1 Safety Plicy Cde fr Cntractrs This dcument cvers the minimum safe wrk practices in regard t fire, safety, security, health, envirnmental and traffic regulatins that gvern prime cntractrs and subcntractrs while wrking within the cnfines f any Flex N Gate facility (the Wrk Site ). These requirements are cnsidered as additins t any safety rules r regulatins f the cntractr. The cntractr shall be respnsible fr infrming each member f their team and all subcntractrs f fire, safety, security, health, envirnmental and traffic requirements and standards at all Flex N Gate facilities. 2 Cntractrs are respnsible fr the cnduct f their persnnel while n Flex N Gate prperty. Failure t maintain prper standards f cnduct at all times may and will result in immediate remval f cntractr persnnel frm Flex N Gate prperty. Upn request, cntactrs are respnsible fr prviding Flex N Gate Systems with their Injury and Illness Preventin Prgram (IIPP) prir t start f jb. Cntractrs shall prepare, cmmunicate t its emplyees, and enfrce the Injury and Illness Preventin Prgram. If yu have questins regarding safety requirements, r abut the safety f a particular peratin r activity, cntact yur supervisr r yur emplyer s Safety Officer. Yur Designated Prject Manager r Flex N Gate Health & Safety Representative are available fr cnsultatin r questins regarding Health & Safety. Definitins Flex N Gate Prject Manager means the individual within Flex N Gate wh has the respnsibility f managing the cntract and the prject the cntractr is wrking n. Designated Prject Manager means the individual wh has been delegated the respnsibility f managing the cntract and the prject the cntractr is wrking n. Flex N Gate Lss Preventin Representative means the designated individual within Flex N Gate wh is respnsible fr managing Flex N Gate s Envirnmental Health and Safety Prgram. General Infrmatin 1. EMERGENCY NOTIFICATION PROCEDURES/ACCIDENT REPORTING In case f any accident, injury r illness, always cntact yur Flex N Gate Prject Manager at the Wrk Site. Cntractrs are respnsible fr prviding first aid t their emplyees, alng with prviding first aid related equipment. If a serius accident has taken place n Flex N Gate prperty, the cntractr shall immediately ntify the Flex N Gate Prject Manager, then submit a written accident reprt. The reprt shuld be directed t the Flex N Gate Prject Manager, as well as the lcal general manager. This written reprt must be received n later than 24 hurs after the incident. 2. AISLES AND STAIRWAYS Cntractrs must cmply with applicable requirements established by law. Stairways and aisles must remain free f debris and cleaned up befre, during and after wrk is perfrmed. 3. HAZARD COMMUNICATION (HAZCOM) Cntractrs must maintain an effective prgram in cmpliance with applicable requirements established by law. Safety Data Sheets (SDS s) fr all chemicals must be submitted t the prject Manager, and apprved prir t use. All chemicals must remain labeled using acceptable labeling methds such as GHS, HMIS r NFPA frmatting. 2

CHEMICALS/CHEMICAL SAFETY The Cntractr will nt dispse f any chemical waste r slutins cntaining chemicals n Flex N Gate prperty withut first cnsulting with the Flex N Gate Envirnmental representative. Chemical waste includes, but is nt limited t: Acids, bases Adhesives, glues, cements, epxies Bleaches, cleaners, detergents Cmpressed and liquefied gas Cre slder (acid, resin) Fluxes Greases, ils Paints, dyes, pigments, fillers Pesticides, herbicides Slvents, thinners Flurescent tubes (Lcal gverning rules apply) VDT s Empty cntainers that nce cntained any chemicals (refer t SDS) Chemical wastes will nt be dumped int strm drains, industrial waste r sanitary waste drain systems. Drains, sewers, sinks, rest rms, trenches, trashcans r the grund shall nt be used fr dispsal f chemicals. Jb sites must be cleaned up when cmpleted and unused r left-ver items remved. General trash such as wd, nails, plastic wrap, etc. must be remved frm the jb site and dispsed f prperly. The facility Envirnmental Representative MUST be cnsulted regarding dispsal f any and all regulated r chemical wastes. The Envirnmental Representative can be cntacted thrugh the designated Prject Manager. The cntractr is respnsible t prvide cpies f manifests and any ther dcumentatin with regard t waste remval, transprt and dispsal. 5. COMPRESSED GAS CYLINDERS All cmpressed gas cylinders shall be legibly marked with the name f the material cntained. Cylinder marking shall cnfrm t the current American Natinal Standards Institute (ANSI) standards and be marked by means f stenciling r stamping. The Cntractr will nt stre gas cylinders n any Flex N Gate premises. Any unusual circumstances which may require cylinder strage must be reviewed and have prir apprval f the Designated Prject Manager and Envirnmental Representative. 3
4 Cmpressed gas cylinders will be secured (rped r chained t an immvable bject) in an upright psitin at all times, except when cylinders are being transprted. All cylinder mvement must be prperly cnveyed with hand carts with secured cylinders. Mvement with ther cnveyance means is strictly prhibited, e.g. Frklifts. Valve prtectin caps will be firmly installed (hand tight) when cmpressed gas cylinders (empty r full) are transprted r stred. Cylinders will be kept a safe distance r shielded frm welding and cutting peratins. Cylinders will nt be placed where they can cntact an electrical utlet. Gas cylinders may nt be stred inside ffice areas. The crrect regulatrs, in prper wrking rder, shall be used fr each type f gas. And shall nt be mdified in any way. Cylinder valves will be clsed when wrk is finished and when cylinders are empty r are being mved. If a gas cylinder develps a leak, immediately remve it t a safe lcatin utside the building. If the leak cannt be crrected, call the Designated Prject Manager. 6. CONFINED SPACE ENTRY Each cntractr must cmply with cnfined space entry requirements established by applicable law. If the scpe f wrk the cntractr is invlved in invlves cnfined spaces, cntractrs are required t include a written cnfined space plan. This plan must be prvided t the designated Prject Manager. Cnfined Spaces include strage tanks, bins, sewers, in-grund vaults, tunnels, manhles, pits r any ther lcatin which: 1. Is large enugh fr wrkers t enter and perfrm certain jbs; 2. Has limited r restricted means fr entry r exit; 3. Is nt designed fr cntinuus ccupancy; A permit required cnfined space is any cnfined space which: 1. Cntains r has the ptential t cntain a hazardus atmsphere; 2. Cntains material that has the ptential t engulf an entrant; 3. Has walls that cnverge inward r flrs that slpe dwnward and taper int a smaller area which culd trap r asphyxiate an entrant; 4. Or cntains any ther recgnized safety r health hazard, such as unguarded machinery, expsed live wires, r heat stress; All wrk in permit required cnfined spaces requires a cntractr generated CONFINED SPACE ENTRY PERMIT. Permits must be reviewed by the Designated Prject Manager prir t entry int a cnfined space. The cntractr shall prvide the required persnnel safety equipment and cmpetent persns. All wrk inside permit required cnfined spaces requires the fllwing safety precautins at a minimum: Additinal requirements may be added based n hazards invlved in particular jbs. 4

A written permit Use f an attendant utside the space and entry supervisr Prper PPE fr the jb (ie: hard hats, fall prtectin, respiratry prtectin, safety glasses, hearing prtectin, glves, chemical prtectin, safety-te bts, etc.) Air mnitring within the space Sufficient lighting and cmmunicatins between the entrant and attendant A rescue plan 7. CONTROL OF HAZARDOUS ENERGY (LOCKOUT/TAGOUT) All cntractrs must cmply with applicable lckut / tagut requirements established by federal, state and lcal laws. Flex N Gate Prject Managers will prvide all cntractrs wh are engaged in electrical wrk a cpy f ur Lck Out/Tag standard if required. If the scpe f wrk the cntractr is invlved in invlves hazardus energy requiring the use f lckut / tagut prcedures, the cntractr must prvide the Prject Manager with a cpy f their Energy Cntrl Plan fr review. The cntractr MUST cmmunicate with the prject manager regarding questins n what, where, when and hw t PROPERLY lckut equipment. Much f the equipment used within Flex N Gate facilities is very technical and requires precise shutdwn / start-up prcedures in rder t prtect bth persnal and equipment frm damage. The Flex N Gate prject manager MUST be cnsulted and invlved in cmmunicating these prcedures t cntractrs prir t wrk being perfrmed. Lckut tags will identify the cntractr s cmpany name, date and the lcked ut emplyee s name. 8. EXCAVATIONS AND TRENCHING All cntractrs must cmply with applicable requirements fr trenching and shring established by federal, state and lcal laws. 9. FLOOR / WALL OPENINGS All cntractrs must cmply with applicable requirements established by federal, state/prvince and lcal laws. Flr penings and penetratins greater than 2 (5cm) must be cvered/prtected. Apprpriate signage/warning - Danger! Flr Hle Cver D nt Remve Flr r wall penings must be guarded t prevent accidental access and fall hazards using a standard barrier (railing) cnsisting f a tp rail at 42 inches, mid rail (between the flr and tp rail) and te bard. The standard barrier (railing) must be cnstructed t supprt a minimum f 200 lbs in any directin. Cautin, r barrier tape may be used as a visual aid, but alne is NOT sufficient in guarding ff flr r wall penings. 5
6 10. HELICOPTER LIFTS Any prpsed usage f helicpters t hist, set, install r mve equipment r materials must have prir apprval frm the Designated Prject Manager. Helicpter lifts may require a permit frm the lcal jurisdictin (city r cunty). The entire building must be evacuated prir t a helicpter lift. 11. LADDERS All cntractrs must cmply with applicable requirements established by federal, state/prvince and lcal laws. Ladders must be inspected fr damage befre use and remved frm Flex N Gate prperty if fund t be damaged in any way. Ladders must be held, and/r tie-ff prcedures used, t reduce r eliminate fall hazards. Ladders f the crrect height and style must be used as apprpriate fr specific jbs. At n time may wrkers stand n the tp rung/step f any ladder. 12. SPECIFIC HAZARD AREAS / SAFE WORK PLANS There are certain areas and peratins n the Flex N Gate site where extra precautins must be taken due t the nature f the hazard. Befre entering int, r wrking n r in, any f the fllwing situatins, the cntractr is required t check with the Designated Prject Manager fr a review f applicable safety, security and health rules. The cntractr must als submit t the Designated Prject Manager a written a written SAFE WORK PLAN. Wrking at heights, in ceiling areas r building rfs; Installatin, remval r mdificatin f electrical, r gas systems; Machinery installatin, remval r mdificatin; High vltage electrical areas; Wrk invlving hazardus chemicals; High nise level areas; Excavatin r trenching; Wrk inside cnfined spaces (as defined in sectin 6); Areas invlving chemical hazards such as plating, labratries, waste treatment areas, etc. Cntractrs shall take apprpriate measures t safeguard and cntrl pedestrian traffic int r near cnstructin areas 13. HOUSEKEEPING Wrk areas must be kept neat and rderly AT ALL TIMES, fr bth safety and security reasns. 6
7 Material shuld be carefully stacked and lcated s that it des nt blck aisles, drs, fire extinguishers, fire blankets, eyewash statins, first aid bxes, fixed ladders, electrical equipment r stairways. Bxes are nt t be stacked ver 4 feet high. D nt allw yur tls, equipment and materials t impact areas ccupied by Flex N Gate. Clean up all nn-hazardus liquid spills and drips immediately. Hazardus material spills shuld nt be handled withut the prper equipment and training. All Hazardus Materials spills are t be immediately cmmunicated t the Site Designated Prject Manager and the Flex N Gate Envirnmental Representative Cncrete frm and scrap lumber and all ther debris will be kept clear f all wrk areas. Cmbustible scrap, waste materials and debris will be remved frm the building n a daily basis, preferably at the time f strip-ut. Overhead strage f debris, tls, equipment, pipes, etc. is prhibited. N lse material will be left in the area abve suspended ceiling panels. Interir cnstructin areas will be swept clean and all trash will be remved at the end f each wrkday. The wrk area will be kept free frm any ptential tripping hazards and nails prtruding frm bards must be remved. All ceiling tiles remved by the cntractr will be replaced immediately after the wrk is cmpleted, in an undamaged cnditin. 14. OVERHEAD WORK / FALL PROTECTION N verhead wrk will be perfrmed when, as a result f that wrk, the pssibility f a falling bject injuring any persn exists. Relcatin f persnnel will be accmplished prir t and maintained thrughut the verhead wrk perid. The cntractr will make all persnnel relcatin requests t the Designated Prject Manager. Cntractr persnnel are nt permitted t crawl alng ductwrk, cable trays, piping r ther building structures. Overhead hazard areas will require the wearing f hard hats fr thse entering the area. Fall prtectin (harness, lanyard, lifelines, etc.) is required fr elevated wrk requiring persnnel t be at levels abve 4 feet frm an adjacent level withut the prtectin f a standard barrier (as defined in sectin 10 Flr / Wall Openings ). D nt secure fall prtectin systems t equipment such as sprinkler systems,, utility piping and electrical cnduit, since they d nt have enugh structural supprt. Apprpriate anchr pints which meet OSHA regulatins must be used fr fall prtectin systems. Fall prtectin is required during use f persnnel lifting devices such as JLG lifts, 7
8 Scissr lifts, Cherry pickers, etc. when elevated abve 4 feet. Frklifts may nt be used t lift peple. 15. PERMITS AND APPROVALS WORK AUTHORIZATIONS The cntractr must wrk with the Prject Manager ensuring that all required permits and apprvals are secured befre wrk begins. Such permits may be required by lcal / state agencies such as building permits r electrical permits. Others may be required by OSHA r Flex N Gate Health and Safety, such as ht wrk permits r cnfined space entry permits. Any wrk which will require any part f the sprinkler system t be deactivated requires special permitting and reprting t Flex N Gate s prperty insurance cmpany. Befre prceeding with this wrk yur prject manager must be infrmed. 16. PERSONAL PROTECTIVE EQUIPMENT All cntractrs must cmply with applicable requirements established by federal, state/prvince and lcal laws. Cntractrs are expected, and required, t prvide all PPE t their emplyees based upn the site s PPE requirements. The emplyees f cntractrs are required t wear required PPE at all times while wrking n Flex N Gate prperty. PPE must be inspected and in safe wrking cnditin at all times. 17. ROOF WORK All cntractrs must cmply with applicable requirements established by federal, state/prvince and lcal laws. Befre accessing a rf, cntractrs must have apprval frm the Prject Manager. While wrking n rftps, cntractrs are expected t fllw the guidelines in Sectin 15 Overhead Wrk / Fall Prtectin f this dcument. Systems such as: fall arrest systems, warning line systems, buddy systems and guardrail systems are expected, and must be used by cntractrs, in rder t prvide prtectin t their emplyees while wrking n rftps n Flex N Gate prperties. Special hazards such as cnduits, piping, ducting, skylights, scrubbers, HVAC, etc. must be identified and guarded t prevent accidental expsure while wrking n rftps. 18. WELDING / HOT WORK Any welding, cutting, sldering r ther pen flame wrk, either inside r utside n Flex N Gate prperty, must utilize a Ht Wrk Permit in rder t dcument prcedures fllwed t reduce fire hazards. Permits must be btained frm the Designated Prject Manager. The cntractr will cntinue the peratin nly as lng as cnditins remain safe. 8
9 All cmbustible materials in the area must be prtected r remved befre beginning wrk. Fire blankets and ther preventive devices must be used in rder t reduce the fire hazards during ht wrk. An apprved fire extinguisher must be within reach f all ht wrk peratins and ther pen flames. Cntractrs must prvide fire extinguishers fr use during their ht wrk peratins, except in emergencies. The cntractr is respnsible fr seeing that a fire watch is maintained thrughut the ht wrk peratin, and fr a designated time afterward as directed by the ht wrk permit. The fire watch is respnsible fr taking apprpriate actin t prevent a fire, t extinguish incipient fires, and t summn assistance frm emergency services if any fire is nticed and/r cannt be extinguished. Nn-cmbustible r flameprf shields r screens must be placed arund the wrk t shield thers frm eye damage and prtect frm direct welding rays r arc. Exhaust ventilatin is required when perfrming welding r cutting in cnfined spaces, r where fumes may present a health hazard r nuisance expsure t yu, Flex N Gate emplyees r thers. 19. UTILITIES It is imprtant that yur wrk des nt affect utility services t the Flex N Gate premises particularly thse in ccupied areas. Cntractr(s) must nt perate any switch r valve, which cntrls building services, prcesses r equipment unless cntractr(s) supervisr has btained written authrizatin frm the Designated Prject Manager. A lckut/tagut system, which cmplies with gvernment regulatins, alng with sectin 8 f this dcument, must be used t identify and secure switches and cntrls in the OFF psitin while wrk is being perfrmed 20. WARNINGS / SIGNAGE Warning, Danger, Cautin, Ntice and ther safety and security signs, ntices and barriers are psted fr yur prtectin. They must be bserved and fllwed at all times. When the wrk being perfrmed has the ptential t expse Flex N Gate emplyees r thers t hazards, cntractrs must prvide warning signs, barriers r barricades wherever such prtectin is required by this dcument, r where safe practice dictates it. Where signs and barricades d nt prvide adequate prtectin, flagmen (r ther additinal) prtectin is required. Multi-lingual signage may be required. 9
10 Security 1. IDENTIFICATION BADGES When yu reprt t wrk n Flex N Gate premises, yu will be issued a temprary identificatin badge that will designate yu as a visitr / cntractr, authrizing yu t enter Flex N Gate cntrlled space. A Flex N Gate identificatin badge is required t be wrn in plain sight at all times by all visitrs and cntractrs wrking n Flex N Gate premises. The badge must be returned t the Security Pst r lbby where it was issued upn departure frm Flex N Gate premises. Unauthrized use f this badge such as giving it t anther persn is prhibited. Cntractrs assigned t wrk at Flex N Gate n a lng-term, daily basis may als need t submit required paperwrk and have their pictures taken and wear a Flex N Gate picture badge. Cntact the Designated Prject Manager t arrange this. Cntractrs are respnsible fr the accuntability and prper use f access cards and badges issued t them and their persnnel. Direct any badging questins r prblems t the Designated Prject Manager. 2. PROHIBITED / RESTRICTED ITEMS Alchlic beverages, nn-prescriptin drugs r prescriptin drugs fr nnmedical use, firearms, ammunitin r any ther material cnsidered a weapn are nt permitted n Flex N Gate prperty. Smking is strictly prhibited in all Flex N Gate buildings, including the rf. 3. CONTROLLED ACCESS Fr Security reasns, entry and exit f Flex N Gate buildings is restricted t thse areas designated as yur wrk area. Entrance t an area ther than where yu are wrking is prhibited, unless authrized by the Designated Prject Manager. Building emergency exits are t be used nly in the event f an emergency. Withut exceptin, there shuld never be an ccasin fr a secured card access, emergency r any ther secured drs t be left prpped pen, unattended, r unlcked. Cntact yur Flex N Gate Designated Representative befre pening and entering any rf access areas. 4. SMOKE DETECTORS The Cntractr is cautined that there are many areas thrughut Flex N Gate that cntain smke detectrs. These detectrs are sensitive t smke, dust and ther airbrne particles, that can either, hamper prper peratins r cause them t alarm. Examples f smke detectr lcatins are: cmputer rms, cafeteria s training and meeting rms, return air ducts and adjacent t fire drs and crridrs. 10
11 The Cntractr must evaluate the cnditins that may result frm any peratin/cnditin that may prduce smke, dust, flying r ther airbrne particles must be previusly apprved by the Designated Prject Manager. The Cntractr will ensure all apprpriate parties, Lss Preventin, Designated Prject Manager, lcal site cntact, etc are ntified when wrking in an envirnment with active smke detectin equipment. If the wrk requires the Cntractr t encapsulate individual smke detectrs, he/she will ensure all accessible smke detectrs, within the wrk area, are bagged prir t any wrk. The Cntractr shall als ensure that at the end f each wrkday that all bags are remved frm the smke detectrs. 5. PARKING / SPEED LIMITS Cntractr(s) shall fllw parking arrangements as instructed by the Designated Prject Manager. Cntractr vehicles will be parked in designated parking areas nly. Parking in reserved spaces, patis, walkways, sidewalks, in frnt f any buildings r radways is strictly prhibited. Imprperly parked vehicles will be twed at the wner s expense. Lading dck areas may be used fr lading and unlading, but afterwards the vehicle must be mved t the designated parking area. 8 mph (12 km/h) speed limit will be bserved at all times while driving in Flex N Gate parking lts and radways. Oversized vehicles, such as dump trucks, garbage trucks, ready-mix rigs, etc. must be equipped with audible back-up alarms. Standards and Cnduct Cntractrs are t prvide t Flex N Gate/r designee, and pay all fees fr, all permits, inspectin r ccupancy certificates, maintenance and peratin manuals, equipment, warranties, etc. Shuld the Cntractr perfrm any wrk that des nt cmply with the requirements f applicable cdes and standards, they shall bear all csts that arise in crrecting such defects. All Cntractrs are required t supply Flex N Gate/r designee with an apprpriate & valid Certificate f Insurance cvering Wrkman s Cmpensatin and Liability Insurance befre they can wrk in the building. It is the respnsibility f the General Cntractr t mnitr cmpliance. Cntractrs must maintain existing plumbing, heating, air cnditining, fire prtectin, and ther existing systems, and must retain all existing functins in service except fr scheduled disruptins. Preferably, all such disruptin(s) f system functins shuld be perfrmed after nrmal business hurs r n the weekend. All wrk invlving cre drilling, spraying, r ther functin that may cause disrupting nise, fumes, r dr, r result in necessary access t any ccupied spaces, must be apprved by the Designated Prject Manager. All penetratins f piping, duct wrk, cnduits, etc. thrugh walls, partitins, and flrs shall be sealed t building cde requirements t maintain the integrity f the structure s 11
12 fire safety rating. Als, any penings in walls and partitins made by the Cntractr fr access t cnstructin wrk shall be patched and/r repaired t Flex N Gate s satisfactin. All cre drill pieces are t be remved by the Cntractr, with the exceptin f fire sprinkler penetratins, which shuld be attached t the pipe fr the inspectr t witness. It shall be the respnsibility f each trade t cperate fully with the ther trades n the jb t keep the jb site in a clean and safe cnditin. At the end f each day s wrk, each trade shall prperly stre all f their tls, equipment, and materials and shall clean up their debris frm the jb site. The cntractr shall prvide a final clean-up f the jb site including walls, light fixtures, windwsills, cunters, flrs, etc. (i) (ii) (iii) Sweeping Cmpund will be used when sweeping t minimize the raising f dust. Failure t use sweeping cmpund may result in the mandatry halting f wrk as enfrced by Flex N Gate s designated persnnel. Cntractrs are respnsible fr remving all cnstructin debris frm the site. The Flex N Gate/r designee trash facility will nt be used fr cnstructin debris. Cntractr will be back-charged fr unauthrized use f Flex N Gate/r designee trash facility. The building janitrial service may be cntracted fr clean-up wrk. Cntractrs shall nt utilize electrical/telephne rms, mechanical rms, r ther unauthrized buildings r paring areas fr strage f tls, equipment, materials, supplies, etc. Cntractrs shall be restricted t designated tilet facilities. Under n circumstances are these facilities t be used fr purpses ther than their intent (n tl cleanup, etc.). Cntractrs shall nt lunge r eat in the building lbbies, hallways, r stairwells. Breaks may be taken either in the assigned wrk area, r in cmmn areas such as a restaurant r snack bar t be determined by the Designated Prject Manager.
