As president of Westside Baseball Park, I would like to congratulate everyone for their accomplishments for making it to the State Tournament. It takes a lot of hard work and dedication to compete at this level. I hope your experience here in Mobile is full of action packed All-Star baseball. I would like to take this opportunity to wish each and every team represented the best of luck in their quest to the next level. 4 Opening Ceremony Hank Aaron Stadium Home of the Mobile BayBears Thursday June 25 th Bolling Brothers Blvd, Mobile, AL Check in - 5:00-6:00 p.m. 6:00 p.m. all coaches & players need to line up on the 3rd base side for call outs Westside Athletic Association is excited to invite all teams, coaches, players, and families to take part in the introduction of teams, dinner, and Minor League baseball game Mobile BayBears vs Tennessee Smokies. We ask that all players and coaches wear their team jersey to represent their park for pregame call outs on the field. Please have one member of your team, either coach or team mom, check in at the table in front of the Stadium between 5-6p.m. to receive your game tickets, armbands or hotdog vouchers. We ask that your coach or team mom distribute these to your family members and friends.

6 Credentials and Coaches Meeting Friday June 26 th at 8:00 a.m. Team Managers will have a mandatory meeting at Westside Baseball Park upstairs over main concession stand. All managers are asked to bring their Tournament Books for the credentials to be checked and tournament fees to be submitted. There will be no gate fees. Please bring To the meeting. 7 TOURNAMENT DOCUMENT REQUIREMENTS AND PREPARATION The following documents are required by Babe Ruth League, Inc. of each team before they are eligible to participate in any sanctioned Babe Ruth League, Inc. post season tournament. 1. Tournament documents are to be placed in a three ring binder in the order listed below. 2. These documents are to be placed in clear plastic, top loading paper savers before being placed in the binder. Page 1: Birth Document** Page 2: Certificate of Insurance (accident and general liability) Page 3: Tournament Roster * Page 4: Letter of League Eligibility Page 5: Medical Releases (4 Copies per Player) Page 6: Pictures (4 5x7 or 8x10 color or black and white) Page 7: Coaches Certification ID Cards or Certificates Page 8: Charter Page 9: Letter of Waivers Page 10: Local League Rosters (tournament team players highlighted) Page 11: Local League Schedule Page 12: Affidavits (to be signed after managers/coaches meeting) Page 13: League Boundaries Page 14: Head Coach & Assistant Coaches Heads up Concussion Certificate Page 15: Concussion Forms on each player signed by a parent or guardian. **Birth Documents: See Babe Ruth Rule 0.04 paragraph 1- A for a list of acceptable documents. Consent for treatment form on each player. MUST BE FOUR PART FORM PROVIDED BY BABE RUTH LEAGUE, INC. *1. Each player s birth document is to be placed in the front of the plastic saver with consent for treatment form in the back or under birth document. They are then placed in the binder in the same order as they are listed on the Tournament Team Roster. Dropping out of the tournament (once committed), Failure to complete a full regulation game, Failure to attend the opening ceremony & leaving the tournament early will result in a $300 fine and your league will not be considered to host a state tournament for two years.

9 Snacks Bug Spray Sunblock DON T FORGET TO INCLUDE YOUR PARK RULES. Directions to Field From (I65 North), Head South on I65, Take exit 3- Airport Boulevard, Head West Take a Left onto Schillinger Road. At your fourth light turn left onto Hitt Road. The entrance to the park is the first road on your left. The tournament is being held in the back of the park. Proceed until you reach the very back of the park. Park Rules 1. No coolers will be allowed in the park. A water cooler will be provided for each team in their dugout. Each team may bring one cooler, but it must remain in the dugout. 2. No outside food may be brought into the park. 3. Any spectator or coach who becomes disruptive will be asked to leave the park immediately. 4. Teams will be allowed 15 minutes to warm up on the field between games. 5. Teams are allowed to bring fans with quiet generators. DRUGS, ALCOHOL AND TOBACCO Alcoholic beverages and drugs are strictly prohibited from all recreational facilities under the Jurisdiction of the City of Mobile, Alabama Parks and Recreation Department. Westside Athletic Association has adopted a zero tolerance policy for drugs and alcoholic beverages, therefore, anyone, to include, players, spectators, coaches and umpires who are found to be using or under the influence of drugs or alcohol at Westside Ball Park will be subject to a minimum thirty (30) day ban from all recreation facilities. Westside Ball Park is a Smoke Free campus CONDUCT Westside Athletic Association has a zero tolerance policy for any acts of violence, therefore, anyone, including coaches, umpires, spectators or players, who participate in or are a party to any act of violence, to include verbal or physical, while participating or observing ball games at Westside are subject to being banned from the facilities. Abusive language, profanity and displays of vulgarity are also prohibited and are subject to be banned from the facility. VEHICLES Automobiles, trucks and other vehicles shall be operated at safe and reasonable speeds and remain on roads designated for their use. Skates, skateboards, and bicycles are not allowed. ADVERTISING Advertising will not be allowed on park property unless specifically approved by Westside Athletic Association, and only in the event of a substantial contribution.

10 ALABAMA BABE RUTH LEAGUE Concussion Information Form A concussion is a brain injury and all brain injuries are serious. They are caused by a bump, blow, or jolt to the head, or by a blow to another part of the body with the force transmitted to the head. They can range from mild to severe and can disrupt the way the brain normally works. Even though most concussions are mild, all concussions are potentially serious and may result in complications including prolonged brain damage and death if not recognized and managed properly. In other words, even a ding or a bump on the head can be serious. You cannot see a concussion and most sports concussions occur without loss of consciousness. Signs and symptoms of concussion may show up right after the injury or can take hours or days to fully appear. If your child reports any symptoms of concussion, or if you notice the symptoms or signs of concussion yourself, seek medical attention right away. Symptoms may include one or more of the following: -Headaches -Amnesia - Pressure in head -Nausea or vomiting -Neck pain -Balance problems or dizziness -Blurred, double, or fuzzy vision -Sensitivity to light or noise -Feeling sluggish or slowed down -Drowsiness -Change in sleep patterns - Don t feel right (Fatigue or low energy) -Sadness, nervousness, or anxiety -Irritability -More emotional -Confusion Concentration or memory problems (forgetting game plays) -Repeating the same question/comment Signs observed by teammates, parents and coaches include: -Appears dazed Shows behavior or personality changes -Vacant facial expression can t recall events prior to hit -Confused about assignment can t recall events after hit -Forgets plays Seizures or convulsions -Is unsure of game, score, or opponent any change in typical behavior or personality -Moves clumsily or displays incoordination Loses consciousness

12 What can happen if my child keeps on playing with a concussion or returns too soon? Athletes with the signs and symptoms of concussion should be removed from play immediately. Continuing to play with the signs and symptoms of a concussion leaves the athlete especially vulnerable to greater injury. There is an increased risk of significant damage from a concussion for a period of time after that concussion occurs, particularly if the athlete suffers another concussion before completely recovering from the first one. This can lead to prolonged recovery, or even to severe brain swelling (second impact syndrome) with devastating and even fatal consequences. It is well known that adolescent or teenage athletes will often fail to report symptoms of injuries. Concussions are no different. As a result, education of administrators, coaches, parents and students is the key to a student-athlete s safety. Concussion Policy: Any health care professional or certified coach may identify concussive signs, symptoms or behaviors of an athlete during any type of athletic activity. Any athlete who exhibits signs, symptoms or behaviors consistent with a concussion shall be removed from the contest and shall not return to practice or play until a medical release has been issued by a medical doctor. If you think your child has suffered a concussion: Any athlete even suspected of suffering a concussion should be removed from the game or practice immediately. No athlete may return to activity on the same day he/she sustained an apparent head injury or concussion, regardless of how mild it seems or how quickly symptoms clear. The athlete may return the following day or anytime thereafter with written clearance from a medical doctor. Close observation of the athlete should continue for several hours. You should also inform your child s coach if you think that your child may have a concussion. Remember it s better to miss one game than miss the whole season. And when in doubt, the athlete sits out. This form is required by Alabama Law established in June The form was revised in April 2012, coinciding with the current AHSAA Concussion Policy. This form was revised in January 2013 and adopted for use by the Alabama Babe Ruth League (ABRL). I have reviewed this information on concussions and am aware that a release by a medical doctor is required before a player may return to play under this policy.
