Complete and mail to your state coordinator your completed packet. A completed packet includes the following: 1. Application Form (Page 3-5) 3. Essay Information (Page 5) 4. Certification Form (Page 6) 5. High School Transcript 6. Five by Seven (3X5) Glossy Photograph 7. Two (1) Letters of Reference: a) extension agent, b) shooting coach or instructor, c) school representative - principal, guidance counselor or teacher, and d) a person outside your family who knows you well.\ 8. 2 Eligibility 4-H Shooting Sports Teen Ambassador Training The 4-H Shooting Sports Teen Ambassador Program affords teens the opportunity to demonstrate their skills in citizenship, leadership, community service and public speaking as they travel serving as advocates and spokespersons for 4-H Shooting Sports at a variety of events and activities. Teens must: Open to shooting sports participants grade 9-13 have a GPA of 2.5 or higher Be enrolled in 4-H and actively participating in a 4-H Shooting Sports Club for a minimum of 2 years. Be willing to attend the 4-H Shooting Sports Avisory Council meetings when invited. Must be present for an interview on Saturday, May 14, 2016 Interviews will begin at 2:00 p.m. and end at 4:00 p.m. Must be present at the Shooting Sports Banquet on May 14, 2016 Attend special programs and activities requested by the NJS4- HSSAC. Give talks about 4-H and Shooting Sports to outside organizations. Possibly move on to the National Level by attending the National 4-H Shooting Sports Ambassador Training (Information non-applicable at this time) Optional attendance at the National Champions (June) Grand Island NE

Your essay should be no more than two pages, double spaced with margins no less than 1 inch, and a font size no less than 12 pt.

6 Certification Form I have reviewed the expectations (see page 2) of a NJ State 4-H Shooting Sports Teen Ambassador and I am willing to devote the time required. I will be active in my club and county 4-H program, as well as maintain an acceptable academic standard in school during my service as a State and/or National 4-H Shooting Sports Teen Ambassador. Further, I am willing to conduct myself in the highest standards expected of a Teen Ambassador. The 2016 New Jersey 4-H Shooting Sports Ambassador will be recognized at the 4-H Banquet on May 14, 2016.
