3 A YES response will allow a selection of one of the following: - Art - Elementary - Math - Music - Philosophy - Science - Special Education - Other School Activity List included on page 5 & 6 A YES response will allow a selection of one of the following: - Dallastown - Leaders Heights - Loganville Springfield - Ore Valley - York Township A YES response will allow a selection of one of the following: - Aerospace - Chemical - Civil - Electrical - Environmental - Industrial - Marine - Mechanical - Nuclear - Other Enter the activities you participated in per year. For related activities where you participated in both activities (such as student council and student council officer) enter both activities separately. If you were a sports manager, enter the sport and year you managed, just as if you were a participant in the sport. Available Activity: Volunteer Club Applicant must provide the year(s) of participation AND number of hours. Other School Activities Not Previously Listed. These activities will be counted toward the Volunteer and Skill Enrichment category, and will be verified by school advisors
4 Please use this section to record activities in which you have participated in the past four years. Activities should be UNRELATED to a school activity and non-school sponsored. Activities must be validated by a parent, guardian, appropriate leader or supervisor. Individual activity descriptions should be entered, along with the total hours. Examples of activities that would qualify in this section: Scouts, private instruction for skill enrichment purposes, sports clubs, and other NON-SCHOOL related activities. Simple attendance is not sufficient for an activity to be counted (i.e. regular church attendance would not be counted, however serving as a volunteer youth leader would be a qualified activity. Leisure hobbies are not to be considered (e.g. knitting, coin collecting), as well as routine exercise activities (ex: jogging, aerobics, weight lifting). ** When calculating hours, activities should not exceed 8 hours a day (i.e. 5 day mission trip would be counted as 40 hours, not 120 hours). Please use this section to record TOTAL paid work experience in which you have participated in the past four years. Work experience must be validated by a parent, guardian, or supervisor. DO NOT include PAID work hours in "Volunteerism and Skill Enrichment" Section of this application. It will NOT be counted. Exclude: Career Internship and HACC Academy during school hours. When calculating total work hours, do not count more than 8 hours a day (i.e. A 5 day work week would be counted as 40 hours, not 120 hours)
