P & I Club with which the vessel is entered 12 Nov 97 and valid until 20 Feb 08. Policy indicated that trading area is gulf of Thailand and inland waters. Deductible is all other claims USD 10,000 each incident. Marine hull insurance (Hull & Machinery) of this vessel was available that period of insurance is valid until 5 July 07 at 12:00 (LT). Master had been traded on local area for many years and appeared to rely more on local knowledge during navigation along the river and intended trading area. He had been joined on this type of oil tanker over 15 years. Anyway, thanks for your kind comment, the barge had already up date the mentionable light list in the navigation charts at the date of inspection.[ ] 4.33 If an EC DIS system is fitted, is it fully operational and are fully operational backup components provided? She is of one continuous welded steel construction with a single hull, double bottom configuration. There is a system to monitor regular atmosphere checks for hydrocabon content and regular sounding of double bottom and recorded Do personnel demonstrate familiarity with the operation and calibration of portable gas detection instruments? Vessel was provided with 1 combustible gas detector and 1 Oxygen analyzer. Evidenced reports on last calibration carried out by external agency on 29 June 06 and 1 Feb 07 respectively At what frequency do personnnel undergo medical examinations? Procedures had been written in SMS manual however barge not handled tank cleaning in normal operation except prior docking and operator used a contactor for tank cleaning Are the dangers associated with tank cleaning clearly understood? Initial Operator Comments As per the inspector report that the air condition unit appeared out of order and is not in use for along time. Moreover, we have the notice no used place on that place and the master had already shown to the inspection at the date of inspection too Has the vessel been issued with an approved Ship Security Plan? Oil record books (I & II) record book were properly entered. Removal of oily bilges in engine room facilitated via oily water separator and recorded. 6.2 Is an approved MARPOL Shipboard Oil Pollution Emergency Plan (SOPEP) or Shipboard Marine Pollution Emergency Plan (SMPEP) provided? Vessel exempted to carry IOPP certificate from Administration include SOPEP. 6.3 Is the vessel provided with a USCG approved Vessel Response Plan (VRP)? Vessel is licensed to trade in Thai coastal water. 6.4 Is suitable equipment provided to deal with small oil spills? Absorbent material (saw dust) adequately stowed and oil spill despersant was available. Contents checklist was available for regular record. 6.5 Are anti-pollution notices posted? Oil spill prevention drills conducted regularly as per shipboard drill plan and activities recorded. 6.8 Is a perimeter spill rail fitted; are scuppers in place and are they liquid tight? The deck boundary coaming is 150 mm in height to contain oil spills. 6.9 Are all the cargo manifolds provided with spill trays? Manifold safe-alls of ample size and fitted with drains and covers Are spill savealls installed around bunker and diesel tank vents? Suitable fixed spill containers placed around bunker / diesel and lubricating oil tanks vents and the height of any savealls are not be greater than the vent If so required, is the vessel provided with a containment boom? [Q6.15 is not applicable to this report] Chapter 7 Structure 7.1 If the vessel is enrolled in a structural survey programme, are records available? Vessel is enrolled annually in a structure programme by the Thai Marine Department. Such department issued a certificate of survey on 12 Dec 06 and valid unit 11 Dec 07 without any survey reports. 7.2 If structural survey records are available, do they record that the hull thickness measurements are within acceptable limits? The hull thickness measurement had been carried out on 6-30 May 03. Thickness measurement reports revealed diminution to be minimal or within limits. 7.3 Are records available to indicate regular inspection and testing of tank coatings and/or stainless steel tanks? The documentary evidence to indicate inspection of tank coatings found in order. C argo tanks were sealed by loading port. She carried MFO. 7.4 Additional C omments. Vessel had not been completed an enhanced survey programme. Hull external appeared intact and satisfactory. Undulation between frames not evident. Some light and moderate dents noted around exposed top-sides and boot-tops. Bow section and stem curvature appeared generally intact and satisfactory. External paint coat on top-side appeared intact. A/F paint on exposed boot-top area appeared intact. Marine fouling not evident on visible hull surfaces. [1 7]
18 Chapter 8 Cargo Handling 8.2 Has a Ship/Shore Safety Check List (SSSCL) been properly completed and have those items that require reinspection, been inspected at the appropriate intervals? Ship/Shore Safety Checklist had not been made prior to the current transfer commencing. Initial Operator Comments The Ship/Shore Safety Checklist for the mentionable shipment had already made by the crew. Negligent in the discharge of the crew duty is warning immediately by Captain. The additional reinforcement from our management team is held during the POCC meeting and safety stand down at BOI port at Bangna yesterday [ ] and we will have the special [HSSE] meeting for this case at the head office on as well. 8.3 Have written loading, discharge or ballast transfer plans, as appropriate, been prepared for the current operations? Discharging plan had not been prepared for the current operation. Initial Operator Comments The same set as 8.2.The discharging plan of the mentionable shipment had already made by the crew. Negligent in the discharge of the crew duty is warning immediately by Captain. The additional reinforcement from our management team is held during the POCC meeting and safety stand down at BOI port at Bangna yesterday [ ] and we will have the special [HSSE] meeting for this case at the head office on as well 8.5 Are legible and up to date pipeline and/or mimic diagrams of the cargo system, inert gas system and venting system available in the cargo control position? Mimic diagrams is available. IGS dose not apply to this vessel. 8.6 Is information readily available to the responsible persons relating to maximum loading rates and venting capacities?
