1 KINNICUM FISH & GAME P.O. BOX 191 CANDIA NH APPLICATION PACKAGE FOR MEMBERSHIP What to Expect and What to bring 1. (preferred) or mail the club your application for review. Make sure your application package is filled out completely a. Vehicle license plate #(s) filled in on application. b. A copy of a government issued photo ID (IE.. Drivers License, Passport) c. One of the following; a criminal background check, listing Kinnicum as the recipient, a current LTC permit, or a receipt of a firearm purchase, from an FFL, within the last 12 months. d. One of the following; a copy of an approved firearms safety course, a hunter education certificate or current hunting license. e. address is clearly written (required). f. Test has been completed. g. Informational letter about yourself attached. 2. You will then be scheduled for an interview, generally the 3 nd Wednesday of the month. We will contact you with plenty of notice to schedule the interview. Bring a blank check for your dues and a self addressed stamped envelope to the interview. 3. Once you have completed the interview, you will be contacted to complete the orientation. 4. The orientation will be a verbal review of the Membership Package and you will be taken out to the ranges for a review of our safety procedures. You will be required to show safe gun handling practices. Make sure you bring a rifle or handgun with 10 rounds. You will need to demonstrate safe handling for the orientation director. 5. Once the orientation and range review is complete, and the application is approved by the membership committee, you can be voted into the club. The vote will be taken by current members at the end of our next regular meeting. If approved, your membership card will be mailed to you if you are not present at the meeting. This card must be displayed at all times, while on club property. Keep this page for your records.

2 KINNICUM FISH & GAME APPLICANT RULES AND REGULATIONS 1. Applicant should be sponsored by a member with 2 years good standing and submit a completed membership package. In addition, an informational letter must be written and attached to the application. Please tell us about yourself, why do you want to join, firearms/sporting experience and what skills you will share to help with the upkeep and improvement of KF&G. The membership committee will review and approve the application. 2. Applicant does not need to be present at the meeting when the applicant is voted upon for membership. 3. Applicant s performance of duties, adherence to posted rules, and range safety will be observed during their membership. If applicant is found to be non-compliant for any reason judged valid by the Board of Directors, membership will be terminated and applicant will forfeit all dues and fees paid. Applicant shall have the opportunity to appeal this ruling at the next regular directors meeting. 4. Applicant must work 5 hours in their first year. 5. Note that if any member is able to answer yes to any one of the four questions on the application form at any time during his/her membership, said membership will be terminated and all dues and fees paid forfeited. 4 LIABILITY RELEASE AGREEMENT Must be completed by all individuals prior to shooting (the Sport ) or upon entry of KINNICUM FISH & GAME CLUB INC. ( KFG ) property by any person. I understand that I am participating in a sport in which certain dangers and risks may arise, including, but not limited to, accidental injury. In consideration of the right to participate in the Sport, and of the services provided to me by KFG and its agents, collectively and individually, I have, and do hereby, voluntarily assume all risks associated with the Sport. I understand the risks and hazards involved. I understand that shooting may be considered a dangerous activity, bearing the risk of potentially life threatening injuries and potential exposure to lead and airborne lead particles. I assume the risk of any and all injuries I may suffer due to the physical condition of KFG, the negligence of KFG, equipment failures, defective equipment, acts of God, and the acts or omissions of shooters or others who may be at, on, or near KFG property, whether with or without the permission of KFG. I agree to indemnify and hold KFG harmless for any injuries. I hereby consent to receive medical treatment, and be responsible for payment of the same, which may be deemed advisable in the event of injury, accident, and/or illness. I understand there will be other persons shooting and/or carrying firearms at KFG and I agree to act prudently and carefully to avoid causing them or myself any injury. I certify that I am, or will be, familiar with the safe use and operation of any firearms I may use while at KFG. I have read and am familiar with the rules of firearm safety, as well as the specific shooting range rules and regulations posted and set forth in the Bylaws of KFG. On behalf of myself, my estate, my heirs, and assigns, I hereby release KFG, its employees, agents, representatives, assigns, members, instructors, and volunteers from any and all liability, regardless of cause, including injuries due to negligence up to and including death, and all property damage or theft I may suffer because of my presence and participation in the Sport at KFG, which may be caused by my own or others negligence. I understand that KFG is not responsible for any lost or stolen property, including pets. I specifically release KFG on behalf of myself, my estate, my heirs, and assigns from liability for any personal injury, death, disability, and acts and/or omissions of whatsoever kind and nature, including, but not limited to, those caused by negligence, including those acts and/or omissions of other participants or persons present at KFG. I hereby certify that I am not prohibited from possessing, selling, owning, using, controlling, or transferring firearms under state or federal law. I further certify that I (i) have not been convicted of a felony that has not been annulled or expunged; (ii) have not been convicted of a violation of 18 U.S.C. 922 that has not been annulled or expunged; and (iii) am not a member of an organization that advocates the violent overthrow of the United States Government. I understand that it is my responsibility to consult with a physician prior to, and regarding, my participation in the Sport at KFG. I represent that I am physically fit and have no medical, psychological, or other condition that would prevent my full participation in the Sport. I understand that this release shall remain in full force and effect so long as I am a member of KFG, a user of its facilities, or a guest on its property.
