2 April 2013 Dear Team Representative: Congratulations to your Championship Team and welcome to the 2013 US Youth Soccer Southern Regional Championship. Each of you players, parents, coaches and managers have worked hard, shown great dedication and made many sacrifices to become State Champions and Premier League Representatives. You have now reached the next level of competitive youth soccer play, the U.S. Youth Soccer Southern Regional Tournament. Have fun and enjoy every minute while you are here! On the field, play hard and play fair. Accept the official s decisions, obey the laws of the game, give it your all and give it your best. Treat your opponent with respect and dignity; remember they have worked hard to get here, too. Take time to enjoy the Edmond/Oklahoma City area, and don t forget the many hundreds of volunteers who have worked countless hours to make this week a special one for you. 6 Team Representative Responsibilities Welcome to the 2013 US Youth Soccer Southern Regional Championships. During your teams stay you will be the direct communication link between your team (including players, coaches, trainers, parents and spectators) and your state representative. The state representative will be your direct link with the tournament. Communications with your state representative Make sure that you know your state representative and all of the responsibilities they have for your team. It is important that you know how to contact your state representative at all times. Your state representative can be a vital link between you and the tournament staff during the tournament, especially in an emergency or crisis. If you have any questions prior to or during the event, please go through your state representative. Your state representative will know who can best answer your question and get you a quick response. Once my team has qualified for the Southern Regional Championships, what do I need to do as team representative? Contact your state representative to get your hotel assignment. They will give you instructions on how to book rooms for your team. As the team representative you will be the sole contact for your team (including parents) with your State Reps and will be responsible for the allocation of rooms. Please contact your State Rep with questions concerning your hotel rooms. Carefully go through this handbook given to you by the state representative. There are several items you will need to share with your team and the parents of your team in this notebook. Please review all of the forms in this handbook. Download the forms online at complete and return them at Registration. Check with team members and their families to place the orders for parking passes. (You will pick up all Parking Passes at Registration. PLEASE MAKE SURE IF PARENTS ORDER SEPARATELY TO INCLUDE TEAM NAME) There will be PRE-ORDERS FOR T-SHIRTS and they will be picked up at registration. (A link will be ed to you for the Pre-order T-shirts) Go through the instructions for registration and make sure that you know when your state has registration. If all of the information is complete and ready, registration should be a quick and easy process. Familiarize yourself and your team with the Regional Rules and Procedures available at Practice Request forms can be found at under the Region III tab/team Information. 7 Once my team arrives, what do I need to do as the team representative? After checking into your hotel, please contact your state representative and give them your room number. Go through all of your registration information one more time. If you have any problems, please let your state representative know in advance so that they can assist you in taking care of the problem. Have a team meeting to make sure that everyone on your team knows when and where he or she needs to be. Opening Ceremonies is required attendance for all players and coaches. Get acclimated to the area and locate the venue for registration and your hotel. Arrive at registration 30 minutes early and check in with your state representative. Please do not come to registration without checking in with your state representative. All Bench Pass Change Forms should be submitted at this time, if not done previously online. To add any Bench Credentialed individual requires a $10 payment. The person being added must present their USYS pass and sign the Liability Release. State Representative must witness. Please make sure to pick up your pre-ordered parking passes and purchase any additional parking passes your team may need before you leave registration. Individuals may also purchase parking passes at Opening Ceremonies and daily passes at a location near the complex, but are encouraged to purchase them through you at registration. Please make sure to pick up your team s goody bags and tournament programs before you leave registration. Opening Ceremonies Please familiarize yourself with the layout of the facility. Please keep your cell phone on at all times during your stay so that your state representative can reach you. Once the tournament starts, what are my responsibilities? Make sure that your team is at the proper field location and that your parents are aware of the Southern Regional Rules and Procedures available at Communicate daily with your state representative. Review instructions for Safety and Emergencies with all team members and leaders. 8 We are Regional Champions! What do I need to do? Your State Representative will meet you after your game to escort your team to the Awards Ceremony. Make sure that your team is properly dressed for the Awards Ceremony and the team pictures afterwards. There will be a table near the Awards Ceremony for you to meet with Brandon Story or designee to pick up your information for the National Tournament. You will then need to meet with a representative of our staff to help you complete your team s travel arrangements for the US Youth National Championship Series. The tournament committee and staff thank you for the time and dedication that you give as the team representative. Patch Placement State Cup Champion patches go on the RIGHT sleeve of the jersey, approximately 1 above the bottom edge of the sleeve. 9 Player Profile Information Entering your Players Profile Information Is an important part of the Tournament. My Teams Home Page You can click the pencil icon to change anything or add anything on your team. Click the roster icon to add players to your team. Click Roster Here you can add players, view players, edit your roster or print your roster. (First and Last names must match what is on the State Roster. Do not use nicknames.) Click add a player to set your team roster. Or you can download blank template and enter your roster information into an excel template. Once you have completed the form, you can import your roster. First, Last Name, Jersey and Registration Number are required fields. ADDRESSES ARE BENEFICIAL TO COLLEGE COACHES AS WELL AS THE OTHER PLAYER COLLEGE INFORMATION (GRADUATION YEAR, GPA, STATUS, ETC.) 13 Registration Procedures and Schedule Thursday, June 20, 2013 Central Plaza 930 E. 2 nd Street Edmond, Ok People Required at Registration State Rep Coach, manager or team representative Anyone needing an additional Bench Pass (not previously requested) These individuals should meet with their State Rep prior to the assigned time. In order to stay on schedule, it is important that all teams arrive at registration on time and with the proper paperwork. Required Check in Forms Coach / Manager / Team Representative must provide: US Youth Soccer Player Pass for every player with a current picture, signature of state official and/or stamped with the official state stamp, laminated individually and placed in alphabetical order US Youth Soccer Pass for each of the four people requesting bench passes (picture, signature of state official and or stamped with state stamp and laminated individually) REGION III MEDICAL RELEASE FORM for each player, with insurance information and parent contact information, in alphabetical order LIABILITY RELEASE FORM (& special instructions for children taking medications or having medical conditions) Multiple copies are ok. 14 Players Please Note: Proof of age for each player is not required at check-in, but must be available within 24 hours if requested or if a protest of eligibility is made. It is strongly suggested that this information be available throughout the tournament. Acceptable proof of age documents include a birth certificate or birth registration issued by an appropriate government agency, passport, alien registration card issued by the US Government, certificate issued by the US Immigration and Naturalization Service attesting to age or a certification of an American Citizen born abroad issued by the appropriate government agency. Not accepted are hospitals, baptismal or any religious certificates. Coaches and Team Officials Four Bench Passes are available for each team. Those individuals must have a valid US Youth Soccer pass and sign the Release of Liability. To add any Bench Passes, the coach/team official must be present to sign Release of Liability. There is a $10 fee for each name addition. The person s US Youth Soccer pass is required, and the State Rep. must witness the signature. Please provide the correct information prior to the deadlines! 21 Oklahoma Sports & Orthopedics Institute Heritage Plaza Benson Rd. Suite 201 Edmond, Ok OSOI will provide all athletic training services during the 2013 Southern Regionals to be held at Edmond Soccer Complex in Edmond, Ok. There will be two athletic training trailers located on site. There will be additional athletic trainers throughout the complex under designated medical tents. OSOI will provide athletic training supplies to all competing athletes (i.e. tape, pre-wrap, band aids, ice bags, etc). Athletes who need to be taped should report to one of the athletic trailers 30 minutes prior to the time that their coach has told them to arrive. OSOI will provide call-ahead services to the emergency room, physician s offices, and after hours care as needed. In addition, we will have five sports medicine physicians on-call throughout the tournament. EMS will be on site during the entire length of the tournament and will work closely with sports medicine staff to provide efficient and appropriate care in a timely manner.
