1 REGISTRATION OPTIONS Welcome to the Summer Oztag Season. This Summer Season we are offering two methods of registration of your team, 1. An online registration System; or 2. The Traditional Registration Form. We will use the online registration system that was used for the recent winter season. We understand that some Team Organisers and/or Players may want to continue to use our method of completing the Registration Form and then paying the registration fee s in person or through bank deposits, so we will also continue with these methods as well as offering the online system. The online system enables Team Organisers to register their team and then players to register into the team. This process simplifies the Team Organiser s job and makes it easier for players to register and pay their registration fees. Please note. Sporting Pulse levies a 3.9% surcharge on the use of its. (this = $4.29 extra) when registering using this method.. TEAM ORGANISERS ONLINE REGISTRATIONS If you organised a team during the Winter 2018 competition you should have received an from us prompting, you to register your team for Summer 2018/19. Please use this link as it will simplify the process substantially. If you did not receive this or require your team code, please send us an and we will forward you a new link. To register a new team please go to: and select New Team Registration, then select Continue when it appears. Please complete the Team Registration form, filling in all mandatory fields ( ) Please pay attention to your address and contact number as we will use this to communicate with you. Once you have registered the team you will receive an with a link you need to supply to your players. This enables them to register themselves into your team, you will also need to register yourself into the team as well. Please note; if you are registering online, each player must pay at the time of registration. TEAM ORGANISERS TRADITIONAL REGISTRATION If you chose not to complete your registration online, you need to complete the registration paperwork in this kit. To be clear, you can either register online with the above link OR you can complete the registration paperwork as per past seasons. The paper version of the registration form is at the back of this pack and will need to be completed and ed or handed back to the Oztag office admin@actoztag.com.au You cannot assume that playing in previous competitions will guarantee your team a spot in any competition. In order to register your team registration form, with signatures and money, must be handed in at Registration or online. Please note that the Committee reserves the right to reject the registration of ANY player, without providing any reason.

You can also the completed registration form to admin@actoztag.com.au and deposit entry fees directly into bank account (please put relevant details on the deposit so we can identify who it is for). (e.g. Last name and team name) NEW FEE STRUCTURE NOW INDIVIDUAL FEES, NOT TEAM FEES The registration fees have been reformatted this season to an individual fee. Please note this has been feedback we have received from Oztag members, due to many team delegates feeling the burden of organising monies from each player and or having to pay the full team amount and losing money. 3 SHORTS PRICE RISE ACT Oztag have absorbed cost increase for shorts over the recent seasons, we now need to introduce a small increase to the shorts to maintain our financial stability. We hope you understand the need for us to ensure we continue to thrive and provide you with competition at a high standard. ACT & Southern Regions are one of the only Oztag regions not to have raised the shorts costs when brought in by AOSA 2 years ago. Shorts have not been raised in over 10 seasons, shorts will now be $30.00 in line with all Oztag regions. 4 REPRESENTATIVE RULES In order to try to maintain the competitiveness of our competitions, and to give all teams a fair go, we have rules in place on how many Representative players that are permitted in teams from Division B down. These rules are in place for everyone s benefit but at times may impact on some teams in an unforeseen manner. If you have any question regarding these rules, or how they may apply to your team or players please don t hesitate to contact us. We are always open to suggestions on improvements or tweaking of these rules, but they must be to everyone s benefits, so if you have any idea s please come and discuss them with us. ALL VENUES A Grade B Grade C Grade D Grade Novice Grades Max Rep Tag Players Unlimited A Rep player is a person who has represented ANY Oztag competition at the previous Senior State Cup (2018) or National Titles (2017) or is a current State or National player (including Indigenous representative teams). Rep Player Exceptions The following rep players only count as ½ a player towards the total number of rep players: Men s players aged Over 40 (Over 45 do not count) Females players aged over 30 (Over 35 do not count) Rep players under 18 (subject to the below exemption) Players selected in an ACT Development or 2 nd representative team DO NOT count towards the rep player total Players U/18 who get selected for a representative team will not count towards the rep player total until they have played in at least 2 senior representative tournaments provided they stay in the same team. As soon as they leave their original team they will be counted as a rep player towards the total. You must indicate on the registration if a player is a Rep Player. This means a Rep Player at ANY OZTAG association. Any team found not identifying any rep players will be subject to sanctions as the committee decides; including re-grading of the team or withdrawal of the player/s in question. FILL IN PLAYERS We understand that at times it can be difficult to have all players turn up so to assist teams we allow fill in players. The rules are: Fill ins can only be used after Round 3. Each fill in player MUST pay a $5 fill-in fee, which covers filling in for 1 game only. They MUST report to the ground controllers, register and sign on BEFORE playing. If they wish to play another game they MUST report to the committee room and pay a further $5 and sign on BEFORE the game The player can play a MAXIMUM of 2 games as a fill in player after which they must pay the balance of a full registration. ACT Oztag reserves the right to reject a fill in player for any reason. ACT Oztag Association reserves the right to alter these conditions as we see fit.

5 NOTES When filling out Registration forms please take note of the following: All registration forms must be completed. Players must be 15 years old before they can register and play in an adult team. Photo ID may have to be produced when signing on for finals or when requested by an official. All teams must consist of a minimum of 11 players. NO REFUNDS will be given. There is a $15 administration fee for players withdrawing after being registered but having played no games. Registrations can be made at any time during the season. Players MUST play a minimum of 5 games to qualify for the finals series. Only players registered in a team may play for that team. Penalties for teams playing players not registered in their team will be severe. Should you need substitute players please see the competition organiser BEFORE THE GAME STARTS. All teams may play at any time slot during the season, but no timeslot guarantee is given. Players may register in more than one team on the one evening, BUT no guarantee is given that games will not clash, please advise the office if you are in more than one team, so they can do their best to cater for your request. The registration of ANY player in ANY team is at the discretion of the committee. You must indicate if a player is a Rep Player or not. This means a Rep Player at ANY OZTAG association. Any team found not identifying rep players will be subject to sanctions as the committee decides, including re-grading or withdrawal of the player (no refunds will be issued). 6 REGISTRATION DEAL If you enter a fully completed* team registration with at least 12 players and fully paid by Friday 29 th September 2018 you will receive a free football. * A fully completed team registration means: All players must complete address, phone number and Date of Birth on registration form in the correct fields. All team details completed at the top of the form, including at least one contact phone number. Fully paid means; you pay the full amount in one transaction. Having part payments from individual players through EFTPOS, Cash and Direct Deposit means a lot of extra work for us. If you can get all the money together and pay it, you get the discount. The above bonus also apply for ONLINE registrations and payments. For ONLINE registrations the players must be registered and fully paid for in accordance with the above numbers and dates for the organiser to receive the discount. There will be NO exceptions to this requirement to receive the bonus of a Free Football. If in any doubt, please call us on

7 PROCEDURES FOR REPORTING A CLAIM 1. The injured player must report any injury sustained whilst playing the game of Oztag to game officials/ Ground Controllers on the night the injury occurred, they will then fill in the required paperwork for your claim. 2. Player to contact Sports Underwriting Australia on or austclaims@aig.com, to place a claim of Injury. Sports Underwriting Australia will explain anything you need to know and a Claim form will then be forwarded to you. 3. Player will not be able to make a claim if they do not have an Oztag Registration Number 4. Private Health Insurance MUST be claimed on first before submitting a claim to Sports Underwriting Australia. 5. Medicare Costs for the Gap cannot be claimed for. Unfortunately, the Health Act does not allow for any general Insurer to provide any cover in regard to medical expenses for which any Medicare benefit is payable. This includes what is known as the Medicare gap. AN EXCESS OF $200 APPLIES TO EACH CLAIM Summary of Insurance Coverage Policy Limit for Medical is $2,500 per injury per annum. Non Medicare medical benefits Covers 50% of the invoice. Physiotherapy benefit is 75% of the invoice. Any expenses must be incurred within 12 months of the insured person sustaining the injury. We agree to abide by all rules as determined by ACT Oztag. We further warrant that we are in a fit state of health to play and understand that while risk management strategies are in place at our venue, I participate in Oztag knowing that injuries may still occur. We also understand that in no event will ACT Oztag be liable for any damages for injury to any players, or loss of income caused by any injury to any players, sustained while playing in any competition organised by ACT OzTag. Any player that has not signed or paid their Individual Registration Fee understands that they are not a Registered player and cannot claim Insurance. 10 CONDITIONS OF PLAY, INCLUDING PLAYER INSURANCE COVER Appendix 10 The Oztag competition is organised and run by The Australian Oztag Sports Association Inc ( Oztag ). Oztag offers to provide access to the competition to Players on provision by Players of the registration fee and agreement to these Conditions of Play. As a Player, I agree and understand that, subject to these Conditions, 1. I play at my own risk. If I have not signed the registration form, I will not be covered by any insurance and I play completely at my own risk. Qualification of any team for the semi-finals will depend upon validly signed and completed registration forms for all Players being submitted. 2. I will not wear football boots with screw in type studs (metal, nylon or spikes). 3. I will wear shoes suitable to playing the sport whilst playing - I will not play in bare feet. 4. I will not wear jewellery (which includes men s watches) while playing. 5. I will not talk to the referee unless I am the captain of a team. 6. I will not sledge either the opposition or my own team members. This is a major offence and will result in me being sent off. 7. I must comply with the referee s directions and decisions. 8. I must play in official Oztag shorts. 9. I must be 15 years of age to play in in adult competition. 10. Any injury I sustain must be reported to the first aid officer on the night of the injury. Claims may be rejected if the injury is not reported on the night. 11. All teams must wear numbered shirts with numbers either clearly identified on both sleeves or with one large number on the back of the shirt. 12. Changes may occur to the competition during the competition. Information regarding any changes will be published on the website ( or on the ACT Oztag FaceBook page. Teams & players are recommended to check these notices as these changes may affect them. Injury Insurance & Liability 1. Oztag has arranged injury insurance cover ( Player Insurance ) for Players through Sportscover (Australia) Pty Limited ACN under authority from American Re-insurance Company ARBN Policy terms are available to all Players and will be provided on reasonable notice to Oztag. A summary of the Player Insurance cover is set out below. 2. Players acknowledge that there is a real possibility of injury in playing in the Oztag competition. The most common injuries are ankle and knee injuries and broken bones but may include any form of injury, including in rare cases, death. 3. Players who desire more comprehensive or other insurance cover must obtain this cover prior to playing. It is up to each Player to determine whether they require further cover in addition to the Player Insurance before they play. As a Player, I warrant that: 1. I am in a fit state of health to play. 2. I will not be under the influence of alcohol or drugs that may adversely affect me while playing. 3. I have no pre-existing medical or other conditions which might cause me injury or be aggravated by playing. 4. I have read and understood these Conditions of Play. 5. I will comply with these Conditions of Play. I acknowledge that if this information is incorrect, the Player Insurance may be voided. Release 1. As a Player, I acknowledge that the Player Accident Insurance may be my only recourse to recover damages for personal injury. 2. I hereby RELEASE Oztag and its associated parties, including without limitation, its members, officers, servants and agents, contractors and referees including any other person engaged by Oztag in any capacity ( the Oztag Parties or, individually, Oztag Party ) from all liability for loss of any nature, including without limitation, all personal injury, including assault or battery, economic loss sustained from personal injury, damage to property, whether arising out of negligence, breach of any statutory duty, breach of contract or at common law. 3. I acknowledge and represent that any Oztag Party regardless of their connection with me may rely on this release. This release from liability applies unless specifically prohibited by statute. 4. Notwithstanding paragraph 14, Oztag expressly excludes all liability for death or personal injury to any Player in so far as such liability is capable of being the subject of any implied warranty under section 74 of the Trade Practices Act 1974 (Cth) and any other state or territory legislation having a similar purpose. 5. Paragraph 10 and the Important Notice on the registration form constitutes a risk warning for the purposes of the Civil Liability Act 2002 NSW and any other legislation having a similar purpose. 6. These Conditions of Play represent the entire agreement and understanding between Oztag and the Player and bind the Player s heirs, successors and assigns. 7. Oztag will not waive or vary any of these Conditions of Play unless it does so by notice in writing. INSURANCE COVER INFORMATION SHEET (effective 22nd June 2011).
