4 Team Manager Responsibilities You must decide between the team manager and the head coach the following responsibilities: -It is wise to have a binder that you can put all your info in to keep it organized. You will need this binder and info with tournaments and games. -You must collect a copy from each player on your team the registration/ medical release form (a copy of one is at the back of this booklet) after they register for UYSA. Put this in your binder. You do not need to collect birth certificates! -Make sure every player is registered properly with Surf and UYSA on time. -Collect roster info from the head coach and keep in a binder, create a team contact sheet. -Make sure the team is properly informed EACH WEEK with updates to schedules and information from the coach even if there are no changes. -Register team for tournaments that your coach has assigned. -Collect money at times from players for tournaments, futsal, etc. -Enter game scores during state play, sometime the coach does this so ask them. -Club pass players and collect the right paper work for this. -Print rosters for games. -Check in for tournaments. -Be early to games to see if the coach needs anything. -Make our club is represented well with good sportsmanship. *It is extremely important to communicate with your coach to make sure all information is correct, you are doing what they need, and that the team is ready to go. 2

5 Treasurer Info, Shalane Low: Registering for Surf: Go to utahsurfsoccer.com and scroll to the very bottom of the page where there will be a Quick Link to Surf Payments. This link will take you to the Sports Illustrated Play payment website. Login (or create an account and add your child if you are new to the system). Click Register to Play/Participate. Choose the child you wish to register (or add new child). Scroll down to select your team session. Choose your team in the drop-down box. Complete the process to checkout and pay. Other Financial Information: 1. Payment plan options are 50% due at registration, 25% due 9/1/16, and the final 25% due 11/15/ Team Managers should add themselves as a team manager on the payment site and follow up with those who have not registered. Anyone NOT listed on your TeamWall has NOT paid. 3. SI Play now has a free app available to download to track your team, add practices and games, track availability, send texts and s, and upload photos, etc., if you are interested. The app is called TeamWall and is available in the app store (very similar to TeamSnap but with no cost). 4. Anyone who needs an alternate payment plan option due to financial hardship may contact the club treasurer directly at utahsurfsctr@gmail.com 5. The club will provide fundraising opportunities throughout the year. Money earned through fundraising will be applied directly to that player's account. If funds have already been paid in full, a reimbursement check will be issued. 6. Payment of tournaments and futsal/indoor sessions will be handled by the teams individually; these do not go through the club. Team Accounts: Below is information on obtaining an individual team account for your Surf teams. Having a separate team account may make it easier to plan and pay for tournaments, futsal, etc., as well as to do individual team fundraisers. Obtain a non-profit tax EIN number. Apply at irs.gov. Search "EIN number" in the search box. Click on "Applying for EIN number." It will walk you through the process. Select other non-profit. Name your non-profit the same as your team (i.e. Utah Surf 04 Premier SB). Using your team s EIN, open a business checking account at a bank or credit union of your choice. Most institutions will give you a limited number of free printed checks. Mountain America has been very easy to work with for these team accounts. Registering for UYSA: 1. Log into the Surf site 2. Click on join surf 3. Click on Utah Surf all access 4. Click on Registration 5. Click on UYSA Registration - top right of screen 6. Click on Player registration and login; create a login if you have not done so in the past 7. Continue as prompted 8. Print receipts the team manager needs the Player participation form. PLEASE REMEMBER TO SIGN IT IN BOTH PLACES. YOU SIGN IT IN THE MIDDLE OF THE PAGE. These forms are used for all tournament check-ins. *please note you must register your players with 2 separate websites, Surf and UYSA. 3

7 Registrar Info, Suzanne Brown: Immediately after tryouts please me at with the following information: 1. Team Name (Please include the name of the team from the previous year if it is a continuing team or let me know if it is a brand new team) 2. Final Roster (If you add to the roster after you send it to me, please let me know) 3. Head Coach, Asst Coaches and Team Mangers (please include contact information) Coaches: 1. You must be a licensed coach or have a license waiver in place before your team can be activated for the current season. There must be a Head Coach and at least one Assistant Coach on the roster in order to activate. It is HIGHLY RECOMMENDED that each team have at least one Team Manager. 2. X League teams (U9 - U12): All coaches must have a minimum of an E License or E-Waiver to coach an X League team. AAA Competition (U13+): If you are coaching a U13+ team you must have a minimum of an E License or an E- Waiver. If you are coaching more than one U13+ team you MUST have a D License or D-Waiver or D-Pending designation AND you must have an asst coach with a minimum of an E License or E-Waiver on each team. 3. To obtain a License Waiver you must complete the application and send it to Holly Gundred at UYSA. The application can be found at under the "Coaches" Tab. You will also need to pay the $25 fee and the link is there as well. 4. All coaches and Team Managers must complete the Heads Up Concussion online training EVERY year. This is a mandatory requirement. The certificate of completion must be uploaded to your UYSA profile before the beginning of the season. If you don't have a current certificate uploaded you will be "red lined" on the game day rosters and unable to coach. The online training video can be found at under the "Coaches" tab. You must watch the "Heads Up Concussion" video and take the quiz. You will be issued a certificate at the end. The certificate MUST be SAVED to your computer and UPLOADED to your coach s profile. The system does NOT save your certificate. If you don't save it to your computer you will have to repeat the training. The training takes about minutes. 5. All coaches and Team Managers must register as a coach/admin in the UYSA system EVERY year. Please complete your UYSA registration as soon as possible. To register as a coach/admin please follow the instructions below: (You must go through the Surf website. This is the only way that it will link your registration to the Surf.) Scroll to the bottom of the home screen to "Quick Links" Click "Registration Links" Under "League Registration" choose UYSA X League (U9 -U12) or UYSA Competition (U13+) Click on the "Registration" tab in the upper right corner of the screen Choose "Coach/Admin" registration and either create a new account or log in to your existing account. Follow the onscreen instructions. You only need to register once...even if you are coaching more than one team. Make sure you have a photo uploaded. No photo = no coaching. You will not show up on the game day rosters. Photos must be close up, front facing, head shots. No sunglasses or hats. If the photo doesn't meet these requirements I will delete it and you will need to upload another photo. 5

8 Player Registration Please give this information to all your parents ASAP. It is very important to have your players register with UYSA as soon as possible. This registration will create your official game day rosters. The deadline for UYSA registration is the last Friday in June. Please make sure to sign your with your team name. I have a lot of teams and many of you coach more than 1 team. It makes it a lot easier when I have a team name. UYSA Registration Instructions: Go to the Surf Website Scroll to the bottom of the home screen to "Quick Links" Click "Registration Links" Under "League Registration" choose UYSA X League (U9 -U12) or UYSA Competition (U13+) This is the age group of the team your player will be playing on...not their age. For example: if your player is playing on a U13 team but they are 12 years old, choose the UYSA Competition link. Click on the "Registration" tab in the upper right corner of the screen Choose "Player" registration and either create a new account or log in to your existing account. Follow the onscreen instructions. You must upload a photo during the registration process. The photo must be a close up, front facing head shot. No sunglasses and not hats. And please no family photos. If the photo does not meet these requirements I will delete the photo and you will need to upload a new photo. If you need to update your photo I have to delete the photo before the system will let you upload a new photo. Utah State Concussion Policy. You must watch the "Heads Up Concussion" video and take the quiz. You will be issued a certificate at the end. The certificate MUST be SAVED to your computer and UPLOADED to your coach s profile. ALL coaches MUST complete this training and ALL team managers...any team administrator. The system does NOT save your certificate. If you don't save it to your computer you will have to repeat the training. The training takes about minutes. It must be done every year. If you don't have the certificate uploaded you will not show up on the game day roster and you won't be able to coach. The video can be found by: Coaches/Referee Heads Up Concussion Training. EVERY team MUST have a head coach and at least one asst coach. I can't activate your team without 2 licensed coaches and the minimum player requirement. If you have an E License or an E Waiver you can only coach on ONE team...whether you are a head coach or an asst coach...only ONE TEAM. If you are coaching more than one team you MUST have some form of a D License a D Waiver, or a D Pending...meaning you are in the middle of taking the D License course. Please do not procrastinate getting your license where it needs to be. I see many of you that are set to coach more than one team but I know you only have an E License or an E Waiver. If you do not have a coaching license please contact Holly Gundred at UYSA asap to apply for a license waiver. You must have a coaching license or a waiver in order to be on the roster. Heads Up Concussion Training and the License rules are not up for discussion or debate. They are UYSA policy and State Law. I can't change these rules. If you don't complete these 2 items you will not be able to coach! UYSA is strictly watching the coaching licenses. They have made it very clear that if there is a license violation I will face disciplinary action. I won't risk it...so please take care of it ASAP. 6

9 Uniform and Equipment Info, Marlo Anderson: Coaches/Managers, Marlo Anderson is over uniforms and equipment. Once your team is finalized after tryouts, please send your rosters to Once they are enter into the soccer.com system you should be getting notification by way of an being sent to each of your team s players within hours prompting them to begin ordering their uniforms. The will come from soccer.com and will look like this; custserv@sportsendeavors.com You may want to notify your team and let them know that an will be coming so they know what it is and that they can start ordering uniforms when they receive it. I will you when I have entered your roster. If you have any questions please let me know at marlo@stardocs.com Field and Alignment, Steve Brown: Alignment: The club has an alignment specialist who compiles alignment information from each team for the Fall/Spring seasons and presents it to UYSA. Specifics information regarding alignment will be sent to each coach prior to the Fall and the Spring alignments. Scheduling Fields: Prior to each season and before season schedules may be compiled the club field coordinator compiles a list of primary and secondary fields for each team. When the season schedule is published the field coordinator will make available schedules for each field so that coaches and team managers may see what times may be available for the rescheduling of HOME games. Prior to submitting a game change request the coach and or team manager must confirm with the field coordinator the availability of the field for the desired date and time, with the referee assignor to ensure referees can be assigned, and with their opponent for their acceptance. AWAY games are rescheduled by your opponent with their field coordinator and with their referrer assignor. See UYSA policies for further details regarding rescheduling. 7

11 How do I reschedule a game? *Review schedules as soon as possible and pursue corrections as soon as possible. Make sure parents and coach are involved, please watch for school breaks and time Please note that you must contact the field coordinator and the club reffs before you decide on a date to see if fields and refs are available. Contact Steve Brown with Surf, utahsurf.clubtm@gmail.com. *RESCHEDULE INFORMATION 1- Teams can initiate two reschedules a. Reschedules cost $50 starting the 1st day of league play b. Reschedule must be COMPLETE before the 1st day of league to not be charged. COMPLETE- Changed on the UYSA schedule 2- Team initiating the reschedule should click the button (does not matter if home or away) 3- Before clicking the button the HOME team must have field and referee assignor approval a. Please and contact all coaches, team managers, field coordinators, and referees before you make any changes. If you are rescheduling for weather, no referees or unplayable fields do not click the reschedule button, follow steps 1-4 and then follow step 4B. Send reschedule information to scheduler@utahyouthsoccer.net. To reschedule for any other reason follow reschedule steps 1-4 and follow 4A. To reschedule ALL games Both teams MUST agree to the schedule online through the reschedule button before a reschedule is complete. (If you do not see it changed on your schedule it is NOT rescheduled!) 1. Contact the opposing team using a. Log in to your UYSA account b. Make sure your picture is highlighted c. Click TEAMS on the tabs below d. On the new tabs Click Tournament and Schedule Apps e. Click Schedules/Game Scoring on the right hand side (by the team you want) f. Click the Opposing Team Name to see contact info g. All contacts from your personal , do not use the reschedule button at this time 2. Find several dates that teams agree on. a. The home team MUST contact their FIELD ASSIGNOR and REFEREE ASSIGNOR for approval b. Send A couple dates that may work for a reschedule 9

12 **NO GAME should be rescheduled (using the button or sending information in) without approval from your (home team) FIELD and REFEREE ASSIGNOR contact your club if you don't know who they are. 3. After the NEW date, time and field are approved by assignors and both teams The team initiating (asking for) the reschedule should do the following: A. Log in to UYSA account B. Make sure your picture is Highlighted C. Click on the TEAM tab below pictures D. Click on the Tournament and Schedule Apps tab E. Click on Schedules/Game Scoring F. Click on the Reschedule Button by the game you have new information for G. Enter ALL information Date, Time including AM or PM, Field Name and Number (all in drop down menus) H. Click Send Request 4. Opposing team MUST log-in follow steps above and click Accept Request or if the information is not correct, Click Send Alternate Request and correct the information. Notify the opposing team that they must now login and Accept. Do not send any information to the state if you completed your reschedule online. It is up to the teams to follow all steps correctly the state is not approving or accepting the reschedules. The schedule will change when the teams have finished all steps correctly, No waiting UYSA reserves the right to move games back to original dates, times and locations if teams did not follow the correct steps. 10

13 How do I Club Pass? The club pass or C-pass may be used to allow a player to play on another team for which they are eligible (age and gender) within the same club. Only coaches and team managers who are rostered to the team in affinity have the access to club pass. 1. Log in to your UYSA account. 2. Make sure your picture is highlighted. 3. Under the family pictures click on the TEAM tab (half way down on the screen) A new window will pop open with 2 tabs 4. On the new tabs click on the TOURNAMENT & SCHEDULE APPS There should be 2 links on the right hand side 5. Click on VIEW TOURNY APP link on the right hand side 6. Click on the PLAYER ROSTER TAB 7. Choose the player you wish to club pass and click the Assign button in that player s row 8. Select the team and game # from the drop down and then Save Click on UYSA CLUBPASS on the top of the column to show a summary of all players going to other teams and coming to your team. If the team you are looking for does not appear in the drop down there may be one of the following problems: The team you are trying to club pass to has a full roster (a team may not deactivate a player to open up a spot for a club pass player) You may be trying to club pass a player to a division they are not allowed to play in. Examples - Premier 1 and 2 players can only club pass to the other premier bracket and to D1 regardless of the age group you are club passing to. - You cannot club pass within the same bracket in which your team participates. - Player's cannot club pass down more than 2 divisions. - Multi-rostered players can only club pass from the team they are primary rostered to, not from their secondary team. 11

14 How do I score a game? Please talk to your coach first if he wants you to enter scores. After a match has been played both the home and away coach and admins have access to input the score and comments about the match. Both coaches may input the score and specific players who scored goals. The comments will only be visible to UYSA office staff and will not be seen by the referee or the opposing coach. The referee of the game is required to input their game report (also including score) within 24 hours of the game ending. Referees will not input which players scored goals but will report the score of the game, any cautions or ejections, and any other important match information. Coaches are not to report red cards of opposing players, but may input red cards of their own players if a referee has failed to do so. Coaches and admins have 3 days (72 hours) to input goal scorers before the game is locked. UYSA cannot update goal scorers for you after the game is locked. If coaches input differing scores the match will show Contested Score (CS) and the results will not finalize until the score of the match is verified with the referee(s) of the match by UYSA office staff. Please contact us if a match shows as Contested Score for more than a week. To score a game: 1. Log into your UYSA coaches/admin account 2. Click on the schedules/game scoring link next to your team s state competition or x-league application in the Tournament Application/Game Scoring section. 3. Find the match just played and click on the score box next to your team name. 4. Enter score for home and away team (if you are entering a score you must enter the score for both teams if you just put in your score and save it will save the opposing team s score as 0 and most likely result in a CS) 5.To enter which player s scored goals choose the player from the select admin/player drop down and then choose goal from the Item drop down and add. If a player scored more than one goal repeat the process. Coaches and Admins will only have 3 days (72 hours) to input goal statistics for their players after a match is completed, after 4 days the game will be locked. UYSA office staff cannot enter goal statistics for your team after the game is locked. What if a game is terminated early? 1. If a match is terminated after fifteen (15) minutes of the second half has elapsed because of the weather or grave disorder on the part of the coaches, players, or spectators of a team that is losing at the point of termination, the match shall be considered a completed match. 2. If a match is terminated at any point because of grave disorder on the part of the coaches, players, or spectators of a team that is winning or tied at the point of termination, such team will forfeit the match If a match is terminated early due to field conditions such as failure of lights, destruction of a goal, or other conditions making the field unplayable, the match shall be rescheduled. A match rescheduled due to early termination shall be replayed in its entirety. Referee fees must be paid again at a match rescheduled due to early termination. 12

15 How Do I Print a Game Day Roster? Only coaches and administrators who are rostered to the team in the system have access to print game day rosters. To print a game day roster: -Go to -Click on Member Login on the top right of the page -Log in to your UYSA Coaching/Admin Account (should still be linked to your family) -The Screen is split in 2 halfs, Pictures on the top, Tabs on the bottom half click the TEAMS tab -A new window will pop up with 2 tabs on the new tabs Click Tournament and Scheduling Apps -There should be 2 links on the right hand side click Game Scoring/Scheduling -Click the Print Roster button by the game The print roster button will appear 24 hours before a game, Teams are encouraged to print as close to game time as possible **Coaches and Admins** This is also where you go to find contact information for the other team for reschedules, click directly on the name of the other team. NOTE: It is no longer required to print a roster for the other coach. Referees should check the date and time stamp on the roster and use the one that is most recent. Teams are strongly encouraged to print close to game time. Adding jersey numbers to your Affinity roster ** Only team coaches or team administrators can add jersey numbers to the Affinity roster** 1. Login to your UYSA account. 2. Under the Teams section of the page, click EDIT for the team you wish to add or change the jerseys numbers. 3. Click on the Tournament tab under the team name and team code. 4. Click the edit button to the right of Fall SCL/IRL, X-league or Spring SCL/ IRL, X-league. 5. Click the Player Roster tab under the team name and team code. 6. Scroll to the bottom of the page and click the Edit Player Team Info button. a. This will bring you to the page with all the players on the roster. 7. Enter in the home jersey numbers and the away jersey numbers. 8. Click the Save Changes button. Note: To print game day rosters, refer back to the Competition Soccer webpage. 13

16 Register For a Tournament, Create a Tournament Roster and Print a Photo Roster 1-Log into UYSA and your account. 2-Click on teams tab. 3-Click on apply to tournament for the team you are registering for a tournament. 4-Click on tournament roster tab and then click on create a new request. 5-Fill out the information for the tournament. 6-Click Save and Continue. -in state tournaments are usually sanctioned by USYS. -for director information you may enter unknown if you don t know. -for out of state tournament enter all information. It will then take you to your roster and here you can edit your roster. -if you need to remove a player click on the red X to remove. -click in the jersey box to add jersey numbers. -to add a guest player or coach click on add player/admin inside org, you will need to know their UYSA ID number. -when all is done with your roster for the tournament then click save and continue in the bottom right corner. 7-Click on the Comments tab then on save and continue. 8-Then click on submit application tab and you can return to your UYSA login page. You should now see the tournament listed in the tournament request section. If you are ready you can print roster (see below) or come back later and print (see below). To print the photo roster: 1-Click on edit and click on print photo roster. 2-Click on select a format and select PDF from the drop down menu. 3-Click on export and save to a file in a known location. 4-Click on team roster tab and then on print team roster. 5-Roster will open up as a PDF, print or save for later. 6-When you are printing photo or tournament rosters please make sure you print the right amount you need to take to the tournament for check in, the tournament will tell you once you have been accepted. 14

17 Concussion Policy UTAH YOUTH SOCCER ASSOCIATION CONCUSSION & TRAUMATIC BRAIN INJURY POLICY: Effective May 10, 2011, a Utah law called the The Protection of Athletes with Head Injuries Act (UCA , et seq.), requires all amateur sports organizations to adopt and enforce a concussion head injury policy. This policy will ensure the consistent and uniform implementation of a well-established concussion management program and return-to-play guidelines for the Utah Youth Soccer Association. Education A concussion is a brain injury and ALL brain injuries are serious. They are caused by a bump, blow, or jolt to the head, or by a blow to another part of the body with the force transmitted to the head. They can range from mild to severe and can disrupt the way the brain normally functions. All concussions are potentially serious and may result in complications including prolonged brain damage and even death if not recognized and managed properly. In other words, even a ding or a bump to the head can be serious. Concussion cannot be physically seen, and most occur without loss of consciousness. Signs and symptoms may appear immediately after the injury or can take hours or days to fully appear. If an athlete reports any signs or symptoms of concussion, or is observed expressing signs or symptoms of a concussion, seek medical attention immediately. Signs and Symptoms of a Concussion Signs Appears dazed Vacant facial expression Moves clumsily or displays incoordination Answers questions slowly Slurred speech Seizures or convulsions Loss of consciousness Symptoms Headaches Pressure in head Nausea or vomiting Ringing in the ears Balance problems or dizziness Blurred, double, or fuzzy vision Sensitivity to light or noise Feeling sluggish or slowed down Feeling foggy or groggy Drowsiness Change in sleep patterns Amnesia Don t feel right Fatigue or low energy Sadness Nervousness or anxiety Irritability More emotional or irritable Confusion Concentration or memory problems Repeating the same question/comment Page 2 of 5 Modified 4/16/13 What to do if someone is suspected to have sustained a concussion: o Notify a coach, parent/legal guardian, responsible adult if it is suspected someone is experiencing signs or symptoms of a concussion. Never ignore a bump or blow to the head. o If a concussion is suspected then an evaluation by a medical professional is required. A qualified health care provider can diagnose a concussion and advise when following return-to-play guidelines is warranted. o Rest! Players suffering from a concussion need time to allow the brain to heal properly. Young developing brains recovering from a current concussion are susceptible to sustaining a second concussion known as Second Impact Syndrome. This condition can cause permanent brain damage, or even death. It is important to rest until approval from a doctor or qualified health care provider is given before fully returning to activity. o Do not take anti-inflammatory medications such as Ibuprofen, Advil, Aleve, or Motrin without the approval from a doctor. These medications thin the blood, and can possibly cause more damage to a brain that has sustained a concussion. Possible consequences for returning a player to the field from a concussion too soon: * Athletes with the signs and symptoms of a concussion should be removed from play immediately. Continuing to play with a concussion leaves the young athlete especially vulnerable to greater injury. As quoted from the Center for Disease Control (CDC), A repeat concussion that occurs before the brain recovers from the first - usually within 15 a short period of time (hours, days, or weeks) - can slow recovery or increase the likelihood of having long-term problems. In rare cases, repeat concussion can result in brain swelling, permanent brain damage, and even death.

18 This more serious condition is called second impact syndrome. Keep athletes with a known or suspected concussion from play until they have been evaluated and given permission to return to play by a qualified health care provider with experience in evaluating for concussion. Remember: It s better to miss one game than the whole season. Concussion Policy Action Plan Utah Youth Soccer Associations and Clubs: Utah Youth Soccer Association (UYSA) is defined as: - UYSA paid and volunteer administrative staff. - All registered UYSA players. - UYSA-affiliated clubs and its members. - UYSA-affiliated independent teams. - UYSA-affiliated recreation programs. Education and Notification Requirements: UYSA adopts these education and notification policies for the management of concussions in youth soccer. o All coaches (paid and volunteer) must be educated in the nature and risk of concussions prior to the first practice/competition. This education shall include signs and symptoms of concussions. All players and the parent(s)/legal guardian(s) of those players are required to sign and return annually an informed consent form relating to the nature and risk of concussions. This shall include the review of the UYSA Concussion Policy which includes the signs and symptoms of concussions, and the UYSA Concussion Clearance Form. o The UYSA Concussion Policy and UYSA Concussion Clearance Form can be found on the UYSA website under Risk Management heading, or TOSH Concussion Information. Action Plan for Head Injuries: UYSA has adopted these policies for the management of concussions in youth soccer. o Any player showing signs or symptoms of a concussion shall be immediately removed from participation/ competition. o All UYSA players sustaining or suspected of sustaining a traumatic brain injury MUST use the UYSA Concussion Clearance Form in order to return to play. o UYSA will not allow any athlete who has sustained or is suspected of sustaining a traumatic brain injury to return to play until UYSA has received a UYSA Concussion Clearance Form for that athlete bearing the signature of a Qualified Health Care Provider. A Qualified Health Care Provider is someone who meets the following requirements: Licensed in the state of Utah under the Division of Occupational and Professional Licensing Act May evaluate and manage a concussion within the health care provider s scope of practice Successfully completed a continuing education course in the evaluation and management of a concussion within 3 years. Appropriate Clearance of Head Injuries: UYSA will follow these guidelines for managing concussions or suspected concussions in youth soccer. o The Qualified Health Care Provider who has cleared the athlete must provide a statement that they are trained in the evaluation and management of concussions. o Optimally, all athletes should progress through the Return-To-Play Protocol (R.T.P.P) on the UYSA Concussion Clearance Form before returning to play, but athletes may return to play without completing the protocol if indicated on the UYSA Concussion Clearance Form by a Qualified Health Care Provider. o The UYSA Concussion Clearance Form must be returned to the appropriate administration before final clearance is granted. The Appropriate Administration is the person(s) responsible for the clearance information depending on the event in which they were injured. UYSA Administrative Staff is administratively responsible for the clearances of head injuries that occur during a UYSA-hosted event. A UYSA-hosted event is defined as: 1. Spring State Cup Tournament 2. Fall State Cup Tournament 3. Presidents Cup Tournament 4. All State Select/ ODP Events 5. US Youth Soccer Region IV and National Events Club, Independent and Recreational teams are administratively responsible for the clearance of head injuries that occur during a UYSA non-hosted event. A UYSA non-hosted event is defined as none of the events listed above as a UYSA-hosted event, and may include, but are not limited to, the following events: 1. Regular season play 2. Practices, scrimmages, tryouts, and camps 3. Clubhosted events and tournaments * Coaches Requirements: o Shall be educated as to the nature and risk of concussions including continuing to play after a concussion or head injury. This education shall include signs and symptoms of concussions. Education materials are available below at no charge. o Shall educate their athletes on the signs and symptoms of concussions and encourage athletes to notify a coach if they or a teammate exhibits those signs or symptoms. o Shall immediately remove any athlete from participation/competition who is suspected of sustaining a concussion. *Shall not allow an athlete who has been removed from play because of a suspected concussion to return to play 16

19 until the athlete has received written clearance from a Qualified Health Care Professional trained in the evaluation and management of concussions. *Parents/Legal Guardian Requirements: o Shall annually review, sign, and return the informed consent form to the Utah Youth Soccer Club for which their child plays before their child participates in any practice or competition. Their signature acknowledges that they have read, understood, and agreed to abide by the UYSA Concussion Policy and UYSA Concussion Clearance Form. *Athlete Requirements: o Shall annually review, sign and return to the Utah Youth Soccer Club for which they play, an informed consent form on concussion prior to initiating practice or competition. o Shall inform a coach or parent if they, or a teammate, exhibit any signs or symptoms of a concussion. o The athlete may not return to play until he or she has been evaluated by and received written clearance to resume participation from a qualified health care provider who is trained in the evaluation and management of a concussion within the health care provider s scope of practice in accordance with this policy. *For questions about the UYSA Concussion Policy, or for printed copies of the policy: Call: , or battridge@utahyouthsoccer.net *For more information regarding sports related concussions, please visit the Centers for Disease Control and Prevention website at: 17
