They will not be accepted if sent by facsimile. 2.3 Notes All entry forms must be completed in accordance with the relevant provisions of the Technical Handbook, otherwise entries will be invalid The maximum and offi cials may not exceed either those described in part II Number of Competitors of this Handbook or the numbers given on the Entry by Number Forms previously submitted The entry forms must be fi lled out in English (except in the Name fi eld where Chinese characters are requested), and typed using CAPITAL LETTERS format The Codes and NOC Codes can be found in part IV s and NOCs Codes. Technical Offi cial TO-B-00a - Offi cials HQ-B-00b - Delegation HQ-B-00a 1 Technical Officials Registration and Measurement Form Individual registration for Headquarter Officials & Team Officials Number of competitors and officials The name (family name and given name), date of birth, place of birth and accreditation number (AD Number) must be identical to the information contained in the Macau 2007, 2 nd Asian Indoor Games Family List Chinese Characters for Name must be provided if it is appropriate When fi lling in the columns titled gender, blood type, etc., mark the relevant space with an x, other information should be typed in the spaces provided When fi lling in the Date of Birth, follow the example below. A birth-date of October 7, 1975 should be written as shown Date of Birth Day Month Year Use metric measurements to record body height and weight, i.e. centimeters (cm) for height, and kilograms (kg) for weight

8 II. Number of Competitors Official s categories must be correctly stated. The maximum numbers of competitors and reserves who can participate in each sport, as specified in the Technical Handbook for the relevant sport, are listed in the following tables Entry by Name Forms must be signed by the competitors, the President or Secretary General of the relevant NSF, and the President or Secretary General of the NOC To allow for non-correctable errors and insuffi cient copies, photocopied Entry by Name Forms are acceptable provided that the information typed on the Form and the signature and Seal are original Changes and / or additional entries should be made by completing a new entry form, which must be received by before the closing date, that is no later than September 14, Notes for Two Demonstration s (Kickboxing and Kurash) After received the approved Entry by Number Forms from the Wako Asian Kickboxing Federation, and the International Kurash Association (IKA). (III 5.3)* NOC for Aerobic Gymnastics competition, may enter a maximum of fourteen (14) competitors in Aerobic Gymnastics competition. A number of staff are limited to 40% of the number of athletes. (III 5.4)* Each NOC may enter a maximum of four (4) men and four (4) women. A number of staff are limited to 40% of the numbers of athletes. (III 5.3)* Team of

For all the Team Events, each team shall be composed of two (2) men athletes and two (2) women athletes with one (1) man reserve and one (1) woman reserve. The athletes shall be arranged in a fixed order Men 1 ; Men 2 ; 1 ; 2. In any particular round, the reserve may be used to substitute an athlete who is not playing. Chess s Xiangqi Men Team 2 Men Individual 1 Men Individual (Rapid) Individual NOC for Xiangqi competition may be composed of no more than six (6) persons: (one (1) Team Manager, four (4) male athletes and one (1) female athlete including alternates. Each NOC can register two (2) male athletes to participate in the Men Teams Event. Each NOC can register one (1) athlete to participate in each individual event. Each athlete can only participate in one event. (III 5.3~5.6) * Rapid Chess Team Classical Chess Team 2 2 1/ /1 From the Classical Chess & Rapid Chess Team Event, the best two scores on each board shall qualify for the individual event provided they must have played all six (6) rounds. For Blitz Chess Individual, not more than two (2) athletes (one man and one woman) can be registered (III 5.3~5.5) * Blitz Chess Team 2 2 1/

The team can enter Compulsory Exercise and Optional Exercise and therefore each NOC may enter a maximum of three (3) teams totally in the Dragon & Lion Dance competition. Dragon Dance may be composed of one (1) Team Manager, one (1) Coach, and fourteen (14) athletes (including substituted athletes and musical instrument players). Southern Lion may be composed of one (1) Team Manager, one (1) Coach, and eight (8) athletes (including substituted athletes and musical instrument players). Electronic s (E-s) Need for Speed: Carbon FIFA NBA Live NOC for E-s competition, may be composed of no more than fi ve (5) persons - maximum one (1) competitor in each event. Each NOC may enter a maximum of one (1) team manager and one (1) accompanies staff in E-s competition. (III 5.3) * Northern Lion Compulsory Exercise 10 Northern Lion may be composed of one (1) Team Manager, one (1) Coach, and ten (10) athletes (including substituted athletes and musical instrument players). (III 5.3~5.4) * Optional Exercise

12 / Event Men / Event Men Extreme s (X s) BMX Freestyle Park Park Best Technical Big Air Vert Flatland Inline Stunt Park BMX Freestyle / Inline Stunt / Skateboard NOC for Extreme s competition, may be composed of no more than thirty one (31) persons - nine (9) officials, ten (10) athletes in BMX Freestyle, eight (8) athletes in Inline Stunt and four (4) athletes in Skateboard. There is no restriction in gender, athletes may choose to participate in the group of events within each discipline (BMX Freestyle, Inline Stunt and Skateboard) with restriction. (III 5.3) * Extreme s (X s) s Climbing Speed Competition s Climbing To take part in the competition, in men and women, Speed Competition must be at least sixteen (16) years of age on 26th of October, NOC for s Climbing, may be composed of no more than six (6) men athletes and six (6) women athletes including alternates. A number of each event is limited to three (3) participants from each NOC. A number of staff are limited to 20% of number of athletes from each NOC. Team events (relays) Each participating NOC may enter one (1) team in each event. representing their NOC for Finswimming competition, may be composed of no more than ten (10) men including two (2) reserve athletes and ten (10) women including two (2) reserve athletes for the relay events, in Finswimming. A number of staff are limited to 20% of numbers of athletes from each NOC. Futsal /4 36 NOC for Futsal competition, may enter a maximum of one (1) men s team and one (1) women s team. may be composed of no more than eighteen (18) persons including alternates (14 athletes and 4 Offi cials). For the Futsal competition, the participating teams are allowed to register eighteen (18) names in view of the fact that some players might incur injuries etc., between the above date and the date of the fi nal registration during the Team Managers Meeting.
