) Proof of Residency: Gas, Water, Electric, or Landline Phone Bill only (NEW participants only) *Any other document submitted that is not listed will not be accepted Note: Be sure you have already made copies of any/all documents that are being retained by SDYFC

If player uses another name, hyphenate the given birth name and other name. i.e. name on birth certificate: Jones preferred name: Roberts first name: Michael ENTER: Jones- Roberts, Michael School and grade should be for the school year Picture in color Clear and visible (not blurry) unobstructed (no hats, sunglasses, helmet, etc. ) Do not use tape or staples parent/guardian name and phone # NO WHITEOUT, RED PEN or TAPE should be used on this official form OR the form will be null and void. If no medical insurance, put SDYFC Must be signed & dated by parent or legal guardian Make sure both are initialed by parent and player for tackle divisions To be filled out by association Eligibility director. Would be helpful to write in pencil: property or NEW here at bottom

MUST be marked IF Yes marked, must get clearance from specialty MD prior to being cleared MUST have MD stamp or business card attached OR player will not be cleared. These 2 dates may be different. Date of Physical MUST cover the season (July 2018-January 2019). Keep in mind physical dates are good for 1 year i.e. if dated 5/2018 will expire 5/2019.

13 CERTIFICATION OF COACHES What I need to bring to Roster Certify each Coach : Coaches Training Certificates; i.e. USA Football Level 1 Footbal Coaches CPR & First Aid Card/Certification (**Head Coach and 1 Assistant coach required to be certified) **NO online courses!! Must be hands on course IMPORTANT REMINDERS ALL coaches MUST have registered with SDYFC in order to be certified on a roster (subject to verification) *background checks completed and cleared A minimum of (2) Coaches must be rostered to each team maximum of (8) A Coach can only be rostered to one (1) team *The ONLY person(s) that can be rostered to an additional team is the President and Coaches Director.

15 FOR THE TEAM: ROSTER 4 copies for each team *conference keeps two (2) copies; Records & Little Scholars ROSTER CERTIFICATION READY How paperwork should be organized FOR EACH ATHLETE: CONTRACT with COLOR PHOTO (no hats and or sunglasses) Original (color) and 1 copy (black & white ok) *conference keeps copy BIRTH CERTIFICATE - 1 copy for NEW participants ONLY PHYSICAL Original and 1 copy (*conference keeps copy) REPORT CARD - 1 copy *conference keeps for Little Scholars **WILL NOT be returned PROOF OF ADDRESS - 1 copy for NEW participants ONLY NOTE: Associations need to ensure they have copies of all documents being placed in the team binders in case of loss and a copy of the player physical and contract on the field while rostering. FOR THE COACH: USA Football Online Training Certificate Level 1 Course Minimum for EACH Coach CPR and First Aid for the Head Coach and one (1) Assistant on each team **SDYFC Registration; to be verified at roster certification or they will be redlined All documents per team must be compiled in the order listed below, loosely, not in sleeves and or a binder. Each coach and athlete s documents should be grouped together -place each grouping in roster order, staff then players. The rostering process will go much smoother if your paper work is organized. Team Rosters Coaching Certificates (each rostered coach) Coaches CPR and First Aid Certification (2 required: Head Coach & 1 assistant coach) Player Contract (Original in front of copy) Player Birth Certificate (NEW participants) Player Physical (Original in front of copy) Report Card; end of previous school-year (17-18) Proof of Address (NEW participants)

16 ELIGIBILITY PROCESS COMPLETION ROSTER READY? Contact Priscilla Gressett for available slots to roster your team(s) before August 17 th. *Roster Certification begins on August 6 th It is the responsibility of each association to have all teams rostered by the deadline date (This means all associations need to have an appointment scheduled to Roster, by August 17 th.) *If any teams are participating in a Pre-Season game the team is REQUIRED to be ROSTERED *Any corrections must be completed prior to participation in a preseason game. (*scrimmages excluded) IMPORTANT REMINDER If you are unable to keep your scheduled certification date/time, please contact Priscilla Gressett at least 24-hours in advance If you are more than 15 minutes late to your scheduled roster certification time, please be aware you may have to wait and or reschedule; we will begin certifying the next scheduled association who is present. No Shows will be fined $50 PER TEAM for each missed appointment.

17 TEAM BINDER ASSEMBLY The following paperwork MUST be in your team binder **ALL documents must be placed in plastic sheet protector sleeves: DO NOT punch holes in documents or forms** Team Documents SDYFC Certified Roster (STAMPED Original) Coaches USA Football Online Training Certificates Level 1 minimum (entire staff can be placed in one (1) plastic sleeve) Coaches Concussion Training Certificates (entire staff can be placed in one (1) plastic sleeve) CPR/1 st Aid Cards (Head Coach and one (1) assistant per team) Player Documents Contract (SDYFC Certified, stamped original) Physical / Medical Clearance Form (original) *Contract and physicals are to be placed in the same sleeve facing opposite sides for each player in roster order. Please do not keep ANY other documents in the Team Binder.

18 IMPORTANT REMINDERS Team Parents and Team Managers are NOT rostered and should not be on the field unless performing the duty of counter/checker they must leave the field once play counts are complete Coach Trainees must be at least 16-years old AND must follow the same eligibility guidelines as players. Max of 4 Coach Trainees per team (does not affect max number of coaches) P.O. Boxes are NOT acceptable addresses (a physical address must be provided) Contracts must have a photo attached (at least one (1) must have a color photo and unobstructed) White out, red pen or tape is NOT PERMITTED on contracts or physicals The contract and the name on the birth certificate must match (do not forget the Jr. or the III in the name). If a player uses another last name, hyphenate the birth given name and name to be used on the contract If participant does not have insurance, SDYFC should be listed in the insurance field -do not leave blank If contract is not completely filled out, the child will be redlined Special waivers are not automatic, special waivers need to be signed the home Association President PRIOR to roster certification. 19 IMPORTANT REMINDERS (continued) Players cannot be on the field without a physician conducted physical form must be turned in to the association prior to participation NO EXCEPTIONS; PARTICIPANT SAFETY RISK If the coaches book is not on the field, there cannot be a practice example when you bring your book to roster, practice for that day must be cancelled or copies of the contract and physicals must be at the practice field We will not accept documents that are fuzzy, blurry, illegible, crooked with only partial information, etc. VERY IMPORTANT: Do not accept documents that you believe have been tampered with or falsified, you put your association and that team at risk (i.e. forfeit of games, suspension, etc) Football player s (ALL DIVISIONS) age cut-off date is 7/31 (see birthday matrix next page) MIN/ MAX allowable participants per Division as follows: 7U MAX 24, MIN 12 8U MAX 26, MIN 14 9U 14U MAX 36, MIN 16 Fielding Multiple Teams: The formation of teams is done on a first come first serve basis on your open registration day. Associations can have a veteran registration day. No Association shall start a second TACKLE team until a minimum of 25 players are assigned to the first team in that division of play, No Association shall start a second FLAG team until 20 players are assigned to the first flag team SUBJECT TO SDYFC APPROVAL AYF does not form SELECT TEAMS, IN OTHER WORDS YOU CANNOT SELECT OR RECRUIT ONLY THE BEST ATHLETES FOR A CERTAIN TEAM.
