This plan supprts that cmmitment by meeting the OSHA (Occupatinal Safety and Health Administratin) requirements fr a cnfined space entry prgram. OSHA s Permit-Required Cnfined Space Standard, Title 29, Cde f Federal Regulatins defines a cnfined space as that which has ne r mre f the fllwing characteristics: Cntains r has the ptential t cntain a hazardus atmsphere Cntains material that has the ptential t engulf Walls that cnverge inward r flrs that slpe dwnward and taper int a smaller area which culd trap r asphyxiate Recgnized health r safety hazard(s) such as unguarded machinery, expsed live wires r heat stress. Examples f these spaces may include tanks, strage bins, s, undergrund vaults, diked areas, manhles, tunnels, equipment husings, ductwrk, and pipelines. II. Knwn Cnfined Spaces Knwn permit required cnfined spaces in buildings perated by Mnre One BOCES are listed in the fllwing table, highlighted in yellw. These spaces are identified at entry pints with a sign: DANGER PERMIT-REQUIRED-CONFINED SPACE DO NOT ENTER Withut Authrizatin and Permit frm O&M Office Call x2991. Other cnfined spaces are listed fr inclusin in annual safety training and they requires ntificatin t the O&M Freman and tw mechanics befre entry. There may be undiscvered cnfined spaces. All emplyees are encuraged t bring any discvered lcatins t the attentin f the O&M Freman fr the safety f all as well as fr inclusin within this plan. The O&M Freman may use the Decisin Chart in the appendix t evaluate the space. Emplyees whse wrk may expse them must be ntified f the existence, lcatin and hazards f the PRCS. If emplyees may be expected t enter PRCS then a written plan and a written space entry permit system must be in place. This plan and its permit frm intend t be the tls t meet these requirements. Management f Knwn Permit Required Cnfined Spaces 1. Knwn PRCS have a sign at the entry which says, DANGER PERMIT-REQUIRED-CONFINED SPACE DO NOT ENTER Withut Authrizatin and Permit. 2. Knwn PRCS will have lcks that are n the master key system. 3. Knwn PRCS will be reviewed at the annual emplyee health & safety meeting in August f every year. 4. New O&M (Operatins and Maintenance) emplyees will receive a review f this plan. IV. Entry Prcedures fr PRCS 1. Identify and evaluate PRCS space hazards befre allwing emplyee entry. 2. Identify the task that needs t be cmpleted in the PRCS; 3. Perfrm apprpriate testing fr the fllwing atmspheric hazards in this sequence: xygen, cmbustible gases r vaprs, and txic gases r vaprs; 4. If a space is thught t have r des have the ptential fr a hazardus atmsphere, n permit may be granted. Mnre One BOCES des nt maintain the equipment fr a safe entry int a hazardus atmsphere s wrk in a hazardus space must be cntracted t an enterprise the credentials and ability manage a hazardus atmsphere. 5. Establish and implement the means, prcedures and practices t eliminate r cntrl hazards necessary fr safe permit space entry peratins; 6. Identify the required PPE and any ther equipment necessary fr safe entry, enter it n the permit and require emplyees t use it; 7. Identify apprpriate prcedures fr summning rescue and emergency services, and preventing unauthrized persnnel frm attempting rescue; 8. Cmplete the PRCS permit applicatin: 9. 5 1. Annually, the O&M Department will review this PRCS plan with its permit prcedures and entry requirements with its Maintenance and Grunds emplyees. Other jb categries may be included t thse wh may be attendants, entry supervisrs, n-site rescue team members, r emplyees wh may ptentially enter the space. 2. As part f this training, nn-permit cnfined spaces will als be reviewed fr prper safety prcedures. 3. The training shuld test that the emplyees f the O&M department can identify a PRCS. 4. PPE (Persnal Prtective Equipment) that might be required fr PRCS will be reviewed and inspected at the annual training sessin. 5. The Directr f Safety & Security will ntify rescue and emergency services f this plan, subsequent updates and its identificatin f PRCS. VI. Reclassifying a PRCS t Nn-PRCS OSHA regulatins als allw permit spaces t be reclassified as nn-permit spaces by if the hazards which had required it t be a PRCS have been eliminated. VII. Duties fr Full PRCS Entry Operatins Entry int any PRCS will require a trained and equipped team that cnsist f: A. Authrized Entrants and they shall be required t: 1. Knw the hazards assciated with the PRCS space and thse hazards effects. 2. Prperly use the equipment required fr entry. 3. Maintain cntinuus cmmunicatin with the attendant. 4. Alert the attendant in the event f an emergency. 5. Knw the evacuatin prcedure if an emergency ccurs. B. Attendants and they shall be required t: 1. Knw the hazards assciated with the PRCS space and thse hazards effects. 2. Maintain an accurate accunt f the authrized entrants. 3. Maintain cntinuus cmmunicatin with the Entrants. 4. Remain at their assigned statin until relieved by anther attendant r until the permit space entry is cmplete. 5. Mnitr cnditins in and arund the permit space. 6. Summn rescue and applicable medical services in the event f an emergency. 7. Perfrm nn-entry rescue prcedures. C. Entry Supervisr and they shall be required t: 1. Knw the hazards assciated with the permit space and thse hazards effects. 2. Verify that the safeguards required by permit are in place. 3. Verify that rescue services are available and that means fr summning them are perable. 4. Cancel the written permit and terminate the permit space entry when required. 5. 6 6. Peridically determine that entry peratin is being perfrmed in a manner cnsistent with the requirements f the permit space entry prcedures and that acceptable entry cnditins are maintained. D. Rescue Persnnel and they shall be required t: 1. Respnd and aid in rescue effrts shuld an emergency situatin arise. 2. Cntact all necessary emergency medical and fire authrities which will be required t lend assistance. VIII. Respnsibilities t Cntractrs 1. When cntractrs are invlved in permit space entry wrk perfrmed in the schls supervised by Mnre One BOCES, the District will infrm them f the fllwing infrmatin and crdinate any entry peratins: 2. The lcatin f the permit spaces at ur facility and that entry int these spaces is nly allwed thrugh a permit space prgram r alternative prcedures r space reclassificatin. Directr f Sustainability will debrief the cntractr at the cmpletin f the entry peratin, r during if a need arises, and if any hazards were cnfrnted r created during their wrk. IX. Cntractr s Respnsibilities t Mnre One BOCES When a cntractr is hired t perfrm wrk in a PRCS in a facility supervised by Mnre One BOCES, the cntractr will btain the fllwing infrmatin frm the District and ensure the fllwing tasks are perfrmed: 1. Obtain infrmatin n the hazards f the permit space and infrmatin frm the previus entry peratins by Mnre One BOCES. 2. Determine if Mnre One BOCES emplyees will be wrking in r near the space. 3. If Mnre One BOCES emplyees will be wrking in r near the space during the peratin, the cntractr will crdinate entry peratins with a Mnre One BOCES representative. 4. Will infrm Mnre One BOCES f the permit space prgram that will be used. 5. Hld a debriefing cnference at the cmpletin f the entry peratin r, if needed, during the entry peratin t infrm Mnre One BOCES f any hazards cnfrnted r created. X. Training 1. Training must be given t each emplyee wh has access r ptential access t a permit required cnfined space. The amunt and type f training needed will depend n the individual s duty assignment. Fr example, sme emplyees may nly be required t knw the existence, lcatin, and danger psed by a permit space. Others may be members f a PRCS team and wuld need cnsiderably mre training. Still thers wuld need training as it pertains t the type f entry prcedures used such as alternative prcedures r reclassifying t nn-permit space prcedures. 7 training is t give emplyees the understanding, knwledge, and skills necessary fr the safe perfrmance f their assigned duties in relatin t the permit required cnfined space f cncern. 2. If a full permit-required cnfined space prgram is required, training is needed n the fllwing tpics: a. Types f cnfined space hazards b. Cmpnents f the written PRCS prgram c. Cmpnents f the PRCS wrk permit d. The need fr prmpt guarding f the entrance pening e. Atmspheric testing equipment including its use, calibratin, and maintenance f. Atmspheric testing prtcl: i. Oxygen, cmbustibles, txins and test at pre-entry, and frequently thereafter if nt cntinuus testing. ii. Check all levels f the space. iii. Methds fr the cntrl r eliminatin f any atmspheric hazards which include inerting, draining and rinsing, purging and cleaning, cntinuus frced air ventilatin. g. Prcedures t fllw if an emplyee detects a hazard h. Evaluatin prcess t be used fr reentry if hazards are detected i. Use f entry equipment j. Persnal prtective equipment that may be required such as full bdy harness, respiratry prtectin, chemical prtective clthing, eye and face prtectin. k. Authrized entrant, attendant, entry supervisr, rescue team respnsibilities l. On-site r ff-site rescue: rescue plan, practice rescue, basic first aid CPR certificatin, full bdy harness with retrieval line attached t mechanical retrieval device m. Prcedures fr annual review f canceled permits n. Dcumentatin f the training XI. Annual Update 1. Every March, the Directr f Sustainability shall slicit cmments frm the O&M Freman, Custdial Supervisr, Maintenance Mechanics, and the Directr f Safety and Security fr this plan s updates and may call a meeting t review the plan and discuss updates. 2. By April 1 f each year, the Directr f Sustainability shall send this updated plan t The Genesee Valley Educatinal Partnership Health, Safety and Risk Management Office t have it reviewed fr cmpliance with current state regulatins. 3.
