1 Guidance in Completing the Risk Management Plan Proforma: Excursion Hazard Identification The following may assist with identifying hazards relating to activities at each stage of an excursion. Consider what could go wrong, that is, the potential injuries or illnesses that could occur. Hazards are the sources of these potential injuries or illnesses. Travel Consider aspects of travel that may present a hazard such as walking to and from the train, crossing the road, transport to the venue. If CBD location, consider travel arrangements in the event of lockdown or evacuation. Venue Consider aspects of the excursion venue that may present a hazard such as location near water, cliffs, crowds, slippery floors. If CBD location, consider possibility of CBD lockdown or evacuation due to evacuation trial or major emergency Excursion Program Activity Consider the activities of the excursion program that may present a hazard such as hazards of bushwalking, collecting leaves, observing animals, swimming, singing at an eisteddfod, climbing, eating at different venues for students with anaphylaxis. Equipment Consider any equipment that may present a hazard such as sporting equipment, high risk equipment at the venue. Environment Consider aspects of the environment that may present a hazard such as weather conditions, natural hazards such as bushfires, floods or storms, the nature of the terrain, plants, animals and insects. Consider common allergens that may pose a hazard e.g. foods, insect stings or bites, latex (e.g. balloons or swimming goggles and caps) that may trigger allergic reactions. These must be considered as part of the risk assessment for students with allergic conditions (anaphylaxis). People Consider aspects of people that may present a hazard such as poor behaviour, the nature of participants such as maturity, age and skill, child protection issues, medical conditions or disabilities. Accommodation Consider aspects of accommodation that may present a hazard such as insufficient supervision, standard of accommodation and amenities, meal menus and allergies, security and child protection issues Other Consider other hazards related to specific excursions such as access to first aid and mobile phone reception. Investigate access to emergency services and equipment. Risk Elimination or Control Risk Assessment Matrix How serious could the injury be? How likely is it to be that serious Very Likely Likely Unlikely Eliminate the risk. Eliminate the item or activity; e.g. do not undertake a particular high risk activity such as abseiling in high wind; do not use high risk equipment. If elimination is not reasonably practicable, control the risk as far as practical using the hierarchy of controls below. Select the highest possible control and/or use a combination of controls to reduce the risk. Substitute the hazard: Replace the activity, material, or equipment with a less hazardous one eg choose an easier bushwalk; substitute a food known to cause severe allergic reactions (for example, peanut butter or tree nuts) with alternative nutritious food. Isolate the hazard: Isolate the hazard from the person at risk; isolate through distance e.g. select a lunch location well away from the water; check if a coastal walk has fencing. Use engineering controls: Have access to equipment to counteract the hazard; consider hiring coaches with seatbelts and ensure these are worn if available; ensure that an appropriately trained person is with the student at all times and has immediate access to an Epipen in the event of an emergency. Use administrative controls: Establish procedures and safe practices e.g. supervision of students, clear rules, instruction in safe methods, training of staff, volunteers and students in the excursion activities or in the use of equipment and qualifications of instructors. Ensure health care plans are reviewed and updated for the excursion for students known to have severe allergic conditions. Discuss student health needs with caterers, in consultation with parents/carers. Use personal protective equipment: Use appropriately designed and properly fitted equipment such as safety goggles, hats and sunscreen and helmets, in conjunction with other control measures identified from above. Encourage students and staff to wear appropriate footwear and protective clothing at all times; students wear medic alert bracelet or necklace where required. This proforma is only a guide and prefilled with information to assist to you in filling out the form. You must ensure this RA is completed directly

2 This Risk Assessment(RA) must be signed off by all staff attending and reviewed by supervisor. RA must be then logged with DP 2 weeks prior to excursion. Attach to the Excursion/Incursion in Sentral/Activities as a saved PDF as Activity name. Students must make their own way to and from Emerton Leisure Centre. - Support Students. HT Support and an SLSO will meet at SCHOOL the Support students who have permission to attend the School Swimming Carnival. Those students will be walked over to Emerton Leisure Centre by the HT Support and an SLSO. - Brief Students on rules and behaviour - Remain on pedestrian pathways and use pedestrian crossings at all times Excursion Coordinator A.Healy HT Support R.Harnett SLSO H.Christense n Prior to walk On excursion VENUE : - Accessibility of venue - Unaware of student attendance - Students getting lost - Lack of qualified staff to deal with an emergency 6 - ELC is easy to access - View roll marking procedures (attached on Sentral) and ed to staff - Staff are scheduled for supervision inside and outside areas of ELC - ELC has staff trained in Senior First Aid, CPR and Anaphylaxis CCMDC Staff ELC Staff Prior to participation

3 training, AUSTSWIM and Life Saving to assist with any emergencies during the Swimming Carnival -Lack of appropriate facilities - Students have to evacuate venue due to emergency - Slip, fall resulting in injury - Falling on hard and/or soft surfaces 6 - ELC has a first aid room, showers, change rooms, shelter and external communication - Organiser brief students on emergency exits and evacuation procedures - In case of emergency Staff proceed to emergency meeting point and mark student lists to ensure all students are accounted for. (ELC evacuation signalled by loud blasts. All students and staff are to assemble in the ELC car park behind the 0m pool). - First aid kit on hand in case of injury - Students are briefed on expected behaviours - Brief students on planned route ACTIVITY - Students unsupervised -Drowning -Dehydration/Sunburn -Medical conditions -Lack of a Risk Assessment 4 - Staff are scheduled for supervision inside and outside areas of ELC - Staff informed of student supervision expectations - Students informed of out of bounds areas - Swimming Carnival permission note requires parents/ caregivers to indicate student s swimming ability (non- swimmer/ nonswimmer) - Swimmers are recorded on a spreadsheet, which is checked by marshalling staff prior to each race. - Weaker swimmers are placed in lanes 1 and 7 and a buddy system is offered Nominated Staff CCMDC students Parent/ Guardian (Permissio n notes) Prior and during activity - Students are informed of what to do if they get in trouble in the water (e.g.: use lanes/ edge, signal for help) - Students are briefed on pool safety rules (e.g.: no running) and expected behaviour. Students not following these rules and expectations will be referred to the IC house patron - Sun safety (e.g.: hat, sunscreen) is discussed at School Assembly, sunscreen is available at venue

4 - Note requires parents/ caregivers to inform the coordinating teacher of any special needs, allergies and medical conditions/ medication. Information is communicated to event Marshalls (using a spread sheet) - If a medical condition puts student at particular risk in the water, a medical certificate must be obtained in order to swim - Swimmers with medical conditions are recorded on a spread sheet, which is checked by marshalling staff prior to each race. - Students with medical conditions are placed in lanes 1 and 7 and a buddy system is offered - A copy of relevant Health Care plans are taken to the Swimming Carnival are located at the recorders table. - Appropriate First Aid available at venue - Senior First Aid officer present - All accompanying staff members are required to read the swimming carnival risk management plan EQUIPMENT -Inappropriate equipment -Lane ropes, rescue aid, kickboards, mobile phone, PA system, wrist bands, cones for designated areas, digital starter gun, sunscreen, some shelter, textures to indicate no permission to swim, first aid equipment.

5 ENVIRONMENT - Lack of shelter - -Sun/Heat - Lack of hydtration - Excessive rain -Grandstand has shelter -Shelter and students advised to bring hats -Canteen, water taps in bathrooms and students advised to keep hydrated. -Shelter and indoor area Food Preparation (If required) - Students gets food poisoning - Students bring their own food. - Buy from ELC canteen Students ELC Prior to/during Allergies/Medication - Students suffers from an allergic reaction - Students requires medication - First Aid Trained Staff present - First Aid kits available - A ventolin and Epi Pen is taken to ELC and is located at the Recording Table - Health Care Plans located at the Recording Table - Mt Druitt Staff Senior First Aid Training located at the Recording Table - (Access to Health Care Plans, Student permission notes, speadsheet indicating any medication conditions and requirements from permission notes. - Children with Anaphylaxis will carry their Epi Pen with them where possible and can ask staff to take responsibility for its safe keeping - Students encouraged to bring own known medication and Staff are made fully aware of student s needs via School permission note. (Any required medication is picked up from the front office and taken to the venue with time required for student consumption). Prior to/during - First Aid Trained Staff present during matches / First Aid kits available - Request to students for those students who have Asthma will carry puffers & spacers with them to and from the venue. - Asthma puffers & single use spacers in First Aid kits PEOPLE - Staff not qualified to deal with injuries - ELC staff are appropriately trained ELC Prior to/during
