1 2017 National Mock Trial Forms Overview Included in this packet are forms you will need for the 2017 National High School Mock Trial Championship. Please note that there are other forms that teams will use for the competition, namely scoring and timekeeping forms, which are included in other handbooks. Below is a brief explanation of the purpose and use for each form included in this packet Team Rosters: Note that there are two separate rosters: one for the Plaintiff and one for the Defense. The forms include a place to indicate the team code, which will be assigned when teams register in Hartford. For each round of competition, you will provide copies of your filled out roster to the presiding judge, the scoring judges, and opposing counsel, for a total of 5 copies per round. It s important to bring the correct number of copies with you to the competitions as staff will not have the ability to make photocopies of any of your forms. We recommend bringing 25 copies of each roster Star Witness & Attorney Certificates: During each round of competition, team members choose who they think was the best witness and attorney from the opposing team and present them with a certificate at the end of the competition round. Copies of the forms will be included in the team s registration packet. Teams are responsible for ensuring they bring the certificates with them to each round of competition Photo & Video Release Form: During the course of the competition, staff and sometimes news media will take photos and/or videos of students participating in the competition. This signed form signifies that Civics First and the National High School Mock Trial Championship, Inc. have been given permission to use photographs and/or videos taken at any of the 2017 competition events for educational and/or promotional materials and publications. Each student (and a parent/guardian for students under 18) must fill out and return this form at their regional competition. The Larry Bakko Professionalism & Civility Award: The nomination ballot will be included in your team s registration packet and will need to be filled out and returned to your Presiding Judge at the end of the 4 th round of competition. Criteria for this award are described in full on the information sheet included with the ballot

6 2017 Photo & Video Release Form This signed Photo & Video Release Form signifies that Civics First and the National High School Mock Trial Championship, Inc. have been given permission to use photographs and/or videos taken during the 2017 National High School Mock Trial Championship for educational and/or promotional materials and publications, in both print and online. All photographs and/or videos used will be portrayed in a positive manner. Each team member must fill out and sign a release form and have a representative from your team turn in all signed forms at the Registration Desk when your team registers on May 10 or 11, 2017 in Hartford, Connecticut. Team members under the age of 18 must also have the form signed by a parent or guardian. I, give permission for photographs and videos taken during the 2017 National High School Mock Trial Championship to be used by Civics First and the National High School Mock Trial Championship, Inc. for educational and/or promotional purposes. By signing this form, I understand that I am waiving any rights of compensation or ownership to any photographs and/or videos. 7 Larry Bakko Professionalism and Civility Award The Larry Bakko Professionalism and Civility Award is sponsored by the National High School Mock Trial Championship and was created to highlight the importance of professionalism among teams participating in our state tournament while honoring Larry Bakko s passion and dedication for Mock Trial. The Team Professionalism Award nomination ballot will be included in your team s registration packet. During the competition experience (inside and outside the courtroom), your team and coaches will observe other participating teams and, using a subjective process, will nominate another team to be considered for the Larry Bakko Professionalism and Civility Award. Teams may take into account the behavior of students, coaches, and spectators associated with other teams when making a nomination for this award. Teams may not campaign for this award. When discussing this award with students, it is hoped that coaches will emphasize the importance of professional behavior inside and outside the courtroom throughout the competition. The discussion of this award will provide you with an excellent opportunity to review the Code of Ethical Conduct and the Rules of the Competition that deal with ethics and decorum. Discussions about this award will provide coaches the opportunity to discuss the high standards of professionalism attorneys are held to as members of the Bar. The qualities that teams nominated for the award must demonstrate inside and outside the courtroom include: A professional demeanor Civility Integrity Honesty Fair play Respect for the competition Respect for fellow competitors Respect for volunteers and all associated with the program inside and outside the courtroom throughout the competition Each team must submit one team ballot nominating another team for the Professionalism Award. A team may not nominate itself. A team may nominate another team that they competed against in one of their rounds, or not, as it sees fit. The NHSMTC Board of Directors will not recommend a particular process by which each team determines their nomination, but suggests that coaches take student input into account when completing the nomination ballot. How a team reaches the decision to nominate another team for this award is completely within the purview of that team. All ballots are due before a team leaves the courthouse and after the fourth round on Saturday. Ballots will be submitted to the courtroom liaison and turned in with the scoresheets. Do not exit the building before submitting the Larry Bakko Professionalism and Civility Award nomination ballot.
