The purpose of this survey was to ascertain the suitability of loading and securing of subject cargo and further to ascertain the suitability of towing arrangements from Spitzer Industries, Houston, Texas to the Installation site in care of installation vessel MSV DEEP BLUE in Walker Ridge Area, Block 249, US Gulf of Mexico. This survey was assigned by Mr. Gus Cassity on behalf of Petrobras American, Inc. and for the account of Petrobras American, Inc. 5 Cascade & Chinook Subsea Project Page 4 Navigation Lights and Day Shapes : Port, starboard and astern light stands fitted with shields and solar charged battery powered lights with day/night photo-senor and switch. Towing Gear : Tow (2) towing padeyes erected on the raised fo'c'sle deck, each with a 90' x 2 1/2" dia. studlink chain bridle leg to a connected to 90' x 2 1/2" dia. studlink chain pigtail. All shackles are 123-ton SWL safety type towing shackles. Emergency Tow Gear : 2 1/4" diameter steel wire shackled into the centerline padeye with a 123-ton SWL safety type towing shackle and faked and clipped into a Flemish coil across the exterior facing of the breakwater. One (1) 3/4" x 450' messenger wire shackled into the emergency tow wire with a 55-ton SWL safety type towing shackle and then lead across bow to starboard and down starboard side to the stern, outboard and over all obstructions and clipped at regular intervals along the length.. Wires terminated on both ends in poured spelter socket. In addition there is an emergency tow wire recovery line consisting of 3" x 250' poly-line coiled on the starboard stern with a recovery buoy attached. Ground Tackle : None. Ballast System : None. 6 Cascade & Chinook Subsea Project Page 5 General Condition As sighted afloat on August 3, 2009, at Spitzer Industries, Houston, Texas, the, BARGE 410 is considered satisfactory for the proposed tow. LOADOUT DETAILS The BARGE 410 was moored starboard side to the quay at Spitzer Industries, Houston, Texas with six (6) 3" diameter synthetic mooring lines. Transport grillage for the two (2) manifolds and two (2) pump stations was pre-installed prior to barge arrival with the manifold grillage mounted on the port bow and the pump station grillage mounted on the starboard stern. Loadout of the manifolds and pump stations was completed by the D/B BIG JOHN and due to the extensive out-reach, rotation of the barge was not required to safely land out all four (4) lifts. The manifolds and pump stations were staged in pair's quayside on set-up blocks, one (1) manifold with one (1) pump station, at opposite ends of the barge. The D/B BIG JOHN was maneuvered into position by two (2) tug boats, perpendicular to the quayside and in front of each lift. The tug boats maintained position while the ancillary lift rigging was connected to the pre-installed lift rigging on each item. The D/B BIG JOHN then lifted the load, the tug boats maneuvered the D/B BIG JOHN alongside the barge, perpendicular to the grillage and the load was set down onto the grillage. This routine was repeated for each of the four lifts. The two (2) manifolds were loaded first and then the two (2) pump stations were loaded second. Immediately upon completion of the loadout, seafastening installation was commenced. However, due to unforeseen clashes between manifold/pump station structural foundation supports and inaccuracies in dimensional control, a large number of the seafastenings required modifications to permit proper fit-up. Upon completion of seafastening installation and final acceptance, the barge was towed to Texas Terminals, Houston, Texas for ballasting operations. The barge was ballasted down to an even keel condition with drafts at 12' fore and aft. This draft allowed the barge to properly lay in alongside the MSV DEEP BLUE offshore for installation of the manifolds and pump stations. Once ballasting operations were completed and the manways were re-sealed on the barge deck, the Tow Approval Certificate was issued to the Master of the M/V MARINER.

The No. 2 block has a maximum capacity of 200 ST at 20' forward of the barge's headlog. The crane has a main mast pinned 37' aft of the headlog and a back-stay mast for topping the main mast. All lifting machinery is deck mounted and diesel engine powered. 9 Cascade & Chinook Subsea Project Page 8 Synopsis of Events August 3, 2009 The undersigned surveyor arrived at Spitzer Industries, Houston, Texas at 0651 hours. The Crowley deck barge, BARGE 410 was moored starboard side to the quay with transport grillages pre-installed for the manifolds and the pump stations. The Cascade East manifold and the Pump Station No. 1 were staged quayside astern of the barge and the Chinook manifold and the Pump Station No. 2 was staged quayside off the bow of the barge. Spitzer Industries was in the process of erecting a gangway for access to the barge. A pre-loadout meeting was attended from 0803 hours until 0818 hours in Spitzer's main conference room with representatives from Technip, Petrobras and Spitzer in attendance. The D/B BIG JOHN arrived alongside the quay at Spitzer Industries at 0854 hours and a loadout JSA was held from 0927 hours until 0936 hours to discuss and address loadout safety. Loadout of the Cascade East manifold commenced at 0945 hours and was completed at 1013 hours with the manifold sitting on the forward transport grillage. Loadout of the Chinook manifold commenced at 1045 hours and was completed at 1113 hours with the manifold sitting on the aft transport grillage. Loadout of the Pump Station No. 1 commenced at 1142 hours and was completed at 1202 hours with the Pump Station No. 1 sitting on the forward transport grillage Loadout of the Pump Station No. 2 commenced at 1224 hours and was completed at 1337 hours with the Pump Station No. 2 sitting on the aft transport grillage with all loadouts completed, the D/B BIG JOHN was released and departed Spitzer Industries. Installation of seafastenings on the manifolds and pump stations commenced at 1358 hours, including modifications to the con-conforming seafastenings. The undersigned surveyor departed Spitzer Industries, Houston, Texas at 1912 hours. August 4, 2009 The undersigned surveyor arrived at Spitzer Industries, Houston, Texas at 0648 hours. Spitzer Industries welders and fitters had worked through the previous night to modify the manifold and pump station seafastenings. Spitzer Industries personnel commenced fit-up and fastening of the modified seafastenings on both manifolds and both pump stations at 0731 hours. Technip personnel, with assistance from Spitzer Industries, commenced installation of the subsea crosshaul rigging on each of the four (4) lifts. At 1138 hours, the undersigned surveyor was informed by Mr. Loren Smith with Signet Maritime that the window of opportunity to shift the BARGE 410 from Spitzer Industries to Texas Terminals had expired and it would be early morning on August 5, 2009 for the next available opportunity. Spitzer Industries personnel continued with fit-up and fastening of the manifold and pump station seafastenings while Technip personnel continued with rigging up the crosshaul rigging.
10 Cascade & Chinook Subsea Project Page 9 Seafastenings on the Pump Station No. 1 were completed and approved for tow at 1351 hours and on the Pump Station No. 2 at 1401 hours. Seafastenings on the Cascade East manifold were completed and approved for tow at 1425 hours and on the Chinook manifold at 1439 hours. The undersigned surveyor departed Spitzer Industries, Houston, Texas at 1526 hours. August 5, 2009 The undersigned surveyor arrived at Spitzer Industries, Houston, Texas at 0648 hours. The BARGE 410 was found to be complete in all aspects and ready for tow to Texas Terminals for ballasting operations. Three (3) inland tow boats, M/V MISS EDNA, M/V OU311 and M/V STUD were alongside the barge to orchestrate the shift to Texas Terminals. The BARGE 410 was let go from the quay at 0755 hours and maneuvered off the quay for Texas Terminals at 0801 hours. The BARGE 410 arrived at Texas Terminals, Houston, Texas at 1011 hours and was moored up alongside the quay, starboard side to at 1033 hours. The offshore tug, M/V MARINER was alongside the quay as well. At 1048 hours, the M/V MARINER shifted in alongside the port side of the BARGE 410 and commenced making up the tow wire connection to the tow bridle. Four (4) 6" ballast pumps were set onboard the deck of the barge at 1149 hours and the necessary manways in way of the compartments to be ballasted were removed. Connection of the M/V MARINER's tow wire to the tow bridle on the barge was completed at 1204 hours. Ballasting of the BARGE 410 compartment No. 2 commenced at 1408 hours and ballasting of compartment No. 6 commenced at 1424 hours. Compartment No. 6 was completed at 1954 hours and compartment No. 2 was completed at 2009 hours. Ballasting of compartment No. 7 commenced at 2017 hours and was completed at 2143 hours, finishing ballasting operations. Final drafts were 11' 10" forward and 12' 0" aft. The Tow Approval Certificate was issued to the Master of the M/V MARINER at 2157 hours with the understanding that the tow was scheduled to depart at 0800 hours on August 6, The undersigned surveyor departed Texas Terminals, Houston, Texas at 2216 hours.

Port bow outboard, forward of the Chinook manifold at Frame No. 7. Detail Securing 1. The Cascade East manifold is secured atop a purpose built transport grillage package, 30' x 20' x 5', fabricated from W12 wide flange beam sections. The transport grillage package is secured to the barge deck by a combination of welded stops and clips with three (3) clips welded along each side, port and starboard and two (2) clips welded along each end, fore and aft. Additionally, there are two (2) stops welded on the inboard sides of the grillage, port, starboard, fore and aft and on the outboard side of the grillage fore and aft. See Drawing No. I-DE TKU-246, Rev. B. The Cascade manifold is secured atop the transport grillage package by a bolt-on clip arrangement. Each clip assembly consists of a 12" x 10" x 1/2" base plate with two (2) 10" x 6" x 1/2" plate clips welded onto the base plate. The base plate is then notched with four (4) through holes to accommodate 1" dia. threaded bolts with lock-washers and backing nuts. The attachment location on the transport grillage package is also notched to accommodate the 1" dia. bolts with the nuts securing the bolts on the

The clip assemblies are arranged in opposing pairs with one (1) on the outboard side of the manifold base and one (1) on the inboard side of the manifold base. In total, there are sixteen (16) clip assemblies with two (2) assemblies' inboard/outboard port, starboard, fore and aft. There was no torque requirement for the bolted connections. 2. The Chinook East manifold is secured atop a purpose built transport grillage package, 30' x 20' x 5', fabricated from W12 wide flange beam sections. The transport grillage package is secured to the barge deck by a combination of welded stops and clips with three (3) clips welded along each side, port and starboard and two (2) clips welded along each end, fore and aft. Additionally, there are two (2) stops welded on the inboard sides of the grillage, port, starboard, fore and aft and on the outboard side of the grillage fore and aft. See Drawing No. I-DE TKU-246, Rev. B. The Chinook manifold is secured atop the transport grillage package by a bolt-on clip arrangement. TOWING ARRANGEMENTS Particulars Tow Make-Up : Connect main tow wire (port tow wire) to pigtail chain on barge using 123-ton towing shackle and tow ahead. Route : Houston Ship Channel outbound to Galveston Rhodes, thence various courses South by East to Walker Ridge Area, Block 249 as safe navigation permits. Weather Forecast : Forecast is considered favorable. Weather Force 2-3 for the next 3-5 days in extended forecast. DAMAGES None. RECOMMENDATIONS None. SURVEYOR'S COMMENTS 1. A large quantity of the bolt-on clip assemblies for both the manifolds and pump stations, required modifications to the both whole positions and well as the length of the clips due to unforeseen clashes between manifold/pump station structural foundation supports and inaccuracies in dimensional control. All of these modifications were completed by Spitzer Industries and NDE was performed to assure quality of the modifications.
