Scpe This prgram cvers all emplyees and ther wrkers that may be invlved in cnfined space entry. When wrk is perfrmed n a nn-wned r perated site, the peratr s prgram shall take precedence. This dcument cvers NC Sturgen emplyees and cntractrs and shall be used n wned premises, r when an peratr s prgram desn t exist r is less stringent. Definitins Acceptable entry cnditins - the cnditins that must exist in a cnfined space t allw entry and t ensure that emplyees invlved with a cnfined space entry can safely enter int and wrk within the space. Attendant - an individual statined utside ne r mre Cnfined spaces wh mnitrs the authrized Entrants and wh perfrms all Attendant's duties assigned in the NC Sturgen Cnfined Spaces Prgram. Attendants must have sufficiently cmpleted and fully understands the Cnfined Space training and is apprved by the HSE Manager t wrk in a cnfined space as an Attendant. Authrized Entrant - an individual wh is authrized by NC Sturgen t enter a cnfined space. Entrants must have sufficiently cmpleted and fully understands the Cnfined Space training and is apprved by the HSE Manager t wrk in a cnfined space as an Authrized Entrant. Blanking r Blinding - the abslute clsure f a pipe, line, r duct by the fastening f a slid plate (such as a spectacle blind r a skillet blind) that cmpletely cvers the bre and that is capable f withstanding the maximum pressure f the pipe, line, r duct with n leakage beynd the plate. Cnfined Space A space that is large enugh and s cnfigured that an emplyee can bdily enter and perfrm assigned wrk; Has limited r restricted means fr entry r exit (fr example, tanks, vessels, clers, strage bins, hppers, vaults, and pits are spaces that may have limited means f entry); and Is nt designed fr cntinuus ccupancy. Duble blck and bleed - the clsure f a line, duct, r pipe by clsing and lcking r tagging tw in-line valves and by pening and lcking r tagging a drain r vent valve in the line between the tw clsed valves. Emergency - any ccurrence (including any failure f hazard cntrl r mnitring equipment) r an event internal r external t the cnfined space that culd endanger Entrants.

Entry - the actin by which a persn passes thrugh an pening int a cnfined space. Entry includes ensuing wrk activities in that space and is cnsidered t have ccurred as sn as any part f the Entrant's bdy breaks the plane f an pening int the space. Entry permit means the written r printed dcument that is prvided by NC Sturgen t allw and cntrl entry int a cnfined space that cntains the infrmatin specified in this prgram. Entry Supervisr - the persn respnsible fr determining if acceptable entry cnditins are present at a cnfined space where entry is planned, fr authrizing entry and verseeing entry peratins, and fr terminating entry as required by this sectin. Entry Supervisrs must have sufficiently cmpleted and fully understands the Cnfined Space training and is apprved by the HSE Manager t wrk in a cnfined space. An Entry Supervisr als may serve as an Attendant r as an authrized Entrant, as lng as that persn is trained and equipped as required by this sectin fr each rle he r she fills. Als, the duties f Entry Supervisr may be passed frm ne individual t anther during the curse f an entry peratin. The Entry Supervisr is respnsible t test and mnitr the atmsphere cnditins. Hazardus atmsphere - an atmsphere that may expse emplyees t the risk f death, incapacitatin, impairment f ability t self-rescue (that is, escape unaided frm a cnfined space), injury, r acute illness frm ne r mre f the fllwing causes: Flammable gas, vapr, r mist in excess f 10 percent f its lwer flammable limit (LFL), (0% is nrmal). Atmspheric xygen cncentratin belw 19.5 percent r abve 23.5 percent, (20.9 % is nrmal). Any ther atmspheric cnditin that is immediately dangerus t life r health. (Ex.-H2S 10%, 0% is nrmal). Nte: Fr air cntaminants fr which OSHA has nt determined a dse r permissible expsure limit, ther surces f infrmatin, such as Material Safety Data Sheets that cmply with the Hazard Cmmunicatin Standard, published infrmatin, and internal dcuments can prvide guidance in establishing acceptable atmspheric cnditins. Ht wrk permit - the written authrizatin t perfrm peratins (fr example, riveting, welding, cutting, burning, and heating) capable f prviding a surce f ignitin. Immediately dangerus t life r health (IDLH) - any cnditin that pses an immediate r delayed threat t life r that wuld cause irreversible adverse health effects r that wuld interfere with an individual's ability t escape unaided frm a cnfined space. Nte: Sme materials -- hydrgen fluride gas and cadmium vapr, fr example -- may prduce immediate transient effects that, even if severe, may pass withut medical attentin, but are fllwed by sudden, pssibly fatal cllapse hurs after expsure. Such materials in hazardus quantities are cnsidered t be immediately dangerus t life r health. Inerting - the displacement f the atmsphere in a permit space by a nn-cmbustible gas (such as nitrgen) t such an extent that the resulting atmsphere is nn-cmbustible. This prcedure prduces an IDLH xygen deficient atmsphere. Islatin - the prcess by which a cnfined space is remved frm service and cmpletely prtected against the release f energy and material int the space by such means as: blanking r blinding; misaligning r remving sectins f lines, pipes, r ducts; a duble blck and bleed system; lckut r tagut f all surces f energy; r blcking r discnnecting all mechanical linkages. Line Breaking - the intentinal pening f a pipe, line, r duct that is r has been carrying flammable, crrsive, r txic material, an inert gas, r any fluid at a vlume, pressure, r temperature capable f causing injury. Nn-Permit Cnfined Space - A cnfined space that des nt cntain r, with respect t atmspheric hazards, have the ptential t cntain any hazard capable f causing death r serius physical harm. Oxygen deficient atmsphere - an atmsphere cntaining less than 19.5 percent xygen by vlume. Oxygen enriched atmsphere - an atmsphere cntaining mre than 23.5 percent xygen by vlume. Permit-Required Cnfined Space - a cnfined space that has ne r mre f the fllwing characteristics: Cntains r has a ptential t cntain a hazardus atmsphere. Cntains a material that has the ptential fr engulfing an Entrant. Has an internal cnfiguratin such that an Entrant culd be trapped r asphyxiated by inwardly cnverging walls r by a flr which slpes dwnward and tapers t a smaller crss-sectin. Cntains any ther recgnized serius safety r health hazard. Permit system - the emplyer's written prcedure fr preparing and issuing permits fr entry and fr returning the cnfined space t service fllwing terminatin f entry. Prhibited cnditin - any cnditin in a cnfined space that is nt allwed by the permit during the perid when entry is authrized. Rescue service - the persnnel designated t rescue emplyees frm Permit-Required Cnfined Spaces. Retrieval system - the equipment (including a retrieval line, chest r full-bdy harness, wristlets, if apprpriate, and a lifting device r anchr) used fr nn-entry rescue f persns frm cnfined spaces. Testing - the prcess by which the hazards that may cnfrnt Entrants f a cnfined space are identified and evaluated. Testing includes specifying the tests that are t be perfrmed in the permit space.

Shall prvide the necessary equipment t cmply with these requirements and ensure that all emplyees are trained n its use. Shall ensure that all cnfined space assessments have been cnducted and dcumented. Shall ensure that prvisins and prcedures are in place fr the prtectin f emplyees frm external hazards including but nt limited t pedestrians, vehicles and ther barriers and by use f the pre-entry checklist verifying that cnditins in the permit space are acceptable fr entry during its duratin. Shall ensure that all Permit-Required Cnfined Spaces permits are psted. Shall ensure an annual review f the prgram including all entry permits issued that during that annual perid. Shall ensure that cnfined spaces are identified prperly as either a Nn-Permit Cnfined Space r a Permit-Required Cnfined Space. Shall ensure that all cnfined spaces that have been identified as n entry have signs that state, DANGER- DO NOT ENTER. Shall ensure signs have been psted at all Permit-Required Cnfined Space areas that state, DANGER PERMIT ENTRY CONFINED SPACE alng with the prper warning wrd such as ASPHYXIANT, FLAMMABILITY r TOXIC HAZARD Shall file all permits at the area ffices fr review. Permits shall be kept n file fr ne year. Affected Emplyee Shall attend Cnfined Space Entry training cmmensurate with their duties and when duties change as required. Shall cmply with all aspects f this prgram. Authrized Entrants, Attendants and Entry Supervisrs may be any NC Sturgen emplyee that is authrized by management t wrk in a cnfined space setting and that has been trained and is prficient in the understanding f prgram requirements. Authrized Entry Supervisr Duties Shall have a tailgate safety meeting, with all wrkers t be invlved in the cnfined space entry and review the jb t be perfrmed and what safety cncerns may be present. Shall cnfirm that all islatin, Lck/ut and Tag/uts have been cmpleted prir t entry int a cnfined space. Shall ensure that the requirements f this prgram are fllwed and maintained. Shall test all atmsphere cnditins prir t entry and shall cmplete and maintain the cnfined space permit frm, and have it accessible fr review n the jb site at all times. Shall ntify NC Sturgen supervisr f entry int a cnfined space, and ntify the supervisr f any changes that may ccur, during an entry. If the cnfined space pses a hazard that cannt be eliminated, the Entry Supervisr must arrange fr a rescue services.

A stand-by rescue team is nt required t be n site fr Nn-Permit Cnfined Space entries. Authrized Attendant Duties Knws the hazards that may be faced during entry, including infrmatin n the mde, signs r symptms, and cnsequences f the expsure. Is aware f pssible behaviral effects f hazard expsure in authrized Entrants. Cntinuusly maintains cmmunicatin and an accurate cunt f authrized Entrants in the cnfined space and ensures that the means used t identify authrized Entrants, and accurately identifies wh is in the cnfined space. Remains utside the cnfined space during entry peratins until relieved by anther Attendant. If mre than ne cnfined space is t be mnitred by a single attendant, the prgram must include the means & prcedures that will be used in rder t enable the attendant t respnd t emergencies in ne r mre permit spaces that he/she is mnitring withut distractin frm all respnsibilities. Attendants may enter a cnfined space t attempt a rescue, if they have been trained and equipped fr rescue peratins as required and nly when they have been relieved by anther authrized Attendant. Mnitrs activities inside and utside the cnfined space t determine if it is safe fr Entrants t remain in the space and rders the authrized Entrants t evacuate the cnfined space immediately under any f the fllwing cnditins: If the Attendant detects a prhibited cnditin; If the Attendant detects the behaviral effects f hazard expsure in an authrized Entrant; If the Attendant detects a situatin utside the space that culd endanger the authrized Entrants; If the Attendant cannt effectively and safely perfrm all the duties required. Summn rescue and ther emergency services as sn as the Attendant determines that authrized Entrants may need assistance t escape frm cnfined space hazards. Takes the fllwing actins when unauthrized persns apprach r enter a cnfined space while entry is underway: Warn the unauthrized persns that they must stay away frm the cnfined space; Advise the unauthrized persns t exit the cnfined space immediately, if they have entered the space; Infrm the authrized Entrants and the Entry Supervisr if unauthrized persns have entered the cnfined space. Perfrms n duties that might interfere with the Attendant's primary duty t mnitr and prtect the authrized Entrants. Authrized Attendants shall nt mnitr mre than ne cnfined space at a time. Authrized Entrant Duties Knws the hazards that may be faced during entry, including infrmatin n the mde, signs r symptms, and cnsequences f the expsure; Uses apprpriate persnal prtective equipment prperly, e.g., face and eye prtectin, and ther frms f barrier prtectin such as glves aprns, cveralls, and breathing equipment;

Is entitled t request additinal mnitring at any time. Maintain cmmunicatin with the Attendants t enable the Attendant t mnitr the Entrants status as well as t alert the Entrant t evacuate if needed; and Exit frm cnfined spaces as sn as pssible when rdered by an Attendant r Entry Supervisr, when the Entrant recgnizes the warning signs r symptms f an expsure exists, r when a prhibited cnditin exists, r when an alarm is activated. Prcedure Nn-Permit Cnfined Space Entry If testing f the cnfined space atmsphere is within acceptable limits withut the use f frced air ventilatin and the space is prperly islated, the space can be entered by fllwing the requirements fr Level I cnfined space entry. Entrants and/r their representative shall be given the pprtunity t bserve and participate in the air mnitring prcess. Entrants shall review and sign the cnfined space permit. Emplyees may enter and wrk in the cnfined space as lng as LEL, O2, and txicity hazards remain at safe levels. Cmplete the NC Sturgen Cnfined Space Entry Permit t dcument that there are n cnfined space hazards. Make this certificatin available t all persnnel entering the space. A trained Attendant must always be utside the cnfined space. The Attendant must mnitr the authrized Entrants fr the duratin f the entry peratin. Exceptin: The Attendant requirements fr Level I cnfined space entry may be exempted, if the jb assessment is perfrmed and has determined that there are n inherent dangers t allw single persn entry. This prvisin is intended t permit field peratins t enter crankcases, shallw valve bxes, cellars, excavatins, etc. withut an Attendant being present and all ther aspects f the entry permit cmplied with. When there are changes in the use and cnfiguratin f a cnfined space that might increase the hazards t the Entrants (e.g., using epxy cating n a tank flr, welding, painting, etc.), re-evaluate the space. If necessary, reclassify the space as a Permit-Required Cnfined Space. Cntinuusly mnitr the cnfined space atmsphere t ensure that it is still safe. The space must nt cntain a hazardus atmsphere while persnnel are inside. If a hazardus atmsphere is detected during an entry, persnnel must immediately evacuate the space. Re-evaluate the space t determine hw the hazardus atmsphere develped. The Entry Supervisr shall cancel the entry permit. Take actin t prtect persnnel befre any subsequent activity t re-enter the space takes place. Reissue the NC Sturgen Cnfined Space Entry Permit befre allwing Entrants t re-enter the space. If necessary, reclassify the space as a Permit-Required Cnfined Space. Ensure that vehicle r ther equipment exhaust des nt enter the space.

Cmplete the NC Sturgen Cnfined Space Entry Permit befre prceeding with wrk in a Permit- Required Cnfined Space. Entrants and/r their representative shall be given the pprtunity t bserve and participate in the air mnitring prcess. Entrants shall review and sign the cnfined space permit. At least ne trained Attendant must always be utside the Permit-Required Cnfined Space. The Attendant must mnitr the authrized Entrants fr the duratin f the entry peratin. Only authrized Entrants may enter a Permit-Required Cnfined Space. All Entrants must sign in and ut n the entry permit when entering and leaving a Permit-Required Cnfined Space. The back f the permit r a sign-in sheet must be used fr this purpse. Pst signs and barricades utside all Permit-Required Cnfined Spaces t ntify persnnel that a cnfined space entry is in prgress and unauthrized entry is prhibited. Cnditins must be cntinuusly mnitred where Entrants are wrking t determine that acceptable cnditins are maintained during entry. If a hazardus atmsphere is detected during an entry, persnnel must immediately evacuate the space. The Entry Supervisr shall cancel the entry permit. Re-evaluate the space t determine hw the hazardus atmsphere develped. Take actin t prtect persnnel befre any subsequent activity t re-enter the space takes place. Re-issue the NC Sturgen Cnfined Space Entry Permit befre allwing Entrants t re-enter the space. Emplyees r their representatives are entitled t request additinal mnitring at any time. The permit must be terminated when the entry peratins are cmplete r when permit cnditins change (i.e., hazardus air mnitring results are nted, unsafe behavirs are bserved, etc.). The minimum rescue equipment required fr Permit-Required Cnfined Space entry is cvered in the Rescue & Emergency sectin f this prgram. Permit-Required Cnfined Space entry peratins will be reviewed when NC Sturgen believes that the requirements f this cnfined space prgram may nt adequately prtect persnnel. If deficiencies are fund in the prgram, the prgram will be revised and persnnel will be trained in the new revisins befre subsequent entries are authrized. Pre-Jb Planning and Space Preparatin The Entry Supervisr must determine that the cnfined space is prperly islated by blinding, discnnecting, and/r by fllwing lcal Lckut/Tagut prcedures. The Entry Supervisr must discuss with all Entrants the hazards f the space, cmmunicatin methds and emergency prcedures during the cnfined space entry. Eliminate any cnditin making it unsafe t pen the equipment t atmsphere.

If applicable, wash, steam, ventilate r degas the cnfined space t prperly free it f pssible cntaminants. Vent vaprs t a safe lcatin. D nt allw unauthrized persnnel t enter a cnfined space. Barricade and/r guard all cnfined spaces t prevent entry f unauthrized Entrants. If perfrming ht wrk in the cnfined space, precautins must be taken cnsistent with the COMPANY Ht Wrk Permit prcedure. Ensure that vehicle r ther equipment exhaust des nt enter the space. Pre-Entry Safety Meeting The Entry Supervisr must declare when the cnfined space is ready fr entry. The Entry Supervisr shall hld a pre-entry safety meeting t discuss all requirements and prcedures with all authrized Entrant(s) and Attendant(s) invlved with the entry. He/she will discuss ther cncerns such as previus cntents, vessel cating, PPE required etc., during this meeting. The Entry Supervisr must crdinate entry peratins when emplyees f mre than ne cmpany are wrking simultaneusly in the cnfined space. This crdinatin is necessary s that ne cmpany s wrk des nt endanger the emplyees f anther cmpany. Equipment Check all wrk equipment t ensure that it has the prper safety features and is apprved fr the lcatins where it will be used. The Entry Supervisr shall ensure that all equipment is prperly maintained in a safe cnditin and that Entrants use the equipment prperly. The fllwing equipment must be cnsidered and may be required when entering a cnfined space: Atmspheric Testing and Mnitring Equipment. Barriers, Shields, and Signs Pst signs and barricades utside all Permit-Required Cnfined Spaces t ntify persnnel that a cnfined space entry is in prgress and unauthrized entry is prhibited. Any signs used must state Danger Permit Entry Cnfined Space alng with the prper warning wrd such as Asphyxiant, Flammability r Txic Hazard. All barricades must be capable f preventing a persn frm inadvertently walking int r kicking an bject int the space. Cmmunicatins Equipment Only use intrinsically safe equipment in areas where a hazardus atmsphere may exist. Use a cmmunicatin system that will keep the Attendant in cnstant, direct cmmunicatin with the Entrant(s) wrking in the cnfined space. Als, use a cmmunicatin system that allws the Attendant t summn help frm rescue r emergency service. Entry and Exit Equipment (Fr example: ladders may be needed fr safe entry and exit).

Lighting equipment used in the cnfined space must be certified safe fr the lcatin. Prtable electric lighting used in wet and/r ther cnductive lcatins (drums, tanks, vessels) must be perated at 12 vlts r less. 120 vlt lights may be used if prtected by a grund-fault circuit interrupter. Persnal Prtective Equipment Ensure that persnnel wear the required persnal prtective equipment. Fr respiratry prtectin requirements, refer t the Respiratry Prtectin Prgram. Rescue and Emergency Equipment Except if prvided by utside rescue services. The Attendants must als have an apprved first aid kit. Vacuum Trucks When used, trucks must be prperly grunded r bnded t prevent static sparks. Ventilating Equipment Lcal exhaust air mvers used t btain acceptable atmspheric entry cnditins (e.g., Cpus air mvers). Other Any ther equipment necessary fr safe entry int and rescue frm permit required cnfined spaces. Air Mnitring Befre an emplyee enters the space, the internal atmsphere shall be tested, with a calibrated directreading instrument, fr xygen cntent, fr flammable gases and vaprs, and fr ptential txic air cntaminants, in that rder. Mnitring f the space must infrm the entrants f the ptential hazards and results and they must participate in the permit review and signing. Air shall be peridically test while cntinuus ventilatin is applied. Any emplyee, wh enters the space, r that emplyee's authrized representative, shall be prvided an pprtunity t bserve the pre-entry testing required by this paragraph. Emplyees r their representatives are entitled t request additinal air mnitring at any time. Ventilatin Cntinuus frced air ventilatin must be used and tested as fllws: An emplyee may nt enter the space until the frced air ventilatin has eliminated any hazardus atmsphere; The frced air ventilatin shall be s directed as t ventilate the immediate areas where an emplyee is r will be present within the space and shall cntinue until all emplyees have left the space; The air supply fr the frced air ventilatin shall be frm a clean surce and may nt increase the hazards in the space. The atmsphere within the space shall be peridically tested as necessary t ensure that the cntinuus frced air ventilatin is preventing the accumulatin f a hazardus atmsphere. Any emplyee, wh enters the space, r that emplyee's authrized representative, shall be prvided with an pprtunity t bserve the peridic testing and may request additinal mnitring at any time. If a hazardus atmsphere is detected during entry each emplyee shall leave the space immediately and the space shall be evaluated t determine hw the hazardus atmsphere develped; and measures shall be implemented t prtect emplyees frm the hazardus atmsphere befre any subsequent entry takes place. Multiple Emplyer Prcedure In rder nt t endanger the emplyees f any ther emplyer, the Entry Supervisr shall:

Infrm the cntractr that the wrkplace cntains permit spaces and that permit space entry is allwed nly thrugh cmpliance with a permit space prgram meeting the requirements f this sectin. Apprise the cntractr f the elements, including the hazards identified and the emplyees experience with the space, that make the space in questin a permit space. Infrm the cntractr f any precautins r prcedures that NC Sturgen has implemented fr the prtectin f emplyees in r near permit spaces where cntractr persnnel will be wrking. Rescue and Emergency Services General Rescue service must be n-site fr immediately dangerus t life and health (IDLH) cnditins while wrk is being perfrmed. Rescue services must be either: Prvided by the hst facility, Prvided by an utside service which is given an pprtunity t examine the entry site, practice rescue and decline as apprpriate, r Prvided by NC Sturgen by selecting a rescue team that is equipped and trained t perfrm the needed rescue services. The Attendant shall rder the ther Entrants nt t mve the injured nr allw untrained r unauthrized wrkers int the space that are nt trained t handle a cnfined space rescue. Material Safety Data Sheet s fr substances that an injured Entrant was expsed t must be prvided t the medical facility treating the injured wrker. Permit-Required Cnfined Space Rescue: When the Attendant becmes aware f the need fr rescue, the Attendant shall immediately summn the nsite rescue team by the agreed upn cmmunicatin methd, verbally, radi r cell phne, withut leaving the vicinity f the cnfined space. The Attendant shall prevent unauthrized persnnel frm attempting a rescue.

The preferred means f prviding rescue service is thrugh the use f a qualified utside rescue service vendr (client hst). The utside rescue service vendr must be: Infrmed f the hazards that they may cnfrnt during a rescue; Prvided access t the Permit-Required Cnfined Space t examine the entry site, practice rescue, and decline as apprpriate. Access t the space allws the rescue service and lcal supervisin t jintly develp apprpriate rescue plans. If the hst peratr is designated t prvide rescue services fr NC Sturgen, the agreement f services must be included in cntract fr the jb. If NC Sturgen emplyees are t perfrm Permit-Required Cnfined Space rescues, they must be: Nn-entry Rescue Prvided and trained in the use f the prper persnal prtective equipment necessary t make the rescue; Prvided PPE at n cst Trained t perfrm the assigned duties; Required t practice making rescues at least nce every 12 mnths; Trained in basic first aid and CPR. A minimum f ne member f the rescue team must hld a current certificatin in first aid and CPR. T facilitate nn-entry rescue, an Entrant must be attached t a retrieval system whenever he/she enters a Permit-Required Cnfined Space with a vertical depth f mre than 5 feet. The retrieval equipment is nt required if it will increase the verall risk f the entry, e.g., creating an entanglement hazard, r will nt cntribute t the rescue f the Entrant. Each Entrant shall use a full bdy harness equipped with a D ring lcated between the shulders r abve the head. Wristlets may be used instead f the full bdy harness, if the use f the full bdy harness is nt feasible r creates a greater hazard and that using wristlets is the safest and mst effective alternative. The retrieval line must be attached t the D ring and the ther end f the retrieval line attached t a retrieval device r fixed pint lcated utside the space s that rescue can begin as sn as the rescuer becmes aware that rescue is necessary. Issuance/Reviewing f Permit Only when all pre-entry requirements are satisfied, the Entry Supervisr shall issue a cmpleted and signed cnfined space permit. The cnfined space permit is valid fr ne shift. In the event f any unauthrized entry, emplyee cmplaints, a hazard nt cvered by the permit, the ccurrence f an injury r near miss the entry permit shall be cancelled and a review shall be cnducted t prvide emplyee prtectin and fr revising the prgram prir t authrizing subsequent entries.

If n cnfined space entries were perfrmed during a 12 mnth perid, n review is necessary. Terminatin and Clsing r Cancelling f Permits The Entry Supervisr shall terminate the cnfined space permit, at the end f the jb peratin, at the end f the shift r when the Entry Supervisr r Attendant determine that cnditins in r near the cnfined space have changed and is hazardus t the Entrants. The Entry Supervisr shall, at the cnclusin f entry peratin, clse ut the permit and prvide the safety department the riginal cpy f the Cnfined Space Permit. Training Training shall be prvided s that all emplyees whse wrk is regulated by this prgram acquire the understanding, knwledge, and skills necessary fr the safe perfrmance f the duties assigned t them. Training shall be prvided t each affected emplyee, befre the emplyee is first assigned duties under this prgram, if a new hazard has been created r special deviatins have ccurred and befre there is a change in assigned duties. The emplyee shall be retrained: Whenever there is a change in cnfined space peratins that presents a hazard abut which an emplyee has nt previusly been trained. Whenever the supervisr has reasn t believe either that there are deviatins frm the permit space entry prcedures required by this sectin r that there are inadequacies in the emplyee's knwledge r use f these prcedures. The training shall establish emplyee prficiency in the duties required by this prgram and shall intrduce new r revised prcedures, as necessary. The supervisr shall certify that the training required by this prgram has been accmplished. The certificatin shall cntain each emplyee's name, the signatures r initials f the trainers, and the dates f training. The certificatin shall be available fr inspectin by emplyees, their authrized representatives, management, clients and the safety department.
