4 Training Master Induction and training Before taking the vessel to sea, any new Master must read and understand this MTOP manual and be trained and conversant in all competencies relevant to the safe and clean operation of the vessel. New masters will complete and sign the New Master Induction Training Form refer Appendix 1. New Masters training and sign off may only be conducted by a competent Master approved by NZNMM. Completed sign off sheets will be stored in each Masters personnel file. New Crew member induction and training New crew must read the Hazard Register and sign the relevant section of the Induction Sheet before going to sea. Within one week of joining the ship, all new crew will read the relevant sections of the manual (as listed in the sign off sheet) and complete and sign the form refer Appendix 2. Crew member training may be conducted by any NZNMM master. Final certification must be approved by NZNMM sailing manager. All relevant documentation will be stored in each crew member s personnel file. Drills & ongoing training Drills will be carried out at intervals not exceeding 12 months. Where it is not practical to undertake a simulated drill, then we will discuss the procedure and familiarise the crew and Masters with the operation of the associated equipment. Master & crew competency will be monitored on an on-going basis by observation, feedback and examining the outcome of any drills, incident accident or hazard reviews. Crew will undertake refresher training if they have not worked on board the vessel within the preceding 6 months or at any time increased competency is required. All crew drills will be recorded primarily in the Vessel Logbook. At a suitable time thereafter, records of all drills will be recorded in the Drills Logbook by the Master & retained on board. Periodically the records of drills will be transferred to the data base held on NZNMM electronic storage system. Any training outcomes that may require changes to procedures contained with this Maritime Transport Operator Plan will be noted on the Record of Maritime Transport Operator Plan reviews. This may result in an immediate review or be considered at the next review. Refresher Training conducted shall be recorded in a form refer Appendix 3 and retained in the crew member s personnel file and electronically by NZNMM. 5 1.3.2 Contact details for shore based personnel responsible for managing trip reports and emergency responses: Name Contact phone Alternative number number Chris MURRAY Brian BUTLER Safe Operating Procedures Our vessels must be operated at all times in a safe and efficient manner observing all Maritime New Zealand in-force maritime and marine protection rules. We also abide by all local Navigation Safety Bylaws. All vessels are to be operated within any plan approval limits and stability requirements at all times. The procedures in this Plan are supplemented by information contained in vessel specific manuals and standard operating procedures. The Master has ultimate authority during operations for safe operation and to decide on, and to take whatever action he/she sees fit to maintain the safety of vessel, crew, passengers and the environment Appropriate shore and vessel-based procedures that address all reasonably foreseeable hazards in the maritime transport operation including, but not limited to, the following: Adverse weather/fog Listen to marine weather forecast. During restricted visibility, the Master should adjust speed, turn on navigation lights, post an extra look out and use all available navigational aids. Gear should be stored low and decks and scuppers kept clear. Don appropriate safety equipment (lifejackets, harnesses etc) as deemed appropriate by the Master. Sunscreen worn if working in exposed conditions. 6 Anchoring/mooring/berthing Only trained crew & trainees under direct supervision may operate anchoring equipment Care to be taken with hands & feet thereby ensuring that they remain clear of dangers. Keep loose clothing out of the way. The correct day shape and/or night lights to be displayed. Watch for surge, loose ropes to be stowed. Refer to Vessel Manuals for operational specifics and approved processes Close quarters At all times Maritime Rules Part 22 Collision prevention apply Controlled beaching Refer to Vessel Manual for operational specifics and approval process Crossing bars As per MNZ guidelines document on MNZ website a copy of which is included in the Vessel Manual. Prior NZNMM approval to be obtained Drug & Alcohol Policy Alcohol is not to be brought on board without the approval of the Master Alcohol is not to be consumed: During the period 8 hours prior to sailing or prior to night watch duties at anchor At anchor when the weather forecast indicates that the vessel could be required to weigh anchor at short notice When vessel is underway When passengers are on board Without the specific approval of the Master All crew to be fit for duty at all times If passengers become disruptive with alcohol consumption, Master to limit alcohol consumption and if necessary return to port. If prescription drugs are to be in use during the trip other than short duration trips, the Master must be informed of the medical condition and drugs prior to sailing The master is to be informed when prescription drugs are being carried and the dosage in the event of an emergency. 7 Crew concerned about the Master s ability to safely operate the vessel due to either drugs and/or alcohol should immediately report their concerns to Museum Management. Non-legal substances are not to be brought on board. Masters are to deny permission for a crew member to sail if they believe the crew member does not meet the above conditions and notify the delegated person Enclosed spaces Regularly open and close escape route hatches and doors to ensure all clips & hinges are working freely from both sides Keep ladders clear and unobstructed. Ventilate where possible Environmental control Protecting the oceans for our future All new crew will be briefed on the legal requirements and waste management practices on board. In the safety briefing at the start of each trip, everyone will be reminded that NO garbage will be discharged into the sea at any time. All crew are responsible for monitoring and ensuring compliance. Garbage No garbage will be discharged into the sea at any time All garbage on the boats will be stored in a suitable secure rubbish bin located in the galley. At the end of each trip all rubbish will be disposed of ashore in a suitable rubbish receptacle. Chemical Waste Small quantities of cleaning solvents are sometimes stored on board. Wherever possible environmentally friendly alternatives are used All wastes associated with the cleaning solvents are stored securely on board and disposed of ashore in a suitable facility. Oily Waste Oily rags or waste will be stored securely and taken ashore NO oily waste is to go overboard at any time. Crew to report any oil leaks from machinery to the Master immediately and take steps to stop the oil leak and contain any spillage. Wherever possible, all leaks from machinery will be fixed immediately. Sewerage Untreated sewerage will not be discharged closer than 500metres from shore, a mataitai reserve or marine farm or closer than /04/2016 NZ Maritime Museum Trust Board 7

8 metres from a marine reserve, or further if an applicable local bylaw is in effect. Bleach and other chemicals will not be put into the toilet except for a very small quantity for cleaning purposes only Fatigue Policy Master to monitor crew for obvious signs on joining the vessel. Where possible, the Master must adjust passage plans to ensure that the vessel is operated safely within the capabilities of the crew. Seasick crew to be placed where they can recover most quickly To prevent dehydration crew to be encouraged to drink water The ability of crew to respond in an emergency should be the determining factor in passage and trip decisions Crew must agree to the appropriate response to fatigue which could include rests period, lighter duties or being relieved of duty for an appropriate period of time All crew to be fit for duty prior to boarding the vessel. Master to monitor crew for signs of fatigue during operations If fatigued, the Master should ensure a proper rest break is given to the crew member If a rest break is not possible, lighter duties should be given to the effected crew member. In rostering crew the Sailing Manager will ensure that crew are appropriately rostered according to their capabilities Going aloft a) All persons going aloft shall wear a harness Prior to going aloft all persons: Will receive training in the use of a safety harness and the basic techniques and safety requirements of going aloft. Passengers & trainee crew will only go aloft under direct supervision of a trained and certified crew member Certified crew will receive a Tophand Endorsement, records of which will be retained in the Going Aloft Training Record Book on board. When working aloft: Care must always be taken to avoid risks to anyone below; Warnings notices must be displayed; Tool and stores must be sent up and lowered by line in suitable containers which should be secured in place to keep tools or material not being used; Tools should be attached by lanyard wherever practical. No tools will be placed where they can be accidentally knocked down and may fall on someone below, nor should tools be carried in pockets from which they may easily fall; When work is complete, warning signs can be removed. 14/04/2016 NZ Maritime Museum Trust Board 8

9 b) Open hatches Keep secured back when open and closed when not in use. Rope off & use extra signage if necessary Passenger Safety All Passengers are given a safety briefing prior to departure, which shall include but not be limited to the following: Lifejacket location and fitting Hazards relative to the vessel Garbage policy Obeying instructions of the crew Emergency procedures First Aid Kit location Actions to take in event of sighting a man overboard Passenger Count Passengers are counted on board by a crew member and the count is then recorded in the ships log. Food hygiene NZNMM contracts an external caterer for food preparation and supply. Crew are trained in basic food hygiene and food handling practises. Disabled Passengers Every reasonably practicable step will be taken to ensure the safety and comfort of disabled passengers. Refer to the individual Vessel Manual for specific guidelines. Safety Signage All signage required by legislation is in place on our vessels and other signage where appropriate is displayed. 14/04/2016 NZ Maritime Museum Trust Board 9

10 Pre departure checks Consider current & forecast weather conditions All crew fit for duty & registered volunteers with NZNMM All crew are briefed on safety procedures and operations for that day Lifejackets in their correct stowage or worn and in good order Vessel is seaworthy & steering is operational All bags & other supplies safely stored & secured Safety Equipment in location & in operational condition. Trip report to shore base person stating POB, intended destination & ETA. Vessel Maintenance is up to date Bilges reasonably dry Bilge & Fire Valves set correctly Log book filled in Master to check the Defect Book prior to departure All listed Spare Parts are on board When vessels undertake extended passages, a Vessels Plan and Food Plan (If applicable) must be completed and handed to the Sailing Master prior to departure Refer to Vessel Manuals for specific checks Post trip checks Turn off all sea cocks not required Close down trip report Fill in ship s log Remove rubbish and dispose ashore Secure mooring lines and lock cabin/hatches. 11 Standing Orders At all times NZNMM Standing Orders will be adhered to. Refer to the Vessel Manual for vessel specific Standing Orders Transferring crew & passengers Set mooring lines so vessel lays parallel to the wharf/pontoon & doesn t swing too much Use gangway if possible & secure ladders or gangways to vessel Use permanent handrails and/or steps on side of vessel if practical Don t take risks Emergency procedures in response to, but not limited to, the following events: Fire Raise alarm Reassure passengers Lifejackets on if not already on Attempt to put out fire Stop ventilation where possible Monitor heat in surrounding compartments & cool if necessary Prepare to abandon ship Report to MNZ as soon as practicable Overdue vessel procedure The Master should contact shore based personnel to update the trip report in the event of the vessel being delayed. If shore based personnel responsible for managing trip reports and emergency responses are concerned about the vessels whereabouts they should first attempt to contact the vessel by all means possible If unable to make contact, they must call: Maritime Radio on ; OR Coastguard on ; OR Rescue Co-ordination Centre of NZ The Master is responsible for ensuring that Spills, Accidents & Incidents are reported to MNZ 14/04/2016 NZ Maritime Museum Trust Board 11

12 Pollution Oil & Oily Waste Oil or oily water is not to be discharged into the sea. The spiller is responsible for managing their oil spill. If any oils spills into the sea or an oil spill is suspected, crew will immediately notify the Master. Any oil on the decks will be cleaned up before the decks are washed. Any oil spill or suspected oil spill will be reported immediately to the Auckland Harbourmaster 24hour emergency duty officer on (Press 0 after hours). The incident should be recorded in the vessel logbook by the Master. Any oil spill or suspected oil spill will be reported immediately to MNZ and all possible actions taken to prevent the spread of the spill Person overboard Raise alarm Immediately throw flotation device into water Appoint lookout to keep pointing to person in the water Manoeuvre vessel within safe proximity of the person overboard Recovery crew to don lifejackets Recover person and keep warm/cpr if necessary Radio for assistance if required Report to MNZ as soon as practicable Medical emergency Ascertain as accurately as possible the extent of the injury or illness If life threatening or serious: Call for immediate assistance Follow strictly any medical advice given to stabilize the patient Proceed with best possible speed toward assistance or nearest port 2 The Master is responsible for ensuring that Spills, Accidents & Incidents are reported to MNZ. 14/04/2016 NZ Maritime Museum Trust Board 12

13 Keep radio contact with shore base and continually update on the situation Assist with evacuation of patient when assistance arrives or at port. Minor Injury/illness: Make patient as comfortable as possible Monitor situation for any signs of deterioration Always recommend or refer patient to a doctor or the hospital on return to home port Mechanical failure, including loss of propulsion or steering Loss of Propulsion If sailing, continue to sail to safe location to anchor and anchor If under power either anchor or raise sails and sail to a safe location to anchor as safety demands; Call for assistance and/or attempt to remedy the situation. Loss of steering If able to use throttles or sails to control direction of vessel, manoeuvre vessel to where safe to anchor and anchor; or If unable to control vessel using throttles or sails, anchor immediately. 14 1.4.3 Hazard Register Hazards are reviewed at least annually, and the Maritime Operation is constantly being monitored to ensure that all hazards are identified and appropriate controls put in place. In addition to identified hazards as per the specific Vessel Manual, and hazards as per the Operator s shore based Hazard Register, other reasonably foreseeable hazards & safety risks in the Maritime Transport Operation are set out hereunder: HAZARD REGISTER Hazard Adverse weather Trips, falling & slips Significant Eliminate or Minimise? Information Management Procedures Storage Our Maritime Transport Operator Plan will be stored electronically on the NZNMM computer server. Review Reviews will be conducted annually or when necessary. Results will be recorded in the Record of Maritime Transport Operator Plan Reviews in Appendix 5 of this document. If changes are made to the Maritime Transport Operator Plan, the hard copy located on-board the vessel will be replaced by the current version as soon as practicable, and before any operations of the vessel. All persons named in the MTOP will be made aware of any changes and will sign the Record of Maritime Transport Operator Plan Reviews to indicate they have read and understood the amendments. 16 Continuous improvement Information considered during reviews will include the outcome of any hazard, incident or accident reports, crew feedback from operations and training, customer feedback, peer discussions with other operators and information contained in Maritime New Zealand publications. Distribution of the Maritime Transport Operator Plan All crew will receive a copy of the Maritime Transport Operator Plan. The Maritime Transport Operator Plan will be available on-board the vessel in hard copy. The Ship s Log is maintained on-board the vessel. Maintenance & Survey Plans will be stored in the office All Ship s Logbooks will be retained in the Museum s archives Procedures for training of, and communication to, all personnel of safe operating procedures in the Maritime Transport Operator Plan to ensure those procedures in the Maritime Transport Operator Plan are understood and implemented effectively: All personnel or crew will read and understand this plan before taking any responsibility within the maritime transport operation. New crew will then sign the induction form (Appendix 2) to record that they have read and understand the document. Feedback is encouraged on the effectiveness of the policies and procedures within, and this information may be used during the regular review process Procedures for checking and ensuring currency of qualifications of personnel, including crew: Crew will provide a copy of their qualifications and certificates to be stored electronically in NZNMM Health & Safety Management System (Riskmanager). Hard copies of certificates will be stored in each person s file. Relevant expiry dates will be checked regularly and the crew reminded of any expiry dates coming up. 17 1.5.4 Procedures for accident and incident reporting: Details of any accidents, incidents and mishaps that may occur will be recorded in the Ship s Logbook immediately or as soon as practicable thereafter. The Master as soon as practicable, shall complete an NZNMM INCIDENT REPORTING FORM (Appendix 3) and hand same to one of the persons responsible for compliance named in this document. The Accident & Incident Register in Riskmanager will be updated at a suitable time. All accidents and incidents will be investigated by NZNMM to determine if corrective action needs to be taken If it is determined that changes to procedures and/or further training is required, this will be undertaken as soon as possible. Any changes to the MTOP as a result of the investigation will be noted in Appendix 5. All Incidents, accidents and mishaps must be reported to MNZ as soon as practicable by the Master Procedures for logging and managing trip reports: Trip reports will be logged with shore based personnel at the beginning and end of each trip, stating Persons On Board, destination and ETA for return to port. 14/04/2016 NZ Maritime Museum Trust Board 17
