1 SLAM Sirens Dance Team Constitution I. SLAM Sirens Dance Team (SSDT) A. This constitution has been written and adopted for the SLAM Academy Middle/High School Sirens Dance Team. SLAM Academy Middle/High School Sirens Dance Team and its members govern the team and its Constitution. B. Henceforth, the name of the organization shall be SLAM Sirens Dance Team (SSDT) C. The purpose of the SSDT is to: i. Create school spirit, pride, good sportsmanship, and loyalty within school and the community ii. Promote interest in school activities and perform at school assemblies, games, and functions iii. Develop responsibility, teach self-respect, encourage honest effort and a drive for personal excellence, and develop character iv. Teach teamwork and pride in a quality performance through maintaining high standards D. There will be a Head Coach i. The coach will be responsible for the daily running and activities of the SSDT ii. The coach will be responsible for administering all disciplinary procedures iii. The coach may place certain restrictions on team members iv. The coach may dismiss any team member or captain from their position at any time E. Team members must adhere to all safety rules. II. III. Eligibility A. Each member must EARN a 2.0 or above in each of their classes on their report cards. i. Failure to pass a class at the end of the 9-weeks will result in academic probation and benching during games until grades have improved. Grades will be monitored on a weekly basis. ii. Failure to pass all classes for a second time will result in removal. B. Each member must submit a completed application form, acknowledgement of constitution, and parental consent/medical release Form BEFORE auditions take place. C. Each member is required to submit a completed physical and be cleared with the athletic department BEFORE auditions take place AND prior to the start of regular practices for the following year. Team Composition A. The team shall consist of a number of qualified candidates to be determined at auditions and based on the tryout procedure and scoring system. B. If a dancer quits then he/she must wait one full calendar year before he/she is allowed to audition for the team again. i.e. was on the team freshman year and quit, must wait until junior year to audition C. There will be an Officer Line selected through a separate audition process.

2 IV. Attendance A. All students who participate in school activities must adhere to the attendance policies as outlined in the student/parent handbook. i. Student not in full uniform the day of a performance or activity will have to forfeit that performance or activity. ii. Students that have to forfeit a performance or activity will need to be present in regular street clothes/school uniform and sit with the coach. B. To participate in a practice, a student must adhere to attendance policies outlined in the student/parent handbook. i. This must be fully documented with the attendance office. ii. If a medical appointment is scheduled during the day it will be excused as long as it is fully documented. C. There is a 5 minute grace period for tardies to practices not scheduled on school days ONLY. Tardies at any other functions, on school days or if a dancer is more than 5 minutes late to practices that are not on school days will incur demerits and any other consequences that the coach sees fit. D. If a member is ill and misses ANY activity, he/she must be at HOME or seeking medical appointments. E. If a member misses ANY practice, he/she will have to forfeit the upcoming performance or activity. There is NO differentiating between excused and unexcused absences an absence is an absence. F. All doctor appointments, other activities that the student is involved in, and/or jobs should be scheduled so they do not interfere with SSDT activities and performances. Emergencies would be the only exception. The definition of emergency will be left to the coach s discretion. G. All team members will attend all performances and activities in their entirety. H. Work is not considered an excuse for missing any activity or performance. I. Homework is not an excuse for missing any practice, activity, or performance. You are expected to budget your time effectively. J. Being grounded and/or any other punishment by a parent is NOT a reason to miss practice, an activity, or performance. Dance is a team sport and when one person is missing it affects the entire team. V. Uniform and Dress Code A. Each member will be expected to come to practice in appropriate and/or required attire. The workout attire must meet school dress code guidelines. B. Each member of the SSDT is expected to pay the full membership dues as outlined in the SSDT Price List. There will be no exchanges or refunds of any kind. C. Each member will be required to maintain good condition and cleanliness of his/her uniform on a weekly basis. D. Each member is required to wear the designated uniform on game and/or performance days. Uniforms must be worn with team shoes. E. No person outside of the team is allowed to wear any portion of team uniforms or costumes at any time. Team members will only wear uniforms and costumes when required to do so. DO NOT wear your dance clothes as regular clothes! F. NO Jewelry is to be worn during any practice, game, or performance.

3 G. Hair is up to the discretion of the coach. Performance makeup shall be worn at all dance functions. Finger and toe nails must be naturally colored for performances (unless otherwise permitted by the coach). VI. VII. Character A. Team members should be leaders within the school and set a good example at all times, especially while in uniform. B. Team members must follow the SLAM Academy Code of Conduct at all times. C. Team members should be above reproach, maintaining good personal appearance and habits that cannot be criticized. i. Team members shall follow SLAM Academy Dress Code at all times. ii. Smoking, drinking, and/or drug use are grounds for immediate dismissal by the coach. iii. Failure to maintain passing grades at the end of the 6 weeks is grounds for probation, suspension and/or dismissal by the coach. iv. Team members not following school rules and regulations could be placed on probation, suspension and/or dismissal by the coach. D. A team member must be courteous and friendly to all other team members as well as the student body and staff. E. A team member must be courteous and friendly to other teams and visitors. F. While at games, events, and performances, members are expected to remain with their team. G. Personal grooming and applying of makeup should be done prior to any event and never at the event or in front of a game/performance crowd. H. When using social media (Facebook, Instagram, Twitter, Snapchat, KIK, etc.), team members need to make sure that what is posted represents the program and more importantly, themselves respectfully. What is goes online becomes public property. The posting of inappropriate pictures and/or behavior may result in suspension and/or removal. Dance Officers/Leaders A. Officers may be chosen at the beginning of each dance year. Each dance year begins in May and ends in April. i. Officers will be selected and ranked based off of scores from a separate audition ii. All team members shall show respect to the Officers. iii. All Officers shall show respect to all team members. B. Officers will be seen as the leaders of the team. i. Officers will lead stretches and conditioning during practices, as well as help lead at games. ii. Officers will lead by example. iii. Officers will have constant communication with the coaches and their teammates. iv. Officers will be held responsible for extra work that may be needed for performances or other team involved activities. v. Officers may be dismissed of their position by the coaches at any time. C. Duties/Responsibilities of Officers i. It is the duty of the officers to carry out the commands of the dance coach. ii. Dance officers will assist the dance coach in checking appearances, assisting in contest auditions, teaching and instructing the team, and making decisions relevant to the team. iii. Officers will be responsible for a squad of members. a. Each officer, besides the Captain will be responsible for the members in their companies.

4 b. They will ensure that each member of their company is informed, prepared and on task. D. Selection of Officers i. Any member who has served or is currently serving at least one year on the team is eligible to audition ii. The selection process will change from year to year. iii. The number of officers for the upcoming year will be determined by the dance coach iv. Officer audition packets will be handed out when the director(s) sees fit E. Termination of Officers i. Failure of an officer to set a proper example and follow the guidelines and procedures of the dance team and the handbook and extracurricular code of conduct of SSDT may result in removal from her office. ii. An officer will be removed from her office upon the accumulation of the 10th demerit and her parent/guardian will be notified of the demotion. VIII. Fundraising A. ALL members are required to participate in ALL fund raising, which is generated by the team. Please do your part so this program can grow. Your participation will help us to have enough funds in order to go to competition. B. Any funds raised by a member will go directly into the dance account and is nonrefundable in the event that a member quits the team or is dismissed. IX. Financial Obligation A. Being a member of the SSDT includes various financial obligations that include warm-ups, costumes, accessories, shoes and practice/performance attire. B. Every attempt will be made to keep items from year to year to help minimize cost. C. Members will be expected to pay and/or seek donations/sponsorships to help fund their participation. D. Financial concerns and/or issues should be addressed with the coach, so that possible arrangements can be made. E. If a team member quits or is removed from the team for any reason, any owed money is still the responsibility of the dancer and parent(s)/guardian(s). F. If a team member quits or is removed from the team for any reason, no refunds shall be issued.

5 SLAM Sirens Dance Team Merit/Demerit System In order to be a productive and successful team, it is necessary that each team member be responsible to a discipline system and code of conduct. The following is a code of behaviors that each dance team member is expected to observe throughout the year as a Siren. The purpose of this system is intended to enhance our organization and its members. MERIT SYSTEM GUIDELINES: Members will have the opportunity to decrease the number of demerits acquired by receiving merits. The merit system is designed to reward members for positive behavior. Guidelines for the merit system are as follows: So that the merit system is not abused, merits will delete demerits if the merits have been earned and recorded PRIOR to the demerits being issued or if the merits are earned within one week (7 days) of the demerit being issued. If a member does not earn enough merits within the given timeframe, the demerit will become permanent. Merits may be obtained during the summer months but may not exceed 50. A maximum of 15 merits may be earned for a single event. If a member should have questions, they should see the dance coach prior to the event in question. Five (5) merits will delete one (1) demerit. Once merits have been used, they will be deleted. Some serious demerits will not be removed by the merit system. These demerits will be considered permanent and will be noted as permanent on the list of demerits. DEMERIT SYSTEM GUIDELINES: Each dance team member will be allowed to accumulate a total of 15 demerits. If a member receives 16 demerits, he/she will be removed from the team for the remainder of the year; at which point he/she will be required to turn in his/her uniform and get a schedule change. Some demerits will be considered permanent; some demerits will be removable according to the guidelines established in the merit/demerit system guidelines. Once a dance team member has received demerits 3 times for the same offense, any following demerits earned for the same offense will be considered permanent and will not be removed by the merit system. Once a dance team member has received 10 demerits he/she will be placed on probation for the next performance. An officer will be demoted from his/her office upon the accumulation of 10 demerits and his/her parent/guardian will be notified of the demotion. In addition, the officer will be placed on probation. Demerits will be issued by the dance coach, captain and that team member s company officer. Before a demerit can be issued by an officer, the dance coach will first be informed. When a member receives a demerit, he/she will receive notification of the demerit and must sign for the demerit within two school days of the offense or within two days of returning to school. If he/she

6 has a question regarding the demerit, he/she may then conference with the dance coach. Conferencing is not the responsibility of the dance coach. Each member may review his/her discipline record at any time throughout the year. Each member must keep a record of his/her merits/demerits. The demerit/merit system will be in force at all times. The demerit/merit system may be used during officer tryouts if the dance coach so desire. The dance coach may choose to use the total number of demerits and merits received throughout the year to help in determining officers for the following year and this total may be used in the officer candidate s score.

7 SLAM Sirens Dance Team Merit System The following is a list of activities for which merits may be received and the number of merits available for each activity. Activity # of Merits Non-required service to school 1 (signed note required from teacher, sponsor, etc.) *Service cannot be for a student aide class and cannot be used toward another activity i.e. student council; furthermore merits cannot be issued for service performed during school hours. Earning an A on a report card Earning a B on a report card Participating in a studio dance class (a monthly log signed by teacher required maximum of 10 per month) Tutoring another student (note required from a teacher/sponsor) 2 per grade 1 per grade 1 per class 1 per hour Service to community 1 per hour of service (signed note required from service director(s) maximum of 15 per single activity) Siren of the Week Service in another school organization (one time only) Attending a non-required school event (signed note from attending principal) 1 per time 2 per organization 2 per event ****The dance coach shall have the authority to give merits for activities which have not been included or which have been inadvertently excluded when necessary. The number of merits will directly reflect the activity.

8 SLAM Sirens Dance Team Demerit System The following is a list of offenses and the number of demerits received for each offense. Permanent Demerits --- May not be removed by merits: Offense: Being assigned Saturday school Being assigned Indoor Suspension # of Demerits 1 per assignment 2 per assigned day Leaving a practice without permission from the dance coach 3 Unexcused absence from practice 3 Skipping a class at SLAM Academy Middle/High School 7 Forging or altering any school issued permit 10 Missing scheduled dance team performance w/out permission from the dance coach 10 Failure to participate in team and booster fundraisers 3-15 Disrespect to coach or other school employee 3-15 Any offense of local, state, or federal laws (at any time) excluding traffic tickets 5-15 Demerits that may be removed by merits Offense: # of Demerits Failure to wear correct practice/day uniform 1 (Including improper jewelry/hair/gum (performances); each item will count as separate demerit [i.e. pants, top]) Late to practice 1 Leaving practice early for any reason with coach s approval 1

9 Turning something in late (not items counted as grades) 1/day Talking after being asked to be quiet 1 Missing a piece of uniform for a performance 1 per piece Lending out any piece of Sirens wardrobe to someone else without permission 2-5 Failure to stand or sit where directed at a performance 1 Eating or drinking during a game without permission 1 Excused Absence to practice with coach s approval 1 Failure of an officer to complete an assignment 2 Being late to a performance including any practice preceding that performance 2 Being out of school dress code 2 Using profanity at any time 3 Public Display of Affection 3 Defined as ANY physical contact (other than hand holding or a hug) with another person while in Sirens clothing and/or any kissing or groping at any time Missing scheduled dance team performance with approval from coach 3 Not wearing uniform (practice or performance) 5 Deliberate disrespect of a reasonable request made by an officer or coach 5 Unladylike conduct --- coach s discretion 2-15 (Behavior unbecoming of any young woman striving to exemplify the standards and ideals of a Siren) ****The dance coach shall have the authority to give demerits for offenses which have not been included or which have been inadvertently excluded when necessary. The number of demerits given will directly reflect the offense. ****The dance coach shall have the authority to use discretion regarding extenuating circumstances.

10 SLAM Sirens Dance Team Merit/Demerit Compliance Form I am aware that as a member of the SLAM Sirens Dance Team, my son/daughter,, will be required to adhere to a merit/demerit system. I have read and understand the merit/demerit system and realize that upon the accumulation of his/her 16th demerit, my son/daughter will be removed from the dance team for the remainder of the year and no refunds for fundraisers, supplies, member dues, etc. will be given. I further understand that if my son/daughter is an officer of the SLAM Sirens Dance Team, he/she will be demoted from his/her office upon the accumulation of the 10th demerit. 11 Important Dance Dates (Mandatory) First Team Meeting: Mandatory meeting for ALL dancers and AT LEAST ONE parent/guardian that make the team. We will discuss the first fundraiser and review summer practice schedule and camp, uniforms and attire, and upcoming payments for member dues, as well as uniform fittings. Initial team dues will be due on this date. Tuesday, May 23 rd, :00 p.m. (sharp) in Dance Room School Year Classes & Practices: Following the selection of the team, SSDT members will be scheduled into the Dance Elective reserved for SLAM Sirens. Sirens practices will be part of the school schedule. In addition to our initial summer practices, after-school practices will begin on Tuesday, August 22 nd. A practice schedule will be included with the welcome packet to team members who make the SSDT. Practices will be held on Tuesdays and Thursdays during the months of August, September, and October. Dancers must be picked up PROMPTLY at the ending practice time. The Coach, nor teachers or staff at the school, are your child s babysitter. Please be respectful of this. If you are unable to pick up your child at the scheduled practice end-time, your child will be sent to Aftercare, and Aftercare will charge the daily rate to your account. Summer Camps/Practice Schedule First Team Practices: Upon the announcement of the team on Friday, May 19 th, our first team practices will be as follows: Monday, May 22 nd 2:45-4:30pm Tuesday, May 23 rd 2:45-6pm o MANDATORY Parent Meeting to follow at 6:00 pm o **INITIAL FEES DUE** Failure to provide deposit will result in dismissal from team. Wednesday, May 24 th 2:45-4:30pm Thursday, May 25 th 2:45-4:30pm Tuesday, May 30 th 2:45-4:30pm Thursday, June 1 st 2:45-4:30pm Tuesday, June 6 th 2:45-4:30pm Sirens Welcome Camp: Thursday, June 8 th 8am-4pm (during/after school) Friday, June 9 th 8am-7pm (Long lunch break + review time from 12-3) Saturday, June 10 th 8am-4pm Cost: $125 per dancer Material: TWO Competition Routines by professional American Dance Alliance choreographers Mandatory. Failure to attend will result in dismissal from team. Sirens Summer Camp June 12 th through June 16 th 8am to 12pm Cost: Free Mandatory. Failure to attend will result in dismissal from team Summer Practices: July 10 th, 11 th, 12 th 8am to 12pm Cost: Free Mandatory. Failure to attend will result in dismissal from team. Fall Workshop: There will be an American Dance Alliance dancer workshop on in September for the cost of $25. Two pep rally routines will be taught. *Completed Physical must be on record in the athletic office before the first practice on August 22 nd, 2016.

12 ACKNOWLEDGEMENT OF CONSTITUTION Please read, sign, and date. Both Dancer and a Parent/Guardian need to read the Constitution; sign, and return this page to Mrs. Spoelstra. (The constitution is available online at THIS PAGE (not the entire packet) must be turned in on or before Monday, May 15 th, 2017 (unless previously discussed with Coach). If this page is not submitted you will NOT be allowed to audition! I have read and understand the terms set forth by the SLAM Sirens Dance Team Constitution. My parent/guardian has read and understands the terms set forth by the SLAM Sirens Dance Team. By the submitting the following signatures, my parent/guardian and I agree to adhere to the conditions set forth by the SLAM Sirens Dance Team Constitution I understand that my son/daughter will be evaluated by qualified judges and we agree to abide by the decision of the judges and advisor. 13 SLAM Sirens Dance Team Release Form Applicant Release: I, (applicant name), have read all rules and regulations that govern the SLAM Sirens Dance Team. As a representative of my school, I understand and agree to abide by them if selected as a dancer for the school year. In addition, I grant the SLAM Sirens Dance Team the right to take photographs of me and my property. Such photographs of me with or without my name may be used for any lawful purpose, including such purposes as publicity, illustration, advertising, and Web content. Signature of applicant: Date: Parental Release: I, the undersigned, have read and fully understand the rules and regulations, which will govern my son/daughter if he/she is chosen to represent SLAM Academy Middle/High School as a dancer. I further understand that this is a co-curricular activity and that regular attendance at all practices, games, special functions/events, and training camp is an expectation of each dancer. In addition, I grant the SLAM Sirens Dance Team the right to take photographs of my child and their property. Such photographs of my child with or without their name may be used for any lawful purpose, including such purposes as publicity, illustration, advertising, and Web content. I hereby give consent to my son/daughter,, to try out for the Dance Team at SLAM Academy Middle/High School and recognize his/her responsibilities and requirements as a leader of his/her school. I understand that if chosen, my son/daughter will be required to fundraise and/or secure funding for all related and required uniforms, attire, supplies, and competition fees. I understand that if my child is removed or quits the team, the financial responsibility still must be met and that no refunds shall be issued. 14 SLAM Sirens Dance Team Dance Team Waiver Form As the parent or legal guardian of, Name of Son / Daughter I agree to the following terms for participation with the SLAM Sirens Dance Team Program: 1. I grant permission for my son/daughter to participate in the SLAM Sirens Dance Team auditions / practice sessions / games / competitions / camps. I realize that dance is an activity where physical illness or physical injury (minimal, serious, or catastrophic) can occur; therefore, any medical / accidental expenses or liability that possibly results from an illness or injury to my son/daughter during the SLAM Sirens Dance Team clinics / auditions / practice sessions / games / competitions / camps will be my responsibility. I further acknowledge and understand that my son/daughter is assuming the risk of such physical illness or injury by his/her participation, and I further release Miami-Dade County, SLAM Academy Middle/High School and its representatives from any claims for personal illness or injury that my son/daughter may sustain during participation in this activity. My son/daughter is properly covered for any accident by the following insurance: Insurance Provider / Policy Number 2. SLAM Academy Middle/High School, its officers, regents, employees, and agents assume no responsibility for supervision of students outside of clinics / auditions / practice sessions / games / competitions / camps. Parent(s)/Guardian(s) are expected to pick up their students in a timely manner. The coach will provide practice times. Unless there is an emergency, parents will need to pick up their students within 10 minutes of being dismissed from clinics / auditions / practice sessions / games / competitions / camps. If there is failure to do so, the coach will call campus police and the student will incur demerits. 3. I hereby authorize the dance coach, athletic trainer, or any other supervising adult to obtain medical treatment for my son/daughter for such an illness or injury during the activity.
