2 WHAT IS THE GOLF CLASSIC & GALA? This year marks the 17 th annual PROVAIL Golf Classic and Gala! The tournament has become an annual Northwest tradition, joining companies from around the Puget Sound to support PROVAIL s mission of supporting the people with disabilities to fulfill their life choices. Golf Classic May 1 st, 2017 Sahalee Country Club Held at Sahalee Country Club in Sammamish, WA on May 1 st, 2017 this tournament cannot be missed. of the Northwest s premier championship venues, Sahalee Country Club is the perfect space for a day of competitive golf and celebration with coworkers and clients. Foursomes will compete for the PROVAIL Championship Trophy and prizes for the hole in one, putting contest, and more! In 2016, we had an amazing turn out for the whiskey tasting, dessert dash, and fabulous live auction items including trips to Mexico all back this year! Join us on Saturday May 20 th, 2017 at the Meydenbauer Center for a cocktail attire event featuring extravagant live and silent auctions, a three-course dinner, and a celebration of PROVAIL s mission. The gala is a great way for golfers and nongolfers alike to gather for an elegant evening in support of PROVAIL. 2

3 WHAT DOES IT SUPPORT? Supporting children and adults with disabilities to fulfill their life choices. Since 1942, PROVAIL has been supporting children and adults with disabilities to live the life they choose. As one of the state's largest private, multi-service agencies committed to meeting the needs of children and adults disabilities, PROVAIL is dedicated to the belief that: Every one of us has the right to pursue the life we choose to live. For people with disabilities, realizing this belief is often faced with great obstacles. With the support of the community, PROVAIL works to ensure that children and adults with disabilities have the tools, technology, and resources they need. PROVAIL s mission is to support people with disabilities to fulfill their life choices. Providing the services needed to support people with disabilities in all major areas of life so that they can live, work, and fully participate in the community of their choice, PROVAIL s programs address gaps in services for these individuals, including those with the most severe disabilities for whom services and resources are limited. Each of our programs fall within three our core focus areas, Mobility and Communication, Employment, and Home and Lifestyle. Through these focus areas, we work to ensure that people with disabilities have the communication and mobility tools they need to engage with their community, employment to build selfsufficiency and a connection to their community, and a home and lifestyle of their choosing. Support for PROVAIL will provide children with the communication devices they need to be successful, make opportunities available for youth and adults with disabilities to pursue community-based employment, and open doors for adults with disabilities to have community-based supportive housing, health, leisure, and wellness options to maximize their independence. Acceptance of the sponsorship agreement shall constitute an agreement to provide the advertising and promotional opportunities described in the sponsorship benefits in exchange for cash sponsorship as indicated. By signing this agreement, the ing Company acknowledges that this agreement had been read and is acknowledge by an authorized party and understands all terms contained herein. Cancellations or changes in the sponsorship agreement are accepted only in writing to PROVAIL, and must be received on or before the ship Closing Date. Any expenses incurred by PROVAIL to promote the ing Company, including golf registration fees, as agreed upon in this agreement incurred before the cancellation with be the ing Company s responsibility. The closing date for 2017 is April 15 th, The ing Company is responsible for co-producing all artwork for co-branded advertisements to ensure the ing Company s and PROVAIL s specifications are met. The ing Company is responsible for submission of advertising copy, logos, and materials by material deadlines. Payment of sponsorship is due and payable 30 days of receipt of invoice. A service change of 1.5% per month will be added to accounts past due, beginning 30 days delinquent and charge monthly thereafter.
