2 Page 2 f WORK ISSUES 1.1 Prtectin f Prperty The Cntractr will be held liable fr all damage t persnal and real prperty as a result f his/her negligence r failure t prvide prtective measures. 1.2 Clean-Up Take measures t prtect adjacent area t the cnstructin area frm dust, dirt and debris. Debris shall nt be allwed t accumulate within r arund the wrk area. All debris shall be dispsed f each day ff the campus r in a cntractr supplied dumpster. Dumpsters shall be supplied by the cntractr unless therwise stipulated in their cntract. They shuld be in gd cnditin (e.g., n rusted hles), cntain peratinal lids, and placed away frm strm drains. JABSOM Kaka ak trash cmpactr and rubbish cntainers may nly be used if prir apprval is given. 1.3 Nise The Cntractr shall endeavr t keep the wrk area as quiet as pssible. If pwer activated tls, screw guns r ther nisy devices must be used t accmplish the wrk, the Cntractr shall ntify the JABSOM Kaka ak Facilities Management Office (FMO) and advise them f the type f equipment t be used and the duratin f the wrk t be dne. At times it will becme necessary fr the Cntractr t be stpped frm making nise r vibratins due t adverse effects that the wrk may have upn JABSOM persnnel r activities. The Cntractr shall stp wrk immediately and cntact the FMO when advised that the wrk is adversely affecting staff, faculty, students r thers. 1.4 Strage All effrt will be made t prvide strage/lay dwn areas adjacent t the wrk area. Hwever, due t the size f the prject r its lcatin, this will nt always be pssible. The Cntractr in cnjunctin with the FMO shall determine a strage area. The Cntractr is fully respnsible fr the safety and security f this area. Strage within the wrk area is permitted; hwever, the strage f flammable material r liquids is nt permitted within any building withut prir apprval. 1.5 Behavir Issues The Cntractr and Cntractr s emplyees shall behave prfessinally at all times. 1.6 Smking Plicy JABSOM Kaka ak is cmmitted t the prmtin f a healthy envirnment fr all students, emplyees and visitrs f the campus. The use f tbacc prducts by any persn while in a building, under a cvered sidewalk r walkway, r within 25 feet f any building entrance/exit drway is prhibited. 1.7 Drug-Free Plicy By executing a cntract t wrk n the JABSOM Kaka ak campus, the cntractr certifies that they shall prvide a drug-free wrkplace fr their emplyees and the emplyees f their sub-cntractrs in accrdance with the laws f the State f Hawaii. 1.8 Interactin with JABSOM Kaka ak Emplyees JABSOM Kaka ak expects its faculty and staff t prvide curteus cperatin with Cntractrs. We expect t receive the same level f cperatin in return. All cntact between JABSOM Kaka ak staff

3 Page 3 f 11 and cntractr s emplyees shuld be curteus and plite. The Cntractr shall ensure that their emplyees and their subcntractr s emplyees cnduct themselves in a prper manner while n campus. Gestures, remarks, cat calls, whistling, r anything else f a dergatry nature will nt be tlerated and may be cause fr the remval f an emplyee frm this campus. 1.9 Operatin f Cntractr Vehicles n Campus Cntractrs may be assigned a designated parking area by the JABSOM Kaka ak FMO. Cntractr vehicles shuld be parked in a manner in which they d nt impede the parking f ther vehicles, blck driveway and sidewalk entrances, emergency exits and entrances (fire lanes), and gated areas. Vehicles shall nt be parked in a manner in which they may cause damage t planted r landscaped areas. Cntractrs shuld always drive their vehicles in a safe manner, fllwing speed limits Accessibility t Sensitive/Secure Areas The wrk area fr cntractrs and their emplyees may require them t enter adjacent areas cnnected t their wrk site. JABSOM Kaka ak has several areas which are prtected by access cntrl r ther security measures. The campus envirnment requires sensitivity t staff, student and research activities when entering areas t perfrm necessary tasks related t the cntracted assignment. Therefre, it is necessary fr cntractrs t cntact the FMO t gain access t these areas withut vilating access cntrl measures. 2.0 ENVIRONMENTAL, HEALTH AND SAFETY ISSUES 2.1 Hazardus Materials There are a variety f hazardus materials handled and stred at JABSOM Kaka ak, specifically in the Bisciences Research Building (BSB). Many are in the frm f chemicals and bilgical materials fund mstly in labratries. Cntractrs must avid creating cnditins that culd disrupt labratry peratins. Befre beginning wrk in r arund a labratry, review and cmply with the fllwing guidelines as apprpriate. a. The FMO shall advise the primary researcher fr the area what wrk is t be dne s apprpriate precautins can be taken. The FMO and/r Kaka ak Envirnmental Health & Safety Office (EHSO) will then discuss these precautins with the Cntractr. b. Avid cntact with r expsure t any lab set-ups in the wrk area. If yu require the area t be cleared, cntact the FMO and/r EHSO. D nt attempt t clear the area yuself. c. Read the labels n the materials yu use and be able t prvide infrmatin t anyne asking abut them. Safety Data Sheets (SDSs) fr all the chemicals yu use are t be furnished t the FMO prir t starting any jb at JABSOM Kaka ak. d. Befre using nxius r drus chemicals r ther materials (glues, epxies, paints, thinners, etc.) advise the FMO. The FMO may advise Kaka ak EHSO f what material is being used, including the lcatin and duratin. Kaka ak EHSO will then be able t address building ccupants questins and cncerns regarding the materials being used and any unusual drs. e. When using chemical cmpunds, fllw all precautins as advised by cntainer labels and SDSs. Establish and ensure prper ventilatin prir t using chemicals fr yur safety and fr the safety f building ccupants, especially when wrking with vlatile and drus chemicals. f. If the chemical cmpund yu are using causes prblems fr building ccupants, yu may be tld t limit r suspend wrk until further ntice.

Yu are expected t: erect prtectin barriers fr verhead wrk that may pse a hazard t peple belw; prtect yur equipment, hses and pwer crds frm inadvertent damage and resulting hazard t anyne in the area; btain immediate assistance fr any accident invlving yur equipment r anyne in the area. Yu must keep emergency numbers handy at all times and knw where t find a phne. h. Yu are expected t use due cautin with and arund any hazardus materials. If yu have questins r cncerns abut this, cntact the FMO. i. Familiarize yurself with the lcatins f the fllwing: Fire extinguishers Fire alarm pull statins Emergency exits Emergency eye washes Emergency shwers First aid kits (lcated at all security desks) AEDs (lcated at all security desks) Telephnes (security s phne numbers are and ; yu must dial 9 first befre dialing 911) 2.2 Waste Handling Any cntractr generated waste n JABSOM Kaka ak prperty needs t be managed and dispsed in accrdance with HiOSH, EPA/DOH and DOT requirements. Cntractrs are respnsible fr the prper handling, strage, transprt, treatment r dispsal f the hazardus wastes and special categry wastes they generate. Examples f hazardus wastes include, but are nt limited t, cleaners, pesticides, paints, glues, thinners, flr r ther surface finishes, remvers r drying agents. Examples f special categry hazardus wastes include mst flurescent bulbs, high intensity discharge lamps, lamp ballasts and batteries that cntain heavy metals (Ni-Cd, lead-acid, mercury, lithium, silver xide, etc.). a. Flurescent lamps are t be remved frm fixtures with care and placed in cardbard bxes. HID lamps are t be remved and placed in cardbard cartns. Take necessary precautins t prevent damage t lamps cntaining mercury. b. Flurescent lamp ballasts may cntain PCBs (plychlrinated biphenyls) and if s, are subject t significant regulatin. If ballasts are labeled Des nt cntain PCBs, then they may be dispsed f as rdinary waste. If ballasts are nt labeled, they must be treated as if they cntain PCBs and placed in an apprpriate cntainer fr dispsal as a regulated waste. It is a vilatin t dispse f nn-pcb ballasts with PCB cntaining ballasts. c. Batteries used by cntactrs are the cntractr s respnsibility and are t be remved frm JABSOM Kaka ak premises when spent. Batteries f any type, size r cnfiguratin must nt be placed in JABSOM trash receptacles r the trash cmpactr. d. Fr any asbests cntaining materials/asbests abatement, cntact JABSOM FMO and EHSO. e. Lead-based paint remved frm structures r their cmpnents shall be cnsidered hazardus waste. In an unexpected situatin Kaka ak EHSO can assist in determining if paint r painted materials cntain lead and must be handled and dispsed f in accrdance with applicable regulatins.

5 Page 5 f 11 f. Tritium gas-cntaining exit-signs, when remved under renvatin wrk, must be cllected and dispsed f prperly as radiactive material. JABSOM FMO and Kaka ak EHSO will assist with this as the need arises. 2.3 Spills Regulatry agencies require cntainment and remediatin f all spills f hazardus materials, including fuels and il. Cntractrs wh spill any such substance n JABSOM Kaka ak premises, r accidentally discharge int a strm drain r sanitary sewer drain, are t immediately reprt the incident(s) t the FMO. In the case f an accidental spill, the cntractr will clean it up as sn as pssible. Fr strm drain discharges, the cntractr will immediately stp and/r redirect the discharge surce. The FMO will reprt all spills f hazardus material and any illicit strm drain discharges t Kaka ak EHSO immediately. In additin, spill ntificatin t state and federal agencies may be required. Cleanup f the cntaminated area must be perfrmed t the regulatry acceptable level. Any clean-up and required testing will be paid fr by the cntractr. Kaka ak EHSO requires apprpriate dispsal and dcumentatin f cleaned up wastes, waste-cntaminated items and sil r ther cntaminated materials. 2.4 Site Safety All cntractr persnnel wrking at the JABSOM Kaka ak campus are required t abide by precautins required f them. Cntractrs must nt create hazards fr JABSOM Kaka ak persnnel and students. In special circumstances where this cannt be avided, prper warning signs and barriers must be erected and supervised by the cntractr during the wrk activity t keep unauthrized persns ut f the area. a. If nn-cntractr persnnel need t enter the wrk area, the Cntractr may be expected t prvide apprpriate persnal prtective equipment fr such visitrs at any time. b. Safety equipment supplied and used by cntractrs is expected t cmply with OSHA requirements. Cntractr emplyees are required t prperly wear and maintain the persnal prtective equipment prvided t them by their management s their persnal safety is nt cmprmised, causing ptential risk t thers. c. Any situatins nted where JABSOM Kaka ak activities create ptential hazards fr the cntractr shuld prmptly be brught t the attentin f the FMO. 2.5 Fire Alarms and Prtectin Systems Prir t perfrming any wrk, cntractrs shuld assess the wrk and the site t determine if any existing fire alarm, sprinkler system, r ther fire prtectin system (haln gas, CO2, dry chemical) culd ptentially be activated r its peratin hindered in any way. Systems may be activated by renvatin dust, smke r heat generating equipment. If it is determined that the fire alarm needs t be temprarily placed ut-f-service t perfrm the wrk, the cntractr must ntify the JABSOM FMO t arrange this. 2.6 Exit Crridrs The exit crridrs f all areas must be kept clear and unblcked at all times regardless f their width. All carts, supplies, ladders, tls, etc. must be kept ut f the crridrs when nt in use. Sme prjects require a cnstructin partitin that captures part f the crridr width. When this happens, it is extremely imprtant that the remaining crridr be clear. 2.7 Welding/Burning/Trch/Sldering

6 Page 6 f 11 JABSOM Kaka ak has an established N pen flames plicy. N pen flame wrk is allwed withut a permit. A cpy f the plicy may be btained frm the FMO. If it is determined that pen flames must be used, the Cntractr can submit an Open Flame Permit Applicatin (Attachment B). 2.8 Gas Cylinders All cmpressed gas cylinders must be transprted, used, and stred prperly in a secure lcatin. All cylinders, full and empty, will be labeled, capped when nt in used, segregated as necessary, and secured in place at all times. D nt stre mre than 5 cylinders tgether in ne lcatin. Remve all cylinders at the cmpletin f the prject; JABSOM will nt dispse f empty cylinders. 2.9 Ersin & Sediment Cntrls JABSOM Kaka ak maintains a strm drain management prgram in cmpliance with its DOH NPDES- MS4 permit. Befre any sil disturbing peratins cmmence, the cntractr will ensure all ersin and sediment cntrl measures are in place and functinal, as per the prject specific ersin cntrl plan. These measures will be prperly cnstructed and maintained thrughut the cnstructin perid. All cntrl measures will be checked weekly in dry perids and within twenty-fur hurs after any saturating rainfall. During prlnged rainfall, daily checking will be cnducted. Illicit discharge int the strm drains is nt permitted. Accidental r illicit discharges t the strm drains must be reprted t the FMO immediately Fire Sprinkler/Hydrant Testing Guidelines Accrding t the MS4 Permit, JABSOM must prevent illicit discharges f nn-strm water frm entering its strm drain systems, which includes water generated during fire sprinkler/hydrant testing. As such, it is very imprtant fr all cntractrs t cmply with the permit requirements by fllwing these prcedures t the best extent practicable during all phases f testing, draining, and equipment setup and breakdwn. 1. Cnduct activity n nn-rainy days and fr the shrtest duratin pssible t minimize discharge vlume. 2. Prtect nearby strm drains (e.g., inlet prtectin). 3. Delineate wrk area fr pedestrian and vehicular safety. 4. Initial hydrant flws must be diverted t landscaped areas until a clear water flw begins. When diverting flws, be careful nt t: create a safety hazard, kill vegetatin, cause ersin, r drain t areas with pr infiltratin. The clear water may be discharged int the strm drain system (r simply left t drain nt the grassy area). 5. If it is impssible t discharge initial flws int a grassy area, then the cntractr may cntain the initial flw using tanks r drums. Never discharge any f the cntained water directly int the strm drain system. Instead, dispse f it: Off-site using the services f a licensed waste transprter; r, Drain nt landscaped areas. 6. Maintain recrds, which shuld cntain the fllwing: Sprinkler lcatin; Persn perfrming the task; Receiving pint (e.g., landscaped area, cntainment tank); and Apprximate amunt f water discharged Chlrdane/Organchlrine Pesticide Testing in Sil

7 Page 7 f 11 Please refer t Attachment A fr additinal infrmatin. 3.0 UTILITIES ISSUES The Cntractr will ntify the PM at least ne week befre the desired date f any utility shut dwn. The Prject Manager will schedule the shut dwn as clse t the desired date as pssible and will ntify the Cntractr f the scheduled date and time. 8 Attachment A University f Hawaii at Mana Fact Sheet fr Handling Technical Chlrdane & Organchlrine Pesticides in Sil Infrmatin fr Design Staff: When t Cnduct Technical Chlrdane & Organchlrine Testing: The design cnsultant will usually identify the need fr this particular testing, which is based n prject requirements. Hwever, sme generic identifiers are listed belw: The prject will be disturbing sil belw, r within tw t three feet, f a building. Chlrdane/Organchlrine pesticides were either knwn r expected t have been applied t a particular area. A building, r grup f structures, is planned fr demlitin. Develpment f Prject Specs: As is usual practice, these shuld be develped by the design cnsultant; and, reviewed by the Envirnmental Health & Safety Office (EHSO). While EHSO has develped example specs, they are nly meant fr reference. The cnsultant must ensure certain elements are included in the specs: Sil Sampling Plan: A sil sampling plan, which details the number, methd (required t take cmpsite samples when pssible), and depth f samples must be sent t EHSO fr review. Identificatin f Prper Lab Testing: Cnsultants must have their designated lab use USEPA Lab Methd 8081A. The lab must reprt the cncentratin f the technical chlrdane mixture (including chlrdane) rather than the individual chemicals assciated with technical chlrdane. The lab shuld still reprt ther, unrelated rganchlrine pesticides, like DDT, which are generally included in the 8081A analysis. Wrk Plan: A wrk plan, which must als be separately submitted t EHSO fr review, shall describe items such as engineering cntrls, sil stckpiling and segregatin, sampling, backfilling prcedures, persnal prtectin requirements, wrk area islatin, air mnitring, cnstructin barriers, wetting methds, decntaminatin prcedures, and emergency prcedures. Lastly, the plan shall include the names and qualificatins f persnnel wh will be managing sil activities at the site. Infrmatin fr Prject Managers: Cntractr Dispsal Dcument: When the cntractr cmpletes handling f the sil (e.g., caps it under 2-3 feet f nn-cntaminated sil; buries it elsewhere n campus; remves it ff campus, etc.), they must prvide written dcumentatin indicating hw the sil was addressed. A cpy f this dcument shuld be given t EHSO. Recrd Keeping: Ntate where sil is buried n utility maps, as-builts, etc.

10 Attachment B OPEN FLAME PERMIT APPLICATION Cntractrs & Facilities The use f pen flames in Kaka ak Facilities is strngly discuraged by the University f Hawaii Envirnmental Health and Safety Office. Every effrt shall be made t find alternatives t using pen flames. In the event that n alternatives are adequate fr yur type f wrk, yu may submit this Open Flame Permit Applicatin fr the temprary use f pen flames. Once cmpleted, submit this frm t JABSOM FMO fr apprval. 11 Attachment B Please check t verify cmpliance: REQUIRED PRECAUTIONS CHECKLIST I have ensured that all flammable materials are stred prperly and away frm pen flame wrk area. All ht wrk equipment has been examined and ensured it is in a safe perating cnditin. Apprpriate PPE/clthing is wrn (nn-cmbustible materials, nn- expsed cuffs r pckets) All gas cylinders have been prperly secured. The valves n all gas cylinders will be clsed when nt in use. All wall and flr penings have been cvered. The pen flame wrk area is clear f cmbustibles r cmbustibles are prtected. Flr f pen flame wrk area is kept clean and free f clutter (11 meter r 35 ft radius). Fire extinguisher is fully charged and perable, and nearest lcatin is knwn. Nearest fire alarm pull statin lcatin is knwn. There is ample ventilatin t remve smke/vapr frm wrk area. A 30 minute bservatin perid f the wrk area shall be maintained fllwing cmpletin f ht wrk peratins in rder t ensure n smldering fires.
