1 NMT HAZARDOUS WASTE MANAGEMENT TEAM A Quick Guide t Labratry Safety Inspectins At NMT This guide is develped t help the faculty, principle investigatrs, labratry supervisrs and labratry assistants quickly assess the safety readiness f labratries. This quick guide will briefly discuss tpics that are addressed in the labratry safety inspectin checklist used by NMT Hazardus Waste Management team. BASIC RESPONSIBILITIES 1.) Faculty, Principle Investigatrs and Labratry Supervisrs Dcumenting apprpriate hazard assessments fr the wrk being perfrmed. Ensuring that apprpriate labratry engineering cntrls are available and wrking. Prviding apprpriate persnnel prtective equipment (PPE) t wrkers. Establishing lab-specific safety plicies/ prcedures fr lab wrkers. Ensuring all wrkers have cmpleted required safety training. 2.) Individuals wrking in a labratry setting Perfrming all wrk in a safe manner Using available PPE and engineering cntrls. Fllwing lab-specific safety plicies and prcedures. Cmpleting safety training assigned by yur supervisr.
2 SAFETY SIGNS AND POSTINGS 1.) ENTRY DOOR POSTINGS Primary Area Cntacts Permit hlder (Chemical/ Radiatin/ Bisafety/ Laser) names and phne numbers (primary and after-hur phne number. Fr areas with multiple permit hlders the cntact infrmatin fr all permit hlders shuld be listed. Secnd Area Cntacts Name and phne number (primary and after-hurs numbers) f at least ne knwledgeable secndary cntact. Other Area Cntacts Depending n the type f lab, additinal cntact infrmatin may be needed. Fr example: Area Hazards Departmental Safety Officer. NMT Radiatin and/r Bisafety Officer. Building Mnitr. NMT Hazardus Waste Management Team. NMT Campus plice. Other cntacts (i.e. 911) Pst general hazards that may be fund in the labratry. Examples include: Other Infrmatin -Crrsives -Explsives -Cmpressed Gasses -Bilgical -Oxidizers -Txics -Radiatin Pst ther entry requirements and pertinent infrmatin such as: Area PPE requirements like Safety Eye Prtectin Required. Nte: If PPE is nt required t be wrn at all times in that area then dn t pst the dr with that requirement. Lab Fd and Drink Plicy There must be a sign psted with N Fd r Drink warning, hwever if there are specific areas within the lab where fd is allwed (i.e. an internal break rm r ffice) the pstings shuld be changed t a Fd and Drink is Allwed Only in Designated Areas warning. Areas inside the lab where fd/drink are allwed must be clearly marked as designated fd/drink allwed areas and be segregated frm areas where hazardus chemicals are being used r stred. 2
3 2.) EMERGENCY PHONE NUMBERS POSTING Emergency phne numbers must be psted near all phnes in the labratry. 3.) LABORATORY REFRIGERATORS, FREEZERS AND MICROWAVE OVENS POSTING Equipment must be psted with a N Fd r Drink sign. 4.) EMERGENCY SAFETY SHOWERS AND SAFETY EYEWASHES Must be clearly marked with a sign. 5.) HYDROGEN GAS CYLINDER POSTING Area near cmpresses gas cylinders f hydrgen must be psted with a HYDROGEN- FLAMMABLE GAS-NO SMOKING- NO OPEN FLAMES sign. 6.) EXIT SIGNS All exit drs must be marked with an exit sign. 7.) AREA EVACUATION MAP A map shwing egress rutes and designated assembly lcatins must be psted in hallways utside f labratries. LABORATORY SAFETY EQUIPMENT 1.) PORTABLE FIRE EXTINGUISHERS Lcated within chemical wrk r strage areas ( in lab r hallway) In hac habitasse platea dictumst. Access must be free f bstructins Prperly munted n a wall and nt sitting n the flr. Fully charged Security seal is intact. Inspected within the last year (tagged). 2.) EMERGENCY SAFETY SHOWERS/ SAFETY EYEWASHES AND/OR SINK DRENCH HOSES Available within 10 secnds f travel time. Access must be free f bstructins. Marked with a sign. Operatinal. Inspected within the last year (tagged). 3.) CHEMICAL SPILL KITS Spill kits, apprpriate fr the chemicals in the area, must be available and dully stcked. Spill prcedures must be available. Wrkers must be trained t lab-specific spill prcedures. 3
4 4.) PERSONAL PROTECTIVE EQUIPMENT (PPE) Apprpriate PPE must be readily available t wrkers. Wrkers must wear apprpriate PPE fr the activity being perfrmed. Wrkers must cmply with psted PPE requirements. 5.) CHEMICAL FUME HOODS Clse chemical fume hd sashes when nt wrking in the hd. When wrking in a hd, keep the sash in the lwest psitin pssible except when adding r remving equipment. Ensure that hds have a wrking flw indicatr (manmeter, paper strip etc.). D nt wrk with hazardus materials in a nn-functining fume hd. Ensure air flw is nt blcked r inhibited prir t wrking. Elevate equipment at least 2 inches using blcks s air can flw underneath Ensure hd baffles r baffle slts are nt blcked Minimize the amunt f materials stred in a hd D nt stre chemicals r chemical waste in fume hds. Keep all materials and supplies at least 6 inches frm the sash pening. Put an Out-f-Service sign n a hd that isn t wrking and ntify yur building mnitr. Put up a warning sign n unattended hds when active peratins are nging. 6.) BIOLOGICAL SAFETY CABINETS D nt wrk with hazardus bilgical materials in a nn-functining bisafety cabinet. Verify that the cabinet has been certified within the last year. Put an Out-f-Service sign n a bisafety cabinet hd that isn t wrking and ntify the NMT Bisafety Officer. 4
5 GENERAL LABORATORY SAFETY 1.) HOUSEKEEPING Area aisles and exit rutes must be clear and free f bstructins (28 inch wide minimum) Area shuld be kept clean and well rganized. Remve excess clutter. 2.) EATING, DRINKING, TOBACCO AND COSMETICS Eating, drinking, using tbacc (Smking, r chewing) r applying csmetics in nt allwed in areas where hazardus chemicals are used r stred. 3.) LABORATORY SECURITY Always keep unattended labratries lcked. Always secure hazardus material when unattended. REQUIRED LABORATORY SAFETY PROCEDURES 1.) LAB-SPECIFIC EMERGENCY AND SPILL RESPONSE PROCEDURES Lab-specific emergency prcedures must be available t wrkers. These prcedures shuld include actins that wrkers shuld take in event f unusual r emergency situatins such as Pwer utages Lab ventilatin failure Equipment emergency shutdwns prcedures Spill respnse 2.) WRITTEN STANDARD/SAFETY OPERATING PROCEDURES (SOPS) Written prcedures must be available t wrkers when handling hazardus material and/r perfrming hazardus prcesses. MECHANICAL EQUIPMENT SAFETY 1.) GENERAL SAFETY Ensure mving parts n mechanical equipment are prperly guarded. Ensure stp switches and/r electrical discnnects are nt bstructed and easily accessible. Secure heavy equipment t prevent it frm tipping ver. Place a warning sign n unattended equipment when it is perating (i.e. Dn t Tuch: Operatin in Prgress ). 5
6 CHEMICAL INVENTORY/ CHEMICAL SAFETY DATA SHEETS 1.) LABORATORY CHEMICAL INVENTORIES The senir staff, PI and faculty must maintain an inventry f all hazardus chemicals in the labratry. Each labratry must maintain a cpy f the chemical inventry in the lab. 2.) CHEMICAL SAFETY DATA SHEETS (SDS/MSDS) SDS s must be maintained in r near the lab fr any hazardus chemicals in the area. SDSs must readily be available t all lab wrkers. Electrnic SDS systems are allwed as lng as all wrkers have access t the system. ELECTRICAL SAFETY 1.) EXTENSION CORDS /POWER CORDS Use wire-mld r a flr cver kit wherever extensin crds crss walkways s they dn t pse a tripping hazard. Extensin crds may nt be used fr mre than 90 cntinuus days. D nt run extensin crds thugh walls, flrs r ceiling panels. Daisy chaining extensin crds, multi-utlet strips and/r surge prtectrs is nt allwed. D nt use frayed r damaged pwer crds r plugs. D nt cut the grund prng ff 3-prnged plugs. 2.) SURGE PROTECTORS / MULTI-OUTLET STRIPS Only use UL-listed multiple utlet strips with a 15 amp circuit breaker. Only use surge prtectrs n equipment that have a very light current lad such as electrnic / cmputer equipment. D nt daisy chain multi-utlet strips with extensin crds. 3.) GROUND-FAULT CIRCUIT INTERRUPTERS (GFCI) A Grund Fault Circuit Interrupter (GFCI) unit must prtect electrical utlets within reach f a sink r ther surce f pen water. These can be installed at the utlet r in the electrical panel. 4.) ELECTRICAL PANELS / ELECTRICAL DISCONNECTS Keep the area arund electric circuit panels and electrical discnnect switches clear f bstructins. There must be at least 30 inches f unbstructed clearance arund them. 6
7 COMPRESSED GAS / GAS CYLINDER SAFETY 1.) COMPRESSED GAS CYLINDERS Cylinders must be secured. An individual chain r cable is recmmended. Cylinders must be capped when nt in use. Regulatrs shuld be remved and the cylinder capped if it is nt ging t be used fr an extended perid f time. Cylinders must be labeled with the cntents. Empty cylinders must be labeled EMPTY 2.) PLUMBING AND TUBE FITTING D nt use Tefln tape n cmpressin fittings. Tefln tape reduces the sealing ability f cmpressin fittings. Ensure that the plumbing / tubing used dwnstream f regulatr is designed t withstand the maximum discharge pressure f the regulatr. Only use plumbing / tubing material that is cmpatible with the gas. 3.) FLAMMABLE/ TOXIC/ CORROSIVE/ OXIDIZING GAS (GENERAL REQUIREMENTS) D nt stre flammable gasses with xidizing gasses. Keep at least 20 feet f separatin (r a wall with a. hur fire rating) between flammable and xidizing gas cylinders. D nt stre mre than 3 xygen and/r flammable gas cylinders in a labratry. Txic and crrsive gasses must be stred in a gas cabinet with a leak detectin system. Txic and crrsive gasses must be exhausted t the utside thugh a ventilatin duct r fume hd. Txic and crrsive gasses must have a Bnnet vent n the regulatr which must be vented t the utside thrugh a ventilatin duct r fume hd. 4.) HYDROGEN GAS (SPECIFIC REQUIREMENTS) Areas near hydrgen gas cylinders must be placarded with a sign saying HYDROGEN- FLAMMABLE GAS-NO SMOKING-NO OPEN FLAMES. Cylinders must be at least 20 feet frm flammable r xidizing materials. Cylinders must be at least 25 feet frm pen flames and ther ptential surces f ignitin. Cylinders must be at least 50 feet frm: Flammable gas strage areas, HVAC intake vents Air cmpressrs. 7
8 CHEMICAL STORAGE/ CHEMICAL LABELING 1.) CHEMICAL STORAGE Stre chemicals n sturdy shelves r cabinets. D nt stack chemicals cntainers n tp f each ther. Physically separate incmpatible chemicals. Use secndary cntainment fr liquid chemicals stred n the flr r near drains. Only use cmpatible materials fr chemical drip trays and secndary chemical cntainers. D nt stre liquids r large glass cntainers (> 500 ml) abve eye level. Chemical cntainers must be clsed and labeled when nt in use. Use certified Lab Safe r Explsin Prf refrigeratrs / freezers fr refrigerated flammable/ vlatile chemical strage. Greater than 10 gallns f flammable liquids must be stred in an apprved Flammable Cabinet. High hazard chemicals (i.e. acutely txic, carcingens, radiactive material) must be stred separately frm ther chemicals in marked strage. 2.) CHEMICAL CONTAINER LABELING Chemicals cntainers must be clsed and labeled when nt in use. Secndary cntainer labels must include: HAZARDOUS CHEMICAL WASTES 1.) LABORATORY WASTE ACCUMULATION POINT (WAP) & WASTE COORDINATORS WAPs must be psted with a sign Waste Crdinatr name and phne number must be included n the sign. Waste Crdinatrs must cmplete EH&S Hazardus Waste Management training. Waste must be stred in the rm it was generated and under the cntrl f the Waste Crdinatr. N mre than 55 gallns f nrmal hazardus waste r 1 quart f acutely hazardus waste (RCRA P-Cdes) can stred at a Waste Accumulatin Pint. 2.) WASTE CONTAINERS, STORAGE & LABELING Cntainers must be clsed and labeled when nt being filled. Unknwn waste shuld be labeled Hazardus Waste: T be Determined. Hazardus Waste tags must be affixed t each waste cntainer befre pickup by NMT Hazmat. Waste cntainer material must be cmpatible with the cntent. Feed lines draining int waste cntainers must frm a snug, airtight fit. D nt fill liquid waste cntainers mre than 75%. Liquid wastes stred n the flr must be in secndary cntainment. Incmpatible wastes must be physically segregated. DO NOT cmbine incmpatible chemical wastes int the same waste cntainer. 8
9 SAFETY TRAINING *Safety Training will be implemented within the next year and the fllwing lists general requirements fr emplyees: 1.) GENERAL REQUIREMENTS Hazmat class: will be required fr anyne wh is wrking with any kind f hazardus material. Lab Standard Training: will be required fr labratry wrkers wrking in labs that have hazardus chemicals. Hazard Waste Management: will be required fr lab safety fficers r waste crdinatrs. Specialized Safety Training: will be required fr wrkers handling specialized hazards such as radiactive r bilgical materials. 9
