1 Centre XYZ Risk Management Policy Principle Aim: The overall policy is designed to ensure the safety of guests while undertaking activities at Centre XYZ. Some activities are regularly supervised by, others are supervised by Guest Representatives (Teachers/Leaders). This document is available as a guide to all in positions of responsibility to achieve the Principle Aim. The method of Risk Assessment and Mitigation is based on the following Risk Rating Matrix and its general response guidelines. The activity will not knowingly be conducted at Centre XYZ or by Centre XYZ personnel. Fatality High. Medium. Low. The activity may only be conducted by following the Risk Management Plan elimination and control measures. Suitably trained and competent personnel must supervise this activity. 2 Centre XYZ Risk Management Plan element for NSW DEET Schools On site Sport & Activities at Centre XYZ This RMP covers On Site Sport & Activities at Centre XYZ. Activities include: Volleyball, Basketball, Pool Table, Table Tennis Table, Bats & Balls, Outside Chess and use of the grass Playing Field. 1. Tripping / twisting Ankle 2. Injury due to falling / unforseen circumstances 3. Insect Bite 4. Getting Lost 5. Sunburn 6. Allergy to plants/grasses The sport and activities covered here are those normally played and experienced by guests. Details specific to each activity are not considered necessary as they are so well known. School staff should make reference to Guidelines for Safe Conduct of Sport and Physical Activity in Schools NSW DEET 1999 for specific requirements of each activity. Centre XYZ provides the facilities and equipment in a Bush Setting they are not promoted as, nor expected to be, competition grade facilities. 1. FA P = Medium 2. FA R = Low 3. FA P = Medium 4. Reduction by Instruction (walk carefully, don t run, push), Correct Footwear (no thongs, sandals use sturdy shoes), On-Site First Aid Kit 2. Reduction by instruction (Walk don t run, look out for each other, follow Instructions and or rules) 3. Reduction by Instruction (no fingers in holes, be aware, don t touch), First Aid Kit, First Aid Trained personnel (SFA Level), insect spray, observe for leeches and ticks (Rare but possible). 4. These activities are all conducted sight of the Centre XYZ Buildings. 5. Keep outdoor experience in full sun to a minimum. 6. Supervision and control of students with known allergies. A 90 Minute activity, Conducted within site of the Centre XYZ Buildings. 1. Becoming Lost 2. Heat Exhaustion 3. Insect/Animal Bite 4. Tripping / twisting Ankle 5. N U = Low Elimination Substitution Isolation Engineering Administration PPE 1. Reduction by Instruction (30-40 Minutes session on map reading, groups of 2 or 3, Stay with partner(s) stay within sight of buildings), Supervision (Ratio of Max 1:20, regular counts, minimum 2 leaders, check all students return), Follow up if group > 5 minutes after all others have finished. 2. Reduction (no activity when temp > 35ºc), having water bottle, wear Hat, allow drink break. Observe walkers for symptoms. 3. Reduction by Instruction (no fingers in holes, be aware, don t touch), First Aid Kit, First Aid Trained personnel (SFA Level), insect spray, observe for leeches and ticks (Rare but possible). 4. Do not conduct in inclement weather. 5. Reduction by instruction (be aware of drops and stay clear, Walk don t run, look out for each other, remain in sight of buildings at all times), Do not conduct in inclement weather. 6. Reduction by application of Sunscreen every 4 hours, Wear Hat. 7. Reduction by briefing guests on nature conservation, flora and fauna protection. As all of the risks identified are in the Low to Medium Risk Matrix this activity may be supervised by a responsible adult who performs the tasks indicated in this plan. This activity MUST be supervised by an adult. 1. Equipment failure 2. Falling off trampoline 3. Colliding with other jumper 4. Injury caused by landing awkwardly 5. Injury mounting/dismounting 6. Reduction by visual check prior to every supervised use. Mat must not have cuts or holes, Springs in place, Spring Cover Matting in place. Support structure intact and sturdy. No shoes on trampoline. 2. Mitigate by use of competent supervisor, allow activity the participant is capable of, no new jumps one spotter on each side of trampoline, Ensure participants know how to stop bouncing. Jump in the middle of the trampoline, Do not jump from the trampoline to the ground 3. Prohibit two or more on trampoline obey signage - one at a time. 4. 5 Centre XYZ Risk Management Plan element for NSW DEET Schools SWIMMING at Centre XYZ This RMP covers Swimming in the 11 x 4m Salt Water Pool at Centre XYZ. This activity MUST be supervised by an adult with CPR qualifications. 1. Drowning 2. Injury caused by slip, fall, jump or dive 3. Cramps 4. Sunburn 5. Suction Injury 6. Exposure to Hazardous Material Human waste product. 7. Exposure to Hazardous Materials Chemicals 8. Illness caused by Chemical imbalance in the water 1. C P = High 2. M+ P = High 3. M P = Medium 4. FA L = Medium 5. M+ R = Low 6. M+ P = High 7. C R = Medium 8. M U = Medium Elimination Substitution Isolation Engineering Administration PPE 1. Mitigate by 2 supervisors must be present at all times. One must have current CPR qualifications. Supervisors may not be distracted participating or reading etc Supervisors must be aware of rescue aids available floats, towels and rings Supervisors must be prepared to enter the water to assist but only as a last resort. Ensure participants know the depths of the pool as indicated Only allow participants with swimming ability to get out of their depth Maximum of 22 swimmers in pool at any one time. Supervisor:participant Ratio must not exceed 1:20. Pool must not be used when bottom cannot be seen (at night. if water cloudy) Do not use in thunderstorm or very high winds. Ensure gate shuts on departure, and is NEVER propped open. 2. No running, diving or jumping into or around the pool obey signage. 3. Mitigate by taking care to be well hydrated and not to have eaten too soon prior to the swim. 4. Reduction by application of Sunscreen every 4 hours, Wear Hat. 5. Do not use pool if suction cleaning device is in pool, do not allow participants with very long hair to stay near the skimmer box, do not allow participants to remove skimmer box cover. 6. If human waste is observed the pool must be immediately vacated, Centre XYZ Staff notified and sanitation occur. Pool will remain closed for one complete sanitation cycle 4 hours. The following are the primary responsibility of Centre XYZ, but all supervisors should ensure that any possibility of occurrence results in vacating the pool area and is reported immediately. 7. Keep Pool Shed (chemicals and pumps etc) locked at all times. 8. Mitigate by monitoring and adjusting pool chemical balance in accordance with best practice principles and council regulations. Equipment failure 2. Falling 3. Rope Burn / entanglement 4. Injury caused by collision 5. Injury mounting/dismounting 6. Equipment Misuse (Bravado) 7. Emotional Trauma 8. Sunburn 1. M+ U = Medium 2. FA L = Medium Elimination Substitution Isolation Engineering Administration PPE 1. Reduction by visual check prior to every supervised use. Seat Belt must be operational, seat running smoothly and at least 10 metres of tow rope. Equipment is checked by Centre XYZ staff regularly to ensure integrity of system. 2. Mitigate possibility by Correct use of seat belt Supervision to ensure no skylarking, swinging of cable and/or chair One person at a time on the whole apparatus ie do not allow cable to be held or swung. 3. Instruct pullers to let Tow Rope go smoothly and together, supervise. Prohibit running beside rope as it may catch runner. 4. Mitigate possibility by Supervision of participants, keeping pathway clear Ensure Tow Rope >10 metres braking effect. 5. Mitigate by mount/dismount with care no jumping assist participants during mount/dismount 6.. Mitigate by ensuring correct of year old males in particular. Centre XYZ staff will warn of these dangers in introductory talk. Supervisor must be competent to supervise the participant age group 7. Reduction by only having participants undertake activity voluntarily, and stop ride on signs of discomfort/fright. 8. Reduction by application of Sunscreen every 4 hours, Wear Hat.
