Lessons Learnt We all know on a daily basis what we should do and hopefully what should be in place including: Meaningful Risk Assessments and COSHH Assessments - know your chemicals including delivery and storage processes Hierarchy and implementation of control measures Standard operating procedures Emergency procedures Evacuation procedures Qualified and experienced staff Suitable training and supervision for staff Safety signage
Lessons Learnt The panel are going to discuss what lessons can be learnt following an incident from their industry: David Stokes Environmental Health Officer Tim Marshall Director, Integro Insurance Crispin Kenyon Solicitor, Weightmans LLP
Lessons Learnt So what happens when things go wrong, examples of recent chemical incidents : Staff mixed wrong chemicals in plant room to create chlorine Residue of one chemical mixed with another in a bucket Chemical reaction led to 8 children and six adults being treated for minor burns costs people were taken to hospital breathing difficulties. following a chemical spill Accidental mixing of chemicals fine + costs Release of noxious gas costs
Lessons Learnt What should be in place for chemicals: Chemicals stored in containers and tanks of compatible construction Equipped with a secondary containment system or bunding which is resistant and certified to hold highly corrosive liquids Adequate PPE such as safety gloves, glasses and respiratory masks for operatives handling the hazardous materials SOP (standard operating procedure) must be available for all operatives to read when using the chemicals Safety signage must be positioned on cabinets, containers and tanks highlighting the hazards each chemicals poses.
Why investigate accidents David Ward-Stokes Technical Officer Environmental Health
How accident investigation can help manage risk Prevent further accidents: identify controls Better understand business hazard and risk Prevent loss Public interest
5 top tips for accident investigation: Investigation is not to apportion blame (even if that is an eventual outcome) Detailed and analytical establish what happened and why If human / employee error is sole cause likely to have missed key points Identify missing / inadequate / unused controls Establish and analyse causes
