2 Cost $725: Includes Lodging, lift tickets, training lanes, coaching, transportation, meals starting with dinner Thursday evening, May 24th and ending with lunch on Monday, May 25th. Payment and registration are due by May 15, 2018 Lodging Silverthorne La Quinta , rooms have been reserved for the athletes. Athletes attending the camp are expected to stay with the group. Meals Breakfast, mid day snack, lunch and dinner are included with the lodging package. Lift Tickets Lift tickets are included in this camp. Please indicate on the form below if you will not need a lift ticket. A discount is given for those that have a pass. Vail Resort Employee dependent passes are available for this camp, however there is a cost of $10 per day to use this pass. Transportation Transportation in the Summit County area is included in camp cost. Athletes will need to make arrangements to get to and from Silverthorne. If enough athletes need it, a vehicle may be provided to/from the Denver airport at some additional cost. Otherwise, athletes can use Colorado Mountain Express, a van service to/from the airport. Athletes should make arrangements to arrive at the Denver airport no later than 2:00 PM on May 24th in order to arrive at the orientation meeting. Athletes should depart Denver airport no earlier than 5:00PM on May 28th to ensure that they won t miss their flight. 5 U.S. Ski & Snowboard Alpine Development Department Team Agreement Team Rules Members of U.S. Ski & Snowboard teams attending competitions on the national quota, traveling with or as part of a competition trip organized by the USSA Regional Development Staff, or attending any training camp or project as part of a U.S. Ski & Snowboard Group are required to abide by the U.S. Ski & Snowboard Code of Conduct. Additionally, team members agree to: Ø Pay in full the project fee before the first day of arrival of the project, unless a payment plan is worked out at the time that the team is named; Ø Fully participate in the organized accommodations; Ø Attend all team meetings and training activities on time and fully prepared; Ø Observe any curfew established by the coaching staff; Ø Not use or possess illegal drugs; Ø Not illegally use or possess alcohol; Ø Not use or possess alcohol in the team accommodations, the team vehicles, or in the presence of team members who are not of legal age; Ø Reside in the room to which they are assigned and will share responsibility with their roommates for damages, cleaning charges, and rule violations that occur in the room when it is not possible to establish individual responsibility; Ø Observe any additional rules established by the head coach of the project Ø Be filmed and photographed and to have his/her image and voice otherwise recorded in any media by the U.S. Ski & Snowboard s official photographer(s), film crew(s), and video crew(s) and by any other entity authorized by USSA and grants to U.S. Ski & Snowboard the irrevocable, fully paid up, worldwide right and license to use, and to authorize third parties to use, in all media, Athlete name, likeness, picture, voice, and biographical information for: (1) news and information purposes; (2) promotion of U.S. Ski & Snowboard and the specific competitions in which Athlete competes; (3) promotion of USSA through its fundraising mission; and (4) to support U.S. Ski & Snowboard s educational and philanthropic efforts through the production of educational and training videos, DVDs and other media. Violations of these rules may result in suspension from the team and in loss of the opportunity to participate in future teams or quota team preparation or competition projects. Agreement I, the undersigned U.S. Ski & Snowboard competitor, have reviewed these rules and the U.S. Ski & Snowboard Code of Conduct, and agree to abide by it and all applicable rules and procedures during in my participation in any national training or competition project during the current season. I agree that a violation of the rules may result in my immediate suspension from a team, and in the loss of all team services including representation at Team Captains and Jury meetings, training, housing, meals, and transportation. I realize that I will receive no refund for the unused portion of these services. If suspended, I will be prepared to make whatever arrangements are necessary in order to continue to compete in the event and/or return home. I realize that actions that are violations of the law may result in my arrest and that it will be my responsibility in such a case to notify my parents or legal guardians and/or to make such arrangements as may be necessary for my release and return home. This agreement shall be valid for all projects in the training and competition season. Athletes under 18 years of age must have the following section signed by either parents or legal guardians, if applicable. We, the undersigned, parents or legal guardians of the above Athlete, having read and understood the above, agree to allow our child to participate in U.S. Ski & Snowboard training or competition teams. We understand and agree that if our child violates these rules and procedures, he/she may be immediately suspended from the U.S. Ski & Snowboard project with the loss of all team services and without refund of fees paid. We agree that in any case where our child s actions result in his/her arrest, that sole responsibility for notifying us rests with our child and the local law enforcement officials, and that we, and not U.S. Ski & Snowboard or U.S. Ski & Snowboard Rocky/Central Region, are solely responsible for such actions as may be required to secure his/her release and return home. 6 U.S. Ski & Snowboard Regional Alpine Development Department Hold Harmless and Indemnity Agreement For and in consideration of the provision of ski racing instruction and training by U.S. Ski & Snowboard, I ( athlete ), and if athlete is under the age of 18 years, the parents or legal guardians of athlete, do hereby covenant and agree on behalf of ourselves, to hold harmless, release, defend, and indemnify U.S. Ski & Snowboard and any of its employees, or volunteer workers of and from any and all claims arising from athletes participation in U.S. Ski & Snowboard Regional events, including, but not limited to racing instruction, racing competition, or traveling to and from races or training camps. We specifically represent that we have read and have understood that this release is intended to serve as a general release of all legal claims against U.S. Ski & Snowboard and U.S. Ski & Snowboard /Regional Alpine Development projects, is to be construed as broadly as possible in favor of U.S. Ski & Snowboard and releases U.S. Ski & Snowboard from all claims, including, but not limited to, those arising from the negligence of U.S. Ski & Snowboard itself. The racer must carry proof of this insurance and have it available at each race or camp so that prompt medical care can be obtained, if ever needed. Agreement We have read and understood the Insurance Policy statement. The insurance policy listed on the Medical Release meets the requirements of the U.S. Ski & Snowboard Insurance Policy and will be maintained in force while the competitor is involved in a U.S. Ski & Snowboard camp or team or while participating in any event on a U.S. Ski & Snowboard quota. We agree that we are responsible for any and all medical charges and we agree that we will promptly reimburse U.S. Ski & Snowboard for any expenses that they or their coaches incur on behalf of the competitor. PLEASE READ CAREFULLY BEFORE SIGNING. THIS IS A RELEASE OF LIABILITY & WAIVER OF CERTAIN LEGAL RIGHTS. 1. The person for whom this season pass is purchased shall be referred to hereinafter as Holder. The Undersigned means only the Holder when the Holder is age 18 or older OR it means both the Holder and the Holder s parent or legal guardian when the Holder is under the age of 18. The Undersigned agree and understand that skiing, snowboarding and using ski area facilities, including the lifts, for any purpose (hereinafter the Activity ), can be HAZARDOUS AND INVOLVES THE RISK OF PHYSICAL INJURY AND/OR DEATH. 2. The Undersigned are advised that a person using any of the facilities of the ski area is considered a skier. The Undersigned acknowledge and understand the dangers and risks of skiing and understand that the Holder, as a skier under Colorado law, ASSUMES ALL INHERENT DANGERS AND RISKS as provided by the Colorado Ski Safety Act (hereinafter Act ). 3. The Undersigned further understand and agree that in signing this Agreement, the Undersigned are expressly acknowledging and assuming additional risks and dangers that may result in property damage, physical injury and/or death above and beyond those outlined in the Act, including but not limited to: Falling; avalanches; cornices; suffocation; crevasses; equipment failure; equipment malfunction; equipment damage; Holder s improper use of equipment; Holder s use of his/her own personal equipment; Holder s failure to maintain or otherwise upkeep his/her own personal equipment; slick or uneven surfaces; surface and sub surface snow conditions; slipping; tripping; loss of balance; rugged mountainous terrain; bumps; moguls; ice; open water; variations in terrain; tree wells; downed timber; stumps; forest growth; rocks of various sizes; holes; debris; marked and unmarked obstacles; drainage channels; varying visibility; unmaintained trails; path and/or trail obstructions; unmarked roads and/or trails; Holder or another acting in a negligent manner that may cause and/or contribute to injury to Holder or others, such as selecting terrain that exceeds his/her ability and not acting within such ability; Holder s failure to comply with signage; collisions with natural or man-made objects or collisions with other people; lift towers; terrain park elements and features; falling objects; encounters with snowmobiles and/or other motor vehicles; becoming lost or separated; lack of shelter; lift loading, unloading, and riding; storms, lightning, hail, snow and other adverse weather; limited access to and/or delay of medical attention; Holder s health condition; strenuous activity; fatigue; exhaustion; dehydration; hypothermia; high elevation; altitude sickness; frostbite; & mental distress from exposure to any of the above. 4. Pursuant to Colorado law, Holder assumes the responsibility of maintaining control at all times while engaging in the Activity. Holder is responsible for reading, understanding and complying with all signage, including instructions on use of lifts. Holder must have the physical dexterity and knowledge to safely load, ride and unload the lifts. Holder assumes the risks of riding the lifts and engaging in activities accessible from the lifts. Further, the Undersigned understand that a minor Holder may use the ski lifts without an adult present. The Undersigned are advised that snowmobiles, snowmaking, and snow-grooming equipment may be encountered at any time, and the Undersigned recognize that falls and collisions occur and injuries are a common and ordinary occurrence of the Activity. 5. The Undersigned acknowledge and understand that the description of the risks listed above are not complete and that participating in the Activity, whether or not described, may be dangerous and may also include risks which are inherent and/or which cannot be reasonably avoided without changing the nature of the Activity. By signing this document, the Undersigned recognize that property loss, injury, serious injury and death are all possible while participating in the Activity. RECOGNIZING THE RISKS AND DANGERS, THE UNDERSIGNED UNDERSTAND THE NATURE OF THE ACTIVITY AND VOLUNTARILY CHOOSE FOR HOLDER TO PARTICIPATE IN AND EXPRESSLY ASSUME ALL RISKS AND DANGERS OF THE ACTIVITY, WHETHER OR NOT DESCRIBED ABOVE, KNOWN OR UNKNOWN, INHERENT OR OTHERWISE. 6. Additionally, in consideration for allowing the Holder to participate in the Activity, THE UNDERSIGNED HEREBY AGREE NOT TO SUE Arapahoe Basin Ski Area or any of their respective successors in interest, affiliated organizations and companies, insurance carriers, agents, employees, representatives, assignees, officers, directors, and shareholders; and the U.S. Forest Service (each hereinafter a Released Party ) for any property damage including but not limited to equipment damage, injury or loss to Holder, including death, which Holder may suffer, arising in whole or in part out of Holder s participation in the Activity. By agreeing not to sue, the Undersigned are releasing any right to make a claim or file a lawsuit against any Released Party. Also, the UNDERSIGNED AGREE TO HOLD HARMLESS AND RELEASE EACH AND EVERY RELEASED PARTY FROM ANY AND ALL LIABILITY and/or claims for injury or death to persons or damage to property arising from Holder s participation in the Activity, including, but not limited to, those claims based on any Released Party s alleged or actual NEGLIGENCE or BREACH of any CONTRACT and/or express or implied WARRANTY. 7. By execution of this Agreement, the Undersigned also AGREE TO DEFEND AND INDEMNIFY/REIMBURSE each Released Party from any and all claims of the Undersigned and/or a third party arising in whole or in part from Holder s participation in the Activity. 8. Holder understands that entering or skiing in a CLOSED area is illegal. The ski area operator shall have the right to confiscate or revoke the privileges conferred by the pass where, in the sole judgment of the Ski Area s representative, the Holder: 1) acts in any manner that endangers or may endanger the safety of Holder or any other person; 2) violates the law; 3) provides ski lessons or related services for compensation without express authorization; or 4) engages in misconduct or creates a nuisance. Such acts may also be prosecuted as a criminal offense. The pass is NOT TRANSFERABLE and CANNOT BE RESOLD. The pass may be confiscated with no re-issue, if in the sole judgment of a representative of the ski area operator; it is used in a fraudulent manner. Re-issued passes may be subject to a replacement fee. The Undersigned acknowledge their affirmative duty to immediately notify the ski area operator if Holder s pass is lost or stolen.

In consideration for allowing Holder to participate in the Activity, the Undersigned AGREE THAT ANY AND ALL CLAIMS for injury and/or death arising from the Holder s participation in the Activity shall be GOVERNED BY COLORADO LAW and EXCLUSIVE JURISDICTION of any claim shall be the DISTRICT COURT OF SUMMIT COUNTY, COLORADO or in the FEDERAL COURT FOR THE STATE OF COLORADO. 10. In the case of a minor Holder, the Undersigned parent or legal guardian acknowledges that he/she is not only signing this Agreement on his/her behalf, but that he/she is also signing on behalf of the minor and that the minor shall be bound by all the terms of this Agreement. Additionally, by signing this Agreement as the parent or legal guardian of a minor Holder, the parent or legal guardian understands that he/she is also waiving certain rights on behalf of the minor that the minor otherwise may have. The undersigned parent or legal guardian agrees that but for the foregoing, the minor Holder would not be permitted to participate in the Activity. 11. By signing this Agreement without a parent or legal guardian s signature, Holder, under penalty of fraud, represents that he/she is at least 18 years of age. If signing as the parent or guardian of a minor Holder, signing adults represent that they are a legal parent or guardian of the minor Holder. 12. The Undersigned understand and acknowledge that this Agreement is a contract and shall be binding to the fullest extent permitted by law. If any part of this Agreement is deemed to be unenforceable, the remaining terms shall be an enforceable contract between the parties. It is the Undersigned s intent that this Agreement shall be binding upon the assignees, subrogors, distributors, heirs, next of kin, executors and personal representatives of the Undersigned. 13. Medical Care. Undersigned authorize the Released Parties and/or their authorized personnel to call for medical care for Holder or to transport Holder to a medical facility or hospital if, in the opinion of such personnel, medical attention is needed. Undersigned agree to pay all costs associated with such medical care and related transportation. 14. Arapahoe Basin is not responsible for lost or stolen passes. Please notify the Arapahoe Basin season pass office immediately if your pass is lost or stolen, at or ARAPAHOE. The HOLDER shall receive up to (1) one replacement at no cost for the 1 st lost or stolen pass. For every other instance, The HOLDER shall be responsible for a $10.00 replacement fee. 15. Holder understands that Arapahoe Basin season passes are NON-REFUNDABLE and NON-TRANSFERABLE under ANY circumstances. 16. Under the penalty of perjury and/or fraud, I hereby represent I am the individual represented below [Holder] or the parent or legal guardian of the minor Holder. ANY FRAUDULENT USE WILL RESULT IN LOSS OF PASS. Fraudulent use is a criminal offense and the pass user may be prosecuted. 17. Media Release: Holder consents to photography and video obtained by both traditional equipment and unmanned aerial systems. Holder releases to Arapahoe Basin Ski Area the right to use Holder s image for purposes including, but not limited to, all print or electronic advertising media and any social media applications.
