Nestled at the base of the Blue Ridge Mountains and just west of Charlottesville, Old Trail Golf Club is the centerpiece of the spectacular Old Trail community. Old Trail Golf Course offers multiple tees to provide a range of course lengths pleasurable for all players. Plus, the Zoysia fairways offer a perfect lie and the bent grass greens are the purest putting surfaces available today, making this a truly unique golf experience. In addition to the wonderful design and immaculate conditioning of the course, our professional team is committed to providing all golfers a superior level of customer service. Old Trail Golf Club is proud to be the home of The Kandi Comer Golf Academy, located on site and offering players of all skill levels a variety of golf instruction options to help them improve their play with an emphasis on the basic fundamentals of the game including a special emphasis on the short game and course management. Within the Old Trail Golf Clubhouse is our own full service restaurant and banquet facility called Restoration and Restoration Hall. These two fun and inviting venues offer the best indoor and outdoor seating with superior panoramic views of the mountains and golf course. Restoration provides an affordable and inviting atmosphere for our golfers, Old Trail Residents and Crozet Community. Restoration Hall is available for rentals by all to host your next birthday party, family event, wedding, dinner or group outing.

2 2017 Old Trail Card Program Old Trail Golf Club is excited to introduce the Flexi Card and the Unlimited Card as a way to enjoy not only the golf experiences, but other unique privileges from several businesses within the Old Trail Community. Unlimited Card: $205 Individual (monthly) / $285 Family (monthly) Benefits: Unlimited golf at no extra charge (not including cart fee) No tee time restrictions Walking is permitted anytime except Friday-Sunday and holidays starting May 15 th through October 31 st before 11:00AM. 20% discount on Pro Shop merchandise (not including balls, bags, clubs and sale items) 10% meal discount in Restoration Restaurant & Old Trail Snack Bar Discounted guest fees (check the pro shop for availability). Optional VSGA Handicap fee of $25 that will be billed annually Additional cardholder s privileges offered throughout the year 25% off on range balls Flexi Card Benefits: $500 (Annually) Benefits: Up to 50% off Posted Golf Rates (check with Pro Shop for current rates). 4 Terms and Conditions 1. Rules and Regulations 1.1. The Cardholder term is for 12 months from the date your application is accepted. Monthly dues will be charged on the first day of each month and the dues will be for the month in which the charge occurs. The undersigned understands that he/she is obligated to keep a valid approved credit card on file with the Club at all times for monthly dues and that the undersigned is responsible for any amounts that are not paid by the credit card company. If at any time the cardholder s dues and/or other charges of the undersigned are 60 days past due, the undersigned understands his/her playing privileges will be suspended until the account is paid in full RENEWAL POLICY; 60 days prior to cardholder s expiration term he/she must inform the club of their desire to terminate, if not then the card will be automatically renewed for the next 12-month cycle from that date The rates stated above are subject to change All cardholders are required to present the Unlimited Card and valid ID at time of check in Old Trail Golf is not responsible for lost or stolen Unlimited Card; a replacement fee would be assessed for a new card Cancellation Policy: No-show and non-cancellation of a reservation within the 24-hour cancellation period is subject to a $25 charge to the credit card number on file. Should there not be an active credit card on file; the Unlimited Card will be suspended until the fees are paid in full. 2. General Understanding 2.1. The undersigned understands the information included in this packet concerning cardholders at Old Trail Golf Club and herby agrees to the cardholder s benefits, rates and Rules and Regulations as posted at the Club. The undersigned further acknowledges that the golf course and other facilities are open to the public and host daily fee golfers, corporate and charity outings on a regular basis and that the golf course may not be available each time the undersigned chooses to play The undersigned hereby acknowledges receipt of these terms and conditions, has read and understood them, and agrees to be bound by them. 3. Delinquent Account 3.1. If the cardholder s account is delinquent, the Club may at its option take whatever action it deems necessary to effect collection, including without limitation, suspension or termination of a cardholder s playing privileges. If the Club commences any legal action to collect any amount owed by the cardholder or to enforce any other liability of the cardholder to the Club, and if judgment is obtained by the Club, the cardholder shall also be liable for all costs and expenses of such legal action and reasonable attorneys' fees, including any fees required in connection with appellate proceedings. 4. Immediate Family Members 4.1. In addition, by executing this Agreement, Applicant hereby voluntarily assumes all risks of accident or damage to Applicant s person or property, and the person or property of Applicant s family and guests, arising out of or relating to the use or occupancy of the facilities including but not limited to errant golf balls, operation of a golf cart, and adverse weather
