2 Welcome to fire walking. You now have an opportunity to join a select band of people who have firewalked! Firewalking is thousands of years old. People walk on fire for many different reasons. For most people it is an opportunity to confront an inner fear, a fear that is representative of all of the fears that they hold. Being scared is OK. It is the ability to take action when you are scared that is the difference between accessing your full potential and settling for an average existence. You will have an opportunity to walk barefoot across a 12 foot coal bed that is burning at about 900 degrees F. The choice is yours and yours alone. 3 THE PRACTICALITIES Dress We recommend comfortable, casual dress including trousers jeans are ideal skirts must not be worn You should be able to roll your trousers up to just below your knees You should bring a warm fleece type top as well Your feet Please do not apply any lotions or oils to your feet in the 36 hours immediately prior to the firewalk We will provide a bucket of water for you to clean your feet after the firewalk your feet will be black from the coals If you wish to bring your own material for cleaning your feet please do so Toe nails may have nail varnish on them Burns It is possible to burn your feet while firewalking While we take every possible precaution to minimise the risk, if you do receive a second degree burn (a blister) we will provide you with a simple first aid kit This is provided without any acceptance of any liability (see attached liability release form)

4 Liability release We will forward a copy of the liability release form You will be asked to sign a copy of this form at the event. When you have signed the release form and handed it in, you will receive a wrist band. Please wear this for the duration of the event. You will not be allowed to firewalk unless you are wearing this wrist band We ask that all Attendees sign the release from, whether you initially plan to fire walk or otherwise Weather If it is wet, we will still endeavour to complete the firewalk if the weather is so bad that it is impossible or represents a risk to anyone, we won t Bryan Dunlop will have sole authority over this decision Alcohol Logistics You will not be allowed to firewalk if you have or we think you have consumed alcohol in the last 8 hours. We depend on you to remove the burnt turf, inform the local fire brigade and supply a water source. Help with building the fire lanes is always appreciated.

5 Voluntary release Form for Fire Walk PLEASE READ CAREFULLY By signing this form you are waiving all rights to: COMPENSATION IN CASE OF INJURY I fully understand and acknowledge that there are no requirements whatsoever to participate in this event, and that I do so of my own free choice. I also understand that to walk across the fire bed can be dangerous if the training and instruction I am given is not adhered to by me. I therefore commit to follow and implement the training and instruction without deviation. I also acknowledge that I have been told that: People have been injured by participating in Fire Walking That there is an inherent risk in Fire Walking I voluntarily choose to Fire Walk, there is a possibility that I myself may receive injuries requiring medical attention In consideration for my participation in this event and by endorsement below, I agree to release and hold harmless Tailor Made Training Limited, their agents/assistants/employees, independent contractors, event hosts/sponsors/organisers, venue/land management and venue/land owners for any liability whatsoever any damage or injury, physical or mental, which I might incur as a result of my voluntary decision to participate in this activity. If I do choose to walk across the fire bed in my bare feet, I affirm that I have not been coerced or persuaded in any way to do so, and acknowledge that I make this decision voluntarily and without reservation. I acknowledge that I am physically and mentally able to perform the physical aspects of the activity and the mental aspects of receiving and understanding the spoken words of the Trainer. Having assessed and understood the this Release Form and understood what is required of me, I hereby elect, by my signature below, to release Tailor Made Training Limited from any claims heretofore and thereafter arising both physically and mentally, from my participation in this event. I also grant Tailor Made Training Limited permission to use statements, video images, photographs and other likenesses of myself resulting in my participation in this event. I also confirm that I understand that the knowledge and content of the session/seminar is protected by copyright. By signing this waiver, I attest that I am over sixteen years of age, am not under the influence of alcohol, prescribed controlled or illegal drugs or have any physical or mental conditions that prevent me from participating in this event. I also confirm that my feet and ankles are free from any lotions, potions and creams and that I have not applied foot treatments to harden or soften the skin in the past 5 days. I acknowledge that I have been given the opportunity to ask questions regarding any aspect of the Release contained herein, and by signing below, I do acknowledge that I have carefully read and understood all aspects of the Release and agree to the terms in their entirety.
