1 Effective Date: 09/30/10 Standard: 10.6 Dcument Number: KUCSH0043 Rev: INTRODUCTION This Standard applies t dust, fibers, mist and fume (i.e. particulates), gas and vapr expsures in the wrkplace, with emphasis n inhalatin as the prime rute f expsure. It cvers particulate and gas / vapr hazard evaluatin, cntrl prgram design and evaluatin (medical surveillance), t ensure that emplyees and cntractrs will nt suffer adverse health effects frm particulates r gas / vaprs, either used r generated by KUC REQUIREMENTS Where a risk assessment indicates the need, a wrkplace airmnitring prgram must be in place. The wrkplace air-mnitring prgram must: Cmply with all relevant requirements in the Ri Tint Standards. Adequately describe air quality f the wrkplace with regard t dust, fiber, mist, fume, gas and vapr emissins. Identify and adequately characterized wrkplace particulate and gas / vapr surces that cntribute t the exceedences f Occupatin Expsure Limits (OELs). Ensure cntrl measures are peridically checked that they minimize emissins and prtect emplyees and cntractrs frm adverse expsure Designated areas will be created where: It is likely that the 95 percent upper cnfidence limit f a Similar Expsure Grup (SEG) mean cncentratin fr agents resulting in chrnic effects, such as ttal inhalable dust, respirable dust, respirable crystalline silica, asbests r nn-asbests fibrus materials exceeds the relevant OEL; r Agents with an acute effect (e.g. CO, SO2, NH3, HF, etc) exceed 50 per cent f the relevant OEL Designated areas must: Be identified and mapped, psted r therwise clearly cmmunicated t emplyees wrking in the area. Signs, where necessary, shall use apprpriate wrding r symbls t identify the hazard. Have a dcumented respiratry prtectin prgram based n risk assessments and standards, which is applied t emplyees, cntractrs, and visitrs. There shall be special cnsideratin given t the sampling f ht / vlatile / pressurized txic prcess streams where they ccur Expsure data fr knwn human carcingens, mutagenic and reprductive txicants, must be statistically validated n an annual basis. Time-weighted average (TWA) measurements ver several shifts, and cnsistent with the wrk-day perid, must be used. If three r mre years f statistically significant data are less than 25 percent f the OEL, r belw the detectin limit, then mnitring frequency can be extended t nce every three years, prvided that prcess and wrk rganizatin remains unchanged Expsure data fr prgressive chrnic cnditins with a knwn cause (requiring lng-term expsure fr an effect t manifest) r suspect carcingens, mutagenic and reprductive txicants, must be statistically valid n an annual basis. If three r mre years statistically significant data are less than 50 percent f the OEL, then mnitring frequency can be extended t nce every three years, prvided the prcess r wrk rganizatin (including maintenance)remains unchanged Where a risk assessment indicates the pssible presence f levels f gas r vapr sufficient t cause health effects in less than ne shift (e.g. cnfined space entry), cntinuus mnitring is required as lng as the ptential fr harm exists Emplyees and Categry 1 cntractrs shall be cvered by a medical surveillance prgram when: Their SEG TWA mean expsure t respirable crystalline silica, ttal inhalable dust, respirable dust, lead, arsenic, cadmium, r asbests dust is greater than 50% f the relevant OEL. The medical adviser cnsiders that it is advisable, r There is a legal requirement fr medical mnitring Where a risk assessment indicates a risk f a respiratry cnditin, assessment prgrams shall include a medical histry f the Page 2 f 7 Kennectt Utah Cpper. 3 Effective Date: 09/30/10 Standard: 10.6 Dcument Number: KUCSH0043 Rev: 3 pulmnary rgan system, chest x-rays, and pulmnary functin tests meeting the fllwing standards: High quality chest x-rays will be taken every 5 years, unless lcal legislatin requires these t be mre frequent. All chest x-rays will be read by a bard certified radilgist. A physician will review any reading suggesting active lung disease. All spirmetry testing will be by trained staff fllwing the American Thracic Sciety guidelines r equivalent and be ffered at a frequency determined by the likely rate f detectable change in lung functin. Abnrmalities that are nt defined by abve shall underg further testing t include but nt limited t: CT/MRI scanning, pen lung bipsy, referral t Pulmnlgist All lead bilgical mnitring prgrams shall meet the fllwing standards: All testing will be f venus bld. Only labratries using an active quality assurance r quality cntrl scheme will be used fr testing. Females f reprductive capacity with a whle-bld lead abve 20µg/dL will be remved frm expsure until the physician declares the wrker fit fr duty, and expsure t lead shuld cease when pregnancy is ntified t the Cmpany. All ther wrkers with a whle-bld lead abve 40µg/dL will be remved frm expsure until the level has fallen belw 30µg/dL, and the physician declares the wrker fit fr duty All bilgical mnitring prgrams fr ther substances (i.e. arsenic and cadmium) shall be dcumented When a risk assessment determines that cntrls r treatment is needed, the fllwing hierarchy f cntrls shall be cnsidered: 1. Remval r substitutin f the hazard. 2. Islatin. 3. Administrative cntrls. 4. Persnal prtective equipment (PPE) There shall be dcumented prcedures fr inspectin, assessment and maintenance f the engineering cntrls t ensure that the equipment cntinues t perate t design specificatins Cntrls shall be f an adequate standard such that surfaces are Page 3 f 7 Kennectt Utah Cpper. The prgram must include: Nt smking, eating r drinking in designated hazard area. Cigarette smking must als be prhibited in all indr areas and wherever peple are likely t be expsed t secnd-hand smke. Washing f hands and face prir t drinking, eating r smking. Shwering at wrk pst shift r after expsure. Laundering f cntaminated clthing by the peratin Abrasive blast cleaning shall be cnducted s as t prtect wrker health and minimize dust emissins. Substitutes shall be used whenever practicable fr abrasives cntaining crystalline silica. Hwever, if such abrasives are used, wrkers shall be aware f the hazards and expsure mnitring cnducted. The hazardus prperties f alternative materials shall be cnsidered befre use Fixed statin mnitrs and alarms shall be installed where apprpriate t warn against accidental r peridic releases f txic gases / vaprs (e.g. HCN, CO, SO 2 ). All affected persnnel shall be trained n the capabilities and limitatins f the mnitrs. All fixed statin mnitrs / alarms shall be identified, listed and included in a peridic schedule f preventive maintenance and testing, including written dcumentatin f calibratin f detectrs. Peridic drills with regard t respnse t sunding f the alarm shall be cnducted. Frequency f drills shuld be based n level f risk Where required, training in the recgnitin f signs and symptms f hazardus particulate and gas / vapr expsure, emergency prcedures and preventative measures must be prvided RESPIRATORY PROTECTION DEVICES (RPD) Where required, there shall be a dcumented respiratry prtectin prgram. 5 Effective Date: 09/30/10 Standard: 10.6 Dcument Number: KUCSH0043 Rev: 3 Peridic inspectin f RPDs, including befre each use; Peridic evaluatin f cleaning, sanitizing, maintenance and strage practices by cmpetent persns; Perfrmance f psitive and negative fit checks befre each use by RPD wearers t ensure that respiratr is functining prperly; and Training at first issue f a RPD and regular refresher training thereafter prvided accrding t regulatry requirements r at least nce every tw years RPDs shall be selected with regard t: The ptential particulate particles size, gas / vapr types, substance txicity, and likely cncentratins. Cmpatibility with the wrk tasks and ther PPE. Cmfrt (as it affects wear-time) and allwance fr adequate cmmunicatin Half-face and full-face air-purifying respiratrs shall nt be used where: The atmsphere is xygen deficient (< 19.5%). The atmsphere is immediately dangerus t life r health (e.g. in areas where CO cncentratins are > 1,500 ppm r NH 4 > 300 ppm). Gases and vaprs are mre than 10 times their OEL r greater than 1000 ppm fr half-face respiratrs, r mre than 100 times their OEL fr full-face respiratrs. Particulates are mre than 5 times their OEL fr half-face respiratrs, r mre than 50 times their OEL fr full-face respiratrs An air-supplied type respiratr must be wrn fr atmspheres that are xygen deficient, cntain unknwn hazards, have cncentratins f gases and vaprs that are unknwn, r culd ptentially exceed Immediately Dangerus t Life r Health (IDLH) values. Fr air-supplied RPDs, breathing air must be effectively filtered and / r islated frm plant and instrument air, and islated frm surces f ptential cntaminants. The quality f the breathing air must be checked fr cnfrmance with natinal standards Fr effective use f negative pressure RPDs (including dispsable RPDs), fit testing must be qualitative and dcumented as a minimum, althugh quantitative fit testing is preferred. There shall be a plicy requiring a clean-shaven face when using a negative r neutral pressure RPD fr rutine tasks r the use f a psitive pressure RPD will be required. A pulmnary functin test is required t determine whether r nt an individual is medically fit t wear a respiratr RESPONSIBILITIES Area Manager is respnsible t ensure: A Risk Management Team develps a Risk Register and that the hazards in their respective wrk areas are defined and a hazard inventry is cmpiled and maintained and risks are cntrlled. Cntrl measures are peridically checked and that they minimize emissins and prtect emplyees and cntractrs frm adverse expsure. Signs are psted in areas where particulate hazards exceed the relevant OEL and clearly cmmunicated t emplyees. Prcedures are dcument fr inspectin, assessment and maintenance f the engineering cntrls t ensure that the equipment cntinues t perate t design specificatins All fixed statin mnitrs / alarms are identified, listed and included in a peridic schedule f preventive maintenance and testing, including calibratin f detectrs. In additin, peridic drills with regard t respnse t sunding f the alarm shall be cnducted HSE is respnsible t: Identify and characterize wrkplace particulate and gas / vapr surces that cntribute t the exceedences f OELs.
