13 Safety Induction Procedures for new staff and students SHIP 2008 Submission All new staff and students For students/personnel ts/pe so e below 21 * Required to fill up Facility Access Exclusion of liability and Indemnity Form * * 1) Lab Supervisor or PI to guide and inform staff/student regarding the safe practices of the lab using the Personnel Safety Checklist Form 2) New staff/student to perform Lab bsafety Assessment on NUS IVLE. Contact Rachel (surtsm@nus.edu.sg) on details for IVLE access. 3) Undergo OSHE Structured Safety Training System. For staff, register for the relevant courses with OHR or through the Training option in mynus staff Portal. For students, register online at OSHE s website: ( Lab specific safety orientation Department specific training NUS OSHE managed safety courses * Upon completion, send a copy of these forms and Lab Assessment results to Juwita at or scan and to surjnr@nus.edu.sg For staff, students and personnel who are on short term projects, this (3) may not be possible due to time constraints.
